Molecular analysis of bacterial contamination on stethoscopes in an intensive care unit

Background
Culture-based studies, which focus on individual organisms, have implicated stethoscopes as potential vectors of nosocomial bacterial transmission. However, the full bacterial communities that contaminate in-use stethoscopes have not been investigated.
MethodsWe used bacterial 16S rRNA gene deep-sequencing, analysis, and quantification to profile entire bacterial populations on stethoscopes in use in an intensive care unit (ICU), including practitioner stethoscopes, individual-use patient-room stethoscopes, and clean unused individual-use stethoscopes. Two additional sets of practitioner stethoscopes were sampled before and after cleaning using standardized or practitioner-preferred methods.
ResultsBacterial contamination levels were highest on practitioner stethoscopes, followed by patient-room stethoscopes, whereas clean stethoscopes were indistinguishable from background controls. Bacterial communities on stethoscopes were complex, and community analysis by weighted UniFrac showed that physician and patient-room stethoscopes were indistinguishable and significantly different from clean stethoscopes and background controls. Genera relevant to healthcare-associated infections (HAIs) were common on practitioner stethoscopes, among which Staphylococcus was ubiquitous and had the highest relative abundance (6.8%–14% of contaminating bacterial sequences). Other HAI-related genera were also widespread although lower in abundance. Cleaning of practitioner stethoscopes resulted in a significant reduction in bacterial contamination levels, but these levels reached those of clean stethoscopes in only a few cases with either standardized or practitioner-preferred methods, and bacterial community composition did not significantly change.
ConclusionsStethoscopes used in an ICU carry bacterial DNA reflecting complex microbial communities that include nosocomially important taxa. Commonly used cleaning practices reduce contamination but are only partially successful at modifying or eliminating these communities.

L2 AND DEAF LEARNERS’ KNOWLEDGE OF NUMERICALLY QUANTIFIED ENGLISH SENTENCES: Acquisitional Parallels at the Semantics/Discourse-Pragmatics Interface

This study assessed knowledge of numerically quantified English sentences in two learner populations—second language (L2) learners and deaf learners—whose acquisition of English occurs under conditions of restricted access to the target language input. Under the experimental test conditions, interlanguage parallels were predicted to arise from acquisitional pressures imposed by derivational economy on universal grammar (UG)–guided semantic interpretation. The results of a task in which participants matched sentences to multiple discourse depictions confirmed the predicted parallels. However, in matching underinformative sentences to depicted contexts, the L2 and deaf learner groups overactivated discourse-pragmatic knowledge. The restriction of indefinite noun phrases to singleton indefinites and the cancellation of scalar implicatures rendered sentences more informative in underinformative contexts, producing incorrect—although principled—interpretations. These results inform English acquisition at the interface of semantics and discourse pragmatics and provide further support that economy pressures yield L2 learner and deaf learner interlanguage parallels as observed, for instance, in learners’ interpretative knowledge of universally quantified English sentences.

English sentences containing the universal quantifiers each, every, and all are highly complex structures in view of the subtleties of their scope properties and resulting ambiguities. This study explored the acquisition of universal quantifier sentences as reflected in the performance of three diverse college-level student groups on a multipicture sentence interpretation task. The participant groups (hearing native speakers, deaf students, and second language learners of English) all exhibited fundamental knowledge of universal lexical, semantic, and syntactic properties that contribute to quantifier sentence interpretation. The native speakers outperformed the deaf and second language groups, whose performance was strikingly parallel. Performance patterns are explained in terms of the influences of derivational economy, including the option to restrict in situ indefinite noun phrases to singleton indefinites. The symmetry effect observed in child language studies was also observed among the college-aged participants in this study. It is explained in terms of a pragmatic challenge in managing contextual information that invokes an unexpected singleton indefinite interpretation. The results contribute to the understanding of sentence comprehension under conditions of restricted learner access to target language input and underscore the value of seeking correlates of theory-internal derivational economy in language acquisition and use.

The present study examined neuropsychological (NP) functioning and associated medical, neurological, brain magnetic resonance imaging (MRI), and psychiatric findings in 389 nondemented males infected with Human Immunodeficiency Virus-Type 1 (HIV-1), and in 111 uninfected controls. Using a comprehensive NP test battery, we found increased rates of impairment at each successive stage of HIV infection. HIV-related NP impairment was generally mild, especially in the medically asymptomatic stage of infection, and most often affected attention, speed of information processing, and learning efficiency; this pattern is consistent with earliest involvement of subcortical or frontostriatal brain systems. NP impairment could not be explained on the bases of mood disturbance, recreational drug or alcohol use, or constitutional symptoms; by contrast, impairment in HIV-infected subjects was related to central brain atrophy on MRI, as well as to evidence of cellular immune activation and neurological abnormalities linked to the central nervous system.
