To determine the outcome of patients with locally advanced cervix cancer treated with curative intent using external beam radiotherapy (EBRT), without brachytherapy.

A chart review was performed of all patients with cervix cancer who received EBRT alone at our centre from 2000 to 2010. Overall survival and local control were evaluated using Kaplan–Meier survival curves.

In total, 22 patients were identified. The median age and follow-up were 56 years and 65 months, respectively. The stage included IB to IVB. Main histology was squamous cell carcinoma (82%). Median tumour size was 5·5 cm. Majority treated with 3D conformal techniques and nine patients (41%) were treated with intensity-modulated radiation therapy (IMRT); 14 patients received doses of ≥65 Gy. Most patients (73%) received weekly concurrent cis-platinum. The major reason for not receiving brachytherapy was locally extensive tumour (59%). The 5-year relapse-free survival and overall survival rates were 57 and 50%, respectively. Seven patients (32%) had a component of loco-regional failure, mainly within the cervix. There was a better outcome among the nine patients treated with IMRT to a median dose of 66 Gy with a loco-regional control of 78%.

Patients who cannot have brachytherapy may still achieve acceptable rates of loco-regional disease control if high radiation doses (>65 Gy) was delivered.

To evaluate the survival outcomes and toxicities experienced by non-metastatic head and neck cancer (HNC) patients receiving modulated radiotherapy (RT).

A total of 608 HNC patients treated consecutively from March 2010 to December 2014 with common subsites (oral cavity, oropharynx, hypopharynx, larynx and nasopharynx) of HNCs formed the study group. Eligible patients included those treated with radical or postoperative RT between March 2010 and December 2014. More than 90% patients received modulated RT [intensity-modulated radiotherapy (IMRT) or volumetric-modulated arc therapy (VMAT)] with concurrent chemotherapy as per stage guidelines. Demographic parameters and disease-related factors were analysed. Disease-free survival (DFS) was calculated from end date of RT till last follow-up or last date of disease control. Overall survival (OS) was calculated from date of registration to last follow-up date if alive. The primary endpoint was survival. The statistical analyses were performed using SPSS version 20.0 and Kaplan–Meier method was used for calculation survival.

Among the evaluable patients, the median age was 60 years (range: 16–93) with male preponderance (male:female – 513:95). Majority were squamous cell carcinoma 93·4% (568/608). The subsites treated were oral cavity 36·8% (224). oropharynx 26·4% (161), larynx 19·7% (120), hypopharynx 10% (62) and nasopharynx 6·4% (41). RT intent was radical in 63·5% (386) and postoperative in 36·5% (222), with 59·5% (362) receiving concurrent chemotherapy. At last follow-up, 348 (57·2%) patients were alive, 169 (27·7%) patients had succumbed to disease and 120 (24·6%) patients had recurrent disease. Out of 120 recurrent cases loco-regional recurrence, nodal recurrence and distant metastases were seen in 62 (51·7%), 25 (20·8%), 33 (27·5%), respectively. In the entire study cohort at 2 year OS and DFS was 80 and 79% whereas 3 years OS and DFS was 70 and 75%, respectively.

In our study, 2 years and 3 years OS and DFS rates are found comparable to the international data with acceptable toxicity profile with the use of modulated RT. It seems to be possible because of stringent departmental protocols and good medical physics support. Our data re-validates need and benefit of advanced RT techniques like IG-IMRT and VMAT for both postoperative and radical HNC treatment at the cost of minimal long-term side effects. Future stringent follow-up and quality of life issues are being considered in a prospective manner.

This study aimed to investigate tolerance dose to organs at risk (OARs) as well as degree of conformity and homogeneity for head and neck cancer patients by using simultaneous integrated boost intensity-modulated radiotherapy technique (SIB IMRT).

This study analysed 15 head and neck cancer patients receiving treatment using inverse planned SIB IMRT technique. Using a beam energy of 6 MV, two dose levels of 70 and 55·4 Gy were used to treat the tumour. Doses of 2 Gy in 35 fractions and 1·68 Gy in 33 fractions were simultaneously delivered for effective planning target volume (PTV1) and boost planning target volume (PTV2), respectively.

Dose distribution in PTV and critical organs lies within tolerance dose guidelines protecting spinal cord, brain stem, optic chiasm, optic nerve, thus reducing the risk of damage to normal tissues. Minor deviation from tolerance limit was observed for parotid glands. This technique provided highly conformal and homogenous dose distribution as well as better sparing of OARs, hence verifying quality assurance results to be satisfactory.

SIB IMRT technique offers best solution for preserving organ function by keeping dose below tolerance level. Treatment of head and neck carcinoma using SIB IMRT is feasible, more efficient, and dose escalation is achieved in a single plan.

This exploration is intended to measure tissue maximum ratios (TMRs) in smaller fields through CC01 detector and to compare CC01 measured TMRs with Pinnacle treatment planning software (TPS) calculated TMRs.

CC01 compact chamber detector was used to measure TMR in water phantom for 6 and 18 MV beam delivered from Varian linear accelerator. Pinnacle TPS was employed in this study to calculate TMR from the measured percentage depth doses data. CC01 measured TMR data was compared with the calculated TMR data at depths from 5 to 20 cm for field sizes varying from 1 to 10 cm2.

For the smallest given field size of 1 cm2, CCO1 measured 13·95% higher TMR value for 18 MV beam than that for 6 MV beam. At 20 cm depth for 1 cm2 field size, TMR due to 18 MV beam was 52·4% higher than the TMR due to 6 MV beam. For 6 MV beam, the maximum difference appeared between the measured TMR and pinnacle calculated TMR was 2·8% and for 18 MV beam, the maximum difference was 4%.

For both 6 and 18 MV beam, there was good agreement between CC01 measured and Pinnacle calculated TMRs for the field sizes ranging from 1 to 10 cm2. This exploration can be extended to the determination of other dosimetric parameters like TARs, TPRs in small fields.

Electronic portal imaging device (EPID) offers high-resolution digital image that can be compared with a predicted portal dose image. A very common method to quantitatively compare a measured and calculated dose distribution that is routinely used for quality assurance (QA) of volumetric-modulated arc therapy (VMAT) and intensity-modulated radiation therapy treatment plans is the evaluation of the gamma index. The purpose of this work was to evaluate the gamma passing rate (%GP), maximum gamma (γmax), average gamma (γave), maximum dose difference (DDmax) and the average dose difference (DDave) for various regions of interest using Varian’s implementation of three absolute dose gamma calculation techniques of improved, local, and combined improved and local.

We analyzed 232 portal dose images from 100 prostate cancer patients’ VMAT plans obtained using the Varian EPID on TrueBeam Linacs.

Our data show that the %GP, γmax and γave depend on the gamma calculation method and the acceptance criteria. Higher %GP values were obtained compared with both our current institutional action level and the American Association of Physicists in Medicine Task Group 119 recommendations.

The results of this study can be used to establish stricter action levels for pre-treatment QA of prostate VMAT plans. A stricter 3%/3 mm improved gamma criterion with a passing rate of 97% or the 2%/2 mm improved gamma criterion with a passing rate of 95% can be achieved without additional measurements or configurations.

Small field dosimetry is complicated and accuracy in the measurement of total scatter factor (TSF) is crucial for dosimetric calculations, in making optimum intensity-modulated radiotherapy plans for treating small target volumes. In this study, we intended to determine the TSF measuring properties of CC01 and CC04 detectors for field sizes ranging from sub-centimetre to the centimetre fields.

CC01 and CC04 chamber detectors were used to measure TSF for 6 and 18 MV photon beam delivered from the linear accelerator, through small fields in a water phantom. Small fields were created by collimator jaws and multi-leaf collimators separately, with field sizes ranging from 0·6 to 10 cm2 and 0·5 to 20 cm2, respectively.

CC01 measured TSF at all the given field sizes created by jaws and multi-leaf collimators for both 6 and 18 MV beams whereas CC04 could not measure TSF for field sizes <1 cm2 due to volume averaging and perturbation effects.

CC01 was shown to be effective for measurement of TSF in sub-centimetre field sizes. CC01 can be employed to measure other dosimetric quantities in small fields using different energy beams.

For palliative radiotherapy treatments, two types of simulation are available at our Centre: conventional (or 2D) and virtual. Each has its advantages: conventional simulation requires less preparation time whereas virtual simulation allows accurate visualisation and identification of target volumes in 3D. We propose a new approach where treatment field parameters are determined on diagnostic CT scans, and then reproduced with reference to a patient’s bony landmarks using conventional simulation. This combines the benefits of both methods. We argue that the slight differences in set-up between diagnostic and simulation CT scans will have little impact on the determination of the target volume in palliative treatment settings.

In total, 12 patients who had diagnostic scans done within a month before their virtual simulation were randomly selected. Both scans were retrieved retrospectively for the study. An independent radiation oncologist contoured the target volumes on both scans and their relative positions were compared by fusing the digitally reconstructed radiographs generated from the respective scans. A 2D Conformity Index (2DCI) was then calculated and tabulated for 0, 0.5, 1.0, 1.5 and 2 cm margins to evaluate the accuracy of this approach and determine the margins required to account for the inherent variability of this method. In addition, the deviation or offset of the centre of field (COF) was also measured and analysed.

The median 2DCI (0 margin) was 0.85. For margins of 0.5, 1.0, 1.5 and 2.0 cm, the median 2DCIs were 0.97, 1, 1 and 1, respectively. The median displacement of the COF in the superior–inferior direction was 0.7 cm and that in the right–left direction was 0.2 cm.

Our pilot study suggests that our approach is feasible. We recommend adding an additional margin of up to 0.5 cm (to the usual treatment margins) to ensure good coverage of the target volume when using this method.

Dermatofibrosarcoma protuberans (DFSP) is a locally aggressive intermediate malignancy.

The purpose of this retrospective analysis is to determine the efficacy of radiation therapy (RT) in local control of DFSP.

The recurrence-free survival (RFS) for 45 patients treated for DFSP at our institution was estimated and compared between surgery alone and postoperative RT groups.

Age range of the patients were in the third and fourth decades; males:females=2:1; most common site: anterior abdominal wall; tumours >5 cm in size in 75%; low grade in 77·8%; margins positive in 31·8% and <5 mm margins in 45·5%. Two-thirds of patients had at least one recurrence before presentation to our institution. RT dose was >50 Gy in 88% of patients. The patients treated with postoperative RT had poorer prognostic factors compared with surgery alone: they were males (17 versus 13 patients), and presented with high-grade tumours (5 versus 1 patients), multiple recurrences prior to presentation (25 versus 20 patients) and positive or <5 mm margins (22 versus 12 patients). Median follow-up for surgery alone group was 17 (1–152) months and for postoperative RT group, this was 54 (5–121) months. RFS at 5 years was 77·1% for surgery alone and 87·9% for postoperative RT group but was not statistically significant. The median time to recurrence was 4 years.

RT delays the time to recurrence in DFSP. RT improves the outcome of DFSP for recurrent tumours and with positive margins.

This is a retrospective study to evaluate the efficacy and safety of routine use of electronic portal imaging device (EPID) in intensity-modulated radiation therapy for localised prostate cancer.

Data from 20 patients with localised prostate cancer treated by radical radiotherapy using intensity-modulated technique in Habib Bourguiba Hospital were analysed to define the action levels for pretreatment planer dose distribution of 100 treatment fields and the set-up errors of 418 portal imaging. Pretreatment planar dose distribution was measured with the EPID. The additional dose from repeated portal imaging was determined with treatment planning system.

For all 100 fields, the predicted and the measured planar dose distribution agrees well with mean±standard deviation value for γmax=2·31±0·57, γavg=0·36±0·07 and γ%≤1=98·94%±0·71%, respectively. For the evaluation of set-up errors, the mean total errors with 1 SD in the lateral, longitudinal and vertical directions were 0·11±0·44 cm; 0·02±0·37 cm and −0·02±0·21 cm, respectively. The imaging additional dose was evaluated as 1 cGy per monitor unit.

EPID is a useful tool to verify pretreatment dose distribution and to assess the correct field position without a significant increase in the absorbed dose due to the repetition of portal imaging.

The purpose of this study was to analyse the comparison of intensity-modulated radiation therapy quality assurance (IMRT QA) using Gafchromic® EBT3 film, Electronic portal imaging device (EPID) and MapCHECK®2.

Pretreatment authentication is the main apprehension in advanced radiation therapy treatment plans such as IMRT.

A total of 20 patients were planned on Eclipse treatment planning system using 6 and 15 MV separately.

Gamma index of EBT3 film results shows the following average passing rates: 97% for 6 MV and 96·6% for 15 MV using criteria of ±5% of 3 mm, ±3% of 3 mm and ±3% of 2 mm for brain. However, by using ±5% of 3 mm and ±3% of 3 mm criteria, the average passing rates were 95·4% on 6 MV and 95·2% on 15 MV for prostate. For EPID, the results show the average passing rates as 97·8% for 6 MV and 97·2% for 15 MV in for brain. In cases in which ±5% of 3 mm and ±3% of 3 mm were used, the average passing rates were 96·6% for 6 MVand 96·1% for 15 MV for prostate. MapCHECK®2 results show average passing rates of 96·4% for 6 and 96·2% for 15 MV, respectively, for brain using criteria of ±5% of 3 mm, ±3% of 3 mm and ±3% of 2 mm, whereas for ±5% of 3 mm and ±3% of 3 mm the average rates are 95·2% for 6 and 94·7% for 15 MV in prostate.

The EPID results are better than the other methods, and hence EPID can be used effectively for IMRT pretreatment verifications.

In non-melanoma skin cancer—that is, basal cell carcinoma (BCC) and squamous cell carcinoma (SCC)—brachytherapy treatment is preferred over surgical excision because of cosmetic reasons, acceptability and preference of patients.

Moulds are prepared of wax to match the size of the lesion. This represents the area to be considered in treatment planning. A total of 85 patients who had either SCC or BCC were treated, and all these patients were classified on the basis of age, gender and origin.

Patients were treated with 39 Gy in 13 fractions (biological effective dose=50·7 Gy). In 52 BCC patients, treatment achieved excellent cosmetic results in 49 cases, with 17 of these patients experiencing Grade-1 skin reactions related to treatment in the first 24 weeks of follow-up. A single patient experienced Grade-II hyper-pigmentation reaction in the third week. In 33 SCC patients, treatment achieved excellent cosmetic results in 28 cases, with 17 of these patients experiencing Grade-I reaction in the first 36 weeks after treatment. Among the remaining four patients, only one developed Grade-II hypo-pigmentation and one patient experienced tumour recurrence near the primary site. The overall outcome efficacy of the treatment was 98·8%.

The treatment outcome not only enhances our confidence in brachytherapy but also improves the patient’s satisfaction regarding cosmetic results and curative output achieved by avoiding a surgical procedure for non-melanoma skin cancers.

The aim of the current study was to (i) to calculate organ equivalent dose (OED) and (ii) to estimate excess absolute risks (EARs), lifetime attributable risks (LARs) and relative risks (RRs) from stereotactic ablative radiotherapy (SABR) for lung cancer to in-field, close to field, and out of field structures.

A total of five patients with T1, T2 (≤4 cm), N0, M0 medically inoperable non-small cell lung cancer were selected for treatment planning. Patient selection criteria were based on RTOG 0236. Five treatment deliveries were investigated: (i) three-dimensional conformal radiotherapy (3DCRT), (ii) intensity-modulated radiotherapy (IMRT), (iii) intensity-modulated radiotherapy with flattening filter free beam (IMRTF), (iv) volumetric modulated arc therapy (VMAT) and (v) volumetric modulated arc therapy with flattening filter free arcs (VMATF). Delineated normal structures included chest wall, left and right lung, trachea, small and large airways, spinal cord, oesophagus and involved ribs. All plans were prescribed to 60 Gy in five fractions to primary planning target volume (PTV) volume so that ≥98% of the PTV received ≥98% of the prescription dose and internal tumour volume received 100% of the prescription dose. The OED for all delineated normal structures was calculated using differential dose volume histograms. Using risk models, the age-dependent LAR’s and RR were calculated. Additionally, the secondary cancer risk for organs inside primary radiation was analysed using sarcoma and carcinoma risk models.

For all patients, the mean V20 volumes from the SABR plans were 4·1% (3DRT), 11·8% (IMRT), and 12·7% (VMAT), respectively. The EAR (combining all organs EAR) for all the organs studied, ranged from 8·5 to 10·6/10,000 persons/year for VMATF and 3DCRT, respectively. The EAR (combining all organs EAR) for all the organs studied, ranged from 8·5 to 10·6/10,000 persons/year for VMATF and 3DCRT, respectively. The absolute EAR difference between IMRT and IMRTF was low ranging from 0·2 to 0·4/10,000 persons-year, whereas delivery difference (IMRT and VMAT) had a significant impact on EAR with absolute difference ranging from 0·5 to 1·0/10,000 persons-year for IMRT and VMAT and 1·1–1·5/10,000 persons-year for IMRTF, VMATF, respectively. The LAR data showed a strong dependence on age at exposure and the LAR decreased as a function of age at exposure. The absolute attributable risk of bone sarcoma was lower with the VMAT plan and was significantly higher with the 3DCRT plan.

From a clinical perspective, it should be concluded that all five solutions investigated in the study can offer high quality of patient treatments and only estimates of radiation-induced malignancies can truly differentiate among them. The results suggested it would be reasonable to use the cumulative LAR difference when needed to select between treatment techniques. In conclusion, the LAR of radiation-induced secondary cancer was significantly lower when using VMATF than when using IMRT for SABR lung patients. VMATF would be the right choice for the treatment of SABR lung patients in terms of LAR. However, more work is required for the specific estimation and long-term validation and updating of the models behind LAR estimation.

To create practical lookup tables containing percent depth dose (PDD) and profile parameters of electron beams and to demonstrate clinical application of the lookup tables to skin cancer treatment to ensure target coverage in a clinical setup.

For 6 and 9 MeV electron energies, PDDs and profiles at clinically relevant depths [i.e., R95 (distal depth of 95% maximum dose), R90, R85 and R80] were measured in water at 100 cm source-to-surface distance for an 10×10 cm2 open field and circular cutouts with diameters of 4, 5, 6, 7 and 8 cm. Then PDD parameters along with profile parameters such as width of isodose lines and penumbra at the clinically relevant depths were determined. Output factors for the cutouts were measured at dmax in water and solid water.

With PDD and profile parameters, dosimetry lookup tables were generated. Based upon the lookup tables, target coverage at prescribed depths was retrospectively reviewed for three skin cancer cases. The lookup tables suggested larger cutouts for adequate target coverage.

Dosimetry lookup tables for electron beam therapy should include profile parameters at clinically relevant depths and be provided to clinicians to ensure target coverage in a clinical setup.

Monte Carlo calculation method is considered to be the most accurate method for dose calculation in radiotherapy. The purpose of this research is comparison between 6 MV Primus LINAC simulation output with commissioning data using EGSnrc and build a Monte Carlo geometry of 6 MV Primus LINAC as realistically as possible. The BEAMnrc and DOSXYZnrc (EGSnrc package) Monte Carlo model of the LINAC head was used as a benchmark.

In the first part, the BEAMnrc was used for the designing of the LINAC treatment head. In the second part, dose calculation and for the design of 3D dose file were produced by DOSXYZnrc. The simulated PDD and beam profile obtained were compared with that calculated using commissioning data. Good agreement was found between calculated PDD (1·1%) and beam profile using Monte Carlo simulation and commissioning data. After validation, TPR20,10, TMR and Sp values were calculated in five different field.

Good agreement was found between calculated values by using Monte Carlo simulation and commissioning data. Average differences for five field sizes in this approach is about 0·83% for Sp. for TPR20,10 differences for field sizes 10×10 cm2 is 0·29% and for TMR in five field sizes, the average value is ~1·6%.

In conclusion, the BEAMnrc and DOSXYZnrc codes package have very good accuracy in calculating dose distribution for 6 MV photon beam and it can be considered as a promising method for patient dose calculations and also the Monte Carlo model of primus linear accelerator built in this study can be used as method to calculate the dose distribution for cancer patients.

To determine the feasibility of an anthropomorphic breast polyurethane-based three-dimensional (3D) dosimeter with cavity to measure dose distributions and skin dose for a commercial strut-based applicator strut-adjusted volume implant (SAVI™) 6–1.

An anthropomorphic breast 3D dosimeter was created with a cavity to accommodate the SAVI™ strut-based device. 2 Gy was prescribed to the breast dosimeter having D95 to planning target volume evaluation (PTV_EVAL) while limiting 125% of the prescribed dose to the skin. Independent dose distribution verification was performed with GAFCHROMIC® EBT2 film. The dose distribution from the 3D dosimeter was compared to the distributions from commercial brachytherapy treatment planning system (TPS) and film. Point skin doses, line profiles and dose–volume histogram (DVHs) for the skin and PTV_EVAL were compared.

The maximum difference in skin dose for TPS and the 3D dosimeter was 4% whereas 41% between the TPS and EBT2 film. The maximum dose difference for line profiles between TPS, 3D dosimeter, and film was 4·1%. DVHs of skin and PTV_EVAL for TPS and 3D dosimeter differed by a maximum of 4% at 5 mm depth and skin differed by a maximum 1·5% between TPS and 3D dosimeter. The criterion for gamma analysis comparison was 92·5% at ±5%±3 mm criterion. The TPS demonstrated at least ±5% comparability in predicting dose to the skin, PTV_EVAL and normal breast tissue.

3D anthropomorphic polyurethane dosimeter with cavity gives comparable results to the TPS dose predictions and GAFCHROMIC® EBT2 film results in the context of HDR brachytherapy.

In developing countries like Pakistan the cost effectiveness and patient convenience in any treatment modality is a question of major concern. The purpose of this study was two-fold; first to report our experience with a high-dose rate Iridium-192 surface mould brachytherapy of keloid scars after surgical excision, using different radiation treatment regimen and second to establish the most convenient and cost effective treatment protocol having no compromise on the treatment outcomes.

From January 2012 to April 2015 a total 51 patients with 65 keloid lesions underwent postoperative Iridium-192 high-dose rate surface mould brachytherapy. The dose regimen used was: 8 Gy in a single fraction, 10 Gy in a single fraction, 15 Gy in three fractions and 18 Gy in three fractions. The median follow-up period was 33 months (range 15–53 months).

The success rates were 57·2, 89·5, 85 and 89·5% for the treatment regimen of 8 Gy/F×1, 10 Gy/F×1, 5 Gy/F×3 and 6 Gy/F×3, respectively. Grade 2 or above radiation induced toxicity was not observed.

The results of this study show that a dose regimen of 10 Gy (biological effective dose=20 Gy) in a single fraction have comparable results with a dose regimen of 15 Gy in three fractions or 18 Gy in three fractions. 10 Gy in a single fraction is therefore the most convenient and cost effective dose regimen for the management of keloid scars in developing countries like Pakistan, while 8 Gy in a single fraction is considered suboptimal and discouraged in practice.

The data used in brachytherapy planning are obtained from homogeneous mediums. In practice, the heterogeneous tissues and materials affect the dose distribution of brachytherapy. It is aimed to investigate the effect of air cavities on brachytherapy dose distribution using a specially designed device.

In this study, the special device designed with different volumes of air and water to be irradiated and measured at different depths using EBT3 Gafchromic films. EBT3 Gafchromic films were preferred for this study because they can be cut to the shape of the experimental geometry, are water resistance and double directional usability.

In our study, sudden dose increases and decreases were observed at the water–air–water interfaces. Increases were 9, 11·8 and 15% in the 13, 18 and 22 mm apparatus, respectively. These effects were expected and the results were consistent with the literature and within the tolerance limits stated in the clinical dose guidelines. The most important result is that the percent depth–dose curve of the radiation passing through the air to the water and only passing through the water medium is different. The average differences were 1·97, 2·97 and 2·31% for the 13, 18 and 22 mm apparatus, respectively.

Although the effect of heterogeneity may be neglected according to clinical guidelines, it is suggested that the dose effect of heterogeneity is taken into account so that the dose can be estimated sensitively. Brachytherapy plans using dose data without considering air gaps may cause erroneous dose distributions due to heterogeneity of tissue.

Due to the increased degree of modulation and complexity of volumetric-modulated arc therapy (VMAT) plans, it is necessary to have a pre-treatment patient-specific quality assurance (QA) programme. The gamma index is commonly used to quantitatively compare two dose distributions. In this study we investigated the sensitivity of single- and multi-gamma criteria techniques to detect multileaf collimator (MLC) positioning errors using the Varian TrueBeam Electronic Portal Imaging DeviceTM (EPID) dosimetry and the ArcCHECKTM device.

All active MLC positions of seven intact prostate patients VMAT plans were randomly changed with a mean value of 0.25, 0.5, 1 and 2 mm and a standard deviation of 0.1 mm on 25, 50, 75 and 100% of the control points. The change in gamma passing rates of six gamma criteria of 3%/3 mm, 3%/2 mm, 3%/1 mm, 2%/2 mm, 2%/1 mm and 1%/1 mm were analysed individually (single-gamma criterion) and as a group (multi-gamma criteria) as a function of the simulated errors. We used the improved and global gamma calculation algorithms with a low dose threshold of 10% in the EPID and ArcCHECK software, respectively. The changes in the planning target volume dose distributions and the organs at risk due to the MLC positioning errors were also studied.

When 25, 50, 75 and 100% of the control points were modified by the introduction of the simulated errors, the smallest detectable errors with the EPID were 2, 1, 0.5 and 0.5 mm, respectively, using the multi-gamma criteria technique. Similarly for the single-gamma criteria technique errors as small as 2, 1, 1 and 1 mm applied to 25, 50, 75 and 100% of the control points, respectively, were detectable using a 2%/2 mm criterion. However, the smallest detectable errors with the ArcCHECK when using the multi-gamma criteria technique were 2, 2 and 1 mm when MLC errors were applied on 50, 75 and 100% of the control points. When only 25% of the control points were affected the ArcCHECK were unable to detect any of the errors applied. No noticeable difference was observed in the sensitivity using the single- or the multi-gamma criteria techniques with the ArcCHECK.

The Varian TrueBeam EPID dosimetry shows a higher sensitivity in detecting MLC positioning errors compared with the ArcCHECK regardless of using the single- or the multi-gamma criteria techniques. Higher sensitivity was observed using the multi-gamma criteria technique compared with the single-criterion technique when using the EPID.

Safety is a mandatory issue during the operation of a nuclear power plant. A nuclear reactor can have some atmospheric dispersion due to any errors in the safety system.

The aim of this study is to estimate the cancer risk incidence for different body organs due to accidentally released radionuclides from Bushehr Nuclear Power Plant (BNPP).

The assumed hypothesis was atmospheric dispersion of radionuclide into the environment due to the safety failure of BNPP. Total effective dose equivalent (TEDE) from radionuclide diffusion in the medium was calculated using HOTSPOT code at two different wind speeds. Finally, the risk of cancer incidence for different organs of male and female sex has been estimated by Biologic Effects of Ionizing Radiation (BEIR) VII model.

The results showed that with increasing the exposure age and attained age, the risk of cancer incidence for different organs is decreased. The value of TEDE was increased at lower wind speed. The most probable organ for cancer incidence at different levels of TEDE in male and female sex was colon and bladder, respectively. On the other hand, prostate and uterus had the lowest radiation sensitivity and cancer risk incidence in male and female sex, respectively. Increasing the wind speed reduces the risk of cancer incidence for all of organs understudy.

Based on the obtained results, it can be concluded that the younger persons are more subject to the cancer risk incidence because of both the intrinsically greater radio-sensitivity of their organs and their longer remaining life expectancy during which a cancer may develop. The overall risk of cancer incidence as well as the site specific solid cancer incidence were highly dependent to the sex of exposed person, so that the female sex was more exposed to the cancer risk incidence at all of the irradiation levels understudy.

Across the history of radiotherapy, with gradual technological progress and various methods of irradiation, the purpose has always been to deliver homogeneously 100% of the prescribed dose to 100% of the target volume containing the identifiable tumour and/or tumour cells potentially present while limiting the dose to adjacent normal tissues.

The formula for triple point conformity scale is: CS3=(V95+V100+V105)/3VT. (a) Lower limit determination: CS3=(VT+0·93 VT+0·0)/3VT=0·643; (b) Upper limit determination: in order to find out an empirical relation in between V105 and VT, we studied over 593 cancer patients of various sites by taking planning target volume as target, and an empirical relation is derived out as: V105/VT=0·0007. Hence, CS3=(VT+VT+0·0007 VT)/3VT=0·6667~0·667.

Upper and lower limits of CS3 have been calculated at 0·643 and 0·667, respectively. Maximum value of CS3 index is recorded 0·656 while minimum value is 0·478.

The CS3 scale constitutes an attractive tool because it could facilitate decisions during analysis of various treatment plans proposed for conformal radiotherapy. Its major advantages are its simplicity and integration of multiple parameters.

The triple point conformity scale (CS3) provides better qualitative information about radiotherapy plans as compared to other conformity indices. This study advises the users to use the CS3 scale to evaluate a conformal radiotherapy plan which encompasses a wide range of relevant clinical volumes, and is able to extract qualitative dosimetric information.
