Reintervention and mortality risk after total anomalous pulmonary venous connection repair

Background:
Management of total anomalous pulmonary venous connections has been extensively studied to further improve outcomes. Our institution previously reported factors associated with mortality, recurrent obstruction, and reintervention. The study purpose was to revisit the cohort of patients and evaluate factors associated with reintervention, and mortality in early and late follow-up.
Methods:A retrospective review at our institution identified 81 patients undergoing total anomalous pulmonary venous connection repair from January 2002 to January 2018. Demographic and operative variables were evaluated. Anastomotic reintervention (interventional or surgical) and/or mortality were primary endpoints.
Results:Eighty-one patients met the study criteria. Follow-up ranged from 0 to 6,291 days (17.2 years), a mean of 1263 days (3.5 years). Surgical mortality was 16.1% and reintervention rates were 19.8%. In re-interventions performed, 80% occurred within 1.2 years, while 94% of mortalities were within 4.1 months. Increasing cardiopulmonary bypass times (p = 0.0001) and the presence of obstruction at the time of surgery (p = 0.025) were predictors of mortality, while intracardiac total anomalous pulmonary venous connection type (p = 0.033) was protective. Risk of reintervention was higher with increasing cardiopulmonary bypass times (p = 0.015), single ventricle anatomy (p = 0.02), and a post-repair gradient >2 mmHg on transesophageal echocardiogram (p = 0.009).
Conclusions:Evaluation of a larger cohort with longer follow-up demonstrated the relationship of anatomic complexity and symptoms at presentation to increased mortality risk after total anomalous pulmonary venous connection repair. The presence of a single ventricle or a post-operative confluence gradient >2 mmHg were risk factors for reintervention. These findings support those found in our initial study.

Good education requires student experiences that deliver lessons about practice as well as theory and that encourage students to work for the public good—especially in the operation of democratic institutions (Dewey 1923; Dewy 1938). We report on an evaluation of the pedagogical value of a research project involving 23 colleges and universities across the country. Faculty trained and supervised students who observed polling places in the 2016 General Election. Our findings indicate that this was a valuable learning experience in both the short and long terms. Students found their experiences to be valuable and reported learning generally and specifically related to course material. Postelection, they also felt more knowledgeable about election science topics, voting behavior, and research methods. Students reported interest in participating in similar research in the future, would recommend other students to do so, and expressed interest in more learning and research about the topics central to their experience. Our results suggest that participants appreciated the importance of elections and their study. Collectively, the participating students are engaged and efficacious—essential qualities of citizens in a democracy.

Objectives
To determine if changes to the Ontario Highway Traffic Act (OHTA) in 2009 and 2010 had an effect on the proportion of alcohol-related motor vehicle collisions (MVCs) presenting to a trauma centre over a 10-year period.
MethodsA retrospective review of the trauma registry at a Level I trauma centre in southwestern Ontario was undertaken. The trauma registry is a database of all trauma patients with an injury severity score (ISS) ≥12 and/or who had trauma team activation. Descriptive statistics were calculated. Interrupted time series analyses with ARIMA modeling were performed on quarterly data from 2004-2013.
ResultsA total of 377 drivers with a detectable serum ethanol concentration (SEC) were treated at our trauma centre over the 10-year period, representing 21% of all MVCs. The majority (330; 88%) were male. The median age was 31 years, median SEC was 35.3 mmol/L, and median ISS was 21. A total of 29 (7.7%) drinking drivers died from their injuries after arriving to hospital. There was no change in the proportion of drinking drivers after the 2009 amendment, but there was a significant decline in the average SEC of drinking drivers after changes to the law. There was no difference in the proportion of drinking drivers ≤21 years after introduction of the 2010 amendment for young and novice drivers.
ConclusionsThere was a significance decline in the average SEC of all drinking drivers after the 2009 OHTA amendment, suggesting that legislative amendments may have an impact on drinking before driving behaviour.

Summary
Executive summary
All explosive volcanic eruptions generate volcanic ash, fragments of rock that are produced when magma or vent material is explosively disintegrated. Volcanic ash is then convected upwards within the eruption column and carried downwind, falling out of suspension and potentially affecting communities across hundreds, or even thousands, of square kilometres. Ash is the most frequent, and often widespread, volcanic hazard and is produced by all explosive volcanic eruptions. Although ash falls rarely endanger human life directly, threats to public health and disruption to critical infrastructure services, aviation and primary production can lead to potentially substantial societal impacts and costs, even at thicknesses of only a few millimetres. Communities exposed to any magnitude of ash fall commonly report anxiety about the health impacts of inhaling or ingesting ash (as well as impacts to animals and property damage), which may lead to temporary socio-economic disruption (e.g. evacuation, school and business closures, cancellations). The impacts of any ash fall can therefore be experienced across large areas and can also be long-lived, both because eruptions can last weeks, months or even years and because ash may be remobilised and re-deposited by wind, traffic or human activities.
Given the potentially large geographic dispersal of volcanic ash, and the substantial impacts that even thin (a few mm in thickness) deposits can have for society, this chapter elaborates upon the ash component of the overviews provided in Chapters 1 and 2. We focus on the hazard and associated impacts of ash falls; however, the areas affected by volcanic ash are potentially much larger than those affected by ash falling to the ground, as fine particles can remain aloft for extended periods of time. For example, large portions of European airspace were closed for upto five weeks during the eruption of Eyjafjallajökull, Iceland, in 2010 because of airborne ash (with negligible associated ash falls outside of Iceland). The distance and area over which volcanic ash is dispersed is strongly controlled by wind conditions with distance and altitude from the vent, but also by the size, shape and density of the ash particles, and the style and magnitude of the eruption. These factors mean that ash falls are typically deposited in the direction of prevailing winds during the eruption and thin with distance. Forecasting ash dispersion and the deposition ‘footprint' is typically achieved through numerical simulation.
