L’éjaculation précoce (EP) touche environ un homme sur cinq. Le projet BibliothEP (évaluation d’une bibliothérapie de l’EP) s’est fixé comme objectif d’élaborer un traitement à la fois efficace, simple, facile d’accès, bon marché, libre de toute toxicité et éventuellement applicable sans l’intrusion d’un tiers.

L’étude s’est déroulée en deux phases. La première comprenait 120 participants souffrant d’EP. Un manuel concis (51 pages A5) d’auto-traitement cognitivo-comportemental de l’EP leur a été donné à lire, suite à quoi ils ont été comparés à 66 sujets laissés en liste d’attente. La seconde phase comparait 36 sujets ayant lu le manuel à 32 autres qui, en plus du manuel, recevaient un bref accompagnement (45 à 90 minutes) par un professionnel de la santé. Les accompagnants n’étaient pas spécialisés en thérapie sexuelle mais ont reçu une formation de 5 heures les habilitant à soutenir et faciliter le processus d’auto-traitement. Les paramètres évalués après traitement comprenaient la latence éjaculatoire auto-estimée, le sentiment de contrôle sur l’éjaculation, la satisfaction sexuelle, la détresse relative à la situation et l’impression subjective d’amélioration.

Six mois après le traitement, tous les participants ont montré des améliorations significatives en comparaison des sujets laissés en liste d’attente. Ces améliorations persistaient à 12 mois. Elles étaient légèrement supérieures pour les participants ayant bénéficié d’une assistance. Des améliorations ont été trouvées pour toutes les formes du trouble mais l’intervention semble avoir été légèrement plus efficace lorsque l’EP était de sévérité modérée. Dans tous les cas, les améliorations du fonctionnement sexuel se sont accompagnées d’une amélioration des cognitions sexuelles.

Le rapport coût/efficacité de la bibliothérapie en fait un instrument thérapeutique idéal en première ligne. Il semble en outre possible de majorer l’efficacité de la formule en la doublant d’une assistance par un intervenant expressément formé à cette fin.

Substance use disorders are highly prevalent among people with schizophrenia. Dually diagnosed patients present with unfavorable course and poor long-term outcomes. Integrated, motivation-based treatment for both disorders in the same setting is considered the treatment of choice. However, integrated treatment programs are not readily available and effect sizes of the programs are modest.

To evaluate an integrated psychosocial treatment program for people with schizophrenia and substance use disorders in the setting of a community psychiatric hospital.

100 in-patients with schizophrenia and substance use disorders were randomized to Integrated Treatment (IT) or Treatment as usual (TAU). The IT group was initially treated in a specialized open ward; upon discharge they were offered treatment in a specialized out-patient program of the hospital. The TAU group was initially treated in another non-specialized open ward; upon discharge they were offered treatment in the non-specialized out-patient unit of the hospital. TAU included pharmacotherapy, medical treatment, supportive psychotherapy and further aids by nursing staff and social workers. IT included all elements of TAU plus manualized group therapy with motivational interviewing, psychoeducation and cognitive-behavioral approaches. Assessments were performed at baseline and after 3, 6 and 12 months.

The IT group had slightly less drop-outs in the follow-up period (non-significant). The IT group was more satisfied with treatment and they developed a higher motivation to reduce substance use. Both groups succeeded in reducing substance use during follow-up, whereas the IT group did slightly better (non-significant).

Hyperprolactinemia is a common adverse effect of typical and some atypical antipsychotics and can induce side effects associated with decreased quality of life and poor adherence to treatment.

Management options are limited and can lead to decompensation of the patient.

Aripiprazole is a recent antipsychotic with partial agonist activity over the dopamine D2 receptors which can be effective in reducing hyperprolactinemia in patients treated with antipsychotics.

Review the existing literature about the effect of adjunctive treatment with low doses of aripiprazole on antipsychotic-induced hyperprolactinemia.

Search for literature about the topic in PubMed and Goocle Scholar databases using “hyperprolactinemia” and “aripiprazole” as keywords.

We found 6 case reports with 8 patients treated with risperidone, paliperidone, zuclopenthixol and fluphenazine that, with 2,5-10mg of aripiprazole, showed a decrease in serum levels of prolactin. 4 open label studies, with a total of 69 patients, all treated with risperidone, also showed a decrease in prolactin levels for adjunctive treatment with 3-10mg of aripiprazole. Two double-blind and two single-blind RCT vs placebo was found, with a sample of 297 patients treated with risperidone or sulpiride and 5-10mg of aripiprazole, and showed decrease in serum levels of prolactin. In all studies treatment with aripiprazole was not significantly associated with an increase in adverse effects or worsening of clinical symptoms.

The antipsychotic-induced hyperprolactinemia could be partially or totally reversed after adding low doses of aripiprazole to current treatment. It seems to be an effective, safe and well tolerated option.

Alors que la majorité des traitements antidépresseurs prescrits en France durent moins de 28 jours , moins de 2 % des travaux étudiant le traitement pharmacologique des troubles de l’humeur explorent les facteurs métapsychologiques associés à l’adhésion . Quelles sont les attitudes aux antidépresseurs des patients observants et demandeurs de soins ?

L’objectif de cette étude était d’évaluer les attitudes aux antidépresseurs chez :

• – des patients hospitalisés pour leur dépression ;

• – ceux se rendant chez leur pharmacien pour chercher leur traitement.

Les attitudes aux antidépresseurs étaient évaluées en utilisant la version courte du Drug Attitude Inventory (DAI-10), chez 98 patients : 40 hospitalisés de leur plein gré en psychiatrie et 58 venant chercher une prescription d’antidépresseur en officine.

L’âge moyen de l’échantillon était de 43 ans, avec 69 % de femmes. Dans l’échantillon hospitalier, la médiane du DAI était de 3,5 (échelle de –10 à +10). Trente pour cent des patients avaient une attitude négative à l’antidépresseur prescrit, dont 10 % de l’échantillon ne connaissait pas le nom. Il s’agissait de la première prescription d’antidépresseur pour 30 % des patients. Dans le volet ambulatoire, la médiane du DAI n’était que faiblement plus positive (4), et 10 % des sujets avaient une attitude négative à l’antidépresseur qu’ils venaient chercher. Vingt pour cent des sujets déclaraient ne pas avoir de suivi de son trouble dépressif.

Cette étude naturalistique met en relief l’ambivalence des patients face aux antidépresseurs. Tandis qu’un comportement montre un engagement fort dans les soins (être hospitalisé en milieu spécialisé, aller chercher une prescription au long cours), les connaissances sur les troubles les justifiant et les croyances envers les thérapeutiques prescrites restent précaires .

Cette ambivalence, reflet des attitudes de la société face à nos médicaments, pourrait être mieux explorée en pratique clinique quotidienne.

Notre étude présente le travail préliminaire de mise en place d’un protocole de psychoéducation chez 12 patients schizophrènes hospitalisés sous contrainte (spdt) et vise à évaluer l’évolution des CLT et l’AT méthode : un protocole psychoéducation de 5 séances concernant la schizophrénie (épidémiologie et mécanismes, signes cliniques, hygiène de vie, traitements, facteurs de rechute et après l’hospitalisation) sur 3 semaines a été proposé à des patients schizophrènes (PANSS et DSM) spdt et traités en monothérapie par antispychotique atypique, dès la 2e semaine d’hospitalisation après avis collégial. Une échelle de CLT (BMQ belief about medicine questionnaire ) et une d’AT (HAQ Helping alliance questionnaire thérapeute et patient Luborsky ) ont été remplies en début et fin de protocole.

La participation des 12 patients étudiés a été assidue durant la totalité du protocole. Le score d’AT s’est amélioré pour la totalité des patients. Les CLT semblaient évoluer dans les 4 domaines (nécessité, préoccupation, surprescription, préjudice) et si les croyances de nécessité restent peu améliorées, il semble que les croyances de surprescription et de préjudice diminuent. Les croyances de préjudice étaient inversement corrélées avec l’AT (p = 0,042).

Ces résultats préliminaires montrent la faisabilité d’un protocole de psychoéducation dès les débuts d’hospitalisation en spdt de patients schizophrènes. Le bénéfice de la psychoéducation semble confirmer l’amélioration de l’AT et la modification des CLT. Cependant, ces résultats doivent être évalués à plus grande échelle et l’influence sur les rechutes ultérieures doit être mesurée.

An epidemiological study in the field, in our territory, isinteresting from an operational perspective. The main objective of this presentation is to describe the socio-demographic characteristics of people who committed suicide in Romania.

Was performed a retrospective study based on data provided by the Romanian Institute of Statistics and “Mina Minovici” Institute. Suicide events occurring between 2009-2013 in Romania were analysed, focusing on age, sex, seasonality, marital condition, occupation status and religion.

Male / Female ratio = 5.52, mean age was 49.82 years, with a range between 8 and 97 years. Suicide is more common in men than in women under 60 years, but after this age the situation is reversed. Suicide was statistically significantly more frequently in rural areas, the retired and unemployed people. In terms of marital status, it is observed that suicide is more common among married people, followed a short distance from the unmarried. From the distribution of suicides according to marital status and sex are significant differences in favor of unmarried men and widows. A secondary level of education predominated, followed by professional and primary. Method suicidal prevalent in both men and women was strangulation.

The study provides a comprehensive analysis of socio-demographic who committed suicide in Romania in the last 5 years. Suicide is more common among rural men, unmarried, unemployed, professional training level.

The question of a continuum or a dichotomy between schizophrenia and bipolar disorders is not clearly elucidated.

The objective of the present study was to identify specific functional connectivity (FC) patterns in schizophrenia and bipolar disorders.

Therefore, the aim was to explore FC within the language production network in response to a verbal fluency task in patients with schizophrenia, patients with bipolar disorders and healthy participants. We hypothesized that prefronto-subcortical FC patterns within the language production network differ between the three groups.

Forty nine participants, comprising 15 patients with schizophrenia, 14 patients with bipolar disorders (DSM-IV) and 20 healthy controls were included in the study. FC was calculated using functional magnetic resonance imaging during a verbal fluency task between the activated pair-seed regions.

Firstly, patients with schizophrenia presented a significantly reduced FC compared to controls within two pair-seed regions (medio-frontal cluster – left subcortical cluster and left fronto-lateral cluster – left subcortical cluster) while bipolar patients were not significantly different from healthy participants. Secondly, patients with schizophrenia compared to patients with bipolar disorders exhibited reduced FC within one pair-seed region (medio-frontal cluster – left subcortical cluster).

Our results support the hypothesis of a specific medio-prefronto-striato-thalamic functional dysconnectivity implicated in the pathophysiology of schizophrenia. This fronto-subcortical dysconnectivity in schizophrenia could underlie language production symptoms observed in this pathology and could be a functional brain marker of schizophrenia.

Few studies focus on peripheral biological markers of personality. Melatonin (MLT), the main hormonal product of the pineal gland, has been used both as a diagnostic and therapeutic element and has been related with chronotype, depression and schizophrenia, among other psychiatric conditions. However, there is a paucity of studies on its use as a personality marker. The present work aims to determine whether serum MLT levels are related with the Eysenck's personality extraversion/introversion (E/I) dimension.

A sample of 100 healthy volunteers participated in the study. The E/I personality dimension was evaluated using the EPQ-BV (Eysenck Personality Questionnaire – Brief Version). Three blood samples were taken (at 09:00, 12:00 and 00:00 h) to measure MLT. MLT was analysed in serum by an ELISA. Serum MLT concentrations are expressed in pg/ml.

MLT levels displayed a clearly circadian pattern, with night levels being significantly higher than day-time levels (00:00: 35.78 ± 23.53 vs. 09:00: 7.78 ± 5.56, 12.00: 3.35 ± 1.94, p < 0.05). Serum MLT levels at 09:00 h were significantly higher than MLT levels at 12:00 h (7.78 ± 5.56 vs. 3.35 ± 1.94, p < 0.05). No significant correlations were found between E/I scores and serum MLT levels at 09:00 (r = – 0.06, p = 0.55) 12:00 (r = – 0.12, p = 0.25) and 00:00 h (r = – 0.01, p = 0.99).

There is no relationship between MLT levels and the E/I personality dimension. Future studies should examine the neuroticism dimension of personality.

En psychiatrie, la contention médicale reste une pratique courante qui peut s’avérer traumatisante pour le patient. Elle doit toujours être associée à une sédation. Nous avons voulu réaliser un état des lieux de cette prise en charge médicamenteuse dans notre établissement.

Pendant 1 mois (janvier 2015), nous avons ciblé les patients sous énoxaparine sodique en préventif grâce au logiciel de prescription (Pharma®). Pour chaque patient, nous avons vérifié qu’il s’agissait bien d’une contention physique. Puis, une analyse des traitements prescrits était réalisée (médicaments, associations, posologies…).

Quatorze patients ont été inclus dans l’étude (8 hommes et 6 femmes, âge moyen : 33 ans). En moyenne, les patients ont été contenus 4 jours [1–10], l’énoxaparine sodique a été initié 24 h [0–72] après le début de la contention et administré pendant 2 jours [0–6]. Les patients ont reçu entre 0 et 3 antipsychotiques différents (cyamémazine, lévomépromazine et halopéridol) indiqués dans les états psychotiques aigus dont le plus prescrit était le cyamémazine (10/14 patients) à une posologie moyenne de 50 mg à j1, 115 mg à j2 et j3. Concernant les benzodiazépines, les patients ont reçu en moyenne une seule benzodiazépine et principalement le lorazépam (7/14) à une posologie de 4 mg à j1, 5,5 mg à j2 et j3 ou le diazépam (4/14) à une posologie de 7,5 mg à j1, 22,5 mg à j2 et 27,5 mg à j3.

Les patients contenus reçoivent donc peu de psychotropes, à posologies faibles par rapport aux doses habituelles en psychiatrie (exemple : cyamémazine jusqu’à 600 mg). Un groupe de travail sur la contention en psychiatrie est actuellement en cours dans l’établissement afin d’émettre des recommandations sur les modalités de prescription des psychotropes pour éviter une contention physique durable.

Many psychiatric disorders including attention-deficit/hyperactivity disorder (ADHD), disruptive behavioral disorders, autism spectrum disorders, and some psychiatric characteristics, such as poor empathizing, are regarded to be related to elevated levels of androgens or androgen sensitivity. Idiopathic hirsutism (IH) is a disease characterized by an increased sensitivity of the pilosebaceous unit to circulating androgens in women. However, no studies investigated the potential association between IH and androgen-related psychiatric conditions.

The purpose of this study was to examine whether IH has a relationship with androgen-related psychiatric conditions.

Thirty seven females with IH and 33 healthy female controls were included in this study. Childhood and present ADHD symptoms of the participants were assessed using the Wender Utah Rating Scale (WURS) and the Adult ADHD Self-Report Scale (ASRS), respectively. The Autism-Spectrum Quotient (AQ) and the Interpersonal Reactivity Index (IRI) were used to assess autistic traits and different aspects of empathy. Hirsutism severity was measured by the Ferriman–Gallwey scoring system.

No significant difference was found between patients and controls on psychiatric questionnaire scores, except for a trend for subjects with IH to show higher levels of school-associated problems than controls according to WURS. The severity of hirsutism was strongly correlated with the WURS irritability subscore and the WURS total score, and moderately correlated with the WURS behavioral problems/impulsivity and attentional deficit subscores.

This study provides preliminary evidence that common etiological factors may be involved in both the development of IH, ADHD, and coexisting disruptive behavioral problems.

Explorer sécurité d’emploi, tolérance et efficacité du palmitate de palipéridone (PP) à doses flexibles chez des patients adultes schizophrènes hospitalisés pour une décompensation.

Étude internationale, prospective, en ouvert, non-interventionnelle de 6 semaines.

Échelles BPRS (Brief Psychiatric Rating Scale), CGI-S (Clinical Global Impression-Severity), PSP (Personal and Social Performance Scale), questionnaire de satisfaction sur le traitement (MSQ-Medication Satisfaction Questionnaire), ESRS (Extrapyramidal Symptom Rating Scale) et évènements indésirables sous traitement (EIST), entre début d’étude (baseline) et dernière observation rapportée.

Trois cent soixante-sept patients analysés (65,9% hommes, âge moyen (± écart-type) 39,8 ± 12,1 ans, 85,8% schizophrénie paranoïde). Au total, 91,6% des patients ont terminé l’étude de 6 semaines. Le délai moyen entre l’admission à l’hôpital et l’initiation du PP était de 9,4 ± 7,7 jours. Le score initial BPRS (50,2 ± 13,6) s’est amélioré de–6,5 ± 8,6 au jour 8 et de–19,3 ± 12,6 à la fin de l’étude (IC 95% = –20,7;–18,0; deux critères avec p < 0,0001). En fin d’étude, 93,6% des patients étaient évalués comme améliorés sur l’échelle CGI-S. Le score de fonctionnement PSP s’est amélioré de 49,4 ± 14,7 à baseline à 14,3 ± 12,4 en fin d’étude (IC 95% = 12,9; 15,8, p < 0,0001). Le score ESRS moyen a diminué de manière significative de 3,7 ± 5,9 (baseline) à 2,0 ± 4,7 en fin d’étude (p < 0,0001). Le pourcentage de patients très ou extrêmement satisfaits par leur traitement antipsychotique est passé de 6,0% à baseline (traitement précédent) à 46,1% en fin d’étude (PP). EIST rapportés chez ≥ 2% des patients: tremblements (2,5%) et schizophrénie (2,2%).

Ces données chez des patients schizophrènes hospitalisés pour décompensation confirment les résultats d’études contrôlées randomisées montrant que le PP à doses flexibles est bien toléré, induit une réponse au traitement rapide et cliniquement significative et une amélioration du fonctionnement. More than 80 % of patients experiencing their first depressive episode will have at least one new episode. Effective interventions to reduce the risk of relapse and recurrence are needed. Psychoeducation is a form of interactive education enhancing knowledge about patients’ illness, including its course, symptoms and treatment. Psychoeducational methods can also act as family intervention – already an evidence-based practice in schizophrenia and bipolar disorder. Only few studies have focused on the effect of psychoeducation, including family psychoeducation, in the prevention of new depressive episodes.

Purpose is to evaluate whether an intervention of psychoeducation for family members, compared to a control intervention, is effective in reducing the risk of new depressive episodes among patients that have achieved remission or partial remission of depressive symptoms after the acute phase of antidepressive treatment.

The project is based on a double-centre, randomized controlled trial where investigator and raters conducting psychometric assessments, will be blinded to treatment allocation. A total of 130 patients with unipolar depression in remission or partial remission will be included together with their closest relative. After baseline assessments, relatives will be randomized to either 4 sessions of a family psychoeducation program or 4 sessions in a social support group without any psychoeducational intervention. Patients will not participate in group sessions and they will continue their outpatient-treatment as usual.

Primary outcome is evaluated after 9 months and include rates of relapse as measured by HAM-D17 and rates of recurrence according to DMS-VI-R and HAM-D17.

Évaluer l’effet du palmitate de palipéridone (PP) à doses flexibles sur les symptômes négatifs, la dépression/anxiété, le fonctionnement et les symptômes extrapyramidaux chez des patients adultes souffrant de schizophrénie non-aiguë, en échec à un traitement par aripiprazole oral.

Étude internationale, prospective, en ouvert de 6 mois.

Variations entre début d’étude (baseline) et dernière observation rapportée sur la PANSS (Positive and Negative Syndrome Scale), la sous-échelle négative de la PANSS, les facteurs de Marder de la PANSS « symptômes négatifs » et « anxiété/dépression », les échelles de fonctionnement PSP (Personal and Social Performance) et Mini-ICF (Mini International Classification of Functioning) et l’échelle ESRS (Extrapyramidal Symptom Rating Scale).

Quarante-six patients analysés (73,9 % hommes, âge moyen 34,4 ± 9,4 ans, 78,3 % schizophrénie paranoïde). Parmi les patients, 67,4 % ont terminé l’étude. Avant l’inclusion, la dose moyenne d’aripiprazole oral était de 22,7 ± 10,7 mg/jour. Le score de la sous-échelle négative de PANSS s’est significativement amélioré de 20,3 ± 5,0 (baseline) à 17,3 ± 6,1 (fin d’étude) (variation moyenne = −3,0 ± 5,0 ; IC 95 % = −4,4 ; −1,5 ; p < 0,0001), ainsi que les scores des facteurs de Marder « symptômes négatifs » (de 19,5 ± 5,8 à 16,6 ± 5,9 ; IC95 % = −4,5 ; −1,3, p < 0,0001) et « anxiété/dépression » (de 10,3 ± 3,6 à,5 ± 2,9 ; IC95 % = −3,0 ; −0,6, p = 0,0031). Le score ESRS s’est aussi amélioré de manière significative (de −0,6 ± 3,4 ; IC 95 % = −1,6 ; 0,4, p = 0,0456). Le fonctionnement du patient s’est amélioré de manière significative sur les échelles PSP (de 58,9 ± 13,4 à 62,9 ± 15,2, p = 0,041) et Mini-ICF (de 19,0 ± 7,78 à 16,1 ± 9,84 ; IC 95 % −5,1, −0,7 ; p = 0,0079). Le seul événement indésirable sous traitement rapporté chez ≥ 5 patients a été l’anxiété (n = 6).

La transition d’un échec au traitement par aripiprazole oral à un traitement par PP à doses flexibles chez des patients souffrant de schizophrénie non-aiguë a été bien tolérée et s’est traduit par une amélioration significative des symptômes négatifs, dépressifs, anxieux et des symptômes extrapyramidaux, ainsi que du fonctionnement du patient.

National siege mentality, underlying the national threat, is surely socially and politically determined. However, we assumed that certain social attitudinal constructs and psychological predisposition might underlie a belief system such as the concept of national siege mentality is.

We tried to answer whether national collective narcissism, anomie, and psychopathic syndrome we developed may prove valuable in understanding the appearance and development of national siege mentality.

The aim of this research were: (1) To develop valid measure instruments that could be useful and efficient tools for assessing attitudinal and psychological tendencies in political-psychological and psychiatric research. (2) To find out the effects of national collective narcissism, anomie, and psychopathic syndrome on national siege mentality.

Confirmatory factor analysis (CFA) were performed to explore factorial and construct validity of the scales used in this research. A questionnaire was administered to randomly selected adults (N=544). Using structural equation modeling, this study examined the effects of exogenous variables in expressing national siege mentality.

Structural model indicated that national collective narcissism, anomie, and psychopathic personality syndrome showed significant effects on national siege mentality (RMSEA=0.07, SRMR=0.05, CFI=0.96, NNFI=0.95).

The scales used in research proved to be reliable and valid measures of the attitudinal and personality constructs. Given the amount of variance of national siege mentality explained by exogenous variables (46 per cent), it is reasonable to assume that collective frustrations, political mistrust, and psychopathological predisposition may be underpinning the belief system labelled national siege mentality.

Les dyskinésies tardives induites par les antipsychotiques sont des mouvements anormaux pouvant survenir à partir de 3 mois de prise médicamenteuse. Leur incidence, estimée à 29,7 %, semble liée à une hypersensibilité des récepteurs à la dopamine. Des thérapies médicamenteuses permettent de juguler ces effets indésirables. Lorsque ces propositions thérapeutiques échouent à soulager efficacement le patient, l’indication de la stimulation cérébrale profonde se pose alors.

À la lumière d’un cas clinique exposant la situation d’un jeune homme traité par antipsychotiques chez lequel des dyskinésies tardives invalidantes sont diagnostiquées, nous mettrons en évidence l’intérêt de la stimulation cérébrale profonde bilatérale pallidale interne lorsque les autres alternatives thérapeutiques ne sont pas efficaces.

Un report de cas complété par une revue de la littérature étayeront nos propos.

L’identification de facteurs de risque de survenue de ces mouvements anormaux doit davantage être considérée par les prescripteurs, pouvant agir sur un axe préventif. La place de la stimulation cérébrale profonde dans le traitement curatif de ces mouvements anormaux est encore marginale du fait des pathologies psychiatriques dont sont atteints ces sujets. Cette technique est pourtant vectrice d’une amélioration conséquente des dyskinésies tardives induites par les antipsychotiques résistantes aux thérapies médicamenteuses.

The epilepsy literature shows a high prevalence of psychological and social disorders which are considered more handicapping than seizures themselves.

1. - To assess the quality of life (QOL) of patients with epilepsy.

2. - Determine the relationship between QOL and level of anxiety, depression and coping strategies.

It was a cross-sectional study involving 50 subjects with epilepsy and followed in the outpatient psychiatry department. The state-trait anxiety inventory was used to assess the level of anxiety, whereas the Beck depression inventory was used to measure the severity of depression. The QOL was assessed using the quality of life in epilepsy inventory (QOLIE -31). The items of the Brief COPE were used to assess coping strategies.

Coping strategies most frequently used were: emotional support (mean score = 5.52), acceptance (mean score = 5.22) and the emotional expression (mean score = 4.18), but none used the strategies of problem avoidance and self-blame.

The average overall score of QOL of our patients was 71.66. It was significantly higher in patients with a minimum level of anxiety (p = 0.012) and those using active coping strategies (p << 0.005), acceptance (p << 0.005) and distraction (p = 0.015). There was no statistically significant relationship between the level of QOL and that of depression.

Our results emphasize the importance of reinforcing strategies of adjustment to illness and the role of the anxiety associated with the unpredictability of seizures; it seems to have an important impact on QOL of patients with epilepsy.

The association between anxiety disorders and verified coronary heart disease (CHD) remains to be clarified.

To report the prognostic association between anxiety disorder subtypes and subsequent major adverse cardiac events (MACE), report anxiety disorder prevalence in CHD, and the efficacy of randomized controlled trials (RCTs).

A comprehensive literature search was conducted for studies reporting anxiety disorder prevalence according to structured interview in CHD samples or MACE and RCTs. From 4,041 articles 42 samples were selected for extraction (8 for MACE prognosis, 39 for prevalence), no RCTs were eligible.

Five generalized anxiety disorder (GAD) studies reported 883 MACE events (combined n = 2851). There was a non-significant association between GAD and MACE risk (Risk Ratio = 1.20, 95% CI .86 – 1.68, p = .28) however the effect size was highly significant in outpatient samples with adjustment for covariates (adjusted hazard ratio = 1.94, 95% CI 1.45 – 2.60, p <.001). No other anxiety disorder subtype was associated with MACE. Prevalence data showed high comorbidity with depression (49.06%; 95% CI 34.28 – 64.01) and substantial heterogeneity between studies and anxiety disorder subtypes. Panic disorder prevalence was higher in psychiatrist/psychologist raters (9.92% vs. 4.74%) as was GAD (18.45% vs. 13.01%). Panic and GAD estimates were also significantly higher in DSM-III-R versus DSM-IV taxonomies (13.40% vs 3.43%, p <001; 11.12% vs. 6.80%, p <.001 respectively).

The paucity of extant anxiety disorder RCTs, alongside MACE risk for GAD outpatients, should stimulate further anxiety disorder intervention in the CHD population.
