The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Background.
Methicillin-resistant Staphylococcus aureus (MRSA) transmission and infections are a continuing problem in hospitals. Although some have recommended universal surveillance for MRSA at hospital admission to identify and to isolate MRSA-colonized patients, there is a need for formal economic studies to determine the cost-effectiveness of such a strategy.
Methods. We developed a stochastic computer simulation model to determine the potential economic impact of performing MRSA surveillance (ie, single culture of an anterior nares specimen) for all hospital admissions at different MRSA prevalences and basic reproductive rate thresholds from the societal and third party-payor perspectives. Patients with positive surveillance culture results were placed under isolation precautions to prevent transmission by way of respiratory droplets. MRSA-colonized patients who were not isolated could transmit MRSA to other hospital patients.
Results. The performance of universal MRSA surveillance was cost-effective (defined as an incremental cost-effectiveness ratio of less than $50,000 per quality-adjusted life-year) when the basic reproductive rate was 0.25 or greater and the prevalence was 1% or greater. In fact, surveillance was the dominant strategy when the basic reproductive rate was 1.5 or greater and the prevalence was 15% or greater, the basic reproductive rate was 2.0 or greater and the prevalence was 10% or greater, and the basic reproductive rate was 2.5 or greater and the prevalence was 5% or greater.
Conclusions.Universal MRSA surveillance of adults at hospital admission appears to be cost-effective at a wide range of prevalence and basic reproductive rate values. Individual hospitals and healthcare systems could compare their prevailing conditions (eg, the prevalence of MRSA colonization and MRSA transmission dynamics) with the benchmarks in our model to help determine their optimal local strategies.

Summary
Analysis of some bibliographical marginalia in a work by one of the founding figures of our discipline enables us to discuss their implications for our knowledge of early-sixteenth-century drama and possibly of early printing in Alcalá de Henares.
Lot 268 at the auction of printed books held at Bonham's in London on 12 March 2002 was a copy of Nicolás Antonio's Bibliotheca Hispana (2 volumes; Romae: ex officina Nicolai Angeli Tinassii, MDCLXXII). Its condition is not good; the catalogue description of them as suffering ‘browning and staining, some mainly marginal tears and repairs, worming to last few leaves of vol. 1 with some loss, vol. 2 with frontispiece repaired on verso, modern buckram, worming to hinges’ is entirely fair. What makes these two volumes exceptionally interesting and important, however, despite their poor condition, is that both are extensively annotated in manuscript by a number of early readers who had a special interest in authors associated with Alcalá de Henares and its university. The verbal annotations take three principal forms: the addition of entirely new entries concerning individuals who are not listed by Nicolás Antonio; the insertion of information to fill blanks left in the record given by him; and the addition of further details to his accounts. There are also some non-verbal annotations, by way of single or double oblique marginal strokes and other marks to draw attention to particular entries; sometimes points are under-lined in the text with or without further marginal comment. In the case of one annotator in particular, numerous of the marginalia added by this reader record details of works written by authors educated and/or teaching at Alcalá, and among these are several that offer details of early dramatic texts from the sixteenth century. In this category, of particular interest are those which give dates or other information not previously recorded in the sources available to us.
Faced with the potential importance of this, it is necessary first to establish a chronology for the annotations, and then to determine both the basis of the annotators’ knowledge of the material they record, and, as far as possible, its reliability.

Summary
In this essay I am resuming an enquiry into the theological reach of Cervantes’s Novelas which I began in 1996 in a study of La fuerza de la sangre (The Power of Blood). This novella, as I understand it, is a kind of Christian mystery tale which unfolds a warning against prejudiced and pessimistic perceptions of a world in which the innocent suffer and evil can seem to prevail. Its essential theme for a shrewd and not too squeamish reader is the hidden immanence of God. Behind it there lies a medieval Scholastic doctrine on the nature of divine sovereignty which was still accepted by seventeenth-century theologians. Pilloried by agnostic thinkers of the European Enlightenment, this doctrine declared that the whole of life is governed by two forms of providence, one of which is the God who wills what is good in life ( providentia approbationis) and the second of which is the God who permits what is evil ( providentia concessionis) whilst still intending good. Hence the novela's explicit theme of a God who ‘permits’ an innocent child to suffer an accident which is instrumental in bringing about the marriage of his parents; and hence (in my interpretation) the darker and, as Cervantes knows, potentially offensive theme of a God who permits the rape of a virtuous woman with the same end in view. Presented in a form which allows a narrow-minded reader to bury his head in the sand, the theme of providential rape is an example of Cervantine free-thinking concerning the theoretical implications of a traditional religious doctrine. In this essay I wish to show how the same doctrine reappears in forms which I think are still more free-thinking in El celoso extremeño.
Cervantine free-thinking is not specifically metaphysical and it is that of a man whose Christian core is undoubtedly fideist. It is nevertheless an attempt at thinking objectively which sometimes crosses the dividing line between safe thought and curiosity in theological matters. It naturally involves a habit of self-detachment.

Ontologies are becoming increasingly important because they provide the critical semantic foundation for many rapidly expanding technologies such as software agents, e-commerce and knowledge management (McGuinness, 2002). The Unified Modelling Language (UML)1 has been widely adopted by the software engineering community and its scope is broadening to include more diverse modelling tasks. This paper discusses the recent convergence of UML and ontologies and suggests some possible future directions.
