Outcomes of Pre-Existing Diabetes in People With/without New Onset Severe Mental Illness: A Primary-Secondary Mental Healthcare Linkage in South London, United Kingdom

Aims
To compare people with diabetes developing severe mental illness (SMI) to those with diabetes alone with respect to risk status, diabetes care receipt, and diabetes-relevant outcomes in primary care.
MethodsData from mental health care (Clinical Record Interactive Search; CRIS) linked to primary care (Lambeth DataNet; LDN) were used. From patients with a type 2 diabetes mellitus (T2DM) diagnosis in primary care, those with a new SMI diagnosis were matched (by age, gender, and practice) with up to five randomly selected controls. Mixed models were used to estimate associations with trajectories of recorded HbA1c levels; Poisson regression models compared total and cardiovascular comorbidity levels and number of diabetes complications; linear regression models compared BMI and total cholesterol levels; conditional logistic regression models investigated microalbuminuria, receipt of a foot or retinal examination, use of statins and receipt of insulin; Cox proportional hazards were used to model incident microvascular and macrovascular events, foot morbidity and mortality.
ResultsIn a cohort of 693 cases with SMI (122 bipolar disorder, 571 schizophrenia and related) and T2DM compared to 3366 controls, all-cause mortality was increased substantially in the cohort with SMI (adjusted hazard ratio 4.52, 95% CI 3.73–5.47; for bipolar 5.59, 3.37–9.28; for schizophrenia 4.42, 3.60–5.44). However, for all the other outcome comparisons, the only significant findings were of reduced foot examination (adjusted odds ratio 0.75, 0.54–0.98) and reduced retinal screening (0.77, 0.61–0.96).
ConclusionHigher mortality suggests increased risk of adverse outcomes for people with pre-existing T2DM who develop SMI, and reduced foot/retinal examinations suggest disadvantaged healthcare receipt. However, other potential explanations for the mortality difference could not be identified from the outcomes analysed, so further investigation is needed into underlying causal pathways.

Introduction
Hemorrhage remains the major cause of preventable death after trauma. Recent data suggest that earlier blood product administration may improve outcomes. The purpose of this study was to determine whether opportunities exist for blood product transfusion by ground Emergency Medical Services (EMS).
MethodsThis was a single EMS agency retrospective study of ground and helicopter responses from January 1, 2011 through December 31, 2015 for adult trauma patients transported from the scene of injury who met predetermined hemodynamic (HD) parameters for potential transfusion (heart rate [HR]≥120 and/or systolic blood pressure [SBP]≤90).
ResultsA total of 7,900 scene trauma ground transports occurred during the study period. Of 420 patients meeting HD criteria for transfusion, 53 (12.6%) had a significant mechanism of injury (MOI). Outcome data were available for 51 patients; 17 received blood products during their emergency department (ED) resuscitation. The percentage of patients receiving blood products based upon HD criteria ranged from 1.0% (HR) to 5.9% (SBP) to 38.1% (HR+SBP). In all, 74 Helicopter EMS (HEMS) transports met HD criteria for blood transfusion, of which, 28 patients received prehospital blood transfusion. Statistically significant total patient care time differences were noted for both the HR and the SBP cohorts, with HEMS having longer time intervals; no statistically significant difference in mean total patient care time was noted in the HR+SBP cohort.
ConclusionsIn this study population, HD parameters alone did not predict need for ED blood product administration. Despite longer transport times, only one-third of HEMS patients meeting HD criteria for blood administration received prehospital transfusion. While one-third of ground Advanced Life Support (ALS) transport patients manifesting HD compromise received blood products in the ED, this represented 0.2% of total trauma transports over the study period. Given complex logistical issues involved in prehospital blood product administration, opportunities for ground administration appear limited within the described system.

Summary
INTRODUCTION
Professor Judith Gardam's distinguished record of scholarship speaks for itself. What is less well known is Judith's record as an inspiring teacher and colleague. To those of us who were fortunate to be her students, Judith conveyed not only her enthusiasm for, and commitment to, international law, but also her intellectual discipline. No student of Judith's could forget her admonitions on the importance to scholars of international law of striving to truly understand its sources. As a colleague, I found supervising doctoral candidates with Judith a pleasure because of her interest in their projects and her dedication to mentoring the next generation of scholars. The humanity that pervades Judith's scholarship and teaching is all the more powerful because of the rigorous and disciplined approach to the law which is her hallmark.
The limits of the powers of the Security Council have been the subject of considerable debate since the rise of the Security Council which accompanied the end of the Cold War. Notwithstanding the absence of any organ competent to engage in a review of the Council's decisions, and irrespective of the political character of many of its most important functions, the Council's powers are nonetheless not unlimited. Indeed, the recent practice to be examined in this chapter has demonstrated that its powers are subject to important collateral limits arising from the human rights obligations of states.
The response of international law to the rise of the Security Council was a topic addressed by Judith Gardam in her 1996 article in the Michigan Journal of International Law entitled ‘Legal Restraints on Security Council Military Enforcement Action’. Here, Gardam argued that, when undertaking military enforcement actions, the Security Council was bound by requirements of proportionality and necessity analogous to those applicable in the jus ad bellum, and also by the rules of international humanitarian law forming the jus in bello. This chapter looks not to uses of force, but to sanctions imposed by the Security Council. Nonetheless, two of Gardam's concluding observations make for a suitable starting point for this analysis. First, she noted the legal character of the Charter:
The text of the Charter, a legal document, is not only compatible with but arguably, through its emphasis on human rights and humanitarian values, requires the Security Council to measure its responses against legal criteria.
