Summary

A 76-year-old man presents to the ambulatory surgery center for cataract extraction. The surgeon informs you that the procedure will be performed under retrobulbar block. Vital signs at the beginning of the case are heart rate (HR) 76, respiratory rate (RR) 15, and blood pressure (BP) 156/76 mm Hg. Sedation is started with a remifentanil 40 μg bolus IV. Shortly after the block is performed, the patient is nonresponsive. What is your differential diagnosis? What are your next steps?
