High Users of Hospital Days

Abstract

Data from the Manitoba Longitudinal Study on Aging were used to focus on the elderly who used 31 or more hospital days in one year and to see if their high use was an isolated event or a persistent pattern by examining their hospital use over a 6-year period. The relationship of several service-provider characteristics to high use was also examined. The majority (58%) of high users were decedents or those about to enter a nursing home and another 22% used 31 or more days in only one of the six years. Indications are that bed/population ratios and physician practice styles are positively related to the hospitalization utilization patterns of high users. These and other findings raise issues requiring further research.

Résumé

Les données provenant du Manitoba Longitudinal Study on Aging (Etude longitudinale sur le vieillissement au Manitoba) ont servi de point de départ à une étude sur les personnes âgées occupant un lit d'hôpital pendant 31 jours ou plus au cours d'une même année, et ont permis de savoir, grâce à l'examen des séjours sur une période de six ans, s'il s'agit d'événements isolés ou de configurations typiques. Le rapport entre les caractéristiques de plusieurs pourvoyeurs de services et l'utilisation extensive de l'hôpital a également été examiné. Dans la majorité des cas (58%) l'utilisation extensive est imputable à des malades décédés ou sur le point d'être admis dans une maison d'accueil pour personnes âgées; dans 22% des autres cas, l'utilisation extensive constitue en événement isolé au cours des six années étudiées. Certaines indications permettent d'établir un rapport positif entre l'utilisation extensive de l'hôpital d'une part et d'autre part le ratio lit/population et le style clinique des praticiens. Ces résultats ainsi que d'autres questions soulevées au cours de l'étude appellent des recherches complémentaires.
