A potential crisis in human fertility is brewing. As societies become more affluent, they experience changes that have a dramatic impact on reproduction. As average family sizes fall, the selection pressure for high-fertility genes decreases; exacerbated by the IVF industry which allows infertility-linked genes to pass into the next generation. Male fertility rates are low, for many reasons including genetics and exposure to environmental toxins. So, a perfect storm of factors is contriving to drive fertility rates down at unprecedented rates. If we do not recognize the reality of our situation and react accordingly, an uncontrollable decline in population numbers is likely, which we'll be unable to reverse. This book will address, in a unique and multi-faceted way, how the consequences of modern life affects fertility, so that we can consider behavioural, social, medical and environmental changes which could reduce the severity of what is about to come.
Conventional theories of ethnic politics argue that political entrepreneurs form ethnic parties where there is ethnic diversity. Yet empirical research finds that diversity is a weak predictor for the success of ethnic parties. When does ethnicity become a major element of party competition? Scholars have explained the emergence of an ethnic dimension in party systems as the result of institutions, mass organizations, and elite initiatives. But these factors can evolve in response to an emerging ethnic coalition of voters. The author advances a new theory: ethnic cleavages emerge when voters seek to form a parliamentary opposition to government policies that create grievances along ethnic identities. The theory is tested on rare cases of government policies in Prussia between 1848 and 1874 that aggrieved Catholics but were not based on existing policies or initiated by entrepreneurs to encourage ethnic competition. Using process tracing, case comparisons, and statistical analysis of electoral returns, the author shows that Catholics voted together when aggrieved by policies, regardless of the actions of political entrepreneurs. In contrast, when policies were neutral to Catholics, the Catholic party dissolved.
The “historical alternatives” approach calls for research into the role of national institutions and public policies in the resilience or decline of industrial districts. Policies in support of small and medium-sized enterprises (SMEs) were launched in various Western economies in the second half of the twentieth century. This article focuses on the paradigmatic Italian case and investigates the importance of government subsidies for SMEs on firms located in a southern and a northeastern district, between 1971 and 1991. This discussion deepens our understanding of the role of national policies in the reemergence of industrial districts in the decades of the Second Industrial Divide. It also indicates the importance of firms’ utilization of subsidies and their ecosystem as complementary to the policy's effectiveness.
To prioritise policy actions for government to improve the food environment and contribute to reduced obesity and related diseases.
Design
Cross-sectional study applying the Food Environment Policy Index (Food EPI) in two stages. First, the evidence on all relevant policies was compiled, through an Internet search of government documents, and reviewed for accuracy and completeness by government officials. Second, independent experts were brought together to identify critical gaps and prioritise actions to fill those gaps, through a two-stage rating process.

A total of seventy-three independent experts from forty-one organisations were involved in the exercise.
Results
The top priority policy actions for government identified were: (i) control the advertising of unhealthy foods to children; (ii) implement the levy on sugary drinks; (iii) reduce the sugar, fat and salt content in processed foods (leading to an energy reduction); (iv) monitor school and nursery food standards; (v) prioritise health and the environment in the 25-year Food and Farming Plan; (vi) adopt a national food action plan; (vii) monitor the food environment; (viii) apply buying standards to all public institutions; (ix) strengthen planning laws to discourage less healthy food offers; and (x) evaluate food-related programmes and policies.
Conclusions
Applying the Food EPI resulted in agreement on the ten priority actions required to improve the food environment. The Food EPI has proved to be a useful tool in developing consensus for action to address the obesity epidemic among a broad group of experts in a complex legislative environment.
This article analyses the gang policies of the first years of the Funes administration in El Salvador, from June 2009 until July 2012. Using securitisation theory, it explains why the administration returned to an emphasis on extraordinary measures, most of them repressive, to deal with gangs. It argues that these measures were the product of an ongoing and dynamic process in which the government was but one of the players in a complex field constituted by numerous actors. The return to repressive measures as well as the support and facilitation of a ‘gang truce’ were not the result of a rational design or a predetermined agenda, but should be seen as a series of moves in a political conjuncture, in which the Salvadorean government needed to communicate to different audiences messages of being in control.
This chapter explores the interaction between technological innovation and the global spread of capitalism from 1848 to 2005. It then examines the interaction of technological innovation, the global spread of capitalism, and the varied ability of nations to "catch up" with the technological and economic leaders in the world economy during this period of more than 150 years. Technology is the integration of knowledge, organization, and technique, directed towards material transformations. The importance of incremental innovation underscores the complex relationship between the appearance of a new technology and its adoption. Innovation in transportation and communications technologies was essential to the growth of domestic and international commerce during the period between 1870 and 1914. The rise of industrial capitalism also changed the process of innovation itself. Government policies exerted a direct and indirect influence on innovation in the post-1850 world. Innovation was spurred by the growth of capitalism, but technology had powerful effects on the global spread of capitalism.
Much of the work of a clinical director carries a large element of common sense and much of this chapter is familiar territory to the experienced clinical director. The views of many groups and individuals need to be considered when drawing up a local maternity services strategy. Broadly, they can be categorised into: the patients or users of the service, the commissioners, policy makers and regulatory bodies, and the professionals who deliver the service. Women who use maternity services form the most important interest group. All maternity services strategies will have to take into account government policies, standards set by professional bodies and limits set by regulators. The third element of strategy development is to obtain the views of all staff groups involved in delivering the service. The most valuable component of a risk management system is incident reporting and it remains so during the implementation of strategic change.
