A peer support dietary change intervention for encouraging adoption and maintenance of the Mediterranean diet in a non-Mediterranean population (TEAM-MED): lessons learned and suggested improvements

Peer support interventions for dietary change may offer cost-effective alternatives to interventions led by health professionals. This process evaluation of a trial to encourage the adoption and maintenance of a Mediterranean diet in a Northern European population at high CVD risk (TEAM-MED) aimed to investigate the feasibility of implementing a group-based peer support intervention for dietary change, positive elements of the intervention and aspects that could be improved. Data on training and support for the peer supporters; intervention fidelity and acceptability; acceptability of data collection processes for the trial and reasons for withdrawal from the trial were considered. Data were collected from observations, questionnaires and interviews, with both peer supporters and trial participants. Peer supporters were recruited and trained to result in successful implementation of the intervention; all intended sessions were run, with the majority of elements included. Peer supporters were complimentary of the training, and positive comments from participants centred around the peer supporters, the intervention materials and the supportive nature of the group sessions. Attendance at the group sessions, however, waned over the intervention, with suggested effects on intervention engagement, enthusiasm and group cohesion. Reduced attendance was reportedly a result of meeting (in)frequency and organisational concerns, but increased social activities and group-based activities may also increase engagement, group cohesion and attendance. The peer support intervention was successfully implemented and tested, but improvements can be suggested and may enhance the successful nature of these types of interventions. Some consideration of personal preferences may also improve outcomes.

Summary
Introduction
Attitudinal shifts fostered by technological advancements pose the greatest challenges to the disposition of records and information today. Some believe, and advocate, that we should keep everything because storage is cheap and we never know when we might need it. Google encourages us with statements like this: ‘Never delete anything, always use data – it's what Google does’ (Kershaw, quoted in Hardy, 2015). However, others believe that ‘even in this world of “never delete anything”, secure, compliant information disposition has its place!’ (Franks, 2017).
If you are a supporter of this second premise, you understand that records cannot be disposed of unless the reasons for creating them in the first place are understood. Disposition can be defined as ‘The destruction or transfer of records as specified by a retention schedule’ (Pearce-Moses, 2018, s.v. disposition).
Retention is the responsibility of the organization to maintain ‘records for a period of time until their authorized disposition by destruction or transfer’ (Pearce-Moses, 2018, s.v. retention). The retention schedule provides a comprehensive list of each group of related records treated as a unit for retention purposes (records series) and the length of time each is to be maintained – regardless of their format (e.g. electronic and physical) and location (e.g. on premises and in the cloud). This holds true, as well, whether the records and information are in the physical custody of the organization or a third party.
In this chapter, we call upon the findings of four studies that explore the principles of records retention and disposition in a cloud environment and identify ten best practices based on those principles.
The first study, Policies for Government Records Produced by a Community Cloud (Barnard, 2018), addressed transparency, access to information and responsibility for records management in the cloud, including the topics of appraisal, retention schedules and disposition. The second study, Preserving Records and Managing their Life-cycle in a Multi-provenance Digital Government Environment – a case study on a government electronic system: SIGEPE (Lacombe Rocha et al., 2017), considered retention and disposition in the context of controlling records in the physical custody of a cloud service provider.

The impact of friends' deviant behavior on early onset of delinquency: Longitudinal data from 6 to 13 years of age

Sociological theories suggest that association with deviant friends is a necessary step on the path to early onset of delinquent behavior, while personality theories imply that deviant behavior will be stable from childhood to adulthood. These two rival hypotheses were tested with boys (N = 758) recruited from 53 schools in low socioeconomic areas of Montreal. Disruptive behavior in kindergarten was assessed by teachers; aggression and likability at ages 10, 11, and 12 years were rated by classroom peers; delinquent behaviors at ages 11, 12, and 13 years were reported by subjects. Best friend dyads were created by having boys independently identify each other as one of four best friends. Three independent samples were created to replicate findings at different ages (10–11 years, 11–12 years, and 12–13 years). Results of LISREL analyses from the three samples indicated that the main path toward early onset of both overt and covert delinquency was from kindergarten disruptive behavior to aggression between ages 10 to 12 years, and to delinquency from ages 11 to 13 years. Best friends' behavioral characteristics were associated with the subjects' own behavioral characteristics between ages of 10 and 12 years, but did not explain the level of self-reported delinquency the following year, when the subjects' own behavioral characteristics had been taken into account. Because friends tend to share the same behavioral characteristics, they are more likely to foster continuity in behavior than change. We suggest that the influence of significant peers other than best friends be investigated and that a categorical approach be used to try to identify subgroups of boys who may be highly responsive to peers' influence.

Eradication of a Large Outbreak of a Single Strain of vanB Vancomycin-Resistant Enterococcus faecium at a Major Australian Teaching Hospital

Objective:
To demonstrate that nosocomial transmission of vancomycin-resistant enterococci (VRE) can be terminated and endemicity prevented despite widespread dissemination of an epidemic strain in a large tertiary-care referral hospital.
Interventions:Two months after the index case was detected in the intensive care unit, 68 patients became either infected or colonized with an epidemic strain of vanB vancomycin-resistant Enterococcus faecium despite standard infection control procedures. The following additional interventions were then introduced to control the outbreak: (1) formation of a VRE executive group; (2) rapid laboratory identification (30 to 48 hours) using culture and polymerase chain reaction detection of vanA and vanB resistance genes; (3) mass screening of all hospitalized patients with isolation of carriers and cohorting of contacts; (4) environmental screening and increased cleaning; (5) electronic flagging of medical records of contacts; and (6) antibiotic restrictions (third-generation cephalosporins and vancomycin).
Results:A total of 19,658 patient and 24,396 environmental swabs were processed between July and December 2001. One hundred sixty-nine patients in 23 wards were colonized with a single strain of vanB vancomycin-resistant E. faecium. Introducing additional control measures rapidly brought the outbreak under control. Hospital-wide screening found 39 previously unidentified colonized patients, with only 7 more nonsegregat-ed patients being detected in the next 2 months. The outbreak was terminated within 3 months at a cost of $2.7 million (Australian dollars).
Conclusion:Despite widespread dissemination of VRE in a large acute care facility, eradication was achievable by a well-resourced, coordinated, multifaceted approach and was in accordance with good clinical governance.
