Glyphosate- and Acetolactate Synthase Inhibitor–Resistant Kochia (Kochia scoparia) in Western Canada

In summer, 2011, we investigated suspected glyphosate-resistant (GR) kochia in three chem-fallow fields (designated F1, F2, F3, each farmed by a different grower) in southern Alberta. This study characterizes glyphosate resistance in those populations, based on data from dose–response experiments. In a greenhouse experiment, the three populations exhibited a resistance factor ranging from 4 to 6 based on shoot biomass response (GR50 ratios), or 5 to 7 based on survival response (LD50 ratios). Similar results were found in a field dose–response experiment at Lethbridge, AB, in spring 2012 using the F2 kochia population. In fall 2011, we surveyed 46 fields within a 20-km radius of the three chem-fallow fields for GR kochia. In the greenhouse, populations were screened with glyphosate at 900 g ae ha−1. Seven populations were confirmed as GR, the farthest site located about 13 km from the three originally confirmed populations. An additional GR population more than 100 km away was later confirmed. Populations were screened for acetolactate synthase (ALS)–inhibitor (thifensulfuron : tribenuron) and dicamba resistance in the greenhouse, with molecular characterization of ALS-inhibitor resistance in the F1, F2, and F3 populations. All GR populations were resistant to the ALS-inhibiting herbicide, but susceptible to dicamba. ALS-inhibitor resistance in kochia was conferred by Pro197, Asp376, or Trp574 amino acid substitutions. Based upon a simple empirical model with a parameter for selection pressure, calculated from weed relative abundance and glyphosate efficacy, and a parameter for seedbank longevity, kochia, wild oat, and green foxtail were the top three weeds, respectively, predicted at risk of selection for glyphosate resistance in the semiarid Grassland region of the Canadian prairies; wild oat, green foxtail, and cleavers species were predicted at greatest risk in the subhumid Parkland region. This study confirms the first occurrence of a GR weed in western Canada. Future research on GR kochia will include monitoring, biology and ecology, fitness, mechanism of resistance, and best management practices.

Summary
Nearly 600 000 women undergo hysterectomy each year in the USA, and more than one-fourth of US women will have a hysterectomy – the second most frequent surgical procedure among reproductive-aged women – by the time they are 60 years old. The average annual rate of hysterectomy per 1000 women aged 15 years and older declined from 7.1 in 1980 to 6.6 in 1987, then holding at 5.5 from 1988 to 1993; the decline from 1987 to 1988 resulted from changes in the data collection used to define the survey. Conditions most often associated with hysterectomy are uterine leiomyomata, endometriosis, and pelvic organ prolapse.
The comprehensive study on hysterectomy published by Wingo and associates from the Centers for Disease Control and Prevention reported that there were 46 deaths among 119 972 women undergoing vaginal hysterectomy. The vaginal approach to hysterectomy was associated with a much lower mortality rate than the abdominal approach. Excluding pregnancy and cancer-related cases, the mortality rate for abdominal hysterectomy was 8.6 per 10 000 women while that for vaginal hysterectomy was 2.7 per 10 000 women. Therefore, hysterectomy should be considered a low-risk operation that can be performed to treat non-pregnant patients and those with benign gynecologic symptoms or disease. Vaginal hysterectomy can be used for many indications, including pelvic relaxation, cervical intraepithelial neoplasias, small leiomyoma, and recurrent dysfunctional uterine bleeding.
The success of a vaginal hysterectomy is directly related to the particular surgeon's experience.

Summary
As the second most frequently performed gynecologic operation, uterine curettage is used for diagnostic and therapeutic considerations and for the following indications:
Polymenorrhea: menstrual cycle interval less than 21 days.
Oligomenorrhea: menstrual cycle interval more than 37 days.
Metrorrhagia: menstrual bleeding longer than 7 days, or interval bleeding.
Menorrhagia: excessive or prolonged menstrual bleeding.
Postmenopausal bleeding or uterine bleeding occurring more than 12 months after the last menstrual period in a menopausal woman.
Breakthrough bleeding or intermenstrual bleeding in a menstrual cycle that is the result of exogenous hormones.
Dysfunctional uterine bleeding: characterized by any abnormal uterine bleeding in the absence of pregnancy, neoplasm, infection, or uterine lesion.
Spontaneous abortion, fetal death in utero, septic abortion, legal termination of pregnancy, dilation and evacuation of gestational trophoblastic neoplasias, incomplete abortion, or inevitable abortion.
The most common of the above complications are incomplete abortion, postmenopausal bleeding, and dysfunctional uterine bleeding. Curettage of the uterus responds to these conditions by removing endometrial or endocervical tissue for histologic study and evacuating products of conception.
It is extremely important that dilation and curettage be performed correctly for the proper indications and with minimal morbidity, as serious complications and even death may result from poor and inappropriate application. While regional and local anesthesia may be used, general anesthesia is usually administered to allow more abdominal relaxation for optimal bimanual examination of the pelvic viscera. The operative time for dilation and curettage is less than 15 minutes. Transfusion is rarely indicated unless significant preoperative hemorrhage has occurred, usually associated with pregnancy.

Hysterectomy is the most common gynecologic operation and the second most common major surgical procedure in the USA. The frequency of abdominal hysterectomy is 1 per 1000 in women younger than 25 years, but rises to 16 per 1000 in women older than 35 years. After the age of 35 years, women usually have completed their childbearing and also have a higher incidence of significant gynecologic disease. The mortality risk for hysterectomy in the USA has been studied by the Centers for Disease Control and Prevention, which reported 477 deaths among 317 389 women having abdominal hysterectomies from 1979 through 1980. The mortality rate for hysterectomy was higher for procedures associated with pregnancy and cancer than for procedures not associated with these conditions (29.2, 37.8, and 6.0 per 10 000 hysterectomies, respectively). Abdominal hysterectomy had a higher mortality rate than did vaginal hysterectomy. Excluding cases related to pregnancy or cancer, the mortality rate was 8.6 per 10 000 women for abdominal hysterectomy and 2.7 per 10 000 women for vaginal hysterectomy.
Simple total abdominal hysterectomy involves the removal of the uterine corpus and cervix through an abdominal incision and is performed for a variety of indications, including uterine leiomyomas, pelvic abscesses, endometriosis, and recurrent dysfunctional uterine bleeding. In addition, simple abdominal hysterectomy is performed for two malignant indications: adenocarcinoma of the endometrium and ovarian cancer. Invasive cancer of the cervix requires a radical abdominal hysterectomy.
