Domesticated cattle were brought to Ireland during the Neolithic. By the early medieval period, 4000 years later, these animals were central to social and economic status in Irish communities and the landscape was organised around cattle husbandry to a degree unattested elsewhere in Europe. How this socio-economic importance developed is unclear. Here, using isotope data spanning six millennia, the authors identify a culturally driven shift towards the creation and management of open pastures, which began in the Iron Age, eventually supplanting woodland grazing. Cattle continued to dominate the economy until the later medieval period when a shift to participate in silver-based trade led to a reassessment of Ireland's unique human-cattle relationship.

Research has shown that framing messages in terms of benefits or detriments can have a substantial influence on intended behavior. For prevention behaviors, positively framed messages have been found to elicit stronger behavioral intentions than negatively framed messages. Research also seems to indicate that certain contextual features contribute to the persuasiveness of a message. In the present research we test how message framing, contextually presented affect and the number of argument factors interact and contribute to the persuasiveness of a health related message. Consistent with our hypothesis, we found that, in our prevention focused task, increasing the number of arguments increased behavioral intentions (BI) for positively framed messages when subjects were cued, via negative affect, to be attentive to the message. This resulted in a significant framing effect for messages with the maximum number of arguments and a negative background picture. An account of contextual influence in persuasive health messages is discussed.

OBJECTIVES/GOALS: The goal of this study was to examine the attitudes of central Pennsylvania rural residents toward data sharing in the setting of the COVID-19 pandemic. We determined their willingness to provide cell phone data and personal information to public health staff investigating COVID-19 cases, as well as their concerns about sharing this information. METHODS/STUDY POPULATION: We used the electronic medical records of an academic healthcare institution in central Pennsylvania to obtain names and addresses of patients who had visited an outpatient clinic or been an inpatient within the prior three years, were 18 years or older, and who resided in a community defined as rural by the Commonwealth of Pennsylvania. The survey included four statements about contact tracing, three statements about intent to receive the COVID-19 vaccine, and validated scales for general trust and for distrust in healthcare organizations. All study variables were summarized to determine their distributions, and bivariate binomial logistic regression was conducted. A multivariable model including all of the independent variables was then fit for each outcome variable. RESULTS/ANTICIPATED RESULTS: The response rate was 19.5%. 95 % of respondents were white, 56% were female, and nearly two-thirds were older than 60 years. Binomial logistic regression showed that both distrust in healthcare organizations and political values influenced respondents willingness to share information with contact tracers as well as their concerns about sharing. In multivariate analysis, political values were a consistent predicator of willingness to share and concerns about sharing. Respondents who indicated that they wanted to get vaccinated as soon as possible were significantly more likely than those who did not want to be vaccinated to be willing to share their cell phone location data and personal data. Conversely, they were less likely to be concerned with these data being shared without their permission. DISCUSSION/SIGNIFICANCE: Understanding rural residents concerns about sharing personal information is critical if we are to develop successful strategies for lessening the impact of COVID-19 and managing future pandemics. This study is a first step in eliciting such concerns in the context of COVID-19and has implications for directing a successful pandemic response.

OBJECTIVES/GOALS: Vaccination for COVID-19 is a primary public health strategy to control the pandemic. In this study, we examined how various sociodemographic variables influence rural residents intentions to receive the COVID-19 vaccine. We also examined the role of distrust in healthcare organizations in these intentions. METHODS/STUDY POPULATION: sing the electronic medical records of an academic healthcare institution in central Pennsylvania, we obtained names and addresses of patients who had been an inpatient or outpatient within the prior three years, were 18 years or older, and who resided in a community defined as rural by the Commonwealth of Pennsylvania. The survey included three statements about the intent to receive the COVID-19 vaccine, an open-ended question about concerns regarding the vaccine, and validated scales for general trust and for distrust in healthcare organizations. All study variables were summarized to determine their distributions, and then bivariate binomial logistic regression analyses were conducted. Responses to the open-ended question were coded and used as variables in the bivariate analysis. RESULTS/ANTICIPATED RESULTS: Respondents reporting conservative political views were more likely (compared to those liberal political views) to never want to be vaccinated for COVID-19. Those who expressed distrust in healthcare organizations were less likely to want to be vaccinated soon. Conversely those who were more trusting said they were more likely to want to be vaccinated soon. Respondents dominant concerns about the COVID-19 vaccine were that it was new and that the process for its development was rushed. Respondents who believed that COVID-19 was a hoax were unlikely to ever want to be vaccinated, while those who distrusted the process in some way (new vaccine or rushed vaccine) were more likely to want to wait to be vaccinated. DISCUSSION/SIGNIFICANCE: These findings confirm the impact of political orientation on COVID-19 vaccination intention and suggest that distrust in healthcare organizations may prevent people from being vaccinated. These data provide evidence that people delaying vaccination hold different beliefs than those who will never vaccinate.

High-resolution analysis of the ice core from Colle Gnifetti, Switzerland, allows yearly and sub-annual measurement of pollution for the period of highest lead production in the European Middle Ages, c. AD 1170–1220. Here, the authors use atmospheric circulation analysis and other geoarchaeological records to establish that Britain was the principal source of that lead pollution. The comparison of annual lead deposition at Colle Gnifetti displays a strong similarity to trends in lead production documented in the English historical accounts. This research provides unique new insight into the yearly political economy and environmental impact of the Angevin Empire of Kings Henry II, Richard the Lionheart and John.

Reccopolis, in central Iberia, is the only archaeologically identified town founded by Germanic newcomers on Roman soil during the challenging socio-political and environmental circumstances of the mid to late sixth century AD. Despite archaeological investigations, doubts have persisted concerning the nature and size of Reccopolis. Recent geomagnetic survey, however, has revealed a dense urban fabric, unexpected new royal palace buildings, an extramural suburb and one of the potentially earliest Islamic mosques in Iberia. Reccopolis now stands as an exceptional example of early medieval urbanism that challenges our perceptions of urban development in sixth-century Europe.

OBJECTIVES/SPECIFIC AIMS: N/A. METHODS/STUDY POPULATION: Over a million people gained insurance in Los Angeles (LA) County under the Affordable Care Act (ACA). The vast majority gained Medicaid—government sponsored insurance with low-cost sharing. LA County also made significant investments in the safety net including a program called MyHealthLA, which provides primary and tertiary care for the residually uninsured including poor undocumented individuals at specific sites. Despite this insurance expansion, approximately 3 quarters of a million people in the county remain uninsured. Regardless of insurance status, nearly a quarter of LA County residents reported having difficulty obtaining needed medical care, and among those making less than the poverty level, 43% had difficulties. There is still much to understand about barriers to obtaining insurance and accessing healthcare in Los Angeles in the post-ACA era. Our primary objective was to understand how safety net patients are obtaining, maintaining and using their insurance after the ACA. Specifically we hope to understand the barriers and drivers of these three processes. RESULTS/ANTICIPATED RESULTS: We conducted a qualitative study of 34 safety net patients with 3 different insurance types in LA County. We conducted in-person interviews with adult patients (ages 18–64 years), who had either MediCal, MyHealthLA, or were unsinsured. Our interview guide was based on existing literature, a previous qualitative study conducted in Massachusetts and input from experts in the field. We pilot tested our interviews in English and Spanish and then recruited our participants from 3 sites: LAC+USC (a publically funded county hospital), The Wellness Center (a resource center for safety net patients), and White Memorial Medical Center (a private safety net hospital). We approached patients in the ED and urgent care waiting rooms and obtained informed consent for this IRB approved study. We excluded patients who were non-English and non-Spanish speaking or too ill to interview. We recorded interviews, which were then transcribed and translated into English by a contracted agency. We analyzed our interviews using a framework approach, which included a set of a priori codes from the literature as well as emerging codes from patient responses. We will check a sample of our transcripts for coding consistency (aiming for an inter-rater reliability of >80%). DISCUSSION/SIGNIFICANCE OF IMPACT: We recruited a diverse group of patients that were demographically representative of those who gained insurance under the ACA (childless adults making less than 138% of the Federal Poverty Level). Our preliminary results (based on 17 transcripts), suggest that patients, regardless of insurance type have difficulty accessing primary care. We identified seven domains under the broader theme of barriers to accessing primary care: finding a primary care clinic or physician (PCP), getting timely appointments, geography and transportation, continuity of care, using the Emergency Department (ED) or urgent care as a PCP, switching PCPs or clinics, and cost or coverage.

The seventh-century AD switch from gold to silver currencies transformed the socio-economic landscape of North-west Europe. The source of silver, however, has proven elusive. Recent research, integrating ice-core data from the Colle Gnifetti drill site in the Swiss Alps, geoarchaeological records and numismatic and historical data, has provided new evidence for this transformation. Annual ice-core resolution data are combined with lead pollution analysis to demonstrate that significant new silver mining facilitated the change to silver coinage, and dates the introduction of such coinage to c. AD 660. Archaeological evidence and atmospheric modelling of lead pollution locates the probable source of the silver to mines at Melle, in France.

Part I of this study synthesized what archaeological features and written evidence indicate about human mortality crises during the later Roman Empire. It argued that one could test competing ideas about mortality crises by assembling and analyzing the archaeological evidence of mass graves of our Type 1, defined as the resting place of more than 5 individuals who were interred more or less simultaneously. Such an archaeological data-set would clarify whether mass death figured significantly in the demographic and cultural landscape of late antiquity. It should, moreover, begin to elucidate when, where, and to what varying extent mortality crises may have been induced by violence, disease, or economic and environmental conditions. Part I reviewed the aggregate patterns of 85 archaeological features in 55 sites. Ten were very large burials (50-526 individuals); burials of between 5 and 9 individuals were most frequent (31 cases). Very large graves were predominantly urban, although large military burials occurred in the countryside. Eyewitnesses described huge mass graves in some of the empire's great cities, but few have been archaeologically documented until now. Nonetheless, the late Roman city that has been most carefully investigated, Jerusalem, has yielded 8 such sites. A third (10) of 29 burials that provide useful evidence document violent death. Although the dating of many features is imperfect, mass graves identified so far peaked in the 6th (24) and 7th (32) c.

The Roman empire was more than a system of institutions, beliefs and socio-economic structures; it was a concentration of human capital, physically located in the demographic strength of the population. Human health and mortality crucially affected, and reflected, the economy. As less optimistic interpretations of Late Roman history regain traction, it becomes important to find ways to test such interpretations, including their biological manifestations and implications. One approach would be to map over time and space large-scale violence and disease-driven surges in mortality, as well as chronic factors that may have more gradually eroded the empire’s human capital. Biomolecular archaeology and pathogen genetics are sparking novel explorations of ancient diseases, which ranged from the spectacularly acute to the chronic – malaria, leprosy or tuberculosis –, not to mention seasonal spikes in more routine gastrointestinal infections and the like. Individually and especially cumulatively, the impact of acute and chronic factors could have been of considerable magnitude. The same is true for environmental conditions; thanks to the advances of paleoclimate science, we now know that they too varied unexpectedly, and surely could affect population health.
