Three adolescent and two adult patients suffering from chronic excited psychoses (either schizophrenia or schizoaffective disorder) resistant to traditional neuroleptics and clozapine were treated with combined clozapine-lithium. Improvement was assessed with the Positive and Negative Symptoms Scale, the Brief Psychiatric Rating Scale and the Clinical Global Impressions, administered before and during combined clozapine-lithium treatment. All patients demonstrated a significant improvement with this combination. There was no occurrence of agranulocytosis, neuroleptic malignant syndrome or other clinically significant adverse effects.

Our aim was to determine the effect of risperidone monotherapy treatment on disability and on the quality of life of 318 schizophrenic outpatients who had been previously treated with other neuroleptics. Patients were assessed at baseline 2, 4 and 8 months using the BPRS, CGI, WHO/DDS and SF-36. BPRS scores showed a significant decrease at month 2, both in the total score and in each of the three clusters, negative, positive and depression/anxiety. WHO/DDS scores significantly decreased both in the overall score (from 51.8 to 37.4, P < 0.0001) and in the four dimensions. SF-36 scale scores and summary measures showed a significant improvement after 8 months of risperidone treatment. Risperidone long-term treatment (8 months) has a positive effect on clinical and on psychosocial outcomes. Females, paranoids patients and patients without history of use-abuse of substances showed greater improvements in quality of life.

The main feature of observational studies is the representation of naturalistic treatment conditions. In contrast to clinical trials, they allow the evaluation and quantification of adverse event profiles of drugs under “real life” conditions. The price for this unquestionable chance is the proneness to distorting factors, which may aggravate the interpretation of the study results. Analysis of observational study results therefore has to control for potentially influential factors and reconsider possible alternatives explaining observed associations. The most important distorting factors, which should be taken into account during analysis and interpretation are under-reporting, event selection, bias, confounding and misusage? Authors and readers of such study results should be aware of this possible sources of error, in order to derive optimal benefit from this study approach.

Female drug dependants (n = 171) and controls (n = 1137) were studied to search for psychiatric morbidity in them. The psychiatric morbidity was found to be 36.3% and 6.9%, respectively. The most common psychiatric disorder found was dysthymic disorder followed by adjustment disorder, anxiety disorder and borderline personality disorder. The diagnosis was significantly dependent on the type of drug used (P < 0.001) and HIV seropositivtty status of the patients (P = 0.04). The findings highlight the relationship of the psychiatric morbidity to the HIV status and female drug users.

An inquiry among the 150 participating trainees and trainers from 24 European countries (mainly from Western European countries), at the Annual EFPT Symposium revealed that in most countries live supervision and observation are not used in training. To make matters even worse, psychiatry trainees hardly get any supervision at all. About one third of the participants never observed a senior psychiatrist in interaction with a patient during the course of their training. Half of the participants were never observed while interacting with a patient during the course of their training. The author elaborates on five of the reasons why live supervision and observation are indispensable tools for training psychiatrists. He concludes that it is unethical to permit trainees to become psychiatrists without this kind of training. For psychiatry to become a profession earning more respect from the other medical professionals and the general public, it is necessary that senior psychiatrists pay more respect to their own profession by giving trainees an adequate training in the craftsmanship of psychiatry.

An open label trial was conducted to study the efficacy of zuclopenthixol decanoate, a D1/D2 antagonist, in the treatment of chronic (> 10 years) neuroleptic-resistant aggressive schizophrenia (n = 10) and psychotic oligophrenic patients (n = 6). A significant reduction was noted in the Brief Psychiatric Rating Scale score, especially in the clusters of hostility-suspiciousness and excitement. After many years of seclusion, ten of the 16 patients were transferred to an open psychiatric department and were allowed to visit their families.
