Geographic Access to Stroke Care Services in Rural Communities in Ontario, Canada

Background:
Optimal stroke care requires access to resources such as neuroimaging, acute revascularization, rehabilitation, and stroke prevention services, which may not be available in rural areas. We aimed to determine geographic access to stroke care for residents of rural communities in the province of Ontario, Canada.
Methods:We used the Ontario Road Network File database linked with the 2016 Ontario Acute Stroke Care Resource Inventory to estimate the proportion of people in rural communities, defined as those with a population size <10,000, who were within 30, 60, and 240 minutes of travel time by car from stroke care services, including brain imaging, thrombolysis treatment centers, stroke units, stroke prevention clinics, inpatient rehabilitation facilities, and endovascular treatment centers.
Results:Of the 1,496,262 people residing in rural communities, the majority resided within 60 minutes of driving time to a center with computed tomography (85%), thrombolysis (81%), a stroke unit (68%), a stroke prevention clinic (74%), or inpatient rehabilitation (77.0%), but a much lower proportion (32%) were within 60 minutes of driving time to a center capable of providing endovascular thrombectomy (EVT).
Conclusions:Most rural Ontario residents have appropriate geographic access to stroke services, with the exception of EVT. This information may be useful for jurisdictions seeking to optimize the regional organization of stroke care services.

Prenatal exposure to selective serotonin reuptake inhibitors and social responsiveness symptoms of autism: population-based study of young children

Background
Selective serotonin reuptake inhibitors (SSRIs) are considered safe and are frequently used during pregnancy. However, two case–control studies suggested an association between prenatal SSRI exposure with childhood autism.
AimsTo prospectively determine whether intra-uterine SSSRI exposure is associated with childhood autistic symptoms in a population-based study.
MethodA total of 376 children prenatally exposed to maternal depressive symptoms (no SSRI exposure), 69 children prenatally exposed to SSRIs and 5531 unexposed children were included. Child pervasive developmental and affective problems were assessed by parental report with the Child Behavior Checklist at ages 1.5, 3 and 6. At age 6, we assessed autistic traits using the Social Responsiveness Scale (n = 4264).
ResultsPrenatal exposure to maternal depressive symptoms without SSRIs was related to both pervasive developmental (odds ratio (OR) = 1.44, 95% CI 1.07–1.93) and affective problems (OR = 1.44, 95% CI 1.15–1.81). Compared with unexposed children, those prenatally exposed to SSRIs also were at higher risk for developing pervasive developmental problems (OR = 1.91, 95% CI 1.13–3.47), but not for affective problems. Children prenatally exposed to SSRIs also had more autistic traits (B = 0.15, 95% CI 0.08–0.22) compared with those exposed to depressive symptoms only.
ConclusionsOur results suggest an association between prenatal SSRI exposure and autistic traits in children. Prenatal depressive symptoms without SSRI use were also associated with autistic traits, albeit this was weaker and less specific. Long-term drug safety trials are needed before evidence-based recommendations are possible.

A vast body of literature shows that maternal depression has long-term adverse consequences for children. However, only very few studies have documented the effect of maternal depression on children's ability to process emotional expressions and even fewer incorporated measures of observed maternal sensitivity to further tease apart whether it is the symptoms per se or the associated impact via maternal sensitivity that affects children's developing emotion-processing abilities. In a large community sample of Dutch preschoolers (N = 770), we examined independent and mediated effects of maternal depressive symptoms and sensitivity on children's ability to recognize emotional expressions using a nonverbal and a verbal task paradigm. Maternal depressive symptoms predicted less accurate emotion labeling in children, while maternal sensitivity was associated with more accurate emotion matching, especially for sadness and anger. Maternal sensitivity did not mediate the observed associations between mothers’ depressive symptoms and children's emotion recognition, and effects were similar for boys and girls. Given that maternal depressive symptoms and sensitivity affected nonoverlapping areas of young children's emotion recognition, prevention and intervention efforts should focus on both alleviating maternal depressive symptoms and improving maternal sensitivity at the same time in order to maximize benefit.

In the preface of the 1569 Polish edition of the Brethren's hymnal, Bishop Andrew Stefan (1528–77) of the early Unitas Fratrum wrote, “Our fathers have taught us not only to preach the doctrines of religion from the pulpit, but also to frame them in hymns. In this way our songs become homilies.” When we consider the tens of thousands of hymns and single stanzas that Moravian writers have composed since 1457, the truth of Stefan's statement becomes obvious. As the Moravian Church has spread around the world and the need for worship materials in additional languages has been realized, new hymns have multiplied, so that any study of Moravian hymnody must acknowledge that it will be limited and incomplete. Yet a broad survey is possible. After an overview of hymnals produced by the Moravians beginning in the early sixteenth century, we will first consider five reasons that Moravians have continued to write new hymns. We will then present a chronological survey of Moravian hymn text writers and composers.
An Overview of Moravian Hymnal Publications
Moravian hymnody itself had already begun with the dawn of the sixteenth century. What has been hailed as the “first Protestant hymnal,” containing eightynine hymns, was printed in 1501 in Prague. It has long been claimed as a hymnal of the Unitas Fratrum, but its connection with the Unity is tenuous, although it did contain eleven hymns by Lukáš of Prague, the most influential theologian of the Unity. In his History of the Unity of Brethren, Rudolf Rícan asserts: “More definitely belonging to the Brethren, and more safely associated with Lukáš, were the hymnals of 1505 and 1519.” The 1505 hymnal is reported to have contained some four hundred hymns, but no surviving copy is known to exist today. The 1519 hymnal was actually edited by Lukáš; but again, no copy is known to survive. In these hymnals, Lukáš incorporated into the Unity's repertoire some Latin hymns of the Catholic church, translating them into Czech; thisis but one example of his position that not everything from the Catholic church was harmful and deserving of rejection.
