Significant neurological symptoms may be overlooked because of the traditional view that the non-Korsakoff's psychosis, middle aged alcoholic misuser is neurologically preserved.

In this study, we wanted to investigate the presence of neurological symptoms in individuals with misuse or dependence on alcohol who were abstinent for at least 1 month.

We used two scales from the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) to ascertain the presence of both neurological symptoms (HEA-2) and symptoms of general health concern (HEA-3) in 70 individuals who had a diagnosis of alcohol misuse or dependence, who were abstinent for greater than 1 month.

Individuals reported significantly more neurological symptoms than general health difficulties (p0.001). We detected neurological symptoms, unlike general health difficulties in individuals who were abstinent from alcohol for 12 months. When we examined diagnostic subgroups, general health difficulties were most present in individuals with anxiety disorders; however, neurological difficulties were present across diagnostic groups.

Neurological symptoms persist to a greater extent than general health concerns in individuals who previously were dependent or misused alcohol.

L’entérocolite nécrosante est une complication rare des antipsychotiques (un cas/2000 patients traités) ; tous les antipsychotiques peuvent y participer (63 % des patients de la littérature étaient traités par antipsychotique atypique), particulièrement lorsqu’ils sont associés à un traitement anticholinergique (antiparkinsonien, antidépresseur imipraminique) ; elle peut être un effet indésirable souvent méconnu, mais toujours grave car elle conduit au décès du patient dans 40 à 60 % des cas. Elle est peu documentée dans la littérature et essentiellement sous forme de cas cliniques.

Nous rapportons le cas d’un jeune patient âgé de 25 ans, pris en charge pour un trouble schizoaffectif depuis une dizaine d’années, non compliant aux soins et plusieurs fois hospitalisé sous contrainte, résistant à plusieurs séquences thérapeutiques. Ce jeune homme avait été réhospitalisé pour une rechute délirante marquée par un vécu délirant persécutif et hypochondriaque avec de multiples cénesthopathies, associées à des troubles du comportement à type d’agitation psychomotrice. Il a présenté un cas d’entérocolite nécrosante alors qu’il était traité par quétiapine et zuclopenthixol ASP, et a dû bénéficier d’une colectomie totale en urgence du fait d’un retard au diagnostic et au traitement, malgré un suivi somatique régulier assuré par un somaticien dans le service.

Le diagnostic est particulièrement difficile du fait de la non spécificité du tableau clinique, d’autant qu’elle est souvent la complication d’une constipation ancienne, que l’évolution peut être rapide, et que le pronostic reste réservé. A partir du cas présenté, nous proposons une revue de la littérature et nous discutons les facteurs de risque, les difficultés du diagnostic, les diagnostics différentiels, la physiopathologie et les préconisations thérapeutiques. Il s’agit d’une urgence médicale qu’il faut apprendre à reconnaître et à anticiper pour éviter une évolution spontanément défavorable.

HIV like other chronic illnesses is an important contributing factor for psychiatric co-morbidity in not only HIV sufferers as well as their caregivers. Few studies show that a significant proportion of caregivers of HIV positive patients had psychiatric illness which is otherwise neglected but in turn hampers the quality of care provided to the HIV positive patient as well as the caregiver himself. This chunk of population if identified at a earlier stage, steps to teach them healthy coping strategies as well as timely initiation of management of psychiatric illness can decrease the burden of disease.

To assess and compare the presence of psychiatric morbidity, stress and quality of life in primary caregivers of HIV patients.

All primary caregivers of HIV positive patient attending the Anti Retroviral Therapy(ART) center during six month period were assessed using a semi-structured Performa, International neuropsychiatric interview English version 5.0.0 (MINI PLUS) scale, PSLES, WHO QOL brief scale,. Data was segregated and analysed using appropriate statistical test.

Out of total 125 respondents 24(19.2%) were found to be depressive. Anxiety disorders were significantly prevalent (8.4%). other psychiatric disorders were somatisation, adjustment disorder etc. PSLES was fairly high and WHO QOL scoring was low in caregivers as compared to control group.

Significantly high number of caregivers was positively screened for psychiatric morbidity, most of them were stress related neurotic disorders. the stress level was more in caregivers and quality of life was compromised as compared to control group.

To assess the presence of communicability and regret in cases of deliberate self harm in a tertiary care hospital. to establish relation of communicability as a preventive sign and regret as a good prognostic sign in cases of deliberate self harm.

All consecutive patients with deliberate self harm presented to the emergency department during a 1 year period were assessed on a preformed Performa designed by authors after being declared physically fit and were screened by Standardized Assessment of Personality Abbreviated Scale (SAPAS), PSLES, HAM-D and HAM-A.

Majority of respondents screened negative by SAPAS scoring. Most of the patient had significant stress in life .Most of them had marital conflicts, family conflicts and economic burden as major stressor leading to suicidal behavior. Verbal communication was the most common mode of communication. Most of the subjects 86.3% (69 out of 80) had severe regret about their attempt of deliberate self harm. It developed a high motivation in them not to attempt suicide next time in life.

Most of the suicidal acts are impulsive in nature and are not associated with any psychiatric co morbidity. Our study showed that if communicability is enhanced and this is considered seriously, providing supportive care by family most of the suicidal acts can be avoided. Regret arising after the suicidal act can be used to motivate the subject of deliberate self harm to avoid repetition of future attempts.

The purpose of this study was to estimate the prevalence and examine the predictors of depression in patients with systemic lupus erythematosus (SLE).

This cross-sectional study was conducted in a rheumatology clinic of university hospital. All SLE patients met the revised American College of Rheumatology (ACR) classification were included in the study. Socio-demographic data and medications were recorded. Disease activity for SLE was assessed with the Mexican-SLE Disease Activity Index (Mex- SLEDAI). All subjects were screened for anxiety and depression by using the Hamilton Anxiety Rating Scale (HAM-A) and the Hamilton Depression Rating Scale (HAM-D), respectively. Multivariate linear regression analysis was used to determine predictors of depressive disorder.

A total of 62 SLE (61 for females and 1 for male) patients participated in the study. Based on the HAM-D and HAMA, rates of depression and anxiety in the SLE patients were 45.2 and 37.1%, respectively. the multiple linear regression analysis revealed that HAM-A score and younger age were significant predictors of depression in the SLE patients.

The finding suggests that depression and anxiety are common in the SLE patients. in addition, high level of anxiety and younger age may increase the risks of depression. Because of the small sample size, further studies should be conducted to warrant these results.

les états mixtes se caractérisent par la coexistence de symptômes maniaques et dépressifs au cours du même épisode thymique. En pratique clinique, l’identification de tels états est essentielle.

Le but de ce travail est d’évaluer notre pratique clinique en matière de traitement de l’accès mixte et de la comparer aux recommandations internationales.

Il s’agit d’une étude rétrospective portant sur les dossiers de patients atteints de troubles bipolaires, hospitalisés pour accès mixte dans notre service, au cours de la période allant de août 2009 à janvier 2012.

Le sexe ratio était de 0,52 (23 hommes/22 femmes). L’âge moyen de nos patients était de 39,4 ans. Chez la majorité des patients, le traitement de l’épisode mixte reposait sur une association médicamenteuse. les combinaisons prescrites étaient les suivantes : neuroleptique classique (halopéridol) et antiépileptique (acide valoproïque) dans 34 % des cas. Neuroleptique classique (halopéridol) et antiépileptique (carbamézapine) dans 23 % des cas. Antipsychotique atypique (olanzapine) et antiépileptique (acide valoproïque) dans 26 % des cas. Antipsychotique atypique (rispéridone) et antiépileptique (acide valoproïque) dans 7 % des cas.

Notre conduite thérapeutique est à première vue, conforme aux recommandations internationales. En effet, la prescription du lithium est déconseillée au cours des épisodes mixtes. les experts privilégient plutôt les antiépileptiques en association aux antipsychotiques atypiques.

La survenue d’épisodes mixtes est souvent associée à une faible réponse au traitement, à une plus grande récurrence des épisodes et à un risque plus élevé de suicide. La reconnaissance de ces états et le recours au traitement approprié sont nécessaires pour améliorer le pronostic de la maladie.

Clinical symptoms, functional recovery and stressful life events of patients with schizophrenia were evaluated to investigate the relationship between life events and functional recovery.

Fifty-three subjects diagnosed with schizophrenia in the remission period were enrolled in the study. Three interviews each at 3 months interval were made with the participants. the participants were evaluated with socio-demographic data form, Positive and Negative Syndrome Scale, Calgary Depression Scale, the Functional Remission of General Schizophrenia Scale at the baseline, third and sixth month interview. the number of stressful life events were determined according to Life Events List.

There was no significant change in the PANNS scores measured at the baseline and follow interview (p>0.05). Depression scores were significantly increased at the follow interviews(p< 0.05). This significant increase mostly resulted from the difference between the baseline and final interview scores. the number of stresful life events and functional recovery were found to be negatively correlated with a medium level of significance at the third month (p< 0.05). There was a negative relationship between the number of life events and functional recovery at the sixth month but it did not reach to a statistical significance (p>0.05). A significant relationship and a negative correlation was found between functional recovery and depression scores at both the baseline and third month interviews (p< 0.05). This relationship was not significant at the sixth month (p>0.05).

Stresfull life events in remitted schizophrenia patients reduces functionality over depressive symptomatology without affecting positive and negative symptoms.

Il s’agit dune femme de 30 ans, hospitalisée pour un trouble des conduites alimentaires de type mixte (avec restriction, et crises de boulimie), et un état dépressif majeur secondaire. Elle bénéficie alors d’un traitement à base de 10 mg d’escitalopram et quitte la clinique améliorée. Quatre mois après sa sortie, elle garde des moments très dépressifs, et, suspectant une réponse partielle à l’escitalopram, je décide de l’augmenter à 15 mg. Après une brève amélioration, son état devient alors plus instable, avec des fluctuations thymiques marquées (alternance de moments euphoriques et plus dépressifs), et l’apparition d’image de suicide (elle se voit se jeter par la fenêtre). Elle décrit que ces images lui sont comme étrangères, et l’envahissent, et qu’elle ne s’y reconnaît pas mais a cependant peur de passer à l’acte. Elle diminue son escitalopram à 10 mg, ce qui permet la disparition des idées suicidaires. Elle reste cependant instable, et nous décidons d’arrêter progressivement le traitement médicamenteux. Pendant plusieurs mois, elle ira alors mieux, et retrouvera une certaine stabilité thymique. Une difficulté de vie causera une rechute boulimique six mois après cet épisode. La patiente reprendra d’elle-même de l’escitalopram et présentera rapidement un état anxieux, avec agitation et insomnie. Nous arrêterons à nouveau l’antidépresseur, et l’hospitaliserons quelques jours, avec une couverture de lormétazépam, ce qui permettra une amélioration de son état.

Ce cas clinique illustre le risque d’apparition d’idées suicidaires sous antidépresseur, mêlé à d’autres éléments suggérant une intolérance psychique à ce produit (labilité thymique, insomnie, agitation), ainsi que le risque à le reprendre chez un patient ayant déjà vécu ce type d’intolérance. Il illustre également le caractère égo-dystonique que peut prendre ce type d’idées suicidaires [1].

The primary aim of this study is to examine an association between apathy and frontal lobe dysfunction in patients with memory problems. We also aimed to look into the association between apathy severity and caregiver burden along with the relationship between apathy and praxis.

This was a retrospective cross sectional study. We selected 160 consecutive patients diagnosed with Alzheimer's dementia and Amnestic Mild Cognitive Impairment who had a comprehensive battery of neuropsychological tests and a behaviour rating scale of interest for this study recorded in the database. Correlation between apathy with and without depression were tested against frontal lobe test including Trail making A, Trial making B, Letter Fluency, Ideational Fluency, Category Fluency, Abstract Thinking and Executive functioning subtest of CAMGOG-R. Similarly correlation analysis was also done to look into association between apathy and caregiver burden as well as praxis.

Statistically significant relationship were found between apathy and executive function scores, ideational fluency scores, abstract thinking and category fluency scores. Relationship between apathy and caregiver burden were highly significant. Further sub analysis, found apathy to be significantly associated with caregiver burden in both AD and aMCI patients.

Apathy is associated with frontal lobe dysfunction especially impaired executive function and ideational fluency even when controlled for depression. It is highly associated with caregiver burden both in AD & aMCI .Hence early recognition and management of apathy is important in improving the prognosis of patients with AD & aMCI as well as their caregiver.

Les patients schizophrènes présentent souvent des manifestations dépressives. La prévalence varie de 7 et 75 %. Elle est à l’origine de rechutes fréquentes et de morbidité grave. L’optimisation du traitement par la prescription d’antidépresseurs est fréquente en pratique clinique de 11 à 43 %.

Mettre en évidence la place des antidépresseurs dans la prise en charge de la dépression chez les patients schizophrènes.

Étude rétrospective incluant tous les patients schizophrènes diagnostiqués entre 2009 et 2010. L’échelle CDSS a été utilisée pour l’évaluation.

Cette étude inclut 53 schizophrènes. Des manifestations dépressives ont été décelées chez 15 % de ces patients. Le traitement antipsychotique était dans 80 % des cas un neuroleptique classique. Le traitement antidépresseur a été prescrit chez la totalité des patients diagnostiqués. La molécule prescrite est dans 75 % des cas un tricyclique (amytriptiline) et dans 25 % un IRS (paroxétine). Le traitement a été instauré au long cours en association avec le neuroleptique. L’évolution était favorable avec diminution de la sympotomatologie dépressive. Un seul cas d’éxacerbation de la symptomatologie psychotique nécessitant une hospitalisation a été noté.

La prescription d’antidépresseur chez un schizophrène qui déprime est bénéfique. Cependant, elle doit être prudente. Elle prend en compte plusieurs éléments tels que le tableau clinique et les thérapeutiques en cours.

Although there are many antipsychotics, resistant schizophrenia is still to be an extremely important problem in psychiatrists' practice.

Finding a new strategy, different from the ones in daily standardized practice, could give better perspectives for the outcome of the cases of resistant schizophrenia.

Adding memantine for about six months to the last antipsychotic used in the treatment of resistant schizophrenia could be used as a method in order to induce a response to the treatment due to modulation of NMDA receptors.

Two study cases: patients diagnosed with schizophrenia paranoid type, many years ago, with many hospitalisations for psychotic episodes admitted into the hospital for a psychotic relapse. the patients were treated with 2 antipsychotics (each for 8 weeks and adequate dose) without response. Both cases had a common approach from the moment of failure of the second antipsychotic: adding memantine 20 mg per day to the second antipsychotic, maintained with the same dose from that moment.

Adding memantine to the last antipsychotic induced the remission in both cases of resistant schizophrenia. This association was made for a period of 6 months and after that the dose of memantine was progressively reduced in 2 weeks. We kept the antipsychotic as a main treatment. the remission was kept during the period of association and one year after the withdrawal of the memantine.

We think that memantine modulation of NMDA receptors in combination with antipsychotic D2 receptor blockade can improve the response of resistant schizophrenia.

Physical health of people suffering from severe mental illness is a neglected area. They suffer more than general population and they die earlier. Looking after such disadvantaged group of people is mandatory, to improve their outcome and quality of life. Monitoring current clinical practice is essential to try to improve it.

37 Records of patients who were on long acting injectable antipsychotics, attending the Taylor centre were selected and reviewed to establish their physical care arrangement check against the POMT-UK. Type of antipsychotics, whether they suffer from metabolic syndrome or not, other physical illness was recorded.

Records lacked any reference to baseline arrangements of physical monitoring. Although they demonstrated high level of physical morbidity, care and records didn’t reflect adequate clinical care to this important problem.

This study demonstrated that Physical care of people with severe mental illness is still substandard and needs to be developed in order to prevent or identify physical problems earlier, to reduce morbidity of these patients and improve their life expectancy and quality of life.

Physical care is an important aspect of care of people who suffer from severe mental illness total care. This study has demonstrated that this area is still neglected. Improvement in this area will improve the global outcome of these disadvantaged people.

La prise en charge des troubles anxieux et de l’insomnie chez la personne âgée a fait l’objet d’une évaluation des pratiques professionnelles (EPP), réalisée prospectivement sur une semaine. Cette étude a révélé que 40 à 66 % des prescriptions chez la personne de plus de 65 ans comportaient des médicaments dits « inappropriés » au regard du rapport bénéfice/risque (liste de Laroche).

Élaborer des recommandations portant sur les moyens thérapeutiques permettant d’appréhender l’anxiété et l’insomnie chez la personne âgée dans un établissement psychiatrique.

Une recherche bibliographique associée à l’expérience de différents cliniciens du groupe EPP, ont permis l’élaboration de stratégies thérapeutiques (médicamenteuses ou non) à privilégier, compte tenu de l’arsenal thérapeutique à disposition. Une validation institutionnelle a été réalisée.

Deux arbres décisionnels synthétisent les différentes étapes de prise en charge de l’insomnie et de l’anxiété chez la personne âgée. Dans un premier temps, la correction des facteurs étiologiques et l’application des règles hygiéno-diététiques sont mises en avant. L’abord psychothérapeutique (thérapie cognitive et comportementale) est également privilégié en amont de la prescription de psychotropes. L’approche pharmacologique n’est envisagée qu’ultérieurement en utilisant les molécules à demi-vie courte, à faible dose sur une courte durée, limitant ainsi le risque d’effets indésirables. Le positionnement des molécules ou classes médicamenteuses les unes par rapport aux autres dans l’insomnie (antihistaminiques, benzodiazépines et apparentés, neuroleptiques), dans l’anxiété (antidépresseurs, benzodiazépines, buspirone, prégabaline, neuroleptiques anxiolytiques) est précisé.

Les données retrouvées dans la littérature relatives à la prise en charge de l’insomnie et l’anxiété chez la personne âgée s’adressent majoritairement à une population de gérontologie sans comorbidités psychiatriques. Ainsi, les recommandations, notamment de la Haute Autorité de santé (HAS) ne sont pas toujours applicables en l’état aux patients présents dans les services de psychogérontologie.

Femme de 63 ans, hospitalisée dans le service de psychiatrie de l’hôpital Henri-Mondor (Créteil) pour un épisode dépressif majeur d’intensité sévère sans caractéristiques psychotiques, dans le cadre d’un trouble dépressif récurrent, résistant à l’escitalopram 20 mg/j et à la venlafaxine 300 mg/j. Une cure de cinq séances d’électro-convulsivotérapie (ECT) a été conduite, avec une efficacité partielle, interrompue pour mauvaise tolérance, et suivie d’une rechute rapide. L’introduction du pramipexole (1,4 mg/j) a été suivie d’une rémission complète, rapide (en 15 jours) et durable (après deux mois de traitement), en association à la lamotrigine (100 mg/j à la fin de notre prise en charge).

Femme de 68 ans, hospitalisée dans notre service pour un épisode dépressif majeur d’intensité sévère sans caractéristiques psychotiques, dans le cadre d’un trouble bipolaire de type 3, résistant à plusieurs lignes de traitement bien conduites, dont un tricyclique (clomipramine). Au début de notre prise en charge, elle est traitée par acide valproïque et quiétiapine. Amélioration rapide (en 15 jours) sous pramipexole (1,76 mg/j), en association à la lamotrigine (50 mg/j à la fin de notre prise en charge) et l’acide valproïque (750 mg/j). Dans ces deux cas, le pramipexole a été efficace sans être associé à un antidépresseur, pour des patients ayant une indication d’ECT. Il n’a pas été constaté d’effets indésirables. La présentation clinique était centrée sur le ralentissement psychomoteur, l’anhédonie, l’apragmatisme, l’anesthésie affective. Ces dimensions symptomatiques pourraient être liées aux dysfonctions du système dopaminergique [4]. Ce tableau clinique pourrait être prédictif de l’efficacité des agonistes dopaminergiques.

The aim of this study was to compare the effectiveness of attribution retraining group therapy (ARGT) versus selective serotonin reuptake inhibitors (SSRI) in the treatment of major depressive disorder (MDD), generalized anxiety disorder (GAD) and obsessive-compulsive disorder (OCD).

109 patients with MDD, GAD and OCD were recruited from the outpatient department of a tertiary referral hospital between 2007 and 2008. Subjects were sequentially recruited and randomized into ARGT group (n=63) and SSRI group (n=66) for an 8-week treatment period. 54 outpatients in ARGT group and 55 outpatients in SSRI group completed the study. All subjects were assessed using Hamilton Depression Scale, Hamilton Anxiety Scale before and after treatment. Yale-Brown Obsessive Compulsive Scale was employed only for OCD subjects. Plasma hormone levels of serotonin, norepinephrine, cortisol, adrenocorticotropic hormone, and brain-derived neurotrophic factor (BDNF) were measured at baseline and at 8 weeks.

Symptom scores were reduced significantly in both ARGT and SSRI treatment groups (p< 0.001) at the end of the treatment course. However the patients in the ARGT group had significantly lower plasma cortisol concentrations compared to baseline (p< 0.05). on the other hand, patients receiving the SSRIs showed significantly increased plasma levels of serotonin (p< 0.05) and BDNF (p< 0.01).

Our findings suggest that ARGT may modulate plasma cortisol levels and take effect to the HPA axis as opposed to SSRIs which may up-regulate plasma serotonin and BDNF levels via a different pathway to produce an overall improvement in the clinical condition of patients.

The dual diagnosis of substance abuse and severe mental illness has been proposed by some authors to highlight the high frequency of such association. Our objectif was to determine the prevalence of dual diagnosis among mentally ill patients. as a secondary purpose, to describe the profile of patient at higher risk to develop dual diagnosis.

683 mentally ill patients were recruited from el Mamoura Psychiatric Hospital, they were assessed by the Arabic version of 6 item K6 scale, those scoring above 13 were submitted to a structured clininical interview SCID-I (Modules A-E), urine screening for drugs of abuse and complete history of substance abuse and other medical disciplines.

22% of the sample fulfill the diagnosis of dual diagnosis. Results reveals higher risk for dual diagnosis in patients younger in age, single, low socioeconomic state and low level of education. Those having dual diagnosis tend to have early onset, longer duration of illness, poor compliance, higher rate of smoking and increased number of relapses.

Double diagnosis is an important issue that needs more focus in mental health services. Awareness of mental health providers about dual diagnosis is highly recommended.
