Summary

A 35-year-old 110 kg, 180 cm male presents for a right total knee replacement. A heavy smoker at two packs per day for 30 years, his past medical history also shows chronic bronchitis and type 2 diabetes, initially managed with oral antiglycemics such as metformin. He is now insulin-dependent and hypertensive. His medications include NPH with Novolog, lisinopril-HCTZ and fluticasone/salmeterol (Advair) Diskus and montelukast (Singulair) daily with albuterol as a rescue inhaler. HR 90, BP 160/75, RR 20, temperature 36 °C. EKG shows normal sinus rhythm, with a heart rate of 75 beats/min with nonspecific ST–T wave changes in V3–V5 and Hgb 14 g/dL.
