Design and implementation of a digital site-less clinical study of serial rapid antigen testing to identify asymptomatic SARS-CoV-2 infection

Background:
Rapid antigen detection tests (Ag-RDT) for SARS-CoV-2 with emergency use authorization generally include a condition of authorization to evaluate the test’s performance in asymptomatic individuals when used serially. We aim to describe a novel study design that was used to generate regulatory-quality data to evaluate the serial use of Ag-RDT in detecting SARS-CoV-2 virus among asymptomatic individuals.
Methods:This prospective cohort study used a siteless, digital approach to assess longitudinal performance of Ag-RDT. Individuals over 2 years old from across the USA with no reported COVID-19 symptoms in the 14 days prior to study enrollment were eligible to enroll in this study. Participants throughout the mainland USA were enrolled through a digital platform between October 18, 2021 and February 15, 2022. Participants were asked to test using Ag-RDT and molecular comparators every 48 hours for 15 days. Enrollment demographics, geographic distribution, and SARS-CoV-2 infection rates are reported.
Key Results:A total of 7361 participants enrolled in the study, and 492 participants tested positive for SARS-CoV-2, including 154 who were asymptomatic and tested negative to start the study. This exceeded the initial enrollment goals of 60 positive participants. We enrolled participants from 44 US states, and geographic distribution of participants shifted in accordance with the changing COVID-19 prevalence nationwide.
Conclusions:The digital site-less approach employed in the “Test Us At Home” study enabled rapid, efficient, and rigorous evaluation of rapid diagnostics for COVID-19 and can be adapted across research disciplines to optimize study enrollment and accessibility.

Are Religious People More Compassionate and Does This Matter Politically?

Analyzing a unique module of the General Social Survey, we test hypotheses that three religion dimensions — affiliation with specific religious traditions (belonging), service attendance (behaving), and religious orthodoxy (believing) are associated with compassionate feelings, and that these feelings carry over into support for government efforts to help the poor, blacks, and the sick. The religiously orthodox report more compassionate feelings toward others than do modernists and, partly because of this, are more supportive of government intervention to help the poor. Yet attending religious services frequently does not increase compassionate feelings and makes people less supportive of government efforts to help the poor. There are no differences among religious traditions in compassionate feelings, and the only difference on economic policy preferences is for Black Protestants to support government assistance to blacks. Compassionate feelings have comparable effects to political ideology and party identification on support for government assistance to the disadvantaged and misfortunate. We conclude that people of faith, variously defined, do not constitute a monolithic “Religious Right” and are potentially open to policy appeals from both political parties.

Granular cell tumour (GCT) is a benign tumour with abundant eosinophilic cytoplasm filled with granules of varying sizes. These granules are the defining characteristic of the GCT and are believed to represent lysosomes in varying stages of fragmentation. The commonly used term granular cell myoblastoma, found in the older literature, is a misnomer because the tumour is clearly not of muscle origin. Among the major theories of origin, some support the tumour's derivation from neuronal tissue, histiocytes, fibroblast or Schwann cells. In the larynx, pseudoepitheliomatous hyperplasia may predispose to confuse the GCT with squamous cell carcinoma.
The most common region of GCT is in the head and neck, accounting for approximately 30 to 50 per cent of all lesions. The larynx is relatively an uncommon location for these tumours, accounting for approximately three to 10 per cent of the reported cases.
Affected patients typically present with persistent hoarseness, stridor, haemoptysis, dysphagia, and otalgia, but the tumour may be asymptomatic and be discovered only incidentally during a routine examination.
Complete excision with an attempt to maintain normal stuctures generally results in cure.
We present the case of a patient with typical features of a GCT of the larynx. The gross appearance, histopathology and brief discussion of the current literature are also presented.
Respiratory Illness in Conference Participants Following Exposure to Rug Shampoo

Persons participating in a conference in a major city motel experienced transient mild respiratory illness associated with their presence in the motel conference rooms. The illness was characterized by coughing, sneezing, sore throat, headache, eye irritation, and other symptoms of exposure to a respiratory irritant. Investigation incriminated a chemical shampoo used to clean the conference room rugs approximately one week earlier. Repeated cleaning of the rugs to remove excess cleaning compound eliminated the problem. Excessive application of the shampoo, coupled with a poorly ventilated environment, apparently produced a chemical concentration sufficient to cause clinical illness.
