    Summary

    Since the publication of the first edition of this book more than two decades ago, the field of neuromodulation has undergone remarkable transformation. Deep brain stimulation (DBS) is now a firmly established treatment for movement disorders, and an increasing body of evidence supports DBS in the treatment of other neurological and psychiatric disorders. When DBS first arrived on the scene, the prospect of using a fully implantable neurostimulation system to deliver targeted, adjustable, nondestructive, and reversible brain modulation was paradigm shifting.

    Summary

    This chapter details factors that should be considered when evaluating each patient's candidacy for deep brain stimulation (DBS). A number of factors need to be assessed in a systematic matter to determine each patient's candidacy for treatment with DBS. Parkinson's disease (PD) patients referred for surgery are at greater risk for psychotic symptoms, as they usually have relatively advanced disease. When considering a patient with essential tremor (ET) for DBS, it is important to make sure the patient meets clinical criteria for the diagnosis of ET as a first step. Determining which dystonia patients are candidates for DBS can be challenging than making this determination for patients with PD or ET. An effective method for arriving at decisions regarding the candidacy of patients for treatment with DBS is to collect the required data for each patient and then convene a conference. These details are discussed by the interdisciplinary/multidisciplinary team.

    The issue of progressive cognitive decline in patients with schizophrenia has been debated. We performed a cross-sectional study of patients with chronic schizophrenia, aged from 18 to 69 years, in order to address this issue. The patients included in this study passed a rigorous screen for any comorbid condition with an adverse impact on central nervous system function. We assessed intellectual deterioration with a battery of neuropsychological tests known to be sensitive to cognitive impairment in progressive dementia. No evidence of accelerated intellectual decline was found. No significant differences were found between the five age-derived cohorts (18–29, 30–39, 40–49, 50–59, and 60–69 years of age) on the Mini-Mental State Examination, Dementia Rating Scale, or other tests sensitive to dementia. While performance on the Boston Naming Test significantly declined with age, this was mainly due to age rather than duration of illness. However, it is important to note that mean performances on the majority of the tests were abnormal across all cohorts studied. These results suggest that intellectual function does not markedly decline during the adulthood of patients with schizophrenia. The course of schizophrenia is more consistent with a static encephalopathy than a dementing disorder.
