Regulatory impact analyses (RIAs) weigh the benefits of regulations against the burdens they impose and are invaluable tools for informing decision makers. We offer 10 tips for nonspecialist policymakers and interested stakeholders who will be reading RIAs as consumers.
1. Core problem: Determine whether the RIA identifies the core problem (compelling public need) the regulation is intended to address.
2. Alternatives: Look for an objective, policy-neutral evaluation of the relative merits of reasonable alternatives.
3. Baseline: Check whether the RIA presents a reasonable “counterfactual” against which benefits and costs are measured.
4. Increments: Evaluate whether totals and averages obscure relevant distinctions and trade-offs.
5. Uncertainty: Recognize that all estimates involve uncertainty, and ask what effect key assumptions, data, and models have on those estimates.
6. Transparency: Look for transparency and objectivity of analytical inputs.
7. Benefits: Examine how projected benefits relate to stated objectives.
8. Costs: Understand what costs are included.
9. Distribution: Consider how benefits and costs are distributed.
10. Symmetrical treatment: Ensure that benefits and costs are presented symmetrically.

Effective preparedness, response, and recovery from disasters require a well-planned, integrated effort with experienced professionals who can apply specialized knowledge and skills in critical situations. While some professionals are trained for this, others may lack the critical knowledge and experience needed to effectively perform under stressful disaster conditions. A set of clear, concise, and precise training standards that may be used to ensure workforce competency in such situations has been developed. The competency set has been defined by a broad and diverse set of leaders in the field and like-minded professionals through a series of Web-based surveys and expert working group meetings. The results may provide a useful starting point for delineating expected competency levels of health professionals in disaster medicine and public health.

The Wisconsin Card Sorting Task (WCST; Heaton, 1980) is commonly used to assess concept formation and set shifting. Cognitive research suggests that set shifting performance is enhanced by a match between a person’s regulatory focus (promotion focus: attempting to earn an entry into a cash drawing; prevention focus: attempting to avoid losing an entry into the drawing) and the task reward structure (gains: attempting to maximize points gained; losses: attempting to minimize points lost). A regulatory match results when attempting to earn an entry by maximizing points or attempting to avoid losing an entry by minimizing losses. We test the hypothesis that performance on a modified WCST is accentuated in younger, healthy participants when there is a match between the global performance incentive and the local task reward structure. As predicted, participants in a match showed better set shifting but equivalent initial concept formation when compared with participants in a mismatch. Furthermore, relative to a baseline control group, mismatch participants were significantly worse at set shifting than were participants in a regulatory match. These results suggest that set shifting performance might be impacted by incentive and task reward factors in ways that have not been considered previously. (JINS, 2010, 16, 352–359.)

INTRODUCTION
It has been four decades since the concept of gene therapy for human disease began to be considered and discussed in earnest. With the advent of recombinant DNA technology in the 1970s and the concurrent discovery that viruses could carry novel genetic information into target cells, gene therapy was seen as having significant future potential. A major step in the development of gene therapy for hematopoietic disorders occurred in 1985 when the laboratories of Bernstein and Anderson simultaneously showed that the genome of mouse hematopoietic stem cells (HSCs) could be permanently modified using a genetically engineered retrovirus capable of integrating into host cell DNA. These results suggested that a patient's own HSCs could be harvested, genetically modified ex vivo, and then returned, perhaps following administration of radiation or chemotherapy to eradicate the diseased HSCs. From that time, it took almost a decade before functional correction of human disease model was demonstrated using retroviral-mediated, HSC-targeted gene transfer. The first such description was that of Heard and colleagues when they demonstrated the successful treatment of a murine model of the lysosomal disorder, mucopolysaccharidosis type VII. Subsequently, murine models of two immunodeficiencies – chronic granulomatous disease and a form of severe combined immunodeficiency due to JAK3 deficiency – were also successfully treated using gene therapy. These successes seemed to fulfill the stated goals of the Orkin and Motulsky NIH panel, which called for more basic science research into gene therapy and the use of murine disease models to demonstrate efficacy prior to clinical trials.
