Summary
What connections can there be between bombs and roses, mines and hampers full of asparagus, between beautifying a grubby convent and extolling the Zeebrugge Raid? The answer is that Helen Beale, a thirty-two-year-old Women’s Royal Naval Service (WRNS) officer in Dover, revealed how these elements counter-intuitively co-existed in World War I Kent. There was heroic male masculinity in dangerous naval operations at sea alongside what might be called ‘traditional femininity’, women who were never too ‘sailorised’ but worked as administrators in backroom offices, and sometimes revealed subjective feelings and domestic preoccupations.
This chapter discusses Beale’s version of naval Dover from February to May 1918 when she established the WRNS there, including founding the hostel that was members’ home from home. Her own (unpublished) words are used wherever possible, contextualised by primary and secondary material about the WRNS and naval history. In those fine spring days of the last year of the war she established a template for the Dover WRNS in the period that followed, even after the war’s end.
Beale was the youngest daughter of a non-conformist family with seven children. Her father, James, was a solicitor who had grown wealthy from negotiating railway contracts, especially the Midland Railway’s St Pancras route. In the 1890s the family moved from London to their new Arts and Crafts house, Standen, in East Grinstead (now owned by the National Trust). As women of their class did, before the war Helen joined the local branch of the Red Cross as a Voluntary Aid Detachment (VAD) nursing assistant and later nursed in Étaples and London.
Looking for something with more scope, in January 1918 this well-travelled VAD switched to the Women’s Royal Naval Service. The WRNS had just been formed after much early resistance to women’s incursion. In December 1914 Vice Admiral David Beatty had expressed a typical view: he had ‘never heard such nonsense’ as the rumour of a possible women’s branch of naval services. He opined that there was plenty of work for women which men could not do. If women were married it was ‘enough to look after their homes and children’.

Background
Despite evidence of gender differences in bipolar disorder characteristics and comorbidity, there is little research on the differences in treatment and service use between men and women with bipolar disorder.
AimsTo use routine data to describe specialist mental health service contact for bipolar disorder, including in-patient, community and support service contacts; to compare clinical characteristics and mental health service use between men and women in contact with secondary services for bipolar disorder.
MethodCross-sectional analysis of mental health patients with bipolar disorder in New Zealand, based on complete national routine health data.
ResultsA total of 3639 individuals were in contact with specialist mental health services with a current diagnosis of bipolar disorder in 2015. Of these 58% were women and 46% were aged 45 and over. The 1-year prevalence rate of bipolar disorder leading to contact with specialist mental health services was 1.56 (95% CI 1.50–1.63) per 100 000 women and 1.20 (95% CI 1.14–1.26) per 100 000 men. Rates of bipolar disorder leading to service contact were 30% higher in women than men (rate ratio 1.30, 95% CI 1.22–1.39). The majority (68%) had a diagnosis of bipolar I disorder. Women were more likely to receive only out-patient treatment and have comorbid anxiety whereas more men had substance use disorder, were convicted for crimes when unwell, received compulsory treatment orders and received in-patient treatment.
ConclusionsAlthough the prevalence of bipolar disorder is equal between men and women in the population, women were more likely to have contact with specialist services for bipolar disorder but had a lower intensity of service interaction.

Suicidal students’ use of and attitudes to primary care support services

Aim
The aims of this study were to improve responses to students in distress and who are feeling suicidal, to help practitioners to increase their responsiveness to those at high risk of suicide and to develop effective responses to those affected by their deaths. The study sought to build a detailed picture of students’ patterns of service use.
BackgroundNational suicide prevention strategies emphasise that suicide prevention requires the collaboration of a wide range of organisations. Among these, primary care services play a key role in relation to suicide prevention for young people in crisis.
MethodsThis study, undertaken between 2004 and 2007, focused on 20 case studies of student suicide that took place in the United Kingdom between May 2000 and June 2005. It adopted a psychological autopsy approach to learn from a wide range of informants, including parents, friends, university staff and the records of coroners or procurator fiscals. Twenty families gave permission for their son’s or daughter’s death to be included in the study and agreed to participate in the study. Informants were interviewed in person and the data were analysed thematically. Analysis of the case study data suggested that in a number of cases students had failed to engage with services sufficiently early or in sufficient depth. Primary care practitioners need to be proactive in communicating concerns about vulnerable students to student support services. At local levels, collaboration between student support and National Health Service practitioners varied considerably and channels of communication need to be developed.
