Summary
This chapter introduces the reader to the economic conditions and challenges of citizens who live in Russia outside metropolitan areas, regional capitals, or rich oil- and gas-producing regions. In the Soviet era, life in villages and small towns was organized around large and often powerful state-run factories and collective farms, which were responsible for providing many public services. The privatization of these enterprises left towns and villages uncertain not only about jobs, but also about who would take over the provision of basic services and duties. As the 1990s wrought havoc on local economies, Russian citizens in these smaller towns and villages sought ways to survive. They tried to rely on subsistence farming and occasional local incomes, and to find new ways to make a living as entrepreneurs or farmers. They worked without salaries to keep their jobs or took advantage of opportunities for labor migration to more prosperous places in Russia.

Summary
Key Learning Points
1. There are more adults than children with congenital heart disease (CHD).
2. Despite improvements in patient survival, adults with CHD remain at a higher risk of morbidity and death, when compared to the general population.
3. When a patient with CHD presents to the intensive care unit, determine the patient’s underlying cardiac condition and any previous intervention, as these will affect the patient’s physiology.
4. Patients with CHD may be more sensitive to the effects of critical illness. Consider the interactions between the patient’s underlying physiology and their critical care condition.
5. Consider early communication with the patient’s cardiac centre to obtain additional patient information and advice regarding management, and to discuss whether the patient requires transfer to a specialist centre.

Summary
Key Learning Points
1. These patients can present with vague symptoms or no symptoms at all.
2. It is important to establish if there is end-organ damage at an earlier stage in the course of treatment.
3. Distinguishing a hypertensive emergency from hypertensive urgencies helps in guiding acute treatment.
4. Aim to reduce the systolic blood pressure within the first hour according to the appropriate recommended level for the condition involved.
5. The choice of pharmacological agent and target blood pressure should be determined by the underlying pathology.

We present an exploration of the integrated stellar populations of early-type galaxies (ETGs) from the ATLAS3D survey. We use two approaches: firstly the application of line-indices interpreted through single stellar population (SSP) models, which provide a single value of age, metallicity and abundance ratio. And secondly, by fitting a linear combination of SSP spectra to our data, smoothly weighted in the free parameters of age and metallicity, thereby inferring a star-formation history of these galaxies. Despite the significant differences in these approaches, we obtain generally consistent results, such that galaxies that are more massive appear older with enhanced abundance ratios using line indices, and have shorter star-formation histories weighted to early times. We highlight two limitations of the index-SSP approach. Firstly the SSP-equivalent ages belie the fact that ETGs are overwhelmingly composed of ancient stars. Secondly, the young stellar contributions implied in our star formation histories are required to obtain realistic UV-optical colours. We remark that, even fitting solar-abundance models, we can recover a star-formation duration that correlates with the measured alpha-enhancement, in agreement with other recent work.
