  • Fluoroquinolone stewardship at a community health system: A decade in review

  • Objective:

    To describe inpatient fluoroquinolone use and susceptibility data over a 10-year period after the implementation of an antimicrobial stewardship program (ASP) led by an infectious diseases pharmacist starting in 2011.

    Design:

    Retrospective surveillance study.

    Setting:

    Large community health system.

    Methods:

    Fluoroquinolone use was quantified by days of therapy (DOT) per 1,000 patient days (PD) and reported quarterly. Use data are reported for inpatients from 2016 to 2020. Levofloxacin susceptibility is reported for Pseudomonas aeruginosa and Escherichia coli for inpatients from 2011 to 2020 at a 4 adult-hospital health system.

    Results:

    Inpatient fluoroquinolone use decreased by 74% over a 5-year period, with an average decrease of 3.45 DOT per 1,000 PD per quarter (P < .001). Over a 10-year period, inpatient levofloxacin susceptibility increased by 57% for P. aeruginosa and by 15% for E. coli. P. aeruginosa susceptibility to levofloxacin increased by an average of 2.73% per year (P < .001) and had a strong negative correlation with fluoroquinolone use, r = −0.99 (P = .002). E. coli susceptibility to levofloxacin increased by an average of 1.33% per year (P < .001) and had a strong negative correlation with fluoroquinolone use, r = −0.95 (P = .015).

    Conclusions:

    A substantial decrease in fluoroquinolone use and increase in P. aeruginosa and E. coli levofloxacin susceptibility was observed after implementation of an antimicrobial stewardship program. These results demonstrate the value of stewardship services and highlight the effectiveness of an infectious diseases pharmacist led antimicrobial stewardship program.

  • Background

    There is limited research on community-based mental health interventions in former Soviet countries despite different contextual factors from where most research has been conducted. Ongoing military conflict has resulted in many displaced persons and veterans and their families with high burdens of mental health problems. Lack of community-based services and poor uptake of existing psychiatric services led to the current trial to determine the effectiveness of the common elements treatment approach (CETA) on anxiety, depression, and posttraumatic stress symptoms (PTS) among conflict affected adults in Ukraine.

    Methods

    We conducted a three-armed randomized-controlled trial of CETA delivered in its standard form (8–12 sessions), a brief form (five-sessions), and a wait-control condition. Eligible participants were displaced adults, army veterans and their adult family members with elevated depression and/or PTS and impaired functioning. Treatment was delivered by community-based providers trained in both standard and brief CETA. Outcome data were collected monthly.

    Results

    There were 302 trial participants (n = 117 brief CETA, n = 129 standard CETA, n = 56 wait-controls). Compared with wait-controls, participants in standard and brief CETA experienced clinically and statistically significant reductions in depression, anxiety, and PTS and dysfunction (effect sizes d = 0.46–1.0–6). Comparing those who received standard CETA with brief CETA, the former reported fewer symptoms and less dysfunction with small-to-medium effect sized (d = 0.20–0.55).

    Conclusions

    Standard CETA is more effective than brief CETA, but brief CETA also had significant effects compared with wait-controls. Given demonstrated effectiveness, CETA could be scaled up as an effective community-based approach.

  • Summary

    Ecosystem science and the systems ecology paradigm co-evolved starting in the late 1960s within the milieu of substantial research funding from the US National Science Foundation-supported US International Biological Program (IBP). Nationally, educational programs focusing on ecosystem structure and functioning, and mathematical modeling, were slow to develop except at Colorado State University (CSU). There, leaders in the Natural Resource Ecology Laboratory (NREL) and the Department of Range Science (DRS) established internationally recognized interdisciplinary programs and outreach in basic and applied ecosystem science and systems ecology. Operating from the sound research base within a major Land Grant University (CSU), the NREL, with IBP funding, supported many graduate students housed in the academic DRS. As the systems ecology approach expanded, other ecosystem-focused research programs developed, and graduate students entered other academic departments. Outgrowths from the early diffused educational training were innovative cross-departmental and cross-college programs addressing the systems ecology paradigm. Recently, a new Department of Ecosystem Science and Sustainability was established housing both graduate and undergraduate programs. As formal academic training developed on-campus, environmental literacy efforts were developed, including: training programs for K-12 students and teachers; online distance education programs; Citizen Science training; and numerous institutes, short courses, and workshops.

  • We read with interest the recent editorial, “The Hennepin Ketamine Study,” by Dr. Samuel Stratton commenting on the research ethics, methodology, and the current public controversy surrounding this study.1 As researchers and investigators of this study, we strongly agree that prospective clinical research in the prehospital environment is necessary to advance the science of Emergency Medical Services (EMS) and emergency medicine. We also agree that accomplishing this is challenging as the prehospital environment often encounters patient populations who cannot provide meaningful informed consent due to their emergent conditions. To ensure that fellow emergency medicine researchers understand the facts of our work so they may plan future studies, and to address some of the questions and concerns in Dr. Stratton’s editorial, the lay press, and in social media,2 we would like to call attention to some inaccuracies in Dr. Stratton’s editorial, and to the lay media stories on which it appears to be based.

  • Summary

    The posterior fossa is an intracranial compartment that houses the cerebellum and the brainstem. Mass lesions and increased intracranial pressure (ICP) in this area can have profound consequences for neurologic, cardiac, and respiratory functions. This chapter presents a case study of a 54-year-old female with complaints of headaches, progressive hearing loss and episodes of aspiration that were increasing in frequency. A diagnosis of neurofibromatosis type 2 was made and a posterior fossa craniotomy was scheduled. Immediate concerns for the anesthetic team included (1) a depressed level of consciousness, (2) the potential for increased ICP, and (3) aspiration on induction. Neurosurgical procedures involving the posterior fossa can be challenging for both the surgical and anesthetic teams. Preoperative assessment and preparation of these patients should focus on the consequences of posterior fossa abnormalities as well as the potentially life-threatening complications that may occur intraoperatively.

  • Summary

    The perioperative pain management for craniotomies can be extremely challenging. This chapter presents a common clinical scenario and offers options for perioperative pain management. It presents a case study of a 52-year-old female American Society of Anesthesiologists class 3 patient presented for clipping of a cerebral aneurysm. The case described is a common example of the complexity frequently associated with neurosurgical patients. The combined regimen provided for analgesia and hemodynamic control, while allowing for an adequate neurologic examination. In addition, opioids were limited, thereby decreasing the risk of postoperative nausea and vomiting. Opioids are a key component of intraoperative and postoperative pain management for craniotomies. Morphine can cause histamine release, which can lead to venodilation and subsequent hypotension. A combination of intravenous analgesics and regional anesthesia can provide excellent pain relief and decrease the wide hemodynamic changes that can accompany anesthesia and surgery.

  • Summary

    The injury spectrum in traumatic brain injury (TBI) encompasses not only the initial insult, but includes the cascade of systemic responses and pathophysiology that occur after the focal or diffuse brain injury. This chapter presents a case study of a 49-year-old male motorcyclist who was evaluated in the emergency room after a motor vehicle collision with an articulated truck. Around the time of computed tomography (CT) scanning, the patient's right pupil became dilated and unreactive. Hypoxic cerebral damage, a common postmortem finding in TBI, is associated with arterial hypoxemia, decreased mean arterial pressure, or cerebral hypoperfusion, occurring as a consequence of shock, intracranial hypertension, or cerebral vasospasm. Although a definitively effective neuroprotective therapy in central nervous system (CNS) trauma remains elusive, through the skilled interactions of prehospital, emergency department, anesthesiology, and surgical personnel, the lives of many critically injured individuals can be saved and their neurologic function preserved.
