Objective: To validate a case definition of multiple sclerosis (MS) using health administrative data and to provide the first province-wide estimates of MS incidence and prevalence for Saskatchewan, Canada. Methods: We used population-based health administrative data between January 1, 1996 and December 31, 2015 to identify individuals with MS using two potential case definitions: (1) ≥3 hospital, physician, or prescription claims (Marrie definition); (2) ≥1 hospitalization or ≥5 physician claims within 2 years (Canadian Chronic Disease Surveillance System [CCDSS] definition). We validated the case definitions using diagnoses from medical records (n=400) as the gold standard. Results: The Marrie definition had a sensitivity of 99.5% (95% confidence interval [CI] 92.3-99.2), specificity of 98.5% (95% CI 97.3-100.0), positive predictive value (PPV) of 99.5% (95% CI 97.2-100.0), and negative predictive value (NPV) of 97.5% (95% CI 94.4-99.2). The CCDSS definition had a sensitivity of 91.0% (95% CI 81.2-94.6), specificity of 99.0% (95% CI 96.4-99.9), PPV of 98.9% (95% CI 96.1-99.9), and NPV of 91.7% (95% CI 87.2-95.0). Using the more sensitive Marrie definition, the average annual adjusted incidence per 100,000 between 2001 and 2013 was 16.5 (95% CI 15.8-17.2), and the age- and sex-standardized prevalence of MS in Saskatchewan in 2013 was 313.6 per 100,000 (95% CI 303.0-324.3). Over the study period, incidence remained stable while prevalence increased slightly. Conclusion: We confirm Saskatchewan has one of the highest rates of MS in the world. Similar to other regions in Canada, incidence has remained stable while prevalence has gradually increased.

A 3-year ecological study of physical, chemical, and biological factors affecting the growth of submersed aquatic weeds was conducted in four irrigation and drainage canals in south Florida. Growth of aquatic vegetation was influenced by light intensity which decreased in a geometric ratio to water depth. The light penetration into the water was influenced by dissolved coloring matter and turbidity. A high concentration of nitrogen was found in three of the canals. Nitrogen entered the canals as sewage effluent discharged into the water and as fertilizer runoff from surrounding farmland. Density of aquatic vegetation harvested at intervals of 6 months from clipped plots varied from zero in a newly dug canal to 22,000 lb green weight/A in an old established canal. During the 3-year study, there was a complete shift of the vegetation from southern naiad (Najas guadalupensis (Spreng.) Magnus) to Florida elodea (Hydrilla verticillata Casp.) in two of the canals, but there was no change in plant species in two canals. No other chemical and physical factors evaluated in the study limited submersed aquatic weed growth in these canals.

A novel method to estimate unsteady aerodynamic force coefficients from pointwise velocity measurements is presented. As opposed to other existing methodologies, time-resolved full velocity fields are not required. The methodology is based on a resolvent-based reduced-order model which requires the mean flow to obtain physical flow structures and pointwise measurement to calibrate their amplitudes. A computationally affordable time-stepping methodology to obtain resolvent modes in non-trivial flow domains is introduced and compared with previous existing matrix-free and matrix-forming strategies. The technique is applied to the unsteady flow around an inclined square cylinder at low Reynolds number. The potential of the methodology is demonstrated through good agreement between the fluctuating pressure distribution on the cylinder and the temporal evolution of the unsteady lift and drag coefficients predicted by the model and those computed by direct numerical simulation.

A novel reduced-order model for time-varying nonlinear flows arising from a resolvent decomposition based on the time-mean flow is proposed. The inputs required for the model are the mean-flow field and a small set of velocity time-series data obtained at isolated measurement points, which are used to fix relevant frequencies, amplitudes and phases of a limited number of resolvent modes that, together with the mean flow, constitute the reduced-order model. The technique is applied to derive a model for the unsteady three-dimensional flow in a lid-driven cavity at a Reynolds number of 1200 that is based on the two-dimensional mean flow, three resolvent modes selected at the most active spanwise wavenumber, and either one or two velocity probe signals. The least-squares full-field error of the reconstructed velocity obtained using the model and two point velocity probes is of the order of 5 % of the lid velocity, and the dynamical behaviour of the reconstructed flow is qualitatively similar to that of the complete flow.

The effect of streamwise-varying steady transpiration on turbulent pipe flow is examined using direct numerical simulation at fixed friction Reynolds number $\mathit{Re}_{{\it\tau}}=314$. The streamwise momentum equation reveals three physical mechanisms caused by transpiration acting in the flow: modification of Reynolds shear stress, steady streaming and generation of non-zero mean streamwise gradients. The influence of these mechanisms has been examined by means of a parameter sweep involving transpiration amplitude and wavelength. The observed trends have permitted identification of wall transpiration configurations able to reduce or increase the overall flow rate $-36.1\,\%$ and $19.3\,\%$, respectively. Energetics associated with these modifications are presented. A novel resolvent formulation has been developed to investigate the dynamics of pipe flows with a constant cross-section but with time-mean spatial periodicity induced by changes in boundary conditions. This formulation, based on a triple decomposition, paves the way for understanding turbulence in such flows using only the mean velocity profile. Resolvent analysis based on the time-mean flow and dynamic mode decomposition based on simulation data snapshots have both been used to obtain a description of the reorganization of the flow structures caused by the transpiration. We show that the pipe flows dynamics are dominated by a critical-layer mechanism and the waviness induced in the flow structures plays a role on the streamwise momentum balance by generating additional terms.

It has been postulated that aging is the consequence of an accelerated accumulation of somatic DNA mutations and that subsequent errors in the primary structure of proteins ultimately reach levels sufficient to affect organismal functions. The technical limitations of detecting somatic changes and the lack of insight about the minimum level of erroneous proteins to cause an error catastrophe hampered any firm conclusions on these theories. In this study, we sequenced the whole genome of DNA in whole blood of two pairs of monozygotic (MZ) twins, 40 and 100 years old, by two independent next-generation sequencing (NGS) platforms (Illumina and Complete Genomics). Potentially discordant single-base substitutions supported by both platforms were validated extensively by Sanger, Roche 454, and Ion Torrent sequencing. We demonstrate that the genomes of the two twin pairs are germ-line identical between co-twins, and that the genomes of the 100-year-old MZ twins are discerned by eight confirmed somatic single-base substitutions, five of which are within introns. Putative somatic variation between the 40-year-old twins was not confirmed in the validation phase. We conclude from this systematic effort that by using two independent NGS platforms, somatic single nucleotide substitutions can be detected, and that a century of life did not result in a large number of detectable somatic mutations in blood. The low number of somatic variants observed by using two NGS platforms might provide a framework for detecting disease-related somatic variants in phenotypically discordant MZ twins.

The aim of this analysis was to investigate the strength of the association between sugar intake and treatment for dental decay in children in Scotland, and the impact of tooth brushing frequency on this association. The Survey of Sugar Intake among Children in Scotland was carried out in 2006 in those aged 3–17 years. Diet was assessed using the Scottish Collaborative Group FFQ, and interviews were carried out by trained fieldworkers who asked about dental health. A total of 1700 interviews were carried out, and 1512 FFQ were returned. Of the children, 56 % had received treatment for decay (fillings or teeth removed due to decay). Intake of non-milk extrinsic sugars (NMES), but not total sugar, increased the risk of having had treatment for decay: adjusted OR 1·84 (95 % CI 1·28, 2·64) for the highest ( ≥ 20·0 % food energy) v. lowest ( ≤ 14·8 % food energy) tertile of NMES intake. This raised risk remained in children who reported brushing their teeth at least twice a day. Compared with children who reported brushing their teeth at least twice a day and were in the lowest tertile of NMES intake, children who reported brushing their teeth once a day or less and were in the highest tertile of NMES intake were over three times more likely to have received treatment for decay (adjusted OR 3·39, 95 % CI 1·97, 5·82). In order to improve dental health in children in Scotland, dental health strategies must continue to stress the importance of both reduced NMES intake and good oral hygiene.

Data on gestation period of 701 indigenous East African and Galla does and pre- and postweaning growth of 810 pure-bred and cross-bred kids were analysed by least squares statistical analysis. Breeds of kids were East African, Galla, Toggenburg × East African, Toggenburg × Galla, Anglo-Nubian × East African and Anglo-Nubian × Galla. Average gestation length was 149 days and was significantly (P < 0·05) affected by type of birth of the kid, year–month of kidding, and age of dam. Does carrying twins had shorter gestation length than does carrying singles. Sex and breed of kid did not have significant effect on gestation length.

Significant (P < 0·05) factors affecting birth weight, weight at 30, 60, 90, 120, 150, 210 and 270 days of age, and preweaning growth rate were breed and sex of kid, type of birth, year–month and age of dam. Male kids grew faster and were heavier than females. Kids born single were heavier and grew faster than twin-born kids. Kids from dams less than 3 years old weighed less than kids from older dams. Cross-bred kids had higher preweaning growth rates than indigenous pure-bred kids. The Anglo-Nubian × Galla cross was heaviest while the East African ranked lowest. The results point to importance of cross-breeding with temperate breeds to increase growth rates of indigenous goats. Comparison of Toggenburg and Anglo-Nubian sires for cross-breeding showed both buck breeds produced kids with roughly equal growth rates. Evaluation of indigenous Galla and East African dams for cross-breeding showed Galla dams produced heavier kids than East African but preweaning growth rates were not significantly different.

Experimental wet chemical approaches to complex an iron atom with two C60 fullerenes, representing a new molecule, dubbed a “bucky dumbbell,” have been demonstrated. The structure of this molecule has been determined by 13C solid-state nuclear magnetic resonance (NMR) and electron paramagnetic resonance (EPR). Furthermore, this structure has been shown to have unique binding sites for dihydrogen molecules with the technique of temperature programmed desorption (TPD). The new adsorption sites have binding energies that are stronger than that observed for hydrogen physisorbed on planar graphite, but significantly weaker than a chemical C-H bond. Further development of these molecules could make them ideal candidates for onboard vehicular hydrogen storage.

Objectives: To assess the volume and range of diagnosis in new patients referred to paediatric cardiac outpatient clinics. Methods: Data was collected prospectively, using a proforma completed at all outpatient clinics over a period of three months. Results: There were 526 new referrals, representing an increase of almost one-fifth compared to 5 years ago. Of the referrals, 78 percent came from hospital doctors, and 22 percent from general practitioners, with 221 of those referred being infants. A heart murmur was the most common reason for referral, representing almost two-thirds of cases. In 372 patients referred (71 percent), the heart was discovered to be structurally normal. The proportion of patients with normal hearts referred by general practitioners and hospital doctors were 81 percent, and 68 percent, respectively (p less than 0.004). There was considerable variation in the pattern of referral between doctors working in different hospitals. Conclusion: New referrals to centres dealing with congenital cardiac malformations are increasing alarmingly, with the majority of the children referred having normal hearts. This increase in demand for specialist services has important implications for resources and training.

A number of two-dimensional time-periodic flows, for example the Kármán street wake of a symmetrical bluff body such as a circular cylinder, possess a spatio-temporal symmetry: a combination of evolution by half a period in time and a spatial reflection leaves the flow invariant. Floquet analyses for the stability of these flows to three-dimensional perturbations have in the past been based on the Poincaré map, without attempting to exploit the spatio-temporal symmetry. Here, Floquet analysis based on the half-period-flip map provides a comprehensive interpretation of the symmetry-breaking bifurcations.

The existing scale for assessing competence in cognitive therapy (CTS) dates from 1988 and only the previous version of 1980 has been validated to any extent. A revised version, the CTS-R, was devised to improve on the CTS by: eliminating overlap between items, improving on the scaling system, and defining items more clearly. Kolb's well-known educational model was used as a guideline. In the new 14-item scale, three new items measure general therapeutic flair, the facilitation of emotional expression, and therapist's non-verbal behaviours (optional). We hypothesized that the CTS-R would prove more user friendly and demonstrate satisfactory reliability and validity. Twenty-one mental health professionals undergoing training in cognitive therapy provided 102 video-tapes of therapy with 34 patients, reflecting three stages of therapy. The tapes were rated by four expert raters, in a balanced design. The CTS-R showed high internal consistency and adequate average inter-rater reliability. Reliability for individual items varied widely among pairs of raters. Validity was demonstrated by improved ratings of competence for trainees who saw patients early and later during the course of training. Although raters found the CTS-R a more useful tool than the CTS and satisfactory reliability and validity were demonstrated, more refinement is needed in item definition. The study has led to modifications in the CTS-R, which are in the process of evaluation.

Two experiments were conducted to test different milking procedures for dual-purpose goats that would be suitable for smallholder farmers. In experiment 1, 105 East African and Galla does that kidded to singles were randomly assigned to three treatments: (a) milking in absence of kid and kid sucking residual; (b) milking in presence of kid and kid sucking residual; and (c) milking once a week and kid sucking all milk 6 days of the week. In experiment 2, 45 Galla does that kidded to singles were randomly assigned to three treatments: (a) weigh-suckle-weigh; (b) milking half udder while kid sucking other half and (c) kid sucking all milk in the evening and milking mornings. Measurements of milk yield, milk sucked and weights of kids were taken for 12 weeks of lactation.

Results were analysed using least-squares techniques for unequal subclass numbers. Milking in the presence of kids increased does' milk yield. This increase was attributed to the stimulation of milk let-down by the presence of kids. Although this method potentially provided most milk for dairy use, less residual milk was left to sustain growth of kids. Estimates of total milk yield by milking once a week underestimated milk yield potential of the East African and Galla breeds. Results indicated an appropriate method for estimating milk potential should combine both milked and sucked milk measurements. Restricting milk available to the kids reduced their growth rates and thus indirectly affected their maturing rates. No milking procedure proved to be superior all-around for the smallholder for milking goats. To resolve this dilemma, use of an optimizing technique including socio-economic weighting of milk for family use relative to milk for kids was proposed.

Total and differential cell counts were obtained for 1710 samples of milk taken from cows in which the stage of lactation was known. A bacteriological examination of the samples was also made. The total and differential cell counts considered together showed no marked advantage over the total cell count alone in the diagnosis of mastitis, except in milk of late lactation.

It is admitted by all critics that the Beowulf is essentially a heathen poem; that its materials are drawn from tales composed before the conversion of the Angles and Saxons to Christianity, and that there was a time when these tales were repeated without the Christian reflections and allusions that are found in the poem that has reached us. But in what form this heathen material existed before it was put into its present shape is a question on which opinions are widely different. In the nature of the case we can look for no entire consensus of opinion and no exact answer to the question; the most that one can expect to establish is at the best only a probability.
