The learning hospital is distinguished by ceaseless evolution of erudition, enhancement, and implementation of clinical best practices. We describe a model for the learning hospital within the framework of a hospital infection prevention program and argue that a critical assessment of safety practices is possible without significant grant funding. We reviewed 121 peer-reviewed manuscripts published by the VCU Hospital Infection Prevention Program over 16 years. Publications included quasi-experimental studies, observational studies, surveys, interrupted time series analyses, and editorials. We summarized the articles based on their infection prevention focus, and we provide a brief summary of the findings. We also summarized the involvement of nonfaculty learners in these manuscripts as well as the contributions of grant funding. Despite the absence of significant grant funding, infection prevention programs can critically assess safety strategies under the learning hospital framework by leveraging a diverse collaboration of motivated nonfaculty learners. This model is a valuable adjunct to traditional grant-funded efforts in infection prevention science and is part of a successful horizontal infection control program.

In species that aggregate for reproduction, the social and fitness costs of movement between groups frequently lead to restricted exchange between breeding areas. We report on four individual humpback whales identified in both the Cape Verde Islands and Guadeloupe; locations separated by an ocean basin and >4000 km. This rate of exchange is rarely encountered between such geographically discrete breeding areas. Two individuals returned to the area where they were originally identified. In contrast, no individuals from the Cape Verde Islands were resighted to the much larger sample from the Dominican Republic, though the migratory distances from the feeding areas are comparable between these areas. The social factors driving the stark difference between groups that is observed here are not clear. Effective conservation requires an understanding of the extent and pattern of movement between population units. The findings presented here suggest that there may well be more than one behaviourally distinct group within the West Indies. More broadly, they argue that considerable caution is warranted in assumptions made regarding the number, boundaries and status of population units based solely on spatial separation or proximity.

Introduction: Individuals who have deep and periventricular white matter hyperintensities may have a higher risk for suicidal behavior. There are mixed results in the literature regarding whether unipolar or bipolar patients who have attempted suicide have more deep white matter hyperintensities (DWMH) or periventricular hyperintensities (PVH) relative to those who have no history of suicide attempts.

Methods: A meta-analysis of studies examining white matter hyperintensities (WMH) in mood disorder patients with and without a history of suicide attempts was performed.

Results: Four studies, including a total of 173 patients who attempted suicide and 183 who did not attempt suicide, were included. A significantly higher number of attempters were found to have WMH than non-attempters. Unipolar depressed patients who had attempted suicide had 1.9 times more DWMH and 2.1 times more PVH than those who did not. Bipolar patients who had attempted suicide had 5.4 times more PVH than those who had not. Taken together, unipolar and bipolar patients who had attempted suicide had 2.8 times more DWMH and 4.5 times more PVH than those who had never attempted suicide.

The present study examined attentional disruption in the presence of negative automatic thoughts specific to panic and social anxiety. Participants with panic disorder (n = 18), participants with social phobia (n = 19), and nonanxious participants (n = 19) completed a dichotic listening task in which they shadowed ambiguous passages heard in their dominant ear while ignoring lists of panic-related, social anxiety-related, or control automatic thoughts that were presented in their non-dominant ear. In addition, they completed a simultaneous simple reaction time task. Both patient groups made more shadowing distortions than the nonanxious group. Panic participants committed more shadowing omissions in trials in which they heard panic-related automatic thoughts as compared to trials in which they heard other automatic thoughts. Results suggest that both patient groups experienced some disruption on this demanding task but that only panic patients exhibited an anxiety-specific attentional bias.

The present study was designed to test the hypothesis that blood fearful and nonfearful individuals would be differentiated by their pattern of recall of schematic and non-schematic prose material. Blood fearful (n=36) and nonfearful (n=40) individuals were presented with five prose passages describing harm or injury. Nine sentences in each passage described events representative of the schema content of most individuals, whereas six sentences in each passage described events that are not representative of typical schema content. Participants read passages that either described themselves experiencing the situations or described themselves witnessing others in the situations and completed a free recall task in both immediate (i.e. 2 minutes) and delayed (i.e. one week) recall conditions. Results did not replicate studies from the cognitive psychology literature on which this study was based. Instead, participants who read passages describing themselves in these situations recalled the gist of the material more accurately than participants who read passages in which they witnessed another individual in the situation. Blood fearful participants recalled the affective tone of neutral passages less accurately than nonfearful participants. Results suggest that activation of schema content has little influence on memory performance, although assessing the affective tone of recalled material detected subtle differences in memory performance between fearful and nonfearful groups.

The present study applied the Deese-Roediger-McDermott false memory paradigm to examine whether anxious and fearful individuals exhibit higher recall and recognition rates of never presented threat words than nonanxious individuals. In Study 1, 39 spider fearful individuals, 28 blood fearful individuals, and 41 nonfearful individuals learned four word lists associated with unpresented target words: “spider”, “blood”, “river”, and “music”. Regardless of whether participants completed only a recognition task or a recall task and then a recognition task, there were no differences as a function of group in the degree to which they falsely remembered unpresented target threat words. In Study 2, 48 socially anxious and 51 nonanxious individuals learned four lists associated with social/evaluative threat unpresented target words and four lists associated with neutral unpresented target words. Similar to the findings from Study 1, groups did not differ in the degree to which they falsely remembered target words. These findings add to an increasingly large literature suggesting that anxious individuals are not characterized by a memory bias toward threat.

The present study examined characteristics of autobiographical memories retrieved by individuals with social phobia (n=15) and nonanxious individuals (n=17). Participants were presented with social threat, positive, and neutral cue words and instructed to retrieve the first specific personal memory that came to mind. Memories were coded for retrieval latency, overgenerality (vs. specificity), and affective tone. Nonanxious individuals, rather than social phobic individuals, demonstrated a bias toward the retrieval of specific negative memories when cued by social threat words. Significant omnibus results were attenuated when self-reported depression was included as a covariate in analyses. These results suggest that memories cued by social threat words are particularly salient for nonanxious individuals but not for individuals with social phobia and that depressive symptoms must be accounted for in studies examining cognitive biases toward threat.

The present study examined the recall of material representative and non representative of schemata for social and evaluative situations. Socially anxious (n = 24) and nonanxious (n = 25) individuals were presented with three positively valenced and three negatively valenced prose passages describing common social and evaluative scenarios. Eight of the sentences in each passage described events representative of the schema content of most individuals, whereas three of the sentences in each passage described events that are not representative of typical schema content. Participants completed a free recall task in both immediate (i.e. 2 minutes) and delayed (i.e. one week) recall conditions. Although there were no group differences as a function of type of content (i.e. schematic, non-schematic), socially anxious individuals were less likely than nonanxious individuals to accurately recall the gist of passages containing negative information in the immediate recall condition. In all, this study provided little evidence for the influence of maladaptive schema content on memory for threatening material in anxious individuals, but it added to an increasingly large literature suggesting that some types of anxiety are associated with an avoidance of processing emotional material.
