Single Dose versus Fractionated Stereotactic Radiotherapy for Meningiomas

Objective:
To evaluate the safety and efficacy of stereotactic radiosurgery (SRS) compared to fractionated stereotactic radiation therapy (FSRT) for meningiomas treated over a seven year period.
Methods and materials:Of the 53 patients (15 male and 38 female) with 63 meningiomas, 35 were treated with SRS and the 18 patients with tumors adjacent to critical structures or with large tumors were treated with FSRT. The median doses for the SRS and the FSRT groups were 1400 cGy (500- 4500 cGy) and 5400 cGy (4000-6000 cGy) respectively. Median target volumes for SRS and FSRT were 6.8 ml and 8.8 ml respectively. The median follow-up for the SRS and FSRT groups were 38 months (4.1-97 months) and 30.5 months (6.0-63 months) respectively.
Results:The five-year tumor control probability (TC) for benign versus atypical meningiomas were 92.7% vs. 31% (P=.006). The three-year TC were 92.7% vs. 93.3% for SRS vs. FSRT groups respectively (P=.62). For benign meningiomas, the three-year TC were 92.9% vs. 92.3% for the SRS group (29 patients) vs. FSRT group (14 patients) respectively (P=.77). Two patients in the SRS group and one in the FSRT group developed late complications.
Conclusion:Preliminary data suggest that SRS is a safe and effective treatment for patients with benign meningiomas. Fractionated stereotactic radiation therapy with conventional fractionation appeared to be an effective and safe treatment alternative for patients not appropriate for SRS. A longer follow-up is required to determine the long-term efficacy and the toxicity of these treatment modalities.

Betel-quid dependence and oral potentially malignant disorders in six Asian countries

Background
Despite gradual understanding of the multidimensional health consequences of betel-quid chewing, information on the effects of dependent use is scant.
AimsTo investigate the 12-month prevalence patterns of betel-quid dependence in six Asian populations and the impact of this dependence on oral potentially malignant disorders (OPMD).
MethodA multistage random sample of 8922 participants was recruited from Taiwan, mainland China, Indonesia, Malaysia, Sri Lanka and Nepal. Participants were evaluated for betel-quid dependency using DSM-IV and ICD-10 criteria and assessed clinically for oral mucosal lesions.
ResultsThe 12-month prevalence of dependence was 2.8-39.2% across the six Asian samples, and 20.9-99.6% of those who chewed betel-quid were betel-quid dependent. Men dominated the prevalence among the east Asian samples and women dominated the prevalence in south-east Asian samples. ‘Time spent chewing’ and ‘craving’ were the central dependence domains endorsed by the Chinese and southern/south-east Asian samples respectively, whereas the Nepalese samples endorsed ‘tolerance’ and ‘withdrawal’. Dependency was linked to age, gender, schooling years, drinking, smoking, tobacco-added betel-quid use and environmental accessibility of betel-quid. Compared with non-users, those with betel-quid dependency had higher pre-neoplastic risks (adjusted odds ratios 8.0-51.3) than people with non-dependent betel-quid use (adjusted odds ratio 4.5-5.9) in the six Asian populations.
ConclusionsBy elucidating differences in domain-level symptoms of betel-quid dependency and individual and environmental factors, this study draws attention to the population-level psychiatric problems of betel-quid chewing that undermine health consequences for OPMD in six Asian communities.

We present the results and analysis of HST-wfpc2 imagery of the starburst galaxy NGC 1569 (d = 2.2 Mpc), which permit unprecedented resolution (~ 1pc) of the ionized gas and stellar population. The primary data in our analysis consists of images taken through narrow-band filters isolating Hβ, Hα, [O iii]λ5007, and [S ii]λλ6717+30, as part of go Program 8133. A variety of color-coded and grey-scaled maps of the morphology, ionization structure, and dust distribution are presented. Unsharp masks of the Hα images show a very chaotic structure for the ionized gas, with numerous filaments and arc-like bright rim features across the entire galaxy, but without significant large-scale ionization variations. Variations in the ionization and line-of-sight reddening occur on smaller scales (~ 10 – 50 pc), with numerous low-ionization semi-stellar knots seen throughout the main body of the galaxy, even within the ‘hole’ in the ionized gas distribution around the central super star clusters previously noted from H i maps. Several of these features have strong [S ii] emission indicative of being supernova remnants. We combine these data with archival wide-band HST-wfpc2 imagery to quantitatively evaluate the source(s) of the (largely photo-) ionized gas.
HST-STIS spectroscopy of the Bubble Nebula, NGC 7635

We report the results of long-slit spectroscopy of the wind-blown bubble and photo-evaporating knots around the O6.5IIIf star BD+60°2522 made with the HST-Space Telescope Imaging Spectrograph. The Of star is the primary ionizing source for the H II region NGC 7635, located in the Perseus Arm. The spectra were taken through a 0″.2 x 52″ slit with low and medium resolution gratings covering the wavelength range 2900-6870 Å. Observations with two slit orientations were made; one across the line of embedded knots to the west of the Of star and the second running from the Of star across the bubble to the NE.
The 2D-STIS spectra permit us to subtract the surrounding H II region's diffuse emission from that of the knots and the bubble, and to study the spatial variations in various emission lines in these features to a resolution of ∼ 0″.2, an order of magnitude improvement over the best ground-based spectra of this object in the literature. We present high spatial-resolution emission-line and line-ratio profiles across the bubble and knots, and compare them with the predicted variations from photo-ionization, photo-evaporation, and wind-shock models. We also present an analysis of temperatures, densities, and abundances in the features from higher S/N spectra extracted over selected lengths of the slit.
