Summary
Genocide is legally defined as the intent to destroy, in whole or in part, a national, ethnic, racial or religious group through targeted killings, forced displacement, sexualized violence and other forms of violence. Although the United Nations formally outlawed this crime in the wake of the Holocaust, genocides date back centuries. Genocides have also continued to occur, with upwards of 40 genocides eradicating millions of individuals since the 1940s.
Given the scope of this violence, this entry addresses the causes of genocide with an emphasis on national, subnational and individual-level risk factors. As many scholars study the crime of genocide, this entry emphasizes criminological findings but also incorporates interdisciplinary work on genocides across rural and urban settings.
National risk factors
Genocides are often, though not always, perpetrated by governments. Accordingly, much research has examined why genocide occurs by analysing the national-level conditions that have been associated with genocide during the twentieth and twenty-first centuries. These include, but are not limited to, political upheaval and threat; state structure and capacity; and ideology and social divisions.
Genocide typically occurs in contexts of political upheaval and threat. For instance, civil wars can lead to genocide. Rural, mountainous regions often foster civil wars because distance from centres of power aids rebellion. When rebellious factions in such regions (or others) initiate a civil war, genocide becomes more likely – either because political leaders attempt to remove insurgent threats or because civil wars can usher in repressive leaders via unconventional regime changes.
Successful coups and revolutions can similarly pave the way for repressive leaders, whilst unsuccessful ones threaten existing leaders, which can provoke harsh reactions. Assassinations, elections, riots and movements against the state also impact upheaval and/ or threat, and countries that previously experienced upheaval, including prior wars and atrocities, are also at higher risk of genocide.
State structure and capacity have also been tied to genocide. Autocratic regimes with few checks and balances often harm their own people, though mixed regimes (that is, those that are not democracies or autocracies) are likewise associated with genocide. Strong state capacity has been linked to several prominent genocides (for example, the Holocaust during World War Two; Rwanda in 1994), as strong governments have the power to exert their will.
Extracorporeal membrane oxygenation and paediatric palliative care in an ICU

Objectives:
Compare rates, clinical characteristics, and outcomes of paediatric palliative care consultation in children supported on extracorporeal membrane oxygenation admitted to a single-centre 16-bed cardiac or a 28-bed paediatric ICU.
Methods:Retrospective review of clinical characteristics and outcomes of children (aged 0–21 years) supported on extracorporeal membrane oxygenation between January, 2017 and December, 2019 compared by palliative care consultation.
Measurements and results:One hundred children (N = 100) were supported with extracorporeal membrane oxygenation; 19% received a palliative care consult. Compared to non-consulted children, consulted children had higher disease severity measured by higher complex chronic conditions at the end of extracorporeal membrane oxygenation hospitalisation (5 versus. 3; p < 0.001), longer hospital length of stay (92 days versus 19 days; p < 0.001), and higher use of life-sustaining therapies after decannulation (79% versus 23%; p < 0.001). Consultations occurred mainly for longitudinal psychosocial-spiritual support after patient survived device deployment with a median of 27 days after cannulation. Most children died in the ICU after withdrawal of life-sustaining therapies regardless of consultation status. Over two-thirds of the 44 deaths (84%; n = 37) occurred during extracorporeal membrane oxygenation hospitalisation.
Conclusions:Palliative care consultation was rare showing that palliative care consultation was not viewed as an acute need and only considered when the clinical course became protracted. As a result, there are missed opportunities to involve palliative care earlier and more frequently in the care of extracorporeal membrane survivors and non-survivors and their families.
4408 Using a human-centered design process to address challenges of engaging pregnant & parenting women with opioid use disorder

OBJECTIVES/GOALS: Using a human-centered approach, IDEO, a nationally-renown human-centered design team, and Research Jam, Indiana CTSI’s patient engagement core, integrated and tailored complimentary programs to address the challenges of engaging mothers with opioid misuse around the time of birth. METHODS/STUDY POPULATION: Gathered data through focus groups, site visits, and one-on-one interviews with key stakeholders: mothers in opioid use recovery, peer recovery coaches, and other people living with or directly affected by opioid use disorder (OUD). RESULTS/ANTICIPATED RESULTS: Themes emerged around stigma (e.g., constant judgment, majority of interactions focused on addiction, addiction comes from bad choices), the healthcare system (e.g., healthcare system bias and stigma, misalignment of services and timing of need, no support for support network), and relating to recovery (very variable but generally ambiguous and uncertain process and outcomes, importance of peer recovery coaches, importance of community resources). Identified themes were used to create insights that informed the underlying concepts of an engagement strategy including support and resources for recovery coaches, and education materials for mothers with OUD. One of human-centered design’s strengths is iteration, and the materials created for this have yet to be tested and refined thoroughly to be meaningful and lasting interventions. DISCUSSION/SIGNIFICANCE OF IMPACT: Considerable insights into the lived experience of those experiencing OUD and those who support these individuals yielded tangible ways to test improved engagement and recruitment of women with OUD at the time of birth.
