Strategies used for the COVID-OUT decentralized trial of outpatient treatment of SARS-CoV-2

The COVID-19 pandemic accelerated the development of decentralized clinical trials (DCT). DCT’s are an important and pragmatic method for assessing health outcomes yet comprise only a minority of clinical trials, and few published methodologies exist. In this report, we detail the operational components of COVID-OUT, a decentralized, multicenter, quadruple-blinded, randomized trial that rapidly delivered study drugs nation-wide. The trial examined three medications (metformin, ivermectin, and fluvoxamine) as outpatient treatment of SARS-CoV-2 for their effectiveness in preventing severe or long COVID-19. Decentralized strategies included HIPAA-compliant electronic screening and consenting, prepacking investigational product to accelerate delivery after randomization, and remotely confirming participant-reported outcomes. Of the 1417 individuals with the intention-to-treat sample, the remote nature of the study caused an additional 94 participants to not take any doses of study drug. Therefore, 1323 participants were in the modified intention-to-treat sample, which was the a priori primary study sample. Only 1.4% of participants were lost to follow-up. Decentralized strategies facilitated the successful completion of the COVID-OUT trial without any in-person contact by expediting intervention delivery, expanding trial access geographically, limiting contagion exposure, and making it easy for participants to complete follow-up visits. Remotely completed consent and follow-up facilitated enrollment.
Optimizing reflex urine cultures: Using a population-specific approach to diagnostic stewardship

Background:
Clinicians and laboratories routinely use urinalysis (UA) parameters to determine whether antimicrobial treatment and/or urine cultures are needed. Yet the performance of individual UA parameters and common thresholds for action are not well defined and may vary across different patient populations.
Methods:In this retrospective cohort study, we included all encounters with UAs ordered 24 hours prior to a urine culture between 2015 and 2020 at 3 North Carolina hospitals. We evaluated the performance of relevant UA parameters as potential outcome predictors, including sensitivity, specificity, negative predictive value (NPV), and positive predictive value (PPV). We also combined 18 different UA criteria and used receiver operating curves to identify the 5 best-performing models for predicting significant bacteriuria (≥100,000 colony-forming units of bacteria/mL).
Results:In 221,933 encounters during the 6-year study period, no single UA parameter had both high sensitivity and high specificity in predicting bacteriuria. Absence of leukocyte esterase and pyuria had a high NPV for significant bacteriuria. Combined UA parameters did not perform better than pyuria alone with regard to NPV. The high NPV ≥0.90 of pyuria was maintained among most patient subgroups except females aged ≥65 years and patients with indwelling catheters.
Conclusion:When used as a part of a diagnostic workup, UA parameters should be leveraged for their NPV instead of sensitivity. Because many laboratories and hospitals use reflex urine culture algorithms, their workflow should include clinical decision support and or education to target symptomatic patients and focus on populations where absence of pyuria has high NPV.

We study the dynamics of turbulent boundary layer flow over a heterogeneous topography composed of roughness patches exhibiting relatively high and low correlation in the streamwise and spanwise directions, respectively (i.e. the roughness appears as streamwise-aligned ‘strips’). It has been reported that such roughness induces a spanwise-wall normal mean secondary flow in the form of mean streamwise vorticity associated with counter-rotating boundary-layer-scale circulations. Here, we demonstrate that this mean secondary flow is Prandtl’s secondary flow of the second kind, both driven and sustained by spatial gradients in the Reynolds-stress components, which cause a subsequent imbalance between production and dissipation of turbulent kinetic energy that necessitates secondary advective velocities. In reaching this conclusion, we study (i) secondary circulations due to spatial gradients of turbulent kinetic energy, and (ii) the production budgets of mean streamwise vorticity by gradients of the Reynolds stresses. We attribute the secondary flow phenomena to extreme peaks of surface stress on the relatively high-roughness regions and associated elevated turbulence production in the fluid immediately above. An optimized state is attained by entrainment of fluid exhibiting the lowest turbulent stresses – from above – and subsequent lateral ejection in order to preserve conservation of mass.

Summary
As on Earth, other solid-surfaced planetary bodies in the solar system display landforms produced by tectonic activity, such as faults, folds, and fractures. These features are resolved in spacecraft observations directly or with techniques that extract topographic information from a diverse suite of data types, including radar backscatter and altimetry, visible and near-infrared images, and laser altimetry. Each dataset and technique has its strengths and limitations that govern how to optimally utilize and properly interpret the data and what sizes and aspects of features can be recognized. The ability to identify, discriminate, and map tectonic features also depends on the uniqueness of their form, on the morphologic complexity of the terrain in which the structures occur, and on obscuration of the features by erosion and burial processes. Geologic mapping of tectonic structures is valuable for interpretation of the surface strains and of the geologic histories associated with their formation, leading to possible clues about: (1) the types or sources of stress related to their formation, (2) the mechanical properties of the materials in which they formed, and (3) the evolution of the body's surface and interior where timing relationships can be determined. Formal mapping of tectonic structures has been performed and/or is in progress for Earth's Moon, the planets Mars, Mercury, and Venus, and the satellites of Jupiter (Callisto, Ganymede, Europa, and Io).

The National Science Foundation's National Science Digital Library (NSDL) Program is a premier collective portal of authoritative scientific resources supporting education and research. With funding from NSF, the Materials Digital Library (MatDL) is a collaborative project being developed by the National Institute of Standards and Technology's Materials Science and Engineering Laboratory (NIST/MSEL), the Department of Materials Science and Engineering at the Massachusetts Institute of Technology (MIT), the Department of Chemical Engineering and the Department of Materials Science and Engineering at the University of Michigan (U-M), with Kent State University and University of Colorado at Boulder providing the materials science informatics and workflow technology backbone. As part of the NSDL program, MatDL aims to supports the interface of materials science information and its cognate disciplines, with an emphasis on soft matter. Initial content of MatDL begins with resources selected from NIST/MSEL. Students and faculty in three types of materials science and engineering (MSE) courses at MIT and U-M are taking part in a pilot to use and contribute to MatDL utilizing domain-specific authoring tools. Given the central and interdisciplinary role of materials science in science and engineering, two goals of MatDL are to: 1.) expand its founding partnership with additional participants from the MSE community; and 2.) facilitate the flow of digital materials related knowledge from laboratories where the most recent research discoveries are taking place to the classrooms where new scientists are being trained.

Patients with apraxia are more impaired when performing transitive pantomimes than intransitive gestures. This dissociation might be related to the differences in movement complexity. Alternatively, the programs for intransitive gestures might be better defined, more widely distributed, or easier to activate than are those for transitive pantomimes. The purpose of this study was to test the complexity versus representational hypotheses. Twenty right-handed normal subjects both performed and discriminated correct from incorrect transitive pantomimes and intransitive gestures. The discrimination was performed by having subjects point at illustrations of hand postures. The subjects performed better when discriminating postures than when performing gestures or pantomimes. On both the production and discrimination tests, subjects performed better with intransitive gestures than transitive pantomimes. Although the finding that even normal subjects had more difficulty performing transitive pantomimes than intransitive gestures might appear to support the complexity hypothesis, that subjects also had more difficulty discriminating transitive than intransitive postures supports the representational activation hypothesis.
