A Three-Month Follow-Up Study Evaluating Changes in Clinical Profile and Attitudes Towards Involuntary Admission

Introduction
Involuntary admission and treatment is often a traumatic experience for patients and there is a wide variation in attitudes towards care even when patients are recovered.
Objectives/aimsThe purpose of this large prospective study was to identify clinical predictors of attitudes towards care during involuntary admission.
MethodsThree hundred and ninety-one consecutively admitted involuntarily patients to three psychiatric inpatient units over a 30-month period were invited to participate in the study. Comprehensive assessments at admission and 3 months after discharge were attained including measures of symptoms, insight, functioning, attitudes towards involuntary admission and coercive experiences. Multiple linear regression modelling was used to determine the optimal explanatory variables for attitudes towards care.
ResultsTwo hundred and sixty-three individuals participated at baseline and 156 (59%) successfully completed follow-up assessments. Individuals improved significantly over time clinically and in their attitudes towards their care. At baseline greater insight (P < 0.001) and less symptoms (P = 0.02) were associated with more positive attitudes towards care as was older age (P = 0.001). At follow-up, greater insight (P < 0.001), less symptoms (P = 0.02) and being older (P = 0.04) were associated with more positive attitudes towards care. More positive attitudes towards care at follow-up were associated with greater improvements in insight over time (P < 0.001) and having a diagnosis of an affective psychosis (P = 0.0009).
ConclusionsThe best predictors of positive attitudes towards care during and after involuntary admission are illness related factors, such as levels of insight and improvement in insight, rather than service or legislation related factors, such as the use of coercive measures, seclusion and restraint.

Objective:
To report a case of meningioma arising from the nervus intermedius.
Methods:This paper comprises a case report, literature review, and discussion regarding the presentation of a nervus intermedius meningioma, comparing and contrasting this to other relevant neoplasms of the internal auditory canal and cerebellopontine angle.
Results:Tumours of the cerebellopontine angle include vestibular schwannomas, facial schwannomas and, more rarely, nervus intermedius schwannomas. The nervus intermedius is a division of the facial nerve at the cerebellopontine angle, with parasympathetic and afferent somatic components. Our patient presented with progressive hearing loss. An ipsilateral internal auditory canal mass at the fundus, as indicated by magnetic resonance imaging and electroneuronography, was suggestive of vestibular schwannoma. Intra-operative dissection revealed a nervus intermedius tumour. Histological evaluation indicated a meningioma rather than a schwannoma.
Conclusion:This is the first reported case of meningioma involving the nervus intermedius. The implications this pathology may have on surgical approach, facial nerve outcomes, and the need for improved pre-operative imaging and intra-operative monitoring are discussed. A review of the current literature on nervus intermedius tumour is provided.

Consolidation of continuous fiber-reinforced titanium aluminide matrix composites (TMC) by the foil/fiber/foil method has traditionally taken an empirical approach utilizing processing cycles derived by simple trial and error. In an effort to reduce the empirical nature of producing TMC, a simple but effective analytical approach is employed. This approach analyzes the effect of fiber and foil geometries on consolidation parameters by combining a physical constitutive creep model with computational methods of interpreting raw materials characterization data. Examples of SCS-6/super a2(Ti-25Al-10Nb-3Mo-lV) and Saphikon/γ-TiAl composites consolidation are discussed by comparing the model predictions with equivalent validation specimen microstructures.

Preemergence applications of 5-bromo-3-sec-butyl-6-methyl-uracil (bromacil) and mixtures containing bromacil controlled both broadleaf and grass weeds best, and increased seed production and quality in side-oats grama [Bouteloua curtipendula (Michx.) Torr.] fields. Other herbicides significantly reduced broadleaf weeds but did not control grass weeds. The most troublesome grasses were foxtails; bristly foxtail [Setaria verticillata (L.) Beauv.] was the most prominent. Weed competition, in plots where herbicides were ineffective, significantly lowered seed production. Excessive rainfall after treatment was probably responsible for decreased weed control and increased crop damage from some of the herbicides. Applications of 2,3,5-trichloro-4-pyridinol (pyriclor) resulted in early crop chlorosis.
