Effects of gestation on ovine fetal and maternal angiotensin receptor subtypes in the heart and major blood vessels

Previous studies in fetal sheep have concluded that (a) the vascular AT1 angiotensin II (Ang II) receptor subtype is present in the external umbilical artery, but not in other systemic blood vessels, and (b) carotid arterial rings contract in vitro in response to Ang II. These contractions are blocked by the AT1 specific receptor antagonist losartan. The aim of the present study was to resolve the apparent contradiction of these earlier conclusions, by examining the distribution of Ang II receptor subtypes in different regions of the ovine fetal cardiovascular system, and to find out at what stage in development AT1 receptors first appear. We measured AT1 and AT2 receptors in hearts, carotid arteries, aortae and umbilical vessels from fetal sheep aged 65-144 days (term ~150 days), and in hearts and aortae from lambs, and adult pregnant and non-pregnant ewes. Both AT1 and AT2 receptors were present in aortae of fetuses > 118 days gestation, and carotid arteries of fetuses > 121 days gestation, while in younger fetuses only AT2 receptors were found. The proportion of carotid artery and aortic AT1 receptors increased with age, while the proportion of AT2 receptors decreased. The internal umbilical artery contained both subtypes, but there was no relationship between receptor density and gestational age. The external umbilical artery had only AT1 receptors. The highest density of Ang II receptors was found in the fetal heart where the AT2 subtype predominated. The density of fetal cardiac Ang II receptors declined with age (r = -0.44, P < 0.02) due to the decrease in the AT2 subtype. The density in late gestation fetal hearts was greater than in lamb or adult hearts (P < 0.001). Our study shows that fetal systemic blood vessels contain AT1 receptors, and we have documented for the first time that the appearance of AT1 receptors is both different in different regions of the fetal cardiovascular system and is developmentally regulated. Together with the in vitro contractile studies, this suggests that Ang II can play an important role in fetal blood pressure regulation via AT1 receptors in the fetal systemic vasculature, as well in the umbilicoplacental vessels. We present the outcome of implantation in the first 100 adult patients treated under the Midland Cochlear Implant Programme. All patients were post-lingually deaf with profound or total hearing loss. Performance was tested in lip-reading, implant only and combined lip-reading and implant modes using BKB sentences, connected discourse tracking (CDT) and environmental sound recognition. Assessments were made at nine and 18 months post-implant.
The dominant aetiologies were idiopathic and meningitis. Meningitis was associated with the greatest numbers of ossified cochleas. Forty-three per cent of cases of partial ossification were identified only at surgery. Four per cent of patients became non-users of their devices, however the majority used their implants for more than 12 hours each day. The mean scores at nine months post-implant, in the implant only mode, were for environmental sound recognition 56.7 per cent, for BKB sentences 46.6 per cent (80 per cent of patients scored above 0 per cent) and for CDT 31.2 words per minute (w.p.m.) (62 per cent scored above zero per cent). In the combined lip reading and implant mode the mean scores, at nine months, were for BKB sentences 81.5 per cent and for CDT 65.8 w.p.m. All results were sustained at 18 months.
Patients reported that implantation significantly reduced their hearing handicap. Pre-operative measures of depression were also significantly reduced at nine months post-implant. Results were sustained at 18 months.
Post-operative audiological outcomes in the electrical stimulation only mode correlated significantly with length of profound deafness. Results suggest that performance outcome is also related to the number of active electrodes.
Effects of isolation on the behaviour, live-weight gain, adrenal capacity and immune responses of weaned red deer hind calves

At weaning at 3 months of age (week 1), 30 red deer hind calves were housed in six groups of five animals at a stocking density of 1·5 m2 per head and maintained in these groups for 4 weeks. At the start of week 5, all calves were immunized with ovalbumin (OVA). Fifteen calves, from three groups, selected at random, were transferred to individual pens which restricted visual and tactile contact with others (ISO) while the remaining animals were kept in their groups (GP). The behaviour, food intake, live-weight gain, antibody and lymphocyte responses in vitro to OVA and lymphocyte responses in vitro to the non-specific mitogen, concanavalin A (ConA), of all calves were assessed in each of weeks 5 to 9. Isolated calves had a lower mean live-weight gain than GP calves (P < 0·001), although there were no differences in food intake. Significantly more time was spent lying (P < 0·001) but less time feeding (P < 0·05) and self-grooming (P < 0·001) by ISO than by GP calves. There was no significant difference between ISO and GP calves in the cortisol response to an ACTH challenge test (10 i.u.) at week 11. Lymphocyte responses and antibody titres to OVA were lower in GP than in ISO calves at weeks 7 (P < 0·05) and 8 (P < 0·05), respectively. In contrast, GP calves had greater lymphocyte responses to the non-specific mitogen, ConA, in weeks 7 (P < 0·05) and 10 (P < 0·001) but not in week 9 compared with ISO calves. Differences in lymphocyte stimulation were attributed to the non-specific mitogenic nature of ConA. Factors such as agonistic interactions evident in group housing may have compromised the antibody and lymphocyte responses to OVA by GP calves but conversely the lack of social contact may have also suppressed behavioural activity in ISO calves.

The Birmingham bone anchored hearing aid team is part of the Birmingham osseointegrated programme. In the first seven years of its existence it has received 309 referrals. Twenty-six per cent had suffered a congenital conductive hearing loss and 74 per cent had an acquired conductive hearing loss; the majority secondary to chronic suppurative otitis media.
This report is of 68 out of 106 adults wearing bone anchored hearing aids (BAHAs). Ninety-eight per cent showed audiological improvement with the congenital group demonstrating marginally the best freefield thresholds and speech discrimination. Questionnaire data as to the patient experience confirms the benefits especially hearing in noise, and comfort, and the vast majority were more satisfied with the bone anchored hearing aid than their previous aid.
