Nurses in the Western world have given considerable attention to the concepts of vulnerability and self-autonomy in recent decades.

A qualitative study was conducted with nurses to analyze the problems found by health care professionals in their ethical practices when providing care to patients with terminal diseases, in particular concerning aspects of self-autonomy and vulnerability, in respect to those patients and health care providers.

Using bioethics approach, an analysis of the context was carried out to apply a rigorous methodology in this studied area and fundamental concepts in ethical practices that are intermix. We recruited a purposive sample of research participants from among nurses who worked at a hospital located in the middle part of Portugal.

The transcribed interviews of 8 nurses were analyzed. The results identify the types of difficulty with ethical nuances, found by health care providers, in which constitutes a challenge to ethical practices. The interviewed nurses emphasized the need to broaden the arguments about this subject with other health care professionals faced with these cultural changes and ethical paradigms.

The staff's ethical reasoning mainly concerned their experience of a gap between their personal ideals of what a dignified end of life should include and what they were able to provide in reality, which could result in conscious stress. Staff members need training and support. End of life care demands competence and teamwork.

La violence fait partie intégrante de la psychiatrie, que ce soit à travers des actes auto-agressif ou hétéroagressifs. Évaluer le risque de récidive chez les patients schizophrènes semblerait, alors, primordial.

Évaluer le risque de récidive d’un groupe de patients suivis pour schizophrénie.

C’est une étude transversale réalisée auprès de 51 schizophrènes hospitalisés au service de psychiatrie de Tahar Sfar à Mahdia. Un questionnaire préétabli a permis d’explorer les données générales et cliniques de l’échantillon. Le risque de récidive de violence a été exploré à l’aide d’une échelle actuarielle VRAG.

Les caractéristiques générales de l’échantillon étaient : une prédominance masculine (84,3 %), un âge moyen de 37 ans et un statut de célibataire (76,5 %). Dix patients avaient des antécédents judiciaires et 6 ont été examinés dans le cadre d’une expertise pénale. Environ 80,4 % des patients ont été hospitalisés selon le mode d’office. Le diagnostic de schizophrénie indifférenciée (35,3 %) était le plus fréquent, suivi de la forme paranoïde (25,5 %) et désorganisée (21,6 %). Une personnalité antisociale a été retenue chez seulement 2 patients. L’âge moyen au début des troubles était de 26 ans. Le score moyen à l’échelle VRAG était de –4,96 ± 6,83 attestant d’un risque faible de récidive. L’étude analytique a retrouvé des liens significatifs entre un score VRAG plus élevé et : le sexe masculin (p = 0,042) ; l’âge jeune (p = 0,002) ; le célibat (p = 0,006) et l’âge précoce au début des troubles (p = 0,021).

L’évaluation de la dangerosité implique un regard longitudinal et qualitatif sur le risque de violence. Identifier les facteurs de risque de violence est nécessaire pour prévenir les récidives.

Cinquante patients ELT et 50 contrôles sains appariés selon leurs caractéristiques démographiques ont été inclus. Pour l’ensemble des participants, la sévérité de la symptomatologie dépressive et apathique était évaluée à l’aide de l’inventaire de dépression de Beck et de l’échelle d’apathie Lilloise (Sockeel et al., 2006).

Chez les patients ELT comparativement aux contrôles, la sévérité de la symptomatologie dépressive et apathique était significativement plus importante. Parmi les patients ELT, 18 (soit 36 %) présentaient une apathie modérée à sévère (i.e. un score > –17), et 14 (soit 28 %) présentaient une dépression modérée à sévère (i.e. un score > 19). Les niveaux de sévérité de la symptomatologie apathique et dépressive étaient positivement associés. L’apathie coexistait avec la dépression chez 8 patients ELT (soit 16 %), et 6 patients ELT (soit 12 %) présentaient une dépression sans apathie. Cependant, 10 patients ELT (soit 20 %) présentant une apathie, ne présentaient pas de dépression.

Cette étude donne des indications sur la prévalence de l’apathie dans le cadre d’une épilepsie du lobe temporal. Cette apathie peut être associée à une dépression. Néanmoins, elle est plus fréquemment relevée indépendamment de toute symptomatologie dépressive associée. D’autres études sont nécessaires afin de mieux caractériser les liens entre, apathie, dépression et épilepsie.

In the last ten years it has sought a connection between the psychoanalysis and the neuroscience. The meeting point between these two branches is the neuropsychoanalysis [1].

A literature search was conducted on March 2014. PubMed and Scopus databases were used to find studies for inclusion in the systematic review. The keywords used for the literature search were: ‘psychoanalysis”, ‘psychoanalytic psychotherapy”, ‘psychodynamic”, ‘psychodynamic psychotherapy”, ‘fmri”, ‘functional neuroimaging” and ‘functional magnetic resonance”. keywords were used together with the logical operator ‘and”. Only studies wich compared functional magnetic resonance imaging results before and after psychoanalytic psychotherapy in adult subjects were taken into account. Two (n=2) potentially relevant studies were obtained for the systematic review.

As shown in the table, Buchheim et al. study results demostrate that the therapy hours modulated the attivaction of brain areas. After treatment, Fischmann et al. found a marked reduction in activation pattern.

The common results of the studies investigated in this systematic review is represented by a reduction, a modulation or a normalization of the activation areas before and after the psychoanalytic psychotherapy.

We aimed to evaluate the outcomes of Petals: a charitable organisation in Cambridgeshire. Petals provides counselling for women and couples who have suffered perinatal bereavement, or trauma during pregnancy or birth. This paper attempts to evaluate the effect of counseling interventions at this difficult time.

Outcomes were recorded in 42 patients using the CORE (Clinical Outcomes in

Routine Evaluation) system. CORE was developed to assess the effectiveness of psychological therapies. CORE-OM (CORE Outcome Measure) involves a

questionnaire that assesses subjective well-being, symptoms/problems, function, and risk to self and others. The CORE-OM questionnaire was completed before and after the counselling sessions.

The CORE-OM scores were summated into a global representation of severity.

Severity decreased in all patients. Symptoms of psychological pathology were also decreased in all cases.

A review of the available literature indicates that little is known about the efficacy of therapy for perinatal bereavement and trauma. These original data suggest convincing efficacy and benefits, but the numbers involved are small. Further trials with greater sample sizes are required.

In a case series of 100 patients presenting to psychiatric services with psychotic symptoms, Dalmau and colleagues identified that often young women, with key features such as tiredness and short-term memory impairment, were found to be suffering from NMDA-receptor autoimmune encephalitis (AIE). However, distinguishing AIE from true psychotic illness can be challenging. As a consequence, there is a need to help clinicians identify potential patients suffering from AIE.

To identify relevant clinical features of AIE and to propose a strategy that will help psychiatrists screen for AIE.

A review of the recent literature, and illustration with a clinical case.

AIE should be considered especially in female patients presenting in the context of headache, fever, and viral illness-type prodromes, with atypical psychotic symptoms such as agitation, bizarre behaviour, disinhibition, and poorly formed and segmented delusions. Neurocognitive symptoms such as rapid language disintegration and short term memory loss should also prompt further investigation. Progression of AIE leads to motor dysfunction (seizures, dyskinesia), and secondarily autonomic instability and hypoventilation. A young female with a prolonged and severe episode of manic psychosis presented with some of these features, which is why we considered AIE as a differential diagnosis.

Clinicians should consider AIE when they are faced with a young female presenting with psychotic symptoms and a history of autoimmune illness, with a protracted and unusually severe illness. Investigating for AIE provides an exciting opportunity for psychiatrists to work with neurologists within a multidisciplinary team.

Depressive episodes are common in bipolar illness. Treatment of bipolar depression is often problematic because standard antidepressants carry a number of disadvantages: apparent lower efficacy than in unipolar depression; risk of inducing mania; risk of inducing rapid cycling. Anticonvulsants such as carbamazepine, valproate and lamotrigine are useful in the management of bipolar disorder and they can prevent episodes of depression. There is less evidence that they have antidepressant efficacy but lamotrigine was shown to produce antidepressant effects in a placebo-controlled trial in bipolar depressed patients. Lamotrigine has a role in acute management of bipolar depression and is probably free of a risk of inducing mania or rapid cycling.

To determine the efficacy of lamotrigine in treatment of patients with bipolar depression.

Investigation of lamotrigine efficacy with bipolar depression in 3 month-follow-up and 6 months follow-up treatment with lamotrigine.

The sample consisted of 30 patients with bipolar depression, age range between 26 and 58. All subjects received monotherapy with lamotrigine in the period of six months. Subjects were assessed prior to therapy and in 3 and 6 months follow-up using the following instruments: The Clinical Global Impressions scale (CGI), and Hamilton Depression Rating Scale (HAM-D-21).

The difference between three assessments on CGI was statistically significant. The results indicate statistically significant reduction of depression on HAMD-21 following six months treatment with lamotrigine.

Lamotrigine proved to be efficient in the treatment of patients with bipolar depression for the subjects in this study.

La maladie à corps de Lewy est la seconde cause de démence. Il persiste malgré cela de fréquentes errances diagnostiques, notamment lors de manifestations initiales psychiatriques.

Nous présentons le cas clinique de M F., 60 ans, hospitalisé pour la seconde fois en psychiatrie suite à des troubles du comportement à domicile. Il avait été pris en charge 3 ans auparavant en ambulatoire pour épisode dépressif majeur avec éléments psychotiques , traité par paroxetine et rispéridone. Il a été dans un second temps hospitalisé devant un tableau hypomaniaque, ayant abouti à un diagnostic de trouble bipolaire et la mise en place d’un traitement par valpromide. Aucun bilan neurologique ou neuropsychologique n’avait été effectué.

Au cours de l’hospitalisation, le patient a présenté des fluctuations cognitives, un parkinsonisme, des idées délirantes systématisées à thématique de persécution et une hypersensibilité aux neuroleptiques. Ont été également notés une anosognosie, des troubles du sommeil , des éléments maniaques atypiques avec des achats compulsifs et une désinhibition. Le traitement thymorégulateur initial associé à la loxapine a été inefficace et mal toléré. L’arrêt de l’antipsychotique a permis une nette amélioration clinique. Un traitement par valproate a été secondairement initié, permettant un bon apaisement comportemental. La biologie, le scanner cérébral et l’IRM cérébrale ont éliminé les étiologies organiques courantes. Le bilan neuropsychologique a orienté le diagnostic (troubles des fonctions visuo-spatiales , de l’attention et des fonctions exécutives) vers une probable maladie à corps de Lewy.

Les éléments cliniques peuvent être en accord avec un trouble bipolaire. Mais le terrain, l’anamnèse, les signes atypiques, la réponse aux différentes thérapeutiques et le bilan neuropsychologique orientent vers une maladie à corps de Lewy. On note donc l’importance d’associer au recueil clinique un bilan neurologique et neuropsychologique en pratique courante.

There is still considerable disagreement as to what makes psychotherapy effective. The controversy about the comparative efficacy of different psychotherapy approaches has generated two different major perspectives on the active ingredients in psychotherapy: the specific ingredients model and the common factors model. However, conceptual analyses and empirical data rather suggest a synergistic view of specific and common factors.

The study aims at contributing to a clearer conceptualization of common factors by analyzing their associations with treatment techniques.

A comprehensive literature search was performed to identify the common factors discussed in psychotherapy research. Psychotherapy experts then rated the degree of associations between these common factors and a set of techniques in a web-based survey. A factor analysis was performed to analyze the dimensional structure of common factors, which underlies the rated associations between common factors and techniques. In addition, the techniques associated with the different dimensions of common factors were analyzed by regression analysis.

The factor analysis reveals that the variety of common factors can be described by a four-dimensional structure: common factors related to 1) cognitive processing, 2) emotional processing, 3) coping and 4) common factors fostering therapeutic alliance or therapy motivation. These dimensions are characterized by specific patterns of associated techniques.

Common factors have great potential to contribute to a clarification of the change mechanisms induced by psychotherapy. However, the construct suffers from major conceptual problems. The presented classification represents a first step towards a conceptual clarification of common factors in psychotherapy.

La dépression du post-partum (DPP) est une pathologie multifactorielle survenant chez 13 à 15 % des femmes dans l’année suivant l’accouchement . Les évènements de vie stressants ont été identifiés comme facteur de risque. Parallèlement, le recours aux traitements pour infertilité ne cesse de croître . Actuellement en France, 1 naissance sur 40 est issue de la procréation médicalement assistée (PMA). Lors de la procédure, le parcours peut être une source de stress chez la femme . Nous avons émis l’hypothèse que le risque de symptômes dépressifs et/ou DPP après aide médicale à la conception pourrait être majoré.

Nous avons réalisé une revue de la littérature des études comparant la survenue de symptômes dépressifs dans le post-partum et/ou DPP entre traitement pour infertilité et grossesse spontanée à partir des bases de données Pubmed, ISI Web of Knowledge et PsycINFO jusqu’en décembre 2014. Nous avons ensuite effectué une méta-analyse des données disponibles sur DPP et aide à la conception, et une méta-analyse secondaire se focalisant sur la PMA (logiciel RevMan5).

Notre revue de la littérature, prenant en compte 18 études, ne met pas en évidence de risque majoré de symptômes dépressifs et/ou de DPP après traitement pour infertilité. Notre méta-analyse sur la DPP, prenant en compte 8 études (n = 2451), ne montre pas de différence significative entre aide médicale à la conception et grossesse spontanée (OR = 0,93 [0,67–1,31], z = 0,40, p = 0,69), sans hétérogénéité entre les études. La méta-analyse secondaire concernant les PMA, sur 6 études (n = 1773), ne retrouve également pas d’association (OR = 1,04 [0,71–1,52], z = 0,18, p = 0,86).

Notre étude ne montre pas de risque majoré de DPP après aide médicale à la conception. D’autres études sont nécessaires afin de mieux connaître les spécificités des DPP après traitement pour infertilité (selon la technique utilisée, l’origine ou la nature de l’infertilité).

Depression is associated with various inflammatory-related physical conditions, such cardiovascular and neurodegenerative diseases. Yet, little is known about the association between depression and autoimmune diseases.

To examine the association between depression and risk of autoimmune disease, investigating the temporal and dose-response nature of these relationships.

A prospective study including approximately 1.1 million people is conducted using linked Danish registries. We have identified 145,217 participants with depression between 1995 and 2012. Survival analyses are used to estimate the relative risk of autoimmune disease among those with, compared to without, depression. Analyses are adjusted for covariates.

Preliminary analyses indicate an association between depression and an increased risk of several autoimmune diseases, with a potential dose-response correlation. Final results are still in progress.

Depression seems to be associated with increased risk of the onset of a range of autoimmune diseases. As such, depression may play a role in the etiology of certain autoimmune conditions.
