Summary

Ms Morrissey was diagnosed with terminal cervical cancer in 2018. In 2009 and 2012 she had received two cervical smear results reported as normal, however, following an audit it was disclosed that both results were incorrect. It was claimed that if the 2009 and 2012 smears had been accurately reported, then the pre-cancerous condition would have been diagnosed and she would have been successfully treated.
