Compromised kidney function is associated with an array of environmental contaminants and pathogens that may be considered for regulation. However, there are few valuation estimates for kidney effects for use in benefit–cost analyses, particularly willingness-to-pay estimates. This paper is one of several surveys valuing morbidity developed by the OECD Surveys to elicit Willingness-to-pay to Avoid Chemicals-related negative Health Effects project, which aims to improve the basis for benefit–cost analyses. We report the results of a stated preference survey valuing reduced the risk of symptomatic chronic kidney disease, filling an important gap in the valuation literature and addressing a need for applied benefits analysis of chemical regulation. The survey was administered to representative samples in each of 10 countries: Canada, Chile, China, Denmark, Germany, Italy, Norway, Türkiye, the United Kingdom, and the United States. The mean (median) WTP for an average reduction of 3.5 in 1,000 of the risk of serious kidney disease over 5 years is $2,609 ($764), corresponding to a mean (median) value per statistical case (VSC) of chronic kidney disease of $805,000 ($224,000). The mean VSC varies between $700,000 for Canada and $1,200,000 for Türkiye.

Asthma is a non-communicable and non-curable lung disease that affects 10% of children and 4% of adults worldwide and is associated with an array of environmental contaminants and chemicals. This article offers values suitable for use in cost–benefit analyses of the willingness to pay (WTP) for reduced severity of asthma in adults and children and in reduced probability of getting asthma for these two population groups, all in the context of reducing chemical exposures. To this end, an online survey was administered between November 2021 and May 2022 to 12 727 respondents from seven countries of the Organisation for Economic Co-operation and Development (OECD). This article applies two stated preference methods for eliciting WTP: the contingent valuation method for reduced asthma severity and choice experiments for reduced probability of getting asthma of various severities. The context for such elicitations was a set of household products that contain fewer hazardous chemicals than what is currently available in supermarkets but are more expensive. The study finds that the WTP for reducing asthma severity in adults by one step, e.g. from “moderate plus” to “moderate”, is USD2022 529 per year on average. The parental WTP for reducing asthma severity in their children is USD2022 PPP 948 per year and is on average 1.8 times higher than their WTP for themselves. The mean value of a statistical case (VSC) of adult asthma which would be applied to predictions of new cases of asthma avoided by a regulation equals USD2022 280 000, while the mean VSC of childhood asthma equals USD2022 430 000.

We quantify the importance of idiosyncratic health risk in a calibrated general equilibrium model of Social Security. We construct an overlapping generations model with rational-expectations households, idiosyncratic labor income and health risk, profit-maximizing firms, incomplete insurance markets, and a government that provides pensions and health insurance. We calibrate this model to the US economy and perform two computational experiments: $\left (i\right)$ cutting Social Security’s payroll tax, and $\left (ii\right)$ modifying Social Security’s benefit-earnings rule. Our findings suggest that health risk amplifies the welfare implications of both experiments: downsizing Social Security always leads to higher overall welfare, but the welfare gain is larger when we account for health risk, and increasing the progressivity of Social Security’s benefit-earnings rule has a larger positive effect on welfare in the presence of health risk. We also find that allowing households additional tools to self-insure against health risk weakens the precautionary motive, so our experiments have similar welfare implications both with and without health risk.

In these polarized and challenging times, not even perceptions of personal risk are immune to partisanship. This article introduces results from a new survey with an embedded social media experiment conducted during the first months of the COVID-19 pandemic in Brazil. Descriptive results show that progovernment and opposition partisans report very different expectations of health and job risks. Job and health policy have become wedge issues that elicit partisan responses. The analysis exploits random variation in the survey recruitment to show the effects of the president’s first speech on national television on the perceived risk and the moderating effect of partisanship. The article presents a framing experiment that models key cognitive mechanisms driving partisan differences in perceptions of health risks and job security during the COVID-19 crisis.

Concern has been expressed that human papillomavirus (HPV) vaccination programs might promote risky sexual behavior through mechanisms such as risk compensation, behavioral disinhibition, or perceived endorsement of sexual activity. This study assesses whether HPV vaccination status is associated with any differences in selected sexual behaviors among young sexually-active women in the US. Our dataset includes young, adult female respondents from questionnaire data collected in the National Center for Health Statistics' National Health and Nutrition Examination Survey from 2007 to 2014. The empirical approach implements a doubly robust estimation procedure, based on inverse probability of treatment weighting. For robustness, we implement several specifications for the propensity model and the outcomes model. We find no consistent association between HPV vaccination and condom usage or frequency of sex. Specifically, we find no evidence that HPV vaccination is associated with condom usage or with whether a person had sex more than 52 or more than 104 times per year. We find inconsistent evidence that HPV vaccination is associated with a person having sex more than 12 times per year. As in previous research, HPV vaccination does not appear to have a substantive effect on sexual behavior among young sexually-active women in the US.

A low resting heart rate across development from infancy to young adulthood relates to greater aggression/hostility. Adult aggression and a high heart rate relate to health risk. Do some aggressive individuals retain low heart rate and less health risk across development while others show high heart rate and more risk? A longitudinal sample of 203 men assessed as teens (age 16.1) and adults (mean age 32.0) permitted us to assess (a) stability of heart rate levels and reactivity, (b) stability of aggression/hostility, and (c) whether change or stability related to health risk. Adults were assessed with Buss–Perry measures of aggression/hostility; teens with the Zuckerman aggression/hostility measure. Mean resting heart rate, heart rate reactivity to speech preparation, and aggression/hostility were moderately stable across development. Within age periods, mean heart rate level, but not reactivity, was negatively related to hostility/aggression. Maintaining low heart rate into adulthood was related to better health among aggressive individuals relative to those with increasing heart rate into adulthood. Analyses controlled for weight gain, socioeconomic status, race, health habits, and medication. Low heart rate as a characteristic of hostile/aggressive individuals may continue to relate to better health indices in adulthood, despite possible reversal of this relationship with aging.

As the largest industrial and population centre in China, the Pearl River Delta is facing a growing threat of heavy metal pollution from local mining and power industries. This study investigates the distribution and potential health risks of copper (Cu), cadmium (Cd), lead (Pb) and zinc (Zn) in paddy soils and rice at four typical sites. The Nemerow synthetic pollution index (PN) of soils from Fogang, Dabao Mountain, Shaoguan and Lechang were 8.40, 9.10, 4.64 and 10.28, respectively, indicating serious pollution at all four sampling sites. The average concentrations of Cu, Cd, Pb and Zn in rice grains were 2.23, 10.98, 29.84 and 1.62 times their corresponding maximum allowable levels, indicating potential health risks to humans. Cd has greater bioavailability because of its high mobility from soil to roots, and its subsequent transfer to grains. Pb mainly accumulates in roots because of its lower translocation rate from roots to grains. The greatest health risk index for Cd and Pb for adults and children was at the Shaoguan site, probably due to pollution from atmospheric deposition. Cd and Pb had greater health risk indices than Cu and Zn at almost all sites, indicating a major health risk to local people.

In 1994, the first World Conference on Natural Disaster Reduction held in Yokohama, Japan affirmed that “Disaster prevention, mitigation, and preparedness are better than disaster response in achieving the goals and objectives of the decade. Disaster response alone is not sufficient, as it yields only temporary results in a very high cost.” Since then, disaster risk reduction has become the mainstay for international development related to disasters.

According to the National Research Council (Washington, DC USA), “Disaster research, which has focused historically on emergency response and recovery, is incomplete without the simultaneous study of the societal hazards and risks associated with disasters, which includes data on the vulnerability of people living in hazard prone areas.” Despite over 25 years of global policy development, the National Academies of Sciences, Engineering, and Medicine (Washington, DC USA) recently noted that, “while some disaster management and public health preparedness programming may be viewed as tangentially related, a multi-sectoral and inter-disciplinary national platform for coordination and policy guidance on involving disaster risk reduction in the United States does not exist.” Today, one of the world’s “seven targets in seven years” as agreed upon in the Sendai Framework for Disaster Risk Reduction is to substantially reduce global mortality by 2030. Significant reductions in health risk (including mortality) have historically required a comprehensive approach for disease management that includes both a preventive and a curative approach. Disaster risk management has arisen as a primary means for the world’s populations to address disaster losses, including those related to health. Prevention has been proven as an effective approach for managing health risk. This report describes the role of disease prevention in managing health risk due to disasters.

Effective disaster risk management requires not only management of the immediate problem (disaster-related injuries and disease), but also of the patient’s risk factors and of the underlying health determinants. This requires an accurate and well-validated process for assessment of the determinants of disease.

Ideally, disaster risk management is based on a prioritization process. Once hazards have been identified, they are assessed in terms of the probability and impact in terms of losses. The hazards associated with the greatest probability and impact loss are prioritized. In addition to prioritization, risk assessment also offers a process for ongoing research involving the interaction of health determinants, risk, and protective factors that may contribute to future adverse health outcomes.

Recently, assessments of health risk have become an integral part of local, state, and national emergency preparedness programs. One of the strengths of these assessments is the convening of multi-sectoral input for public health decision making and plans. However, this diversity of input also creates challenges in development of a common nomenclature for assessing and communicating the characteristics of this risk. Definitions remain ambiguous for many of the key indicators of disaster risk, especially those applied to health risk.

This report is intended as a primer for defining disaster-related health risk. This framework is discussed within a nomenclature that is consistent with international standards for risk management and public health prevention.
