Across academic medicine, including psychiatry, women are underrepresented in senior positions. Various reasons have been put forward, for example the lack of high-ranking female role models or mentors and a reduced rate of career progression for women compared with men. Mentoring has been shown to be a popular and feasible intervention which can improve the success of those perceived as disadvantaged groups (in this case women) by having an important impact on personal development, career guidance and research productivity.

Summary
The importance of gynaecological morbidity resulting from reproductive tract infections among diverse populations of women in developing countries is well documented (Cates, Farley and Rowe, 1985; Koenig et al., 1998; Schulz, Cates and O'Mara, 1987; Wasserheit, 1989). Less well researched, but of growing concern, is the problem of infection-related reproductive tract morbidity among men, which is discussed in Chapter 4. Despite a growing consensus that reproductive tract infections are an important health issue, a perplexing array of problems faces those who seek to describe the extent of these conditions. The literature reveals tremendous variability in the reported prevalence of specific infections, some of which undoubtedly arises from real differences in the underlying epidemiology. Of equal importance, however, may be differences in the clinical definitions used to classify and describe various types of gynaecological morbidity, the training of the health care providers who elicit symptoms and record clinical observations, and the thoroughness with which a practitioner examines any client.
This chapter discusses the limitations of defining and observing reproductive tract morbidity in community-based research, and makes recommendations for future practice. We begin by discussing definitions of ‘normality’ and ‘abnormality’ as they apply to gynaecological morbidity, using cervical ectopy as an example. We then illustrate the problems of differences in diagnostic criteria between studies and inter-observer variation. Investigators are urged to use standardized definitions of clinically diagnosed morbidity that are both well-supported by evidence from the literature and easily applied in clinical practice

Introduction: why men?
Recent years have seen several shifts in the emphasis and direction of reproductive health programmes. There has been a paradigm shift away from narrow concerns on demographic targets (through a concentration on population control and the delivery of family planning services) towards a more client-centred individual approach which is, in theory at least, both holistic and responsive to an individual's needs and well-being (Pachauri, 1999). This shift has arisen from separate directions that have combined to challenge the largely demographic imperative of lowering population growth rates towards a broader refocusing on human welfare and individual choice, and the public health goal of reducing reproductive and gynaecological morbidity (Collumbien and Hawkes, 2000). Proponents of the wider conceptual framework have defined reproductive health to include both ‘family planning and sexual health care’ (United Nations, 1994). Operationalizing this definition at a programme level implies provision of ‘the widest range of services without any form of coercion’. However, most of the policy and programme initiatives arising from this paradigm shift have so far been directed at women.
Much of this focus on women is largely historical. From the development of modern methods of contraception to the delivery of family planning programmes and the monitoring of outcomes via fertility surveys, women have been the central focus (Becker, 1996). Only recently has there been a recognition that a broadening of the reproductive health agenda away from fertility control to incorporate components such as management of sexually transmitted infections (STIs) or effective behaviour change communication, will necessitate the inclusion of men into previously predominantly female domains

A phlebotomist with nosocomially acquired human immunodeficiency virus infection developed tuberculosis 10 months after exposure to multidrug-resistant Mycobacterium tuberculosis during a nosocomial outbreak. Healthcare workers with immunosuppression are at increased risk of tuberculosis if infected and, if exposed, should be considered for preventive therapy regardless of tuberculin skin-test status.
