Hybrid procedure of hypoplastic left heart syndrome, comprising ductus arteriosus stenting and bilateral pulmonary artery banding, is a good surgical option for initial palliative procedure for high-risk patients for Norwood procedure. However, ductal stenting may cause retrograde aortic blood flow obstruction. Furthermore, complete removal of stent while performing the Norwood procedure make the operation more difficult. We report a case that overcame these problems using a novel surgical technique.
This study examines the presence of bacterial contamination on surgical gloves and suggests appropriate measures for an aseptic surgical environment. To prevent glove contamination during surgery, surgeons and assistants should change gloves periodically, and scrub nurses should be careful when opening packages and handing over implants.
A 65-day-old girl presented to the emergency room with lethargy, requiring emergency venoarterial extracorporeal membrane oxygenation for refractory cardiogenic shock. Initially, hypoplastic left heart syndrome was suspected. However, cor triatriatum with a pinpoint opening on the membrane was diagnosed based on a detailed echocardiographic examination. After membrane resection, the left heart size was restored. However, follow-up echocardiography performed 4 months later showed occlusion of both upper pulmonary veins and stenosis in both lower pulmonary veins. Hybrid balloon angioplasty was performed in all pulmonary veins, and stents were inserted into the right upper and lower pulmonary veins. Despite repeated balloon angioplasty, all pulmonary vein stenosis progressed over 6 months and the patient expired while waiting for a heart-lung transplant. Even after successful repair of cor triatriatum, short-term close follow-up is required for detecting the development of pulmonary vein stenosis.
The “Fast track” protocol is an early extubation strategy to reduce ventilator-associated complications and induce early recovery after open-heart surgery. This study compared clinical outcomes between operating room extubation and ICU extubation after open-heart surgery in patients with CHD.
Methods:
We retrospectively reviewed 215 patients who underwent open-heart surgery for CHDs under the scheduled “Fast track” protocol between September 2016 and April 2022. The clinical endpoints were post-operative complications, including bleeding, respiratory and neurological complications, and hospital/ICU stays.
Results:
The patients were divided into operating room extubation (group O, n = 124) and ICU extubation (group I, n=91) groups. The most frequently performed procedures were patch closures of the atrial septal (107/215, 49.8%) and ventricular septal (89/215, 41.4%) defects. There were no significant differences in major post-operative complications or ICU and hospital stay duration between the two groups; however, patients in group I showed longer mechanical ventilatory support (0.0 min vs. 59.0 min (interquartile range: 17.0–169.0), p < 0.001). Patients in Group O showed higher initial lactate levels (3.2 ± 1.7 mg/dL versus 2.5 ± 2.0 mg/dL, p = 0.007) and more frequently used additional sedatives and opioid analgesics (33.1% versus 19.8%, p = 0.031).
Conclusions:
Extubation in the operating room was not beneficial for patients during post-operative ICU or hospital stay. Early extubation in the ICU resulted in more stable hemodynamics in the immediate post-operative period and required less use of sedatives and analgesics.
There is growing evidence for the use of acceptance-commitment therapy (ACT) for the treatment of obsessive-compulsive disorder (OCD). However, few fully implemented ACT have been conducted on the neural mechanisms underlying its effect on OCD. Thus, this study aimed to elucidate the neural correlates of ACT in patients with OCD using task-based and resting-state functional magnetic resonance imaging (fMRI).
Methods
Patients with OCD were randomly assigned to the ACT (n = 21) or the wait-list control group (n = 21). An 8-week group-format ACT program was provided to the ACT group. All participants underwent an fMRI scan and psychological measurements before and after 8 weeks.
Results
Patients with OCD showed significantly increased activation in the bilateral insula and superior temporal gyri (STG), induced by the thought-action fusion task after ACT intervention. Further psycho-physiological interaction analyses with these regions as seeds revealed that the left insular–left inferior frontal gyrus (IFG) connectivity was strengthened in the ACT group after treatment. Increased resting-state functional connectivity was also found in the posterior cingulate cortex (PCC), precuneus, and lingual gyrus after ACT intervention Most of these regions showed significant correlations with ACT process measures while only the right insula was correlated with the obsessive-compulsive symptom measure.
Conclusions
These findings suggest that the therapeutic effect of ACT on OCD may involve the salience and interoception processes (i.e. insula), multisensory integration (i.e. STG), language (i.e. IFG), and self-referential processes (i.e. PCC and precuneus). These areas or their interactions could be important for understanding how ACT works psychologically.
The COVID-19 pandemic and pandemic-related measures have impacted the lives and work-related activities of Antarctic researchers. To explore these impacts, we designed, piloted and disseminated an online survey in English, Russian, Spanish and Chinese in late 2020 and early 2021. The survey explored how the pandemic affected the productivity of Antarctic researchers, their career prospects and their mental wellbeing. Findings exposed patterns of inequities. For instance, of the 406 unique responses to the survey, women appeared to have been affected more adversely than men, especially in relation to mental health, and early-career researchers were disadvantaged more than their mid- or late-career colleagues. Overall, a third of the research participants reported at least one major negative impact from the pandemic on their mental health. Approximately half of the participants also mentioned that the COVID-19 pandemic had some positive effects, especially in terms of the advantages that working from home brought and opportunities to attend events, network or benefit from training workshops online. We conclude with a series of recommendations for science administrators and policymakers to mitigate the most serious adverse impacts of the pandemic on Antarctic research communities, with implications for other contexts where scientific activities are conducted under extreme circumstances.
Predicting the course of depression is necessary for personalized treatment. Impaired glucose metabolism (IGM) was introduced as a promising depression biomarker, but no consensus was made. This study aimed to predict IGM at the time of depression diagnosis and examine the relationship between long-term prognosis and predicted results.
Methods
Clinical data were extracted from four electronic health records in South Korea. The study population included patients with depression, and the outcome was IGM within 1 year. One database was used to develop the model using three algorithms. External validation was performed using the best algorithm across the three databases. The area under the curve (AUC) was calculated to determine the model’s performance. Kaplan–Meier and Cox survival analyses of the risk of hospitalization for depression as the long-term outcome were performed. A meta-analysis of the long-term outcome was performed across the four databases.
Results
A prediction model was developed using the data of 3,668 people, with an AUC of 0.781 with least absolute shrinkage and selection operator (LASSO) logistic regression. In the external validation, the AUCs were 0.643, 0.610, and 0.515. Through the predicted results, survival analysis and meta-analysis were performed; the hazard ratios of risk of hospitalization for depression in patients predicted to have IGM was 1.20 (95% confidence interval [CI] 1.02–1.41, p = 0.027) at a 3-year follow-up.
Conclusions
We developed prediction models for IGM occurrence within a year. The predicted results were related to the long-term prognosis of depression, presenting as a promising IGM biomarker related to the prognosis of depression.
The COVID-19 pandemic poses a major threat to mental health and is associated with an increased risk of suicide. An understanding of suicidal behaviours during the pandemic is necessary for establishing policies to prevent suicides in such social conditions.
Aims
We aimed to investigate vulnerable individuals and the characteristics of changes in suicidal behaviour during the COVID-19 pandemic.
Method
We retrospectively reviewed the medical records of patients with suicide attempts who visited the emergency department from February 2019 to January 2021. We analysed the demographic and clinical characteristics, risk factors and rescue factors of patients, and compared the findings between the pre-pandemic and pandemic periods.
Results
In total, 519 patients were included. During the pre-pandemic and pandemic periods, 303 and 270 patients visited the emergency department after a suicide attempt, respectively. The proportion of suicide attempts by women (60.1% v. 69.3%, P = 0.035) and patients with a previous psychiatric illness (63.4% v. 72.9%, P = 0.006) increased during the COVID-19 pandemic. In addition, patients’ rescue scores during the pandemic were lower than those during the pre-pandemic period (12 (interquartile range: 11–13) v. 13 (interquartile range: 12–14), P < 0.001).
Conclusions
Women and people with previous psychiatric illnesses were more vulnerable to suicide attempts during the COVID-19 pandemic. Suicide prevention policies, such as continuous monitoring and staying in touch with vulnerable individuals, are necessary to cope with suicide risk.
The present study enhances knowledge of the biodiversity of diatoms in Chuuk, Micronesia following our 2018 study on the seaweed-associated diatoms. We collected planktonic samples of diatoms from five sites of reef flats using a 20 μm mesh net, and two samples of seaweeds with epiphytes by hand from an islet on the barrier reef. In addition, the seaweed-associated diatoms from our 2018 study were analysed using scanning electron microscopy. A total of 109 diatom taxa are documented in the present study. Of these, 70 species were from net samples, and 39 species from the seaweed-associated diatoms. Thirty-one species are newly recorded from Micronesian waters. Most taxa are benthic or tychoplanktonic; euplanktonic diatoms were rare. The occurrence of benthic diatoms from the water column might be related to the Chuuk environmental conditions which include shallow water, strong light intensity and high grazing pressure, to which benthic diatoms seem to be able to better adapt than planktonic diatoms.
The coastline of the Korean Peninsula is influenced by three major oceanographic ecoregions, including the estuarine Yellow Sea ecoregion on the west coast, the warmer and saline East China Sea ecoregion on the south coast, and the cold East Sea ecoregion on the east coast. The influence of these marine ecoregions on the distribution of intertidal barnacles has not been extensively studied. The present study examines the biogeography of thoracican barnacles from intertidal and shallow subtidal zones, along the coasts of Korea. Twenty-one species in seven families were identified, including three species of coral-associated barnacles. Species composition varied significantly in the three marine ecoregions. Multivariate analysis showed barnacle assemblages were significant among the three ecoregions, although there are large overlaps of clusters between the Yellow Sea and East China Sea ecoregions. The estuarine species, Fistulobalanus albicostatus, occurred mainly in the Yellow Sea ecoregion; warm-water species, Tetraclita japonica, and sponge inhabiting barnacles Euacasta dofleini were observed in the East China Sea ecoregion; and cold-water species, Balanus rostratus and Perforatus perforatus, were found in the East Sea ecoregion. Four invasive barnacle species were recorded and the European barnacle Perforatus perforatus expanded its range northward from its recorded distribution nine years earlier. The cold-water species, Chthamalus dalli and Semibalanus cariosus, previously recorded in the East Sea ecoregion, were absent in the present survey. A trend of increasing seawater temperatures in Korean waters may have a significant impact on the distribution of cold-water species and enhance the northward invasion of P. perforatus.
Cognitive theories of obsessive–compulsive disorder (OCD) stress the importance of dysfunctional beliefs in the development and maintenance of the disorder. However, a neurobiological understanding of these cognitive models, including thought-action fusion (TAF), is surprisingly lacking. Thus, this functional magnetic resonance imaging study aimed to investigate whether altered functional connectivity (FC) is associated with the TAF paradigm in OCD patients.
Methods
Forty-one OCD patients and 47 healthy controls (HCs) participated in a functional magnetic resonance imaging study using a TAF task, in which they were asked to read the name of a close or a neutral person in association with positive and negative statements.
Results
The conventional TAF condition (negative statements/close person) induced significant FC between the regions of interest (ROIs) identified using multivoxel pattern analysis and the visual association areas, default mode network subregions, affective processing, and several subcortical regions in both groups. Notably, sparser FC was observed in OCD patients. Further analysis confined to the cortico-striato-thalamo-cortical (CSTC) and affective networks demonstrated that OCD patients exhibited reduced ROI FC with affective regions and greater ROI FC with CSTC components in the TAF condition compared to HCs. Within the OCD patients, middle cingulate cortex–insula FC was correlated with TAF and responsibility scores.
Conclusions
Our TAF paradigm revealed altered context-dependent engagement of the CSTC and affective networks in OCD patients. These findings suggest that the neurobiology of cognitive models corresponds to current neuroanatomical models of OCD. Further, they elucidate the underlying neurobiological mechanisms of OCD at the circuit-based level.
The effects of CaTiO3 (CT) and BaZrO3 (BZ) modification upon the crystal structure and electromechanical properties of lead-free Bi0.5Na0.5TiO3–SrTiO3 piezoelectric ceramics were compared within a doping range of 0–4 mol%. The different effects of CT and BZ modification upon the phase transition are clearly observed in the polarization and strain hysteresis loops. The CT-modified specimens maintain strong ferroelectricity without any abnormal enhancement in the electric field-induced strain. However, the addition of as little as 1 mol% BZ induces a transition from a nonergodic relaxor phase to an ergodic relaxor phase, thus resulting in disruption of the ferroelectric order and the generation of a high field-induced strain. The present authors believe that the substitution of large ions (such as Zr4+) into the B-sites, rather than the A-sites, of the Bi0.5Na0.5TiO3-based ceramics plays a significant role in the phase transition behavior.
This work quantitatively analyses vessel traffic service (VTS) communications in ports and suggests improvements for more efficient control of the service. For this purpose, analysis of VTS communications was performed on VHF channel 12 in Busan North Port, South Korea. This communications service follows the queue of M/G/1 (the arrivals have a Poisson distribution, the service time is characterized by a general distribution, and with a single server). The degree of congestion of the communication channel was shown as the utilisation rate of the queue, which was 67·7% at peak times and 29·6% at non-peak times. To reduce congestion in the communication channel, we propose to separate the peak time control channel, exclude passing reporting, and decrease the reporting time. With separation of the peak time control channel, the utilisation rate decreased by 41·1%. The utilisation rate decreased by 5·7% when passing reporting was omitted, and by 8·3% when reporting time was reduced by 60%. The results of this study can be used as basic policy data to improve VTS, including reinforcement of the VTS officer's role and adjustment of the control report contents.
Predicting the isotopic modification of ice by melting processes is important for improving the accuracy in paleoclimate reconstruction. To this end, we present results from cold room laboratory observations of changes in the isotopic ratio (D/H and 18O/16O) of ice cubes by isotopic exchange between liquid water and ice in nearly isothermal conditions. A 1-D model was fit to the isotopic results by adjusting the values of two parameters, the isotopic exchange rate constant (kr) and the fraction of ice participating in the exchange (f). We found that the rate constant for hydrogen isotopic exchange between liquid water and ice may be greater (up to 40%) than that for the oxygen isotopic exchange. The range of the rate constant obtained from four melt experiments is from 0.21 to 0.82 h–1. The model results also suggest that f decreases with the increasing wetness of the ice. This is because with increasing water saturation in ice, water may be present only in the small pores or some of the water that was exchanged with ice may be bypassed, decreasing the effective surface area over which the isotopic exchange can occur. The relationship between the two water isotopes (δ18O vs δD) was observed and modeled and the slope was <8, which is significantly different from the slope of the meteoric waterline. We note that these slopes were obtained without considering the sublimation process.
The Drygalski Ice Tongue presents an ~80 km long floating obstacle to alongshore flows in the Victoria Land coastal ocean region of the Western Ross Sea. Here we use oceanographic data from near to the tongue to explore the interplay between the floating glacier and the local currents and stratification. A vessel-based circuit of the glacier, recording ocean temperature and salinity profiles, reveals the southwest corner to be the coldest and most complex in terms of vertical structure. The southwest corner structure beneath the surface warm, salty layer sustains a block of very cold water extending to 200 m depth. In this same location there was a distinct layer at 370 m not seen anywhere else of water at ~−1.93°C. The new observations broadly, but not directly, support the presence of a coherent Victoria Land Coastal Current. The data suggest the northward moving coastal current turns against the Coriolis force and works its way anticlockwise around the glacier, but with leakage beneath the glacier through the highly ‘rippled’ underside, resulting in a spatially heterogeneous supply to the Terra Nova Bay Polynya region – an important location for the formation of high-salinity shelf water.
Personality may predispose family caregivers to experience caregiving differently in similar situations and influence the outcomes of caregiving. A limited body of research has examined the role of some personality traits for health-related quality of life (HRQoL) among family caregivers of persons with dementia (PWD) in relation to burden and depression.
Methods:
Data from a large clinic-based national study in South Korea, the Caregivers of Alzheimer's Disease Research (CARE), were analyzed (N = 476). Path analysis was performed to explore the association between family caregivers’ personality traits and HRQoL. With depression and burden as mediating factors, direct and indirect associations between five personality traits and HRQoL of family caregivers were examined.
Results:
Results demonstrated the mediating role of caregiver burden and depression in linking two personality traits (neuroticism and extraversion) and HRQoL. Neuroticism and extraversion directly and indirectly influenced the mental HRQoL of caregivers. Neuroticism and extraversion only indirectly influenced their physical HRQoL. Neuroticism increased the caregiver's depression, whereas extraversion decreased it. Neuroticism only was mediated by burden to influence depression and mental and physical HRQoL.
Conclusions:
Personality traits can influence caregiving outcomes and be viewed as an individual resource of the caregiver. A family caregiver's personality characteristics need to be assessed for tailoring support programs to get the optimal benefits from caregiver interventions.
To examine the hypothesis that the association between vitamin D deficiency and depressive symptoms is dependent upon total cholesterol level in a representative national sample of the South Korean population.
Design
This was a population-based cross-sectional study.
Setting
The Fifth Korean National Health and Nutrition Examination Survey (KNHANES V, 2010–2012).
Subjects
We included 7198 adults aged 20–88 years.
Results
The incidence of depressive symptoms in individuals with vitamin D deficiency (serum 25-hydroxyvitamin D<20 ng/ml) was 1·54-fold (95 % CI 1·20, 1·98) greater than in individuals without vitamin D deficiency (serum 25-hydroxyvitamin D ≥20 ng/ml). The relationship was stronger in individuals with normal-to-borderline serum total cholesterol (serum total cholesterol<240 mg/dl; OR=1·60; 95 % CI 1·23, 2·08) and non-significant in individuals with high serum total cholesterol (OR=0·97; 95 % CI 0·52, 1·81) after adjustment for confounding variables (age, sex, BMI, alcohol consumption, smoking status, regular exercise, income level, education level, marital status, changes in body weight, perceived body shape, season of examination date and cholesterol profiles).
Conclusions
The association between vitamin D deficiency and depressive symptoms was weakened by high serum total cholesterol status. These findings suggest that both vitamin D and total cholesterol are important targets for the prevention and treatment of depression.
Decreased hemoglobin levels increase the risk of developing dementia among the elderly. However, the underlying mechanisms that link decreased hemoglobin levels to incident dementia still remain unclear, possibly due to the fact that few studies have reported on the relationship between low hemoglobin levels and neuroimaging markers. We, therefore, investigated the relationships between decreased hemoglobin levels, cerebral small-vessel disease (CSVD), and cortical atrophy in cognitively healthy women and men.
Methods:
Cognitively normal women (n = 1,022) and men (n = 1,018) who underwent medical check-ups and magnetic resonance imaging (MRI) were enrolled at a health promotion center. We measured hemoglobin levels, white matter hyperintensities (WMH) scales, lacunes, and microbleeds. Cortical thickness was automatically measured using surface based methods. Multivariate regression analyses were performed after controlling for possible confounders.
Results:
Decreased hemoglobin levels were not associated with the presence of WMH, lacunes, or microbleeds in women and men. Among women, decreased hemoglobin levels were associated with decreased cortical thickness in the frontal (Estimates, 95% confidence interval, −0.007, (−0.013, −0.001)), temporal (−0.010, (−0.018, −0.002)), parietal (−0.009, (−0.015, −0.003)), and occipital regions (−0.011, (−0.019, −0.003)). Among men, however, no associations were observed between hemoglobin levels and cortical thickness.
Conclusion:
Our findings suggested that decreased hemoglobin levels affected cortical atrophy, but not increased CSVD, among women, although the association is modest. Given the paucity of modifiable risk factors for age-related cognitive decline, our results have important public health implications.
Re-examination of the holotype male of Zehntneriana villosa (Zehntner, 1894) (from Ambon, Indonesia) shows that Japanese specimens previously referred to this species should be designated as a new species, Zehntneriana tadafumii sp. nov. The new species differs from Z. villosa in several characters, including the carapace, epistome, third maxilliped and thoracic sternum. Here, we redescribe and illustrate Z. villosa s. str. and the new species. In addition, the taxonomy of Zehntneriana Ng & Takeda, 2010, is also discussed.
There is increasing evidence of a relationship between underweight or obesity and dementia risk. Several studies have investigated the relationship between body weight and brain atrophy, a pathological change preceding dementia, but their results are inconsistent. Therefore, we aimed to evaluate the relationship between body mass index (BMI) and cortical atrophy among cognitively normal participants.
Methods:
We recruited cognitively normal participants (n = 1,111) who underwent medical checkups and detailed neurologic screening, including magnetic resonance imaging (MRI) in the health screening visits between September 2008 and December 2011. The main outcome was cortical thickness measured using MRI. The number of subjects with five BMI groups in men/women was 9/9, 148/258, 185/128, 149/111, and 64/50 in underweight, normal, overweight, mild obesity, and moderate to severe obesity, respectively. Linear and non-linear relationships between BMI and cortical thickness were examined using multiple linear regression analysis and generalized additive models after adjustment for potential confounders.
Results:
Among men, underweight participants showed significant cortical thinning in the frontal and temporal regions compared to normal weight participants, while overweight and mildly obese participants had greater cortical thicknesses in the frontal region and the frontal, temporal, and occipital regions, respectively. However, cortical thickness in each brain region was not significantly different in normal weight and moderate to severe obesity groups. Among women, the association between BMI and cortical thickness was not statistically significant.
Conclusions:
Our findings suggested that underweight might be an important risk factor for pathological changes in the brain, while overweight or mild obesity may be inversely associated with cortical atrophy in cognitively normal elderly males.
