The evidence-based practice movement has led to higher quality intervention studies being reported in the health sciences literature (see, for example, Mosely et al., 2002), and consequently being available for uptake by practising health professionals. Yet interventions in applied settings need to satisfy more than the methodologically-based criteria associated with being categorised as a ‘well-established treatment’ (Chambless et al., 1996). Some of these additional criteria were recently described by King and Ollendick (2007). The current article extends these criteria so as to include attributes of interventions that (ideally) are required when designing or delivering services within rehabilitation settings.

This selective review shows that childhood obsessive-compulsive disorder (OCD) is a serious condition that requires early identification and treatment. Initially, we examine the relationship of childhood rituals to OCD, and discuss recent findings on the epidemiology of childhood OCD. The DSM-IV criteria for OCD are presented, along with recent findings on psychopathology. Clinical management of childhood OCD is a challenging and difficult task for mental health professionals. We briefly describe and evaluate advances in behavioural treatment — mainly in vivo exposure and response prevention. The adjunctive use of antidepressant medication is also examined. Whilst these clinical innovations are promising, further controlled evaluations are necessary before the efficacy of behavioural intervention can be confidently asserted.

In this study, we examined the prevalence and nature of nonclinical panic attacks in 649 Australian youth and explored the relations between such attacks and measures of social support, stress, anxiety, depression, and fear. Full-blown attacks (attacks involving four or more symptoms with rapid onset) were reported by 104 of the youth (16%). Attacks were reported more frequently by girls than boys; however, age was unrelated to panic-attack status. Heightened levels of anxiety and fear, as well as stress in the family and lack of family support, were related to attack status. Path-analytic procedures supported a working model consisting of stress, social support, and emotional distress as related to panic status. Limitations of the self-report data on panic attacks and the other measures used in this study are acknowledged.

We evaluated the empirical validation model of target selection for two boys with interpersonal problems. Behaviours previously identified as distinguishing popular from rejected boys on a role-play measure of social behaviour were targeted for change in these boys who had histories of peer rejection. We trained three of these empirically-derived behaviours — appropriate body orientation, adequate speech intonation, and providing reasons for denying unreasonable requests — in a multiple-baseline-across-behaviours design. On a role-play measure of social behaviour, improvements in the targeted behaviours occurred upon, but not before, introduction of the intervention, confirming the controlling effects of the intervention. Behavioural improvements generalised to untrained role-play scenes and to a novel role-play partner, and were maintained for two of the three behaviours at 4- and 6-month follow-up assessments. The boys also showed improvements in parent-reported child behaviour from pre- to post-training. The study demonstrates use of an empirical approach to the selection of treatment targets for children.

Compared to the study of adult fears, childhood fears have not been extensively investigated in terms of their psychophysiological bases. However, limited findings suggest that children exhibit psychophysiological reactivity to fear-eliciting stimuli. Other data suggest that fear imagery produces psychophysiological arousal and that youngsters may be trained in fear imagery. Psychophysiological measures have also been used in the evaluation of desensitisation as seen in a limited number of case studies, single-subject experimental analyses and group outcome comparisons. In general, psychophysiological changes have been reported that are suggestive of reduced autonomic arousal. Methodological and theoretical issues are discussed including the selection of psychophysiological measures and the desynchrony between measures of fear.
