Aim:
To determine whether environmental house calls that improved indoor air quality (IAQ) is effective in reducing symptoms of chemical intolerance (CI).
Background:Prevalence of CI is increasing worldwide. Those affected typically report symptoms such as headaches, fatigue, ‘brain fog’, and gastrointestinal problems – common primary care complaints. Substantial evidence suggests that improving IAQ may be helpful in reducing symptoms associated with CI.
Methods:Primary care clinic patients were invited to participate in a series of structured environmental house calls (EHCs). To qualify, participants were assessed for CI with the Quick Environmental Exposure and Sensitivity Inventory. Those with CI volunteered to allow the EHC team to visit their homes to collect air samples for volatile organic compounds (VOCs). Initial and post-intervention IAQ sampling was analyzed by an independent lab to determine VOC levels (ng/L). The team discussed indoor air exposures, their health effects, and provided guidance for reducing exposures.
Findings:Homes where recommendations were followed showed the greatest improvements in IAQ. The improvements were based upon decreased airborne VOCs associated with reduced use of cleaning chemicals, personal care products, and fragrances, and reduction in the index patients’ symptoms. Symptom improvement generally was not reported among those whose homes showed no VOC improvement.
Conclusion:Improvements in both IAQ and patients’ symptoms occur when families implement an action plan developed and shared with them by a trained EHC team. Indoor air problems simply are not part of most doctors’ differential diagnoses, despite relatively high prevalence rates of CI in primary care clinics. Our three-question screening questionnaire – the BREESI – can help physicians identify which patients should complete the QEESI. After identifying patients with CI, the practitioner can help by counseling them regarding their home exposures to VOCs. The future of clinical medicine could include environmental house calls as standard of practice for susceptible patients.

We have combined global magnetohydrodynamic (MHD) simulations of the solar wind and magnetosphere interaction with an implicit particle-in-cell simulation (PIC) and used this approach to model magnetic reconnection at both the dayside magnetopause and in the magnetotail plasma sheet. In this approach, we first model the magnetospheric configuration driven by the solar wind using the MHD simulation. At a time of interest (usually when a thin current sheet has formed in the MHD simulation), we load a large particle-in-cell simulation with plasma and fields based on the MHD state. We use the MHD results to set the boundary conditions on the PIC simulation. The coupling between the two models is one way – the PIC results do not change the MHD results. In these calculations, we use the UCLA global MHD code and the iPic3D implicit particle-in-cell code. In this paper we describe this technique in detail. As an example of this approach, we present PIC results on reconnection in the magnetotail during a magnetospheric substorm.

Objectives.
We sought to estimate influenza vaccination coverage among healthcare workers (HCWs) in the United States during 1989-2002 and to identify factors associated with vaccination in this group. The Advisory Committee on Immunization Practices (ACIP) recommends annual influenza vaccination for HCWs to reduce transmission of influenza to patients at high risk for serious complications of influenza.
Design.Analysis of cross-sectional data from 1989-2002 surveys conducted by the National Health Interview Survey (NHIS). The outcome measure was self-reported influenza vaccination in the past 12 months. Bivariate and multivariate analysis of 2002 NHIS data.
Setting.Household interviews conducted during 1989-2002, weighted to reflect the noninstitutionalized, civilian US population.
Participants.Adults aged 18 years or older participated in the study. A total of 2,089 were employed in healthcare occupations or settings in 2002, and 17,160 were employed in nonhealthcare occupations or settings.
Results. The influenza vaccination rate among US HCWs increased from 10.0% in 1989 to 38.4% in 2002, with no significant change since 1997. In a multivariate model that included data from the 2002 NHIS, factors associated with a higher rate of influenza vaccination among HCWs aged 18-64 years included age of 50 years or older (odds ratio [OR], 1.6; 95% confidence interval [CI], 1.1-2.1), hospital employee status (OR, 1.5; 95% CI, 1.2-1.9), 1 or more visits to the office of a healthcare professional in the past 12 months (OR, 1.5; 95% CI, 1.1-2.2), receipt of employer-provided health insurance (OR, 1.5; 95% CI, 1.1-2.1), a history of pneumococcal vaccination (OR, 3.9; 95% CI, 2.5-6.1), and history of hepatitis B vaccination (OR, 1.9; 95% CI, 1.4-2.4). Non-Hispanic black persons were less likely to be vaccinated (OR, 0.6; 95% CI, 0.5-0.9) than non-Hispanic white persons. There were no significant differences in vaccination levels according to HCW occupation category.
Conclusions.Influenza immunization among HCWs reached a plateau during 1997-2002. New strategies are needed to encourage US HCWs to receive influenza vaccination to prevent influenza illness in themselves and transmission of influenza to vulnerable patients.

The conservation significance of the Caribbean island of Anegada in the British Virgin Islands is highlighted in this study of the distribution of plant species in two major habitats, sand dunes and limestone pavement. In 104 plots along 27 transects located around the western salt ponds of the Anegada Ramsar site, 133 plant species were recorded, including five regional endemics. The limestone pavement supported large populations of Acacia anegadensis, endemic to Anegada, and Cordia rupicola, known only from Anegada and Puerto Rico (although the Puerto Rican population is thought to be extirpated). The sand dunes supported a large population of Metastelma anegadense, also endemic to Anegada. Two other regional endemics were recorded within the limestone cays, Leptocereus quadricostatus, previously known from only one locality in Puerto Rico and Malpighia woodburyana, restricted to a few small populations on islands on the Puerto Rican Bank. For both of these species Anegada supports the largest known individual population.

Six abandoned colonies of Adélie penguin (Pygoscelis adeliae) were excavated near Palmer Station, Anvers Island, Antarctic Peninsula, to investigate the occupation history of this species. Sediments from each site yielded abundant fish bones and otoliths and squid beaks that represent prey remains deposited by penguins during the nesting period. Radiocarbon analyses indicate that colony occupation began prior to the Little Ice Age (LIA; 1500–1850 AD), with the oldest site dating to 644 yrs before present (BP; average reservoir-corrected date with Is range, 603–679 yr BP). Food remains indicate that the non-euphausiid prey of penguins consisted primarily of a mesopelagic squid (Psychroteuthis glacialis) and two species of fish (Pleuragramma antarcticun and Electrona antarctica). The relative abundance of the first two prey taxa varied significantly among six sites (X2>34.6; df = 10; P <0.001) with colonies dating prior to the LIA having greater representation of squid, and less of silverfish, than those occupied during the LIA. Data from control excavations at three modern colonies indicate a diet similar to that of the pre-LIA sites. These results suggest that Adélie penguins may have changed their diet in response to warming and cooling cycles in the past. In addition, only Adélie penguins are known to have nested in the Palmer Station area prior to the l950s; gentoo (Pygoscelis papua) and chinstrap (P. antarctica) penguins now breeding in this region have expanded their ranges southward in the Peninsula within the past 50 yrs, in correlation with pronounced regional warming.

In a medium-sized Canadian city, 581 randomly selected households were contacted and responded to a survey on the impact of parental alcohol problems. Twenty-two per cent of the respondents indicated that at least one of their parents had a drinking problem. The biological father was affected in 81%. Compared with the rest of the sample, the adult children of problem drinkers were younger but they did not differ in income or education. Adult children of problem drinkers were more likely to have parents who were divorced or separated; to be divorced, separated, or remarried themselves; to be heavy drinkers and have indications of alcohol problems; and to use more sources of help for problems with stress and anxiety and problems with alcohol. They did not differ from those without parental drinking problems on measures of current positive and negative affect.
