Emotion dysregulation and impulsivity are considered an hallmark of personality pathology (Livesley & Jang, 2000), even though research has mainly regarded Borderline Personality Disorder (BPD; e.g., Linehan, 1993). Recently, some scholars proposed that facets of emotion dysregulation and impulsivity are likely to be associated with different PD traits across all clusters (Garofalo et al., 2014; Sarkar & Adshead, 2006), highlighting significant association with all PDs included in the DSM-5 (APA, 2013).

We sought to expand extant research exploring whether also PDs not included in the DSM nosography, yet clinically relevant and suggested for future research, were associated with facets of emotion dysregulation.

In a community sample, we tested the unique associations between facets of emotion dysregulation and impulsivity with traits of 3 PDs Not Otherwise Specified: Sadistic PD, Depressive PD, and Passive-aggressive PD.

We recruited 399 community-dwelling participants (mean age= 37.91; 56.6% male). They were administered the Difficulties in Emotion Regulation Scale (DERS; Gratz & Roemer, 2004), the Barratt Impulsiveness Scale (BIS-11, Patton et al., 1995) and the Millon Clinical Multiaxial Inventory (MCMI-III; Millon, 2006).

Multiple regression analyses showed that different facets of emotion dysregulation were associated with all PD traits considered. Further, impulsivity accounted for a significant amount of additional variance in Sadistic and Passive-aggressive features above and beyond emotion dysregulation.

Emotion dysregulation and impulsivity seemed to confirm their role as relevant features of personality pathology across different forms of PDs.

Une analyse groupée de 3 essais évaluant la non-infériorité d’étifoxine dans le trouble de l’adaptation avec anxiété (TAA) a été entreprise pour explorer les caractéristiques associées à la réponse au traitement.

Le critère principal d’efficacité était le score d’anxiété sur l’échelle d’Hamilton (HAM-A) à la fin de l’étude (j28) ajusté sur sa valeur initiale (j0). L’évolution entre j0 et j28 du HAM-A (ΔHAM-A) était l’un des critères secondaires. Une méthode Bayésienne de combinaison de modèles (BMA) estime la probabilité d’inclusion a posteriori (PIP) des caractéristiques initiales communes aux essais dans un modèle prédictif de la réponse. La différence minimale cliniquement importante (DMCI) est définie comme le ΔHAM-A moyen parmi les patients « légèrement améliorés » à j28 sur un CGI-I.

Au total, 545 patients ayant reçu pendant 4 semaines de l’étifoxine ou un traitement anxiolytique comparatif (buspirone ou benzodiazépine) ont été analysés. Les patients légèrement améliorés au CGI-I ont une différence de score HAM-A entre j0 et j28 de 10 en moyenne. L’anxiété et la dépression initiales sont identifiées comme prédicteurs forts de la réponse (PIP = 100 %), le sexe comme prédicteur potentiel (PIP = 69 %). L’âge, la sévérité initiale (CGI-S), le domaine de stress, n’ont pas d’influence (PIP < 18 %). Plus l’anxiété initiale du patient est élevée, plus elle diminue à j28. À l’inverse, un score de dépression élevé à l’inclusion est associé à une anxiété plus élevée à j28.

Les diminutions de l’anxiété plus importantes chez les patients initialement les plus anxieux sont de nature à conforter la prescription d’un traitement. Cette analyse a permis une meilleure caractérisation de la réponse au traitement des patients présentant un TAA, susceptible d’orienter le médecin traitant.

Depression is common in patients suffering from somatic diseases particularly chronic, life threatening and disabling diseases such as renal impairment requiring hemodyalisis. It contributes to further deterioration of the quality of life. The goal of consultation psychiatry is to advise screening and early interventions for this most often treatable disorder.

To assess and compare presence and severity of depression, and subjective assessment of QOL between patients on hemodialysis and healthy subjects.

This study included 80 subjects age between 19 and 82 (× 49.6 ± 14.2) on continuous hemodyalis and 40 subjects from the general population of Zenica. Assessment instruments used: Beck Depression Inventory – BDI; SF36- QOL, and sociodemographic questionnaire.

The subjects in the experimental group had significantly higher (p<0.05) average score on BDI, 17.1 ± 9.4, corresponding to mild to moderate depression compared with the subjects in control group who scored an average of 8.5 ± 7.0 (range 2-39), which corresponds to absence of depression. Comparison of results on a scale for assessing the quality of life SF 36, indicates that the subjects in the control had a higher average score or higher quality of life compared to the experimental group.

The research results indicate a significant difference in the presence of depressive symptoms and in the subjective assessment of QOL in patients on hemodialysis compared to healthy subjects. This justifies routine screening for depression in this group of patients.

Deficits in social cognition are related to functional disability in schizophrenia. Therefore, new treatment programs should address these deficits in schizophrenia.

The goal of this study was to test if neurocognition, social cognitive deficits and functional disability improve after group cognitive remediation with a neuropsychological tool: the REHACOP program.

One hundred and one patients with schizophrenia were randomly allocated into experimental or control groups. The patients allocated on the experimental group (N= 52) received a group cognitive rehabilitation treatment using REHACOP. They attended 36 sessions of 90 minutes during three months. During the same time and frequency, patients under control condition (N=49) received Occupational activities. Both groups received treatment as usual (TAU). Patients underwent clinical, neuropsychological, social cognitive (Theory of mind, emotional perception, attributional style and social perception) and functional outcome (UPSA) pre- and post treatment assessments.

Repeated measures of MANOVA showed that Group (REHACOP vs occupational therapy) x Time (pre vs post-treatment) interactions were significant for neurocognition total score (F=16.54, p<0.001), social perception (F=8.05, p<0.01), theory of mind (F=14.19, p<0.001) and UPSA (F=12.14, p<0.001) showing that experimental group obtained significant improvement when compared to controls. Attributional style did not show significant improvements.

Our results suggest that REHACOP is an effective group cognitive remediation program for minimizing existing cognitive, social cognitive and functional disability.

Psychiatric comorbidity is an important aspect of neurological disorders. It affects about 30-50% of neurologic patients but is frequently underrecognized.

Our objective was to determine the prevalence and severity of the symptoms of mental disorders in neurologic in-patients.

Between May and September 2014, all neurologic in-patients of a university neurologic center were asked to complete two self report questionnaires for assessing symptoms of mental disorders, namely the Beck Depression Inventory (BDI) and the Brief Symptom Inventory (BSI), which allow to assess a range of nine different psychiatric domains. We performed a multivariate covariance analysis in order to relate the type and frequency of symptoms of mental disorders with the neurological discharge diagnosis, while age, gender, and duration of in-patient treatment served as putative covariates.

Of all responders (n = 157), 51% stated to have suffered from psychological distress within the past seven days, and 43% indicated depressive symptoms (21% mild, 17% moderate, 5% severe). The mean global severity index GSI (M = 0.64, SD = 0.52) exceeded the 1 SD range of healthy persons but was lower than that of psychiatric in-patients known from the literature. Furthermore, our subanalysis revealed different patterns of symptoms of mental disorders between neurologic patients with degenerative, vascular, demyelinating or epileptic disoders.

Psychometric measurement is useful to characterize the burden of the symptoms of mental disorders and will be used to further develop the psychiatric liaison services.

Although many international guidelines emphasize the need to provide psychoeducational family intervention (PFI) together with pharmacological treatments for optimal management of bipolar I patients, this intervention is not routinely provided in mental health centres.

The Department of Psychiatry of the University of Naples SUN has promoted a study to identify obstacles for the implementation of the PFI in the routine care.

To identify mental health professionals’ benefits and difficulties in the implementation of PFI.

The Family Intervention Schedule was administered to mental health professionals at baseline, at T1, T2, T3, T4, T5 supervision meetings.

Twenty-two mental health professionals were recruited, 45% were psychiatrists, with a mean age of 37.4(±7.3). The participants had been working in psychiatry for a mean of 9.1 (±6.5) years and the majority of them stated to be routinely in contact with patients and their relatives. At the beginning of the study, the highest levels of difficulties regarded:1)the integration of PFI with other work responsibilities;2)the lack of time to run the intervention;3)the identification of suitable families. Among benefits, participants reported an increased feeling of confidence in relation to their work (p<.05) and an improvement in the relationships with the service users’ families (p<.01). Difficulties tended to decrease over time, being absent at the end of the protocol.

Although mental health professionals recognize that PFI has a positive impact on their work, many organizational obstacles prevent its routine use. A possible solution may be the promotion of peer-led PFIs, as already done by the US NAMI.

La rTMS du DLPFC gauche a été réalisée à haute fréquence (20 Hz, 90 % du seuil moteur, 20 trains de 2 secondes avec des intertrains de 60 secondes), de manière quotidienne, selon une méthode de repérage visuelle. Les ECT ont été réalisés de manière bitemporale. Une titration a été réalisée afin de définir individuellement le seuil épileptogène de chaque patient.

Méthode de repérage visuel « 10–20 ». Le patient 1 était une femme de 27 ans atteinte d’un trouble bipolaire de type 1 diagnostiqué à l’âge de 22 ans. Elle présentait un EDM évoluant depuis 9 mois. Elle était traitée par aripiprazole et lithium, après avoir reçu plusieurs thymorégulateurs et antidépresseurs. Les symptômes étaient un ralentissement psychomoteur majeur, une thymie triste avec pleurs, de l’anxiété et des idées suicidaires. Elle bénéficia de 10 séances de HF-rTMS du DLPFC gauche suivis d’ECT bitemporale. Les idéations suicidaires disparurent et la thymie s’améliora nettement après la troisième séance d’ECT. Initialement à 25 le score de Hamilton diminua à 2 après 5 ECT puis à 0 après 10 ECT. Le patient 2 était une femme de 39 ans souffrant d’une dépression chronique résistante, avec des antécédents de tentatives d’autolyses graves. Son traitement était composé de sertraline, lithium et loxapine. À son admission, elle présentait un ralentissement psychomoteur, une anhédonie, une anxiété, des troubles du sommeil ainsi que des idées suicidaires. Elle reçut 7 séances de HF-rTMS avant les ECT. La patiente fut sortante de l’hôpital après la sixième séance d’ECT. Initialement à 19 le score de Hamilton diminua à 3 après 5 ECT puis à 2 après 10 ECT. Pour les deux patientes, les crises électriques furent plus longues, avec des intensités de stimulation moindre et les améliorations cliniques plus rapides comparés aux autres patients présentant un état clinique et un traitement par ECT similaires, sans effet secondaire notable. Caractéristiques des crises électriques.

La neuro-excitabilité corticale a été étudiée chez des patients présentant un EDM. Plusieurs études ont retrouvé une diminution de l’ICI (inhibition intracorticale) et du CSP (période de silence corticale) au décours de l’EDM, impliquant un rôle probable du système GABAergique dans son ensemble. De plus, la normalisation du taux de GABA préfrontal est associée à la rémission dans l’EDM. L’ensemble de ces résultats est en faveur d’une diminution de la transmission synaptique GABAergique dans la dépression. Des auteurs ont retrouvé une augmentation du taux de GABA cérébral ainsi que de l’activité GABA-B après un traitement par ECT chez l’homme. La rTMS augmente également le CSP et les patients déprimés répondant à l’HF-rTMS du DLPFC gauche (20 Hz) ont un CSP et une ICI augmentées. Les modifications de l’ICI sont corrélées avec l’amélioration du score de HAMD (Hamilton Rating Scale for Depression). Les travaux d’Otis et Mody indiquent que les changements du CSP ne sont détectables qu’après une importante activation des interneurones inhibiteurs, suggérant que l’activation des récepteurs GABAb nécessite une exposition prolongée au GABA ou une forte concentration de GABA. La HF-rTMS du DLPFC gauche et les ECT sembleraient donc avoir une activité GABAergique corticale commune avec une majoration du taux cérébral de GABA et des modifications de neuro-excitabilité. On peut s’attendre que la HF-rTMS du DLPFC gauche expose de manière répétée les récepteurs GABA à leur substrat et aurait un rôle d’amorce avant la réalisation d’ECT, permettant une meilleure efficacité du traitement par ECT comme dans le cas de nos deux patients.

En 2013 et 2015, Wachtel et al. soulignaient l’intérêt de l’électroconvulsivothérapie en tant que traitement de référence du syndrome catatonique dans le trouble du spectre autistique (TSA). Cependant, les risques de cette thérapeutique nous encouragent, en l’absence de critère de malignité, à l’utilisation en première et deuxième lignes d’un traitement pharmacologique, notamment en cas d’échec des benzodiazépines ou de comportements hétéro-agressifs associés.

Nous rapportons les cas de syndromes catatoniques résolutifs sous traitement pharmacologique chez 3 patients atteints de TSA. Le 1er cas fut résolutif dès 1,25 mg/j de lorazépam. Après échec du lorazépam, les 2 autres le furent respectivement sous amantadine 300 mg/j et quétiapine 100 mg/j. Notre 3e cas présentait des comportements hétéroagressifs non imputables à une cause organique.

Ces cas illustrent l’intérêt du lorazépam dans la prise en charge de la catatonie dans les TSA et celui de l’amantadine en cas d’échec des benzodiazépines. De même, la présence concomitante de comportements hétéro-agressifs, malgré un bilan organique normal, peut parfois conduire à l’emploi d’un traitement antipsychotique, mais avec la nécessité de précautions particulières dans le syndrome catatonique. En l’absence d’étude sur le sujet, les quelques données recueillies dans le trouble du spectre schizophrénique ainsi que notre 3e cas nous incitent à préconiser l’emploi, dans ces situations, d’un nombre restreint d’antipsychotiques atypiques comme la quétiapine et la clozapine.

Le traitement pharmacologique, tel que le lorazépam, l’amantadine et certains antipsychotiques atypiques, présentent un réel intérêt dans le traitement de 1re et 2e lignes du syndrome catatonique dans le trouble du spectre autistique. L’électroconvulsivothérapie reste le traitement de référence à utiliser en cas de critère de malignité ou d’échec de ces traitements.

Attention deficit hyperactivity disorder (ADHD) has been found to be related to overweight/obesity in children and adolescents.

However, it is a heterogeneous disorder, and the relationships between the dimensions of ADHD and overweight/obesity are not clear.

The aim of this study was to explore which dimensions of the disorder are specifically associated with overweight/obesity.

The study sample consisted of 300 treatment naive children with ADHD and 75 healthy controls aged 7–17 years. The ADHD module of the Kiddie Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Versionwasused to diagnose ADHD. The severity of ADHD symptoms was assessed via Conners’ Parent Rating Scale (CPRS). The weight, height, and breast-feeding duration of the study samples and controls were recorded. The body mass index (BMI) of the participants was categorized according to the national age and sex speci?c reference values.

The rate of overweight/obese children was higher in the ADHD group. The association between ADHD symptoms and BMI percentile scores was evaluated using structural equation modeling. In that model, it was observed that the inattention and oppositionality subscores of the CPRS had a positive predictive effect on the BMI percentile scores of the children. However, the duration of breastfeeding showed a negative predictive effect on the BMI percentile scores.

These results showed the importance of inattention, oppositionality and breastfeeding durationon overweight/obesity in children and adolescents with ADHD. Longitudinal studies are needed to more fully understand this relationship and the mechanisms underlying the association between ADHD and overweight/obesity.

Previously, a number of observational studies have found no association between contact with services and improved outcomes in children with psychopathology, despite the existence of effective interventions. The British Child and Adolescent Mental Health Survey (BCAMHS) 2004 is a study of 5,325 children aged 5-16, with data on parent-reported mental health-related public sector service contacts and psychopathology over three years, allowing exploration of the association in a large community-based dataset.

• Describe the trajectories of children in contact and not in contact with services over three years from 2004-2007

• Explore the association between mental health-related service contact over the study period and outcomes in terms of parent-rated psychopathology

This study is a secondary analysis of children's trajectories by service contact status (no contact, contact 2004 only, contact 2007 only, contact 2004 & 2007). The main measure of psychopathology over time was the well-validated Strengths and Difficulties Questionnaire. Multivariable linear regression was used to examine the association between contact and outcomes in children with a psychiatric disorder at baseline in 2004.

Children in contact with services demonstrated the highest levels of psychopathology over time, and the 2007-only contact group displayed a trajectory of increasing difficulties. Overall, contact with services was not associated with improvement in outcomes, following adjustment for measured confounders.

The effectiveness of contact alone with services was not demonstrated in our findings. Despite methodological limitations, this underlines the need for continuing development and consistent implementation of effective interventions, alongside initiatives evaluating outcomes within services.

The use of CBT remains the recent techniques in Algeria and its introduction in the therapeutic arsenal field; already insufficient, finds resistances from the part of some practitioners. It is about the study showing the interest of cognitive and behavioural treatment in the panic disorder with agoraphobia.

It is about the comparative study of the two types of the population presenting the diagnosis of a panic disorder with agoraphobia. The first group will be treated by antidepressors and the others by the cognitive behavioural treatment. The first population estimated to 50 patients receive only the antidepressors (anafranil) and the other of 50 patients receive the technique of cognitive and behavioural treatment. The two populations will be selected according to the randomisation principle. The study duration is of 3 months and the assessment is done at a day 0, 7, 14, 30 and 90 according to Cottraux anxiety scale and file automatic thought of Beek. The data (given in formation) of scales of two groups will be compared before the first day and the end of the medical care. This comparison will be done by statistical inductive tools for each group 5 to determine if it has a therapeutic effects or not) and between two groups to determine if the psychotherapic access (approach) possess equal therapeutic effects better than chemotherapy.

The CBT widely finds its place in Algeria because it offers others characteristics: less onerous, limited in time, easy to practise, variability of techniques. The contribution of the patient in his therapeutic project with an active way. All the patients can be benefited whatever is their associeted organic defects, in reverse the medicines or the indesirable effects and the contra-indications limit its utilization. In Algeria, the practice of this structured psychotherapy is rare and it will be wished that short cycles of formation must be prodigal for the treated personal with psychiatry (nurses, psychologists, students, psychiatrists and general practitioners). To think of introducing a specialized psychotherapy courses for the medical students at the end of the cycle in frame of medical psychology module.

Pharmacotherapy is essential for the treatment of children and adolescents with Attention Deficit Hyperactivity Disorder (ADHD). There are only two metaanalyzes available in the literature where bupropion (BUP) was also included and compared to atomoxetine (ATX) and methylphenidate (MPH) in the treatment of ADHD in youths.

There is a lack of comparative effectiveness research among ADHD medications in terms of efficacy, where BUP is compared with ATX, lisdexamfetamine (LDX) and MPH.

The main aim of this work was to compare the efficacy of these drugs in children and adolescents using a metaanalysis.

A literature search was conducted to identify double-blind, placebo-controlled, noncrossover studies of ADHD. A systematic electronic literature search of PubMed (1975–April 2014) and clinicaltrials.gov with full text (1981–April 2014) was conducted. Drug efficacy was calculated based on the standardized mean difference (SMD). Treatment score was the primary endpoint.

28 articles and 27 trials met inclusion criteria and were sufficient for inclusion in the metaanalysis. BUP 0.32 (95% CI, -0.05, 0.69) showed small efficacy, ATX 0.68 (95% CI, 0.59, 0.76), and MPH 0.75 (95% CI, 0.52, 0.98) showed modest efficacy in reducing ADHD symptoms and LDX showed high efficacy 1.28 (95% CL, 0.71, 1.84).

The results suggest that LDX has the best efficacy and has promising potential for treating children and adolescents with ADHD. Effect sizes should not be the only evidence for clinicians when choosing ADHD medication. More research is needed for a better clinical evaluation of BUP.

L’évolution du phénomène est observée en croisant plusieurs approches : dispositifs de surveillance sur les phénomènes émergents (TREND) ou les produits nouveaux ou dangereux (SINTES) ; outils de veille sur Internet développés dans le cadre du projet I-Trend (analyse de l’offre et des discours sur les forums, enquête en ligne) ; plus récemment, premières données valides en population générale (baromètre santé).

L’identification de nouvelles molécules se stabilise en France autour de 3 par mois mais s’accélère au plan européen (101 en 2014). Les cannabinoïdes restent majoritaires mais les familles chimiques se diversifient. Les sites de vente se rendent de plus en plus accessibles et sécurisants pour les acheteurs tout en développant des accès vers des espaces plus discrets (deep-web). Les substances observées sur le marché « physique » demeurent peu nombreuses même si leur accessibilité progresse en espace festif. La consommation se développe selon une cinétique plus lente que l’offre, presque uniquement parmi les personnes déjà usagers de drogues. En 2014, 4 % des 18–34 ans auraient consommé un cannabinoïde synthétique. L’offre demeure d’une complexité extrême pour la majeure partie des usagers et la méconnaissance des produits et des risques s’accroît lorsqu’on s’éloigne du noyau des usagers « psychonautes ». Hors milieu festif, l’usage a émergé parmi certains « anciens » héroïnomanes insérés.

Le phénomène « NPS » accélère la mutation du champ vers des usages de plus atomisés, laissant les usagers plus isolés face à la pression d’une offre de plus en plus complexe.
