Gestión de casos: una revisión crítica de las publicaciones de evolución

Resumen

La gestióin de casos ha sido una parte integral de la práctica psiquiátrica en los Estados Unidos durante más de una década y ha generado una gran cuerpo de publicaciones. La aplicación de los principios de gestión de casos a la asistencia a personas que sufren trastornos psiquiátricos se está haciendo cada vez más popular en el Reino Unido y Europa, y actualmente están comenzando a aparecer publicaciones. Sin embargo, no se han hecho afirmaciones definitivas sobre la eficacia de la gestión de casos debido a diversos problemas conceptuales y metodológicos. El presente artículo es una revisión crítica de las publicaciones de evolución de gestión de casos. Se revisa la evolución comunicada en el contexto del diseño del estudio y las características del servicio. Los autores concluyen que la práctica de la gestión de casos puede tener al menos algunas consecuencias en el uso de los servicios por parte de los pacientes (incluida una notable disminución en los días de estancia en el hospital), la satisfacción con los servicios, el compromiso con ellos, y las redes y relaciones sociales cuando la gestión de casos se libra como un servicio clínico directo con una proporción de personahpacientes alta. Se propone un conjunto de recomendaciones para la práctica futura y la presentación de investigaciones en gestión de casos.
