Influence of feedback and attitude of patient and clinician toward outcome measurement.

This presentation is about the influence of feedback of treatment results to patient and clinician during the treatment process and their attitude toward the introduction of the two measurement instruments HoNOS and OQ in a clinical non-academic mental healthcare setting. Feedback in which results of the treatment are fed back to clinician and patient could influence the results of the treatment. It is important to know the attitude of both patients and clinicians towards the introduction of the instruments affecting the clinical care process. If patients are not willing to participate in filling in questionnaires during their treatment and clinicians are objecting toward feeding back treatment results to patients during treatment the effectiveness of outcome measurement will not be that effectives. Knowledge of the attitudes and the factors that play a role in the implementation can be of great importance for the success of the implementation. In this presentation a study will be presented on the attitude of patients concerning outcome measurement and feeding back the treatment results.

This presentation is about implementing a new system into the clinical care process. It is not an easy task to do. The implementation of the outcome measurement instruments the HoNOS and the OQ is made visible with a case history in an outpatients clinic for anxiety and mood disorders in an non-academic setting. The difficulties and challenges during the implementation process are discussed. The term 'operations management” will be introduced and how operation management can help to overcome these hurdles and transform them into challenges. OM could be a tool that can make effects visible and acceptable during the implementation process.

L’internat est une période de stress chronique élevé pour les étudiants en médecine et des travaux récents révèlent une prévalence importante des trouble psychiatriques au cours de cette période [1,2]. L’Association française fédérative des étudiants en psychiatrie (AFFEP) a mené une enquête afin de faire un état des lieux sur la prise en charge des internes de psychiatrie en souffrance psychologique au cours de leur cursus. Trois groupes ont été interrogés : les référents AFFEP, qui représentent les internes de psychiatrie dans chaque subdivision, les coordinateurs locaux et interrégionaux du DES de psychiatrie et du DESC de psychiatrie de l’enfant et de l’adolescent. Entre 2013 et 2015, 86 internes ont présenté des difficultés psychologique soit environ 4 % des internes. Cela s’est principalement traduit par des difficultés ou un absentéisme en stage, la mise en arrêt de travail, et quelques rares cas de suicides ou tentatives de suicide ont été décrits. Les premières personnes à signaler les difficultés étaient majoritairement le médecin chef de service, l’interne lui-même ou son co-interne. Une fois les difficultés signalées, des mesures professionnelles étaient mises en place dans 35 % des cas, et 15 % des internes concernés bénéficiaient de soins. La médecine du travail était peu sollicitée, tandis que le coordinateur local, les co-internes, et l’association locale représentaient les principaux intervenants. Pour tous, ce sont les coordinateurs locaux qui sont jugés les intervenants les plus légitimes dans ces situations. Les dispositifs d’évaluation de la santé des internes, en particuliers les comités médicaux, étaient peu connus des référents même s’ils sont jugés utiles par les internes et les coordinateurs locaux. Du fait de la prévalence importante des internes concernés, de l’hétérogénéité et du manque de prises en charge , il semble crucial de proposer des recommandations consensuelles avec les enseignants universitaires, favorisant le parcours professionnel et l’accès aux soins pour ces internes.

La prise en charge des internes en situation de souffrance psychique comporte plusieurs étapes. La première est celle de la reconnaissance de l’état de souffrance par le sujet lui-même et/ou par son entourage proche. La seconde consiste à aider l’interne à s’adresser à la ou aux personne(s) la(les) plus qualifiée(s) pour analyser avec lui la situation et/ou lui apporter l’aide requise. Une troisième étape consiste à mettre en œuvre les moyens thérapeutiques et/ou de prévention nécessaires avec l’ensemble des personnes ressources. Chacune de ces étapes comporte des obstacles qu’il importe de connaître pour les surmonter : difficulté à reconnaître l’état de souffrance ; difficulté à faire la part entre un simple trouble de l’adaptation lié à une situation professionnelle particulière et un trouble psychique nécessitant l’intervention d’un spécialiste ; difficultés d’accès aux spécialistes, mais aussi difficultés rencontrées dans la mise en œuvre d’éventuelles mesures de prévention dans le lieu de stage, enfin. Chacune de ces étapes peut impliquer l’intervention de multiples intervenants (famille et amis, pairs, enseignants, coordonnateur, psychologues, psychiatres), dont il importe de coordonner l’action dans le respect de la plus grande confidentialité. Le syndicat des internes des hôpitaux de Paris (SIHP) a mis en œuvre depuis le début de l’année 2015 et en concertation avec le coordonnateur du DES de psychiatrie un dispositif d’aide aux internes en souffrance psychique intitulé « SOS-Psychiatrie ». Cette initiative permet à un interne en souffrance psychique de s’adresser, dans un premier temps, à un pair en la personne d’un membre du SIHP. L’interne pourra alors être orienté vers une des consultations d’évaluation organisées par les services de psychiatrie du réseau, avant d’être si besoin redirigé vers un suivi. Depuis sa création, 6 internes ont pu bénéficier de ce dispositif.

In this presentation the use of new technologies are discussed. What is needed to support the clinical care process and how can information technology (IT) help to fit in e.g. measurement instrument, like the HoNOS and OQ, seamlessly in the process. The aim is to make things work more transparent and if possible more efficient and cost-effective. The presenter will show how to overcome the different challenges when IT is used for embedding the above mentioned instruments in clinical practice to enhance the quality of the process.

Les troubles anxieux des sujets âgés de plus de 65 ans ont une prévalence estimée entre 3,2 % et 14,2 % et représentent une source de souffrance importante, de baisse d’autonomie et impliquent un surcoût pour la société. Les troubles anxieux des sujets âgés sont fréquents mais souvent sous-diagnostiqués du fait d’une expression différente par rapport aux jeunes (nécessitant l’utilisation des mesures validées chez les sujets âgés) et de la comorbidité somatique et psychiatrique fréquentes. La plupart de ces troubles anxieux ont débuté avant l’âge de 41 ans (90 %) moins de 1 % débutent après l’âge de 65 ans. Parmi les troubles anxieux des sujets âgés les phobies spécifiques (incluant l’agoraphobie) et le trouble anxieux généralisé (TAG) seraient les plus fréquents suivis de PTSD. La prévalence de l’agoraphobie est élevée et son expression suggère un sous-type de l’âge avancé. Plusieurs barrières sont source de sous-diagnostique et donc de sous traitement pour les troubles anxieux des personnes âgées : la dépression comorbide (fréquente et source de résistance au traitement), les troubles cognitifs, les troubles somatiques (présents chez plus de 80 % des sujets > 65 ans) ainsi que la difficulté à différentier les symptômes de l’anxiété des personnes âgés des changements psychologiques et physiques survenant avec le processus du vieillissement (ex. pattern du sommeil). Les troubles anxieux des personnes âgées sont source d’une plus grande difficulté dans la vie quotidienne, d’une faible adhésion aux traitements et sont à risque élevé de dépression comorbide et de suicide, de chutes, d’impotence physique et fonctionnelle et de solitude. Par ces différentes voies, les troubles anxieux sont source d’augmentation de la dépendance des personnes âgées. Ces données impliquent l’importance d’une détection appropriée de ces troubles fréquents chez les personnes âgées et la mise en place des traitements adaptés ainsi que des mesures de prévention efficaces.

Ces troubles sont souvent sous-diagnostiqués et sous-traités chez les sujets âgés, leur expression différente par rapport aux adultes jeunes étant due aux comorbidités somatiques et psychiatriques fréquentes (notamment la dépression) et à la présence des troubles cognitifs. Les troubles anxieux des sujets âgés représentent une cause non reconnue d’incapacité et de risque de mortalité et ils ont été associés avec des maladies cardiovasculaires, des accidents vasculaires et le déclin cognitif. Les mécanismes de l’anxiété chez les sujets âgés diffèrent de ceux impliqués chez les jeunes adultes à cause des modifications neurobiologiques liés à l’âge ainsi que des facteurs de stress environnementaux plus fréquents avec l’avancées en âgé. La plupart des troubles anxieux chez les sujets âgés sont chroniques et habituellement débutent plus tôt dans la vie à l’exception du trouble anxieux généralisé (TAG) et de l’agoraphobie qui peuvent avoir un début tardif. Le TAG est le trouble anxieux le plus prévalent chez les sujets âgés. Les principaux facteurs prédictifs de TAG à début tardif sont le genre (femme), les événements de vie adverses récents, les problèmes de santé chroniques (respiratoires, cardiaques, déclin cognitif) et les maladies mentales chroniques (dépression, troubles anxieux) . En plus les événements de vie adverses précoces (perte parentale/séparation, maladie mentale parents) étaient associés indépendamment avec l’incidence du TAG à début tardif . Le TAG du sujet âgé est caractérisé par une réponse aux traitements plus médiocre tant à la pharmacothérapie qu’aux thérapies cognitivo-comportementales. Cette différence a été attribuée aux changements neurobiologiques liés à l’âge . Ces données suggèrent l’importance d’une meilleure détection de ces troubles fréquents chez les personnes âgées à l’aide d’outils spécifiques et la mise en place des stratégies thérapeutiques adaptées.

The general population prevalence and incidence of late-life agoraphobia was estimated and its clinical characteristics and risk factors described using data from the French ESPRIT study. One thousand nine hundred and sixty-eight persons aged 65 and above were randomly recruited from the electoral rolls of the district of Montpellier. Prevalent and incident agoraphobia diagnosed by a standardized psychiatric examination and validated by a clinical panel was assessed at base-line and over 4-year follow-up. The one-month prevalence of agoraphobia was estimated at 10.4% of whom 10.9% reported having the first-episode at age 65 or over. During the 4-year follow-up 11.2% of participants without agoraphobia at base line were classified as cases giving an incident rate of 32 per 1000 person-years. These 132 incident late-onset cases were associated with higher incident rates of anxiety disorders and suicidal ideation. Only two incident cases had past or concurrent panic attacks, which was not significantly different from non-cases. The principal base-line risk factors for incident cases derived from a multivariate model incorporating all significant risk factors were younger age of onset (OR = 0.94; 95% CI 0.90–0.99, P = 0.02), poorer visuospatial memory performance (OR = 1.60; 95% CI 1.02–2.49, P = 0.04), severe depression (OR = 2.62; 95% CI 1.34–5.10, P = 0.005) and trait anxiety (OR = 1.73; 95% CI 1.03–2.90, P = 0.04). No significant association was found with cardiac pathologies. We conclude that agoraphobia has high prevalence in the elderly and unlike younger cases, late-onset cases are not more common in women, and are not associated with panic attacks, suggesting a late-life subtype. Severe depression, trait anxiety and poor visuospatial memory are the principal risk factors for late-onset agoraphobia.

Negative symptoms and neurocognitive function are from mild to moderately correlated, and both are correlated to poor functioning. In order to understand more of the underlying nature of these relationships and come up with better treatments the study of the different negative sub-symptoms, their relationship to different cognitive tests and how they together influence functioning can be of value. Apathy or reduced motivation/avolition is one of the negative sub-symptoms that have been given increased attention during the last years. Results from two studies will be presented, both with first episode patients from the ongoing TOP/NORMENT study in Oslo, Norway. The first study explored the correlation between apathy and different neurocognitive domains. The second study explored the pattern of influence from neurocognition and apathy together with other variables upon psychosocial functioning at start of treatment and after one year. In study one it was found that apathy only had significant correlation to tests of executive function and no other cognitive tests. In study two the pattern of influence was the same at start- and after one year with apathy explaining the greatest variance; in addition verbal memory and gender had significant influence. The first study confirms that apathy is only linked to tests of executive function, a result also found in other brain disorders. The second study shows independent contribution from both apathy and specific cognitive areas to poor functioning. These findings can be used in tailoring more specific treatments, understanding more of the underlying mechanisms and shows the importance of focusing on the negative sub-symptoms and the different cognitive tests.

Through treatment of BPD is complicated, many CBT therapist can learnschema therapy (ST) principles and strategies to used them as an additionaltool. ST began as an extension of Beck's cognitive therapy model and has grownto become a unique integrative treatment for the personality disorders. Aschema is a extremely stable, constant negative pattern which develops duringchildhood and is elaborated during person's life. There are two main schemaoperations: healing of schema and schema maintenance.STdevotes considerable attention to modes, the predominant emotions, schemas, or coping reactions working for an individual at a particular time. The aim of the therapy is to engage in schema healing processes, which areintended to reduce the early maladaptive schemas and coping styles, and buildup the person's healthy side. The history of the schemas, modes and copingstrategies are systematically discussed, their origin are explored, they are linked to current problems, and theopportunity of modifying is explored. In treatment phase, the therapistflexibly uses cognitive, emotional/experiential, behavioral, andrelational/interpersonal strategies to change schemas and change maladaptivecoping styles with healthier forms of behavior. Schema therapists use therelationship itself quite extensively, in two main ways. First, it is a area inwhich behaviors modes, and schemas can be observed, assessed, and modified. Second, the relationship is used as a 'corrective emotional experience”. Through what schema therapy terms 'limited reparenting,” the therapist acts inways that supply as an cure to early unmet needs.

Le trouble de stress post-traumatique (TSPT) est un trouble sous-diagnostiqué chez le sujet âgé. La prévalence du TSPT est estimée à environ 2 % dans cette population . Il est très souvent associé à des comorbidités psychiatriques : au premier rang desquelles l’épisode dépressif majeur, possiblement accompagné d’idéations suicidaires, mais aussi les troubles anxieux (trouble anxieux généralisé, trouble panique avec agoraphobie), ou encore aux mésusages d’alcool, et de benzodiazépines . Ce trouble impacte la qualité de vie et entraîne une invalidité significative , affectant principalement 5 dimensions de fonctionnement : la compréhension et la communication, le déplacement, les soins personnels, l’activité et la participation sociale [1,2]. Il existe également des comorbidités somatiques importantes au premier rang desquelles les pathologies cardiovasculaires. Celles-ci sont le plus souvent en lien avec une augmentation des troubles métaboliques : le diabète, l’hypertriglycéridémie, l’hypertension . Ces comorbidités sont le plus souvent sous-diagnostiquées. Les sujets présentant un TSPT ont ainsi un risque accru d’infarctus du myocarde ou d’angine de poitrine [3,4]. Au-delà de la prise en charge du trouble psychiatrique, ces données rappellent l’importance de prendre en charge le sujet dans sa globalité. Il semble donc particulièrement important de mieux dépister ce trouble ainsi que ses comorbidités fréquentes chez le sujet âgé afin d’améliorer la qualité de vie et de diminuer la morbi-mortalité du TSPT. La prise en charge du TSPT chez le sujet âgé repose sur un traitement pharmacologique, au premier rang desquels les ISRS, et sur les psychothérapies. En particulier, les thérapies cognitives et comportementales peuvent être une proposition d’autant plus intéressante qu’il existe des comorbidités addictives ou somatiques.

Si les outils numériques et particulièrement les tablettes tactiles ont été rapidement investies pour la prise en charge des troubles du spectre autistique et d’autres formes de troubles du développement, leur utilisation raisonnée devant la multitude des applications ne va pas sans difficulté. Construit en cohérence avec les recommandations HAS et les programmes scolaires de l’éducation nationale, le programme LearnEnjoy développé en France est un support particulièrement bien structuré pour guider cliniciens et éducateurs dans leur travail. En s’appuyant sur la construction et le fonctionnement de cet outil, la session développera sa thématique en décrivant l’environnement théorique, le contenu des applications, l’utilisation dans le cadre de recherches en psychologie du développement, et le suivi dans plusieurs institutions de la mise en œuvre de ce programme. Olivier Bourgueil, psychologue spécialisé en analyse fonctionnelle du comportement (ABA) montrera comment le contenu des activités a été construit en prenant en compte les spécificités cognitives, attentionnelles et motivationnelles des enfants avec TSA. Les applications sont autant un espace pour le travail des compétences de l’enfant (ou de l’adulte) qu’un guide pour les accompagnants, parents, éducateurs et rééducateurs. Sylvain Moutier, professeur de psychologie cognitive, décrira un protocole de recherche en cours sur les troubles des fonctions exécutives utilisant ce support numérique, recherche visant à mieux connaître les particularités développementales de certains handicaps cognitifs et à améliorer les remédiations en fonction des difficultés observées. Un des atouts essentiels du travail avec LearnEnjoy repose sur le dispositif d’évaluation inclus dans le programme, enregistrant les progressions de l’enfant quel que soit l’endroit où il travaille. Cela renforce les liens de travail entre accompagnants et familles et permet au clinicien de documenter l’évolution des progrès. Gaële Régnault conclura la session en présentant les résultats encourageants de travaux menés dans 22 établissements médicosociaux.

Countertransference occurs in CBT when the relationship with the patientactivates automatic thoughts and schemas in the clinician, and these cognitionshave the potential for influencing the therapy process. Countertransferencealso occurs during supervision and is an indispensable part of the supervisor's response to the supervisee. CBT is typically short-termtreatment – intensity of transference is usually muchlower than in longer-term, dynamically oriented psychotherapy. Neverthelessduring the long-term CBT of the personality disorder or other complex cases, high intensity of transference and countertransference can develop.

Schema therapy shares the view thatschemas are crucial to understanding of personality disorders, but also canhelp to understand the emotional reaction of therapist. A mode is the set ofschema operations that are in one moment functioning for a person. It is acircumscribed complex pattern of emotional, cognitive a behavioral experiences, which operate in typical situations. When therapist suspects that countertransferencemay be developing, he/she could try to identify her/his automatic thoughts andschemas. More comprehensive approach is to quickly identify in which modehim/her are at that moment and reflect it such reaction is for the patientbenefit or not. Understanding therapist countertransference reactions andtheirs management are a significant point of supervision. Self-reflection and realizing the countertransference can therapist help toovercome it and may be necessary for overcoming stagnation in therapy.

Understanding own mode and theirflipping into the therapeutic session is an important tool in psychotherapy andsupervision.

Childhood trauma may be accompanied by biologicalchanges that are caused by the stressful events. Once the events take place, amount of inner changes happen in the child. Thought suppression, developmentalregressions, deliberate avoidance, sleep problems, exaggerated startleresponses, fears of the mundane, irritability, and hypervigilance are prominentamong these. Terr (1991) describes four characteristics related to childhoodtraumas that appeal to last for long periods of life. These are visualizedmemories of the traumatic event, trauma-specific fears, repetitive behavioralpatterns, and changed attitudes about others, life, and the future. The intendof therapist during imagery rescripting is to improve the patient memorizingthe traumatic events and expressing affective experience and then help him/herto rescript experience to less painful. Therapist helps the patient tounderstand, how symptoms connected with the events from childhood and how theyare interconnected with actual problems in life (Smucker & Neiderdee 1995). Therapeutic process can be divided into of several steps (Prasko et al 2012c, Vyskocilova & Prasko 2012b):

1. 1. formation of the therapeutic atmospheres (with feelings of security and control, acceptance, approbation, validation of any emotions);

2. 2. description of the painful memories;

3. 3. formulating the needs of the child in this situation;

4. 4. discussing 'safety person”, who could help the child;

5. 5. imagination the event rescripted with the experience of better resolution in imagination – rescripting the story;

6. 6. general calm down.

Imagery with rescripting techniques that focus onchanging unpleasant memories have also been used as main components of schematherapy programs

“Autism” refers to a wide variety of disabilities, with numerous possible clinical signs and heterogeneous origins. Nevertheless, beyond those diverse clinical signs, a consensus exists concerning the necessity of interventions, particularly educational interventions, for people with autism . In a society such as ours, turned towards digital technologies and tactile devices, (e.g. ), it is important to question the use of these technologies with people with developmental disabilities. Especially as some authors pointed the utility of these devices for teaching skills . Various theoretical corpuses and elements of the scientific literature were taken into account and integrated to develop the LearnEnjoy applications, created initially for children with autism. These facts and knowledge, linked to the peculiarities of people with autism, to the functional approach of language, to the fundamental principles of learning, to an ABA approach, were integrated from the first stages of the development of the applications, to create tools having solid scientific foundations. This way, the LearnEnjoy apps give the users (i.e. the “teachers”) the possibilities of teaching in a progressive and coherent way, different skills such as language (receptive, expressive), imitation, play and motricity, cognitive, academic skills or even independence skills. They also allow the progress in each area to be shared with the parents and the whole team, a necessary feature for the implementation of global and coordinated interventions. Finally, and maybe more importantly, these applications were created so as to specifically foster the contact between the person with autism and the “teacher”. This way the apps, at the same time, reduce the risk of pervasiveness of the tactile tablet, while favouring, just as much for the person with autism than for the accompanying person, the development of a positive, structured and structuring social relationship.

Children with autism spectrum disorder (ASD) have serious difficulties to ignore visual and auditory distractors, or to inhibit ongoing activity on behalf of a new one, or to extract themselves from a routine. Such so-called executive functions enable us to control ourselves and to consider things from multiple points of view. They also involve paying attention, remembering what we need to remember to pursue our goals, thinking flexibly and not going on automatic, exercising inhibition. Then, the observed executive inefficiency of ASD could be one of the main cause of perseverating behaviors in daily life and school activities. If the question of the efficient evolution of these executive functions from childhood to neurotypical adulthood has been addressed in many cognitive development researches, very few studies have focused on the atypical development of ASD patients. Following Diamond et al. , it is important to note that beyond acquiring always more knowledge, it is crucial to be able to inhibit reactions that get in the way of learning something new. Using preliminary data, we will explain how LearnEnjoy apps could become an essential basis of an innovative experimental paradigm, aiming at a better understanding of the atypical executive development of school-age ASD children. Based on new executive digital apps such as Stroop or Flanker Tasks testing for executive inhibition, the main goal of this scientific project is to show evidence of the possibility of executive training in children with ASD. As executive function skills predict children's success in life and in school , such a new scientific study should allow us to envisage creating innovative remediation protocols for improving the deliberate, goal-directed control of behavior of ASD patients.

Meaningful cognitiveand simultaneously experiential technique for working with deepemotional schemas, formatted in childhood, is writing therapeutic letters. Inclusions of emotion regulation skills in the treatment especially forpatients with personality disorder enhance the efficacy of CBT. Therapeuticletters are intended to extend the work of therapy beyond the consultingroom door by continuing the meaning-making that occurred in a therapeuticconversation.

We usedtherapeutic letters to help the patient identify difficult feelings, processthem in a way that maybe they could not in a therapy session, and finally getrelease and freedom from them.

Typicaltherapeutic letters are address to important close persons from thepatient life, like parents, siblings, a partner and friends, children. Thepatient uses the letter to impress his/her feeling and needs, inclusivedefenses of his rights in past and present.

Patientwrites a letter at home, and he brings it then into the session and reads it tothe therapist. These letters are not intended to be sent to thatimportant person, but to process strong emotions inside the patient.

In fact thepatient writes a letter to his inner representation of an important person,who was connected with the development of the maladaptive schemas. Theletter-writing process is inherently collaborative and enables the patient towork at his or her own pace while also facilitating patient empowerment.
