Long-Term, Open-Label Extension Study of Guanfacine Extended Release in Children and Adolescents with ADHD

Introduction:
Guanfacine is a noradrenergic agonist that is believed to improve symptoms of attention-deficit/hyperactivity disorder (ADHD) through selective actions at α2A-adrenoceptors in the prefrontal cortex. A recent double-blind, multicenter trial supports the efficacy and safety of guanfacine extended release (GXR) for pediatric ADHD. This long-term, open-label extension was conducted to study the safety profile and effectiveness of GXR for up to 2 years.
Methods:Subjects were 240 children 6–17 years of age with a diagnosis of ADHD who participated in the preceding randomized trial. GXR was initiated at 2 mg/day and titrated as needed in 1-mg increments to a maximum of 4 mg/day to achieve optimal clinical response.
Results:The most common adverse events were somnolence (30.4%), headache (26.3%), fatigue (14.2%), and sedation (13.3%). Somnolence, sedation, and fatigue were usually transient. Cardiovascular-related adverse events were uncommon, although small reductions in mean blood pressure and pulse rate were evident at monthly visits. ADHD Rating Scale, Version IV, total and subscale scores improved significantly from baseline to endpoint for all dose groups (P<.001 for all comparisons, intent-to-treat population).
Conclusion:Long-term treatment with GXR was generally safe for up to 24 months of treatment, and effectiveness was maintained over this treatment period.

Field-average based recommendations have been a common practice for nitrogen (N) rate recommendations. The problem is that, even when the same amount of N fertilizer is applied at the recommended rate, yields differ from year to year, the result being poor use efficiency. Reduced fertilizer use efficiency can result in unnecessary costs to producers, especially with a likely price of US$ 1·10 per kilogram of N fertilizer anticipated within the next few years, largely due to rising oil and natural gas prices. It has been estimated that by the year 2025 the consumption of N fertilizer will increase by 60–90%, with two-thirds of this being applied in the developing world. In light of this, methods that increase N use efficiency and profitability of farms, while reducing environmental impact, are no longer simply commendable but required in developing countries. The present review assesses the temporal variability in winter wheat with respect to N nutrition and presents improved methods, including the calibration stamp (CS), N-rich strips (NRS) and ramped calibration strips (RCS) to overcome this variability. These methods can be used either as visual tools or along with NDVI handheld sensors for accuracy. Unlike the CS and NRS, the RCS method is designed to include more pre-plant N rates in strips to improve top-dress N estimation. Hypotheses as backgrounds to these methods have been developed and tested in the USA and Mexico. However, the simplicity of these technologies suggests that many farmers in both developed and developing countries can readily apply them.

Background
Even at the first episode of psychosis, high expressed emotion (EE) characterises over half of patient–carer relationships. This study compared a carer appraisal model of EE with the ability of illness factors to predict EE at the first episode.
AimsTo investigate the utility of a carer appraisal model of EE in first-episode psychosis.
MethodWe compared high- and low-EE carers of people who had first-episode psychosis (n=46).
ResultsHigh EE in carers was associated with higher avoidant coping, higher subjective burden and lower perceived patient interpersonal functioning. Patient illness factors and carers ‘distress levels were not associated with EE.
ConclusionsEven at the first episode, carers' psychological appraisal, not patient illness factors, is influential in determining high EE. Carers ‘appraisal of their situation should be a primary target to lower or prevent high EE in early intervention for psychosis.
