Des études ont montré un usage non optimal des antidépresseurs en France [1,2]. Une cohorte d’adultes ayant débuté un traitement antidépresseur a été mise en place pour suivre leur devenir.

La cohorte inclut les adultes du régime général d’assurance maladie, ayant eu ≥ 1 délivrance d’antidépresseurs en 2011. Ont été exclus les patients « prévalents » (≥ 1 délivrance en 2009/2010 d’antidépresseur/antipsychotique/thymorégulateur/médicament de la dépendance/stimulant ; hospitalisés avec motif psychiatrique (F04–F99) en MCO/RIMP/SSR (2006–2010) ; en ALD, arrêt de travail ≥ 6 mois, invalidité, pour motif psychiatrique).

Près de 950 000 adultes (2,5 %) ont débuté un traitement antidépresseur en 2011 (âge moyen 50 ans, 2/3 de femmes). Un médecin généraliste était premier prescripteur pour 90 %. Le délai entre le début de traitement et la consultation suivante (généraliste/psychiatre) était de 23 jours pour les patients avec ≥ 3 délivrances. Dans l’année suivant l’initiation du traitement, 12 % des patients avaient eu une consultation en psychiatrie libérale. Les molécules les plus fréquemment prescrites étaient escitalopram (33 %), paroxetine (15 %), amitriptyline (11 %) fluoxetine (7 %), venlafaxine (7 %). La médiane de traitement était de 28 jours (< 6 mois : 83 %), 40 % n’avaient eu qu’une délivrance sur l’année, 13 % deux, 47 % ≥ trois. La part des personnes socialement défavorisées était un peu plus élevée parmi celles n’ayant eu qu’une ou deux délivrances. Le taux d’instauration variait du simple au double entre départements. Plus ce taux était élevé, plus l’arrêt de traitement était fréquent après un mois ou deux (r = −0,34, p = 0,0005).

Un traitement antidépresseur est débuté annuellement chez 2,5 % des adultes mais interrompu avant 6 mois dans 4 cas sur 5, et suivi d’une consultation dans les 15 jours dans moins du tiers des cas. Les disparités sociales et géographiques de recours et suivi du traitement sont importantes. Les facteurs de chronicité (traitement au long cours, rechutes, hospitalisations, invalidité…) et la qualité du suivi seront analysés.

La non-adhésion diminue largement l’efficacité des antidépresseurs [1,2], dont la représentation évolue dans un contexte particulièrement défiant et médiatiquement tendu.

L’objectif de cette étude est de :

– évaluer l’adhésion aux antidépresseurs chez des patients hospitalisés pour épisode dépressif majeur ;

– explorer les représentations que les patients ont des antidépresseurs et de la dépression, ainsi que la perception de la stigmatisation aux troubles mentaux ;

– analyser la relation entre les attitudes face aux antidépresseurs et des paramètres sociodémographiques et cliniques.

L’adhérence était évaluée chez 40 patients en utilisant la version courte du Drug Attitude Inventory (DAI-10), complétée par un questionnaire mesurant les connaissances, craintes, impact des média et stigmatisation liés aux antidépresseurs. Des entretiens d’investigation étaient ensuite menés à l’aide de celui-ci.

L’âge moyen de l’échantillon est de 43 ans, dont 27 % d’homme. Il s’agit d’un premier épisode pour 40 % des patients. La médiane du DAI est de 3,5 (échelle de −10 à +10), et 30 % des patients ne sont pas adhérents. Les hommes de l’échantillon ont une plus mauvaise représentation des antidépresseurs (−2 VS 4 ; U de Mann-Whitney = 90,50 ; p = 0,0035). Soixante-dix pour cent des patients ont des craintes par rapport à leur antidépresseur (prise de poids et dépendance au premier rang). Vingt pour cent des patients n’ont pas dit à leur entourage qu’ils prenaient des médicaments contre la dépression.

Une intervention à de multiples niveaux pourrait augmenter les connaissances des patients ainsi que de l’opinion publique [3]. Une collaboration spécifique entre journalistes et psychiatres permettrait une meilleure connaissance et une diffusion plus représentative des enjeux de santé mentale dans les média [4]. Des actions de santé publique et initiatives citoyennes pourraient aussi être profitables aux patients.

L’adhérence aux antidépresseurs peut largement être améliorée, la stigmatisation restant une barrière aux traitements et à la prise en charge.

To compare sensorimotor learning in stable schizophrenia patients, healthy age- and sex-matched controls and elderly controls in two variations of the classic Rotary Pursuit.

In Circle Pursuit (true motor learning), a target circle, rotating along a predictable circular path on the computer screen, must be followed by manipulating the pen on the writing tablet. The task consists of two trials of six rotations each, with target speed gradually increasing. In the eight-trial Figure Pursuit (motor and sequence learning), subjects learn to draw a complex figure by following a target circle that moves at a fixed speed along an invisible trajectory around several goals. Tasks were administered thrice (interval 2-7days) to 32 stable schizophrenia patients (S), 16 healthy age- and sex-matched controls (Y) and 16 elderly controls (>65y; E) and recorded with a digitizing tablet and pressuresensitive pen (200Hz frequency; 0.2mm spatial accuracy). Learning speed was assessed in a repeated-measures ANOVA of outcome variable accuracy (% of time cursor is within target).

All groups exhibited learning effects and significant group differences in accuracy existed (E<S<Y, p<0.01) in both tasks. However, within-subject analyses revealed a significant effect of group*trial number (p<0.01) and group*session (p0.02) only in Figure Pursuit, indicating a significant difference in learning between groups in this task.

In our Pursuit continuous sensorimotor task, group differences exist between schizophrenia patients and elderly and young healthy controls. When a sequence learning component is added (Figure Pursuit), there is also significant difference in the amount of learning.

Nous ne constatons aucune différence significative entre le groupe contrôle et le groupe consolidation MBCT en termes de âge de début de troubles (25 ± 11 vs 27 ± 10), de nombre de TS, d’hospitalisations (2 ± 4 vs 2 ± 2), ou de nombre d’épisodes maniaques (6 ± 7 vs 7 ± 8) et dépressifs (12 ± 12 vs 13 ± 16).

Après avoir décrit les modalités de ce groupe, nous évaluerons le profil des patients qui y participent (âge moyen de 48 ans ± 10) au regard de la régularité et de l’assiduité en termes de participation et de présence. Nous présenterons l’évolution des compétences de pleine conscience acquises et développées en lien avec leur assiduité et à l’instauration de leur pratique de méditation à la fois formelle et informelle.

The aim of the present study was to explore any heterogeneity in the 6-month clinical response in antipsychotic drug-naive schizophrenia patients, and to determine predictors of that outcome.

467 antipsychotic drug-naive schizophrenia patients were included in France nationwide and followed up over 6 months. In order to identify trajectories of clinical response, a latent class growth analysis was performed using the Clinical Global Impression-Severity (CGI-S) scores at baseline, 1, 3 and 6 months. Regression models were used to identify predictors of trajectory membership.

Five trajectory groups were identified: a rapid response group (n=45), a gradual response group (n=204), patients remaining mildly ill (n=133), patients remaining very ill (n=23) and a group with unsustained clinical response (n=62). Predictors of the 6-month clinical response were baseline CGI-S score (odds ratio 3.1; 95% confidence interval, 2.1-4.4) and negative symptoms (OR 1.5; 95%CI, 1.2-1.9). The sole predictor of rapid response as compared to gradual response was employment (OR 2.5; 95%CI, 1.2-4.9).

Clinical response in schizophrenia patients 6 months after a first-ever antipsychotic drug initiation is heterogeneous. Therapeutic strategies in first episode should take account of symptoms severity and of early clinical response, in order to maximize the chances of recovery.

Adolescence is recognized as a period of emotional turbulence. In a post-war country like Kosovo it may be more evident in the experience of stress and depression.

The investigation of levels of stress and depression in adolescence

The examination of relationships between stress, depression and several socio-demographic factors such as age, place of residence, gender and academic success in adolescents.

It is a quantitative correlational study which measures the level of stress and depression among adolescents. The measures included the Scale of stressful life events for children (stress-D) and the CDI Scale (Children's Depression Inventory). The purposeful sample was selected of 246 students of secondary schools of Malisheva (Kosovo). 49.2% males / 50.8% females, aged 15-20 years. Data processing with SPSS 17.0 and MicrosoftExcel2007.

18% of adolescents did not have depression, 79% low-level, 2% average and 1% high level of depression. 62% had high levels of stress, 25% medium and 10% low levels; 3% reported no stress. The level of stress is significantly positively correlated with gender (girls +; r=.14, p<.05), and success in school (better success +; r=.24, p<.01), and significantly negative correlated with residence (village +; r=-.15, p<.05 and age (the youngest +;r=-.18, p<.01). Level of depression was significantly negatively correlated with residence (village +; r=-.13, p=.05). There is no significant correlation between levels of stress and depression.

Adolescents are experiencing high levels of stress. Young females living in rural regions, who are also more academically successful are also more vulnerable to stress and require more attention from psychological services.

La présence d’une symptomatologie cognitive marqueur trait dans les troubles bipolaires est actuellement reconnue dans la littérature scientifique [1]. Ces déficits cognitifs spécifiques, présents dès le premier épisode et persistant tout au long de la vie, ont un impact fonctionnel important sur la vie les patients [2]. La remédiation cognitive est une thérapie prometteuse pour cette population en demande. Une diminution des troubles cognitifs et fonctionnels des patients bipolaires suite à une thérapie de remédiation cognitive a été observée [3,4]. Cependant, les programmes proposés actuellement présentent un intérêt limité en raison d’un manque d’adaptation à la spécificité des troubles cognitifs prévalents dans les troubles bipolaires [4].

Notre objectif est d’observer sur le plan cognitif et fonctionnel, l’effet d’un programme de remédiation cognitive spécifiquement développé pour les patients souffrant d’un Trouble Bipolaire.

Nous présentons une première étude de cas d’un patient ayant bénéficié du programme ECo, actuellement en cours de validation dans le cadre d’étude contrôlée, randomisée en double aveugle. Une évaluation neuropsychologique ainsi que des échelles de fonctionnement psycho-social ont été administrées avant et après la thérapie. Le programme ECo se compose de 24 séances individuelles comprenant deux séances de psychoéducation sur les troubles cognitifs et des séances de généralisation des stratégies acquises à des situations de la vie quotidienne. Ce programme cible majoritairement les déficits cognitifs des patients souffrant d’un trouble bipolaire : mémoire et apprentissage verbal, flexibilité, planification, inhibition, raisonnement, vitesse psychomotrice et attention [2].

Les améliorations observées sur le plan cognitif et fonctionnel permettent d’émettre l’hypothèse du bénéfice d’un programme spécifiquement adapté aux difficultés des patients souffrant d’un trouble bipolaire. Notre étude de validation en cours permettra de préciser l’intérêt, la spécificité et les bénéfices de cette prise en charge.

Over the Last decades outpacients services become a substitute for psychiatric hospitals in Brazil. For most patients, Psychiatric emergency Services represents the entry point into health care system and an essential source of treatment. The Psychiatric Emergency Service of the University Hospital of Santa Maria (SEP/HUSM) has operated since 1972 and serves the 537,745 inhabitants of 31 municipalities in southern Brazil.

To describe the profile of SEP/HUSM visits in the years 2010 and 2011.

Define the profile and the quality of care of visits to SEP/HUSM. Evaluate the characteristics of the visitas in SEP/HUSM during the years 2010 and 2011 to compare the statistics with other services.

This is a cross-sectional study of 7853 visitas, with the data of a questionnaire defined in the SEP/HUSM, associated with the records of the HUSM and using demographics and clinical variables. Research approved by the ethics committee HUSM.

The majority of the patients are from Santa Maria (83,9%), male (52%), 25-44 years old, had Mood Disorders (39%) and Substance Dependence (28,3%), came to the SEP/HUSM during the weekdays, and were sent to the observation room (21.5%) or hospitalized (18,9%).

The profile of the care provided in the SEP/HUSM is similar to other centers. There is a great demand by patients without the appropriate profile of treatment in the SEP/HUSM, probably due to problems in the other units of the mental health system.

Les patients reçus au centre d’évaluation et de traitement des troubles de l’humeur complexes (CETTHC) sont adressés par leur médecin, souvent en échec thérapeutique après plusieurs lignes de traitement. Compte tenu de la polymédication et de la fréquente consommation de substances psychoactives, il est fondamental de réaliser une expertise pharmacologique. Le CETTHC et le service de pharmacologie du CHU de Nantes ont rendu possible les « consultations pharmacologiques » afin de compléter l’évaluation clinique des patients. À travers deux exemples, nous soulignons l’importance de l’expertise pharmacologique pour l’optimisation du traitement des patients.

Les patients pris en charge au CETTHC sont reçu en consultation par un pharmacologue. Celui-ci réalise une expertise basée sur un historique médicamenteux précis, une évaluation de l’observance, de la tolérance, de l’adhésion du patient, ainsi que des interactions pharmacocinétiques et pharmacodynamiques. Nous présentons deux expertises pharmacologiques réalisées au cours du premier semestre 2014.

Le premier exemple souligne l’importance de l’analyse des interactions pharmacocinétiques médicamenteuses. Le deuxième souligne la nécessité d’évaluer précisément les consommations (tabac, alcool et autres substances). Dans les deux cas, l’efficacité du traitement était diminuée. L’expertise pharmacologique a conduit à des propositions d’adaptations thérapeutiques précises et pertinentes.

La pharmacologie est une discipline transversale. Les pharmacologues ont une place privilégiée à saisir dans les services cliniques, en particulier ceux accueillant des patients en échec thérapeutique. À l’époque de la médecine personnalisée, disposer d’un regard centré sur le traitement en complément de l’évaluation psychiatrique représente un enjeu majeur.

The burden of neuropsychiatric disorders in the elderly is high, considering patients, their families, and close or extended networks. In Portugal, the 10/66-Dementia Research Group population-based research programmes are running since 2011, with the community prevalence study. The protocol allows for valid diagnoses of dementia and depression, using comprehensive assessments which include the Geriatric Mental State- AGECAT.

We aimed to analyse informal caregiving arrangements and the psychological experience of caregiving in a subsample drawn from the ongoing 10/66 studies.

We report on 580 residents aged 65 + years of a defined catchment area in Portugal (Mora). Assessments included questionnaires on demographic and caregiving issues, the Self-Report Questionnaire (SRQ) on psychological distress and the Zarit Burden Interview (ZBI) on the caregiving experience.

In this subsample, 94 participants were in need of informal caregiving (dementia accounted for 28 cases, depression for 31, and other chronic physical/psychiatric conditions for the remainder). Most primary caregivers were family relatives (mostly wives and daughters) and were living with the patient. A large number were elderly people themselves (mean age 64.1±16.3years). Median scores were 3 on the SRQ (range 0-16) and 8 on the ZBI (range 0-66). Those who were caring for participants with more severe disabilities scored significantly higher on both measures.

These preliminary results of the 10/66 epidemiological community studies support previous suggestions that caregiver strain is also high in subgroups of community samples. Most overburdened families (and individual caregivers) lacked appropriate, tailored interventions. Final results will be available soon.

Individuals with bipolar disorder (BD) are differentially affected by insulin resistance. Individuals without history of mental disorder with insulin resistance usually have high peripheral concentrations of phenylalanine (PHE) and tyrosine (TYR) together. Catecholamine dysfunction is described to be a state-dependend phenomen in patients suffering from BD. Catecholamines are synthesized from essential amino acids PHE and TYR which are biotransformed to dopamine and subsequently converted to nor/adrenaline. Amino acid dysregulation may be a possible mediator of insulin resistance in BD.

Peripheral PHE and TYR concentrations were investigated in euthymic adults with BD. Amino acid differences between normal and overweight individuals with BD were evaluated and outcomes were correlated with the measures of glucose homeostasis.

Mean plasma PHE to TYR ratio (PHE/TYR) was at the upper limit of the normal range in the whole sample. Enrolled subjects with PHE/TYR beyond the limits of normal exhibited the highest number of prior affective episodes. Sex-specific differences were noted as overweight BD females showed different profiles than normal-weight women. In the overweight females, PHE and TYR concentrations were significantly higher compared to normal-weight women. Significant correlations were noticed between PHE, TYR and PHE/TYR with insulin/Homeostasis Model of Assessment (HOMA)-IR in the whole sample and the subgroup of BD women.

These significant differences in gender, amino acid pathways and in correlations with immune marker as well as insulin function have not been reported previously. Taken together, increased levels of PHE in BD should be considered when adjudicating diabetes risk especially in women.

La stimulation magnétique transcrânienne répétée (rTMS) est une technique thérapeutique non invasive de stimulation du cerveau, utilisée pour le traitement de la dépression résistante. Les études ont montré son efficacité sur la diminution de l’intensité de la dépression [1]. Mais, à ce jour, peu d’études se sont intéressées à l’évolution au cours de ce traitement, de facteurs psychologiques tels que les schémas dysfonctionnels représentés par les attitudes dysfonctionnelles [2] et les schémas précoces inadaptés [3] et qui sont impliqués dans la dépression.

Déterminer s’il y a une évolution des schémas précoces inadaptés (Young, 1990, 1999) et des attitudes dysfonctionnelles [2] avec le traitement rTMS.

Vingt-trois patients dépressifs traités par rTMS au cours d’une cure de 10 jours, avec ou sans traitement médicamenteux. En début et en fin de cure, puis un mois après la fin de la cure rTMS, ils ont rempli une batterie de questionnaires composés du MINI, du BDI-13, de l’HDRS, de la HAD, du STAI, de la DAS, de l’YSQ-S1 et de l’ESDV-5.

La rTMS confirme son efficacité sur les symptômes anxio-dépressif. Les attitudes dysfonctionnelles diminuent significativement un mois après la cure rTMS. En revanche, les schémas précoces inadaptés restent stables. La qualité de vie n’augmente modestement qu’un mois après la cure rTMS.

Bien que la rTMS permette une diminution significative de l’intensité de la dépression et l’anxiété, une vulnérabilité psychologique est maintenue au travers de schémas dysfonctionnels. Une psychothérapie associée au traitement et visant le changement de ces pensées négatives semble nécessaire, notamment afin d’éviter les rechutes dépressives. D’autres études avec un plus grand échantillon devraient examiner l’impact de la rTMS sur certains facteurs psychologiques à court, moyen, et long terme.

La stimulation magnétique transcrânienne (rTMS) est un traitement efficace de la dépression résistante. Sa place relative par rapport aux ECT dans la stratégie thérapeutique est questionnée ainsi que leur possible complémentarité.

Nous avons procédé à une synthèse de la littérature sur Pubmed fin juin 2014 avec la combinaison des mots clés suivants : ECT, mECT, rTMS, versus, adjuvant, add-on, comparative, efficacy, maintenance treatment, relapse, longitudinal follow-up, depression.

La rTMS peut être aussi efficace que l’ECT à coût équivalent sous certaines conditions (durée > 4 semaines, plus de 1200 pulses par séance, moins d’échecs médicamenteux antérieurs, et surtout absence de symptômes psychotiques). Les mécanismes d’action sont en partie communs : neurogenèse, sécrétion de facteurs neurotrophiques, transmission monoaminergique, gabaergique et glutamatergique, régulation de l’axe hypothalamo-hypophysaire et régulation de l’activité cérébrale frontal et limbique. L’action rapide de la rTMS sur le risque suicidaire reste à démontrer mais elle présente probablement un meilleur profil de tolérance cognitive en cure initiale, voire un effet procognitif. Dans les deux cas le taux de rechute est estimé entre 40 et 50 % à 3 mois en l’absence de prophylaxie adéquate. Le traitement d’entretien par rTMS consiste le plus souvent en 2 séances sur une journée de façon hebdomadaire sans consensus actuellement sur cette question. Bien que les ECT de maintenance soient bien tolérées, certains patients fragiles du fait de co-morbidités, ou réticents à certains traitements psychotropes, ou craignant les anesthésies, pourraient bénéficier de rTMS en relais des ECT. Plusieurs séries de cas plaident en ce sens.

Des essais prospectifs longitudinaux avec présence de groupes contrôles sont souhaitables, contrôlant sur la durée les traitements et les aspects cognitifs de façon objective et comparative. La rythmicité, les sous-groupes cliniques éligibles, et les traitements associés restent à préciser.

Évaluer l’efficience du palmitate de palipéridone (PP) par rapport aux antipsychotiques les plus communément utilisés en France.

Un modèle médico-économique a été développé afin de simuler la progression d’une cohorte de patients atteints de schizophrénie à travers quatre états de santé (« Stable-traité », « Stable-non-traité », « En-rechute », « Décédé »). PP a été comparé à rispéridone injectable à libération prolongée (ILP), aripiprazole ILP, olanzapine ILP, halopéridol décanoate et olanzapine orale (OO). Les coûts, les années de vie pondérées par la qualité de vie (« Quality-adjusted-life-year » ; QALY) et le nombre de rechutes ont été estimés sur cinq ans selon une perspective tous payeurs. Supposés stabilisés suite à une décompensation clinique, les patients initiaient un antipsychotique et passaient en phase de prévention de la rechute en cas de succès après trois mois. Ils/elles pouvaient arrêter leur traitement après une rechute, un manque de tolérance ou par choix, et passer sur la ligne de traitement suivante jusqu’à la troisième ligne (c.-à.-d. clozapine). Afin de prendre en compte l’observance, les probabilités de rechute en phase de prévention ont été calculées à partir de taux d’hospitalisation sur des données françaises en vie réelle. Les données de tolérance et d’utilité ont été dérivées d’études internationales, et les coûts de sources françaises. La robustesse des résultats a été testée via des analyses de sensibilité.

À 5 ans, PP est le moins coûteux des ILP et est associé à un surcoût de 249 € par rapport à OO. Rispéridone ILP et PP sont associés aux plus grands nombres de QALY. PP domine tous les autres ILP en termes de rechute évitée hormis olanzapine ILP.

PP est le moins coûteux des antipsychotiques ILP en France. OO est l’antipsychotique le moins coûteux, mais est associée à un nombre plus faible de QALY gagnées et de rechutes évitées comparé aux antipsychotiques ILP.

In studies of adult patients with attention deficit hyperactivity disorder (ADHD) atomoxetine (ATX) has demonstrated substantial improvements in ADHD symptomatology using Conners’ Adult ADHD Rating Scales (CAARS). The pattern suggests incremental response over time with no clear plateau of response.

To identify patterns of response to ATX in adult ADHD patients and to describe those trajectories over time

To determine if patients have distinct response trajectories using CAARS in two populations, short term (12 weeks) and long term (24 weeks) treatment data.

Data from 2502 ATX patients, who had an investigator-rated CAARS total score at ≥ short or long term time point, The numbers of trajectory clusters for short term (n=2502) and long term (n=1139) data were identified using hierarchical clustering methods. Linear mixed modelling was used to describe those different trajectories over time.

Using CAARS total, 4 trajectory clusters were identified in short term treated patients and 5 in long term. Three out of 4 short term trajectory clusters (representing 84% of patients) and 4 out 5 long term (representing 96%) showed more successful trajectories. In general clusters with less improvement were those with the worst baseline CAARS and minimal initial improvement. Distinct trajectory patterns of response were found that were incremental over time in all clusters.

Adult ADHD patients receiving atomoxetine have individual trajectories of response that can be divided into 4 short term trajectories and 5 long term trajectories. Further analysis is ongoing to describe these cohorts.

Well-conducted treatment trials and data on predictors of treatment response are both lacking for ADWA, despite the high prevalence of the condition. While benzodiazepines are frequently prescribed for anxiety disorders, etifoxine is also an effective agent not associated with dependence. A randomized controlled trial of etifoxine versus alprazolam for ADWA was undertaken, aiming at comparing efficacy and safety of the two treatments, and investigating predictors of the clinical response.

Two hundred and two adult outpatients with ADWA were enrolled at primary care settings sites in South Africa. Patients were followed for 4 weeks of treatment, and for an additional week after treatment discontinuation. The primary outcome measure was the Hamilton Anxiety Rating Scale (HAM-A) and was assessed by using a non-inferiority analysis. Secondary outcomes included the Sheehan Disability Scale (SDS) and the Clinical Global Impressions-Change Scale (CGI-C). Predictors of response were investigated with a multivariate logistic regression.

Both etifoxine and alprazolam dramatically decreased the HAM-A score at 4 weeks. The difference between treatment groups in HAM-A was of 1.78 [90% CI; 0.23, 3.33] in favor of alprazolam. The two treatments were equally efficacious in patients with more severe anxiety symptoms at baseline. Between week 4 and week 5, after treatment discontinuation, HAM-A scores continued to improve in the etifoxine group, but increased in the alprazolam group (P = 0.019); no difference was found between the two groups regarding the secondary outcomes. No significant predictors of treatment response were found. Patients treated with alprazolam experienced more treatment-related adverse events, mostly of central nervous origin, and especially after medication discontinuation.

The analysis of the clinical outcome of a sample of subjects with schizophrenia, after 25 years of evolution.

. Highlighting some clinical and socio-demographic aspects that might play a role in the heterogeneity of the actual clinical status.

The study sample includes 50 subjects, admitted for the first time in the Psychiatric Clinic of Timisoara during 1985–1987, who were diagnosed with schizophrenia at onset and currently have the same diagnosis (2012). The following parameters were analysed: sociodemographic (gender, age at onset, education, family and professional status) and clinical (type of onset, symptomatology, number of admissions).

The onset was insidious in the majority of cases. There were statistically significant differences between the types of symptoms at onset and the actual clinical status.

The clinical outcome in schizophrenia can vary significantly among patients, even if the diagnosis remains stable over time.
