We applied Narrative Exposure Treatment (NET) to a severely traumatized Kosovar refugee living in a Macedonian refugee camp during the Balkan War. NET is a pragmatic short-term approach that integrates effective therapeutic components deriving from Cognitive Behaviour Therapy and Testimony Therapy. Outcome was evaluated by clinical examination and the Posttraumatic Stress Diagnostic Scale (PDS). Three sessions of NET were enough to afford considerable relief, although some PTSD symptoms remained. Our experience indicates that Narrative Exposure is a promising and realistic approach for the treatment of even severely traumatized refugees living in camps. In addition, it can prove valid testimonies about human rights violations without humiliating the witness.

ABSTRACT
This chapter considers how the P50, Nl, and P300 components of the auditory event-related brain potential (ERP) are related to startle. Although both blink and ERPs can be elicited by startling noises, blinks are muscular and ERPs are neural. Comparing startle blinks and ERPs reveals at what point the neural processes supporting them diverge. Each ERP component is evaluated against variables known to affect startle-stimulus intensity, stimulus rise time, habituation, and leading stimuli presented at both short and long lead intervals. ERPs recorded in startle modification paradigms suggest that the relationship between startle blinks and ERPs is weak. P50, Nl, and P300 are all inhibited by short lead intervals, but with different time courses from startle; only the fronto-central P300 is facilitated with long lead intervals.
Introduction
Psychophysiologists tend to specialize. Those who study event-related brain potentials (ERPs) usually do not study startle, perhaps because startle produces tremendous blinks and muscle contraction “artifacts” on the scalp, often much larger than ERPs. Yet startling stimuli elicit ERPs, and by comparing blinks and ERPs elicited by startling stimuli, we will learn about the dependence and interdependence of the neural pathways that support these two responses.
Developments Encouraging Study of Startle with ERPs
Two developments have encouraged looking at ERPs in conjunction with startle. First, parallels have been observed between startle modification and suppression of P50; schizophrenics have been found to differ from normals in both startle inhibition by lead stimuli at short lead intervals and P50 suppression (Chapter 11).

We evaluated the feasibility of recording multiple physiological anxiety measures during a flight and how well they could distinguish flight phobics from controls. Benefits of baseline adjustment and transformation for all variables and adjustment of heart rate by ventilation to give additional heart rate were calculated. Effect size, one measure of the power to discriminate groups, was between 1.1 and 1.7 for heart rate measures. Although respiratory rate and minute ventilation, indicators of hyperventilation, did not differ between groups, phobics paused more during inspiration than did controls. Phobics also showed more skin conductance fluctuations and less respiratory sinus arrhythmia. Self-reported anxiety was a more powerful discriminator than physiological measures, a result that may be partially explained by how phobics were selected. These results indicate that monitoring of multiple physiological systems outside the laboratory is practical and informative. Physiological measures of psychological importance can be quantified accurately in a noisy, changing, unsupervised ambulatory setting.
