Provision of critical care and resuscitation was not practical during early missions into space. Given likely advancements in commercial spaceflight and increased human presence in low Earth orbit (LEO) in the coming decades, development of these capabilities should be considered as the likelihood of emergent medical evacuation increases.
PubMed, Web of Science, Google Scholar, National Aeronautics and Space Administration (NASA) Technical Server, and Defense Technical Information Center were searched from inception to December 2018. Articles specifically addressing critical care and resuscitation during emergency medical evacuation from LEO were selected. Evidence was graded using Oxford Centre for Evidence-Based Medicine guidelines.
The search resulted in 109 articles included in the review with a total of 2,177 subjects. There were two Level I systematic reviews, 33 Level II prospective studies with 647 subjects, seven Level III retrospective studies with 1,455 subjects, and two Level IV case series with four subjects. There were two Level V case reports and 63 pertinent review articles.
The development of a medical evacuation capability is an important consideration for future missions. This review revealed potential hurdles in the design of a dedicated LEO evacuation spacecraft. The ability to provide critical care and resuscitation during transport is likely to be limited by mass, volume, cost, and re-entry forces. Stabilization and treatment of the patient should be performed prior to departure, if possible, and emphasis should be on a rapid and safe return to Earth for definitive care.
Apolipoprotein E (APOE) E4 is the main genetic risk factor for Alzheimer’s disease (AD). Due to the consistent association, there is interest as to whether E4 influences the risk of other neurodegenerative diseases. Further, there is a constant search for other genetic biomarkers contributing to these phenotypes, such as microtubule-associated protein tau (MAPT) haplotypes. Here, participants from the Ontario Neurodegenerative Disease Research Initiative were genotyped to investigate whether the APOE E4 allele or MAPT H1 haplotype are associated with five neurodegenerative diseases: (1) AD and mild cognitive impairment (MCI), (2) amyotrophic lateral sclerosis, (3) frontotemporal dementia (FTD), (4) Parkinson’s disease, and (5) vascular cognitive impairment.
Genotypes were defined for their respective APOE allele and MAPT haplotype calls for each participant, and logistic regression analyses were performed to identify the associations with the presentations of neurodegenerative diseases.
Our work confirmed the association of the E4 allele with a dose-dependent increased presentation of AD, and an association between the E4 allele alone and MCI; however, the other four diseases were not associated with E4. Further, the APOE E2 allele was associated with decreased presentation of both AD and MCI. No associations were identified between MAPT haplotype and the neurodegenerative disease cohorts; but following subtyping of the FTD cohort, the H1 haplotype was significantly associated with progressive supranuclear palsy.
This is the first study to concurrently analyze the association of APOE isoforms and MAPT haplotypes with five neurodegenerative diseases using consistent enrollment criteria and broad phenotypic analysis.
This paper examines individual differences in constraints on linguistic variation in light of Labov's (2007) proposal that adult change (diffusion) disrupts systems of constraints and Tamminga, MacKenzie, and Embick's (2016) typology of constraints. It is shown that, in pooling data from multiple speakers, some of the complexity in structured community variation may be overlooked. Data on rhoticity from speakers of Bristol English are compared to 34 previous studies of rhoticity in varieties of English around the world. Constraints found to be consistent across varieties are also found to be consistent across speakers of Bristol English, whereas those that differ between varieties also differ between individuals, implying that only those which differ are truly part of the grammar, and that these are indeed disrupted by diffusion.
Increased use of dicamba and/or glyphosate in dicamba/glyphosate-tolerant soybean might affect many sensitive crops, including potato. The objective of this study was to determine the growth and yield of ‘Russet Burbank’ potato grown from seed tubers (generation 2) from mother plants (generation 1) treated with dicamba (4, 20, and 99 g ae ha−1), glyphosate (8, 40, and 197 g ae ha−1), or a combination of dicamba and glyphosate during tuber initiation. Generation 2 tubers were planted near Oakes and Inkster, ND, in 2016 and 2017, at the same research farm where the generation 1 tubers were grown the previous year. Treatment with 99 g ha−1 dicamba, 197 g ha−1 glyphosate, or 99 g ha−1 dicamba + 197 g ha−1 glyphosate caused emergence of generation 2 plants to be reduced by up to 84%, 86%, and 87%, respectively, at 5 wk after planting. Total tuber yield of generation 2 was reduced up to 67%, 55%, and 68% when 99 g ha−1 dicamba, 197 g ha−1 glyphosate, or 99 g ha−1 dicamba + 197 g ha−1 glyphosate was applied to generation 1 plants, respectively. At each site year, 197 g ha−1 glyphosate reduced total yield and marketable yield, while 99 g ha−1 dicamba reduced total yield and marketable yield in some site-years. This study confirms that exposure to glyphosate and dicamba of potato grown for potato seed tubers can negatively affect the growth and yield potential of the subsequently grown daughter generation.
Infection preventionists at Minnesota hospitals were surveyed to determine whether they had Legionella water management plans. Of 137 hospitals, 84 (61%) responded. Among them, 27% hospitals had a water management plan, 21% regularly sampled for Legionella, and 51% had knowledge of ASHRAE Legionella prevention standards. Significant changes are needed to protect patients from nosocomial infection.
The principal aim of the establishment and free exercise clauses of the First Amendment was to preclude congressional imposition of a national church. A balance was sought between states' rights and the rights of individuals to exercise their religious conscience. While the founding fathers were debating such issues, the potential for serious conflict was confined chiefly to variations among the dominant Christian sects. Today, issues of marriage, child bearing, cultural diversity, and corporate personhood, among others, suffuse constitutional jurisprudence, raising difficult questions regarding the nature of beliefs that qualify as 'religious', and the reach of law into the realm in which those beliefs are held. The essays collected in this volume explore in a selective and instructive way the intellectual and philosophical roots of religious liberty and contemporary confrontations between this liberty and the authority of secular law.
Stonehenge is a site that continues to yield surprises. Excavation in 2009 added a new and unexpected feature: a smaller, dismantled stone circle on the banks of the River Avon, connected to Stonehenge itself by the Avenue. This new structure has been labelled ‘Bluestonehenge’ from the evidence that it once held a circle of bluestones that were later removed to Stonehenge. Investigation of the Avenue closer to Stonehenge revealed deep periglacial fissures within it. Their alignment on Stonehenge's solstitial axis (midwinter sunset–midsummer sunrise) raises questions about the early origins of this ritual landscape.
