Only 6 to 8 % of the UK adults meet the daily recommendation for dietary fibre. Fava bean processing lead to vast amounts of high-fibre by-products such as hulls. Bean hull fortified bread was formulated to increase and diversify dietary fibre while reducing waste. This study assessed the bean hull: suitability as a source of dietary fibre; the systemic and microbial metabolism of its components and postprandial events following bean hull bread rolls. Nine healthy participants (53·9 ± 16·7 years) were recruited for a randomised controlled crossover study attending two 3 days intervention sessions, involving the consumption of two bread rolls per day (control or bean hull rolls). Blood and faecal samples were collected before and after each session and analysed for systemic and microbial metabolites of bread roll components using targeted LC-MS/MS and GC analysis. Satiety, gut hormones, glucose, insulin and gastric emptying biomarkers were also measured. Two bean hull rolls provided over 85 % of the daily recommendation for dietary fibre; but despite being a rich source of plant metabolites (P = 0·04 v. control bread), these had poor systemic bioavailability. Consumption of bean hull rolls for 3 days significantly increased plasma concentration of indole-3-propionic acid (P = 0·009) and decreased faecal concentration of putrescine (P = 0·035) and deoxycholic acid (P = 0·046). However, it had no effect on postprandial plasma gut hormones, bacterial composition and faecal short chain fatty acids amount. Therefore, bean hulls require further processing to improve their bioactives systemic availability and fibre fermentation.

What are the constraints, cues, and mechanisms that help learners create successful word-meaning mappings? This study takes up linguistic disjunction and looks at cues and mechanisms that can help children learn the meaning of or. We first used a large corpus of parent-child interactions to collect statistics on or uses. Children started producing or between 18-30 months and by 42 months, their rate of production reached a plateau. Second, we annotated for the interpretation of disjunction in child-directed speech. Parents used or mostly as exclusive disjunction, typically accompanied by rise-fall intonation and logically inconsistent disjuncts. But when these two cues were absent, disjunction was generally not exclusive. Our computational modeling suggests that an ideal learner could successfully interpret an English disjunction (as exclusive or not) by mapping forms to meanings after partitioning the input according to the intonational and logical cues available in child-directed speech.

Glacier monitoring has been internationally coordinated for more than 125 years. Despite this long history, there is no authoritative answer to the popular question: ‘Which glaciers are the largest in the world?’ Here, we present the first systematic assessment of this question and identify the largest glaciers in the world – distinct from the two ice sheets in Greenland and Antarctica but including the glaciers on the Antarctic Peninsula. We identify the largest glaciers in two domains: on each of the seven geographical continents and in the 19 first-order glacier regions defined by the Global Terrestrial Network for Glaciers. Ranking glaciers by area is non-trivial. It depends on how a glacier is defined and mapped and also requires differentiating between a glacier and a glacier complex, i.e. glaciers that meet at ice divides such as ice caps and icefields. It also depends on the availability of a homogenized global glacier inventory. Using separate rankings for glaciers and glacier complexes, we find that the largest glacier complexes have areas on the order of tens of thousands of square kilometers whereas the largest glaciers are several thousands of square kilometers. The world's largest glaciers and glacier complexes are located in the Antarctic, Arctic and Patagonia.

In this 2019 cross-sectional study, we analyzed hospital records for Medicaid beneficiaries who acquired nonventilator hospital-acquired pneumonia. The results suggest that preventive dental treatment in the 12 months prior or periodontal therapy in the 6 months prior to a hospitalization is associated with a reduced risk of NVHAP.

The foundational differences of steady and unsteady jets issued into a laminar boundary layer crossflow are considered. Jets have been used widely for flow control applications, due to their ability to enhance mixing and mitigate separation, but it is unclear what role jet steadiness plays in flow control effectiveness. Here we compare experimentally unsteady (synthetic) and steady rectangular jets issued into a flat-plate laminar boundary layer with varying orifice pitch and skew. The coherent streamwise vortices produced by unsteady jets were shown to be much stronger than those produced by steady jets, despite producing similar flow patterns. These differences are rooted in how vorticity is generated in the orifice, through either a Stokes layer (unsteady) or a Blasius boundary layer (steady). Exploring the time- and phase-averaged vorticity transport equation reveals that the time-varying vorticity term is the reason for the enhanced vortical structure. When considering flow control metrics, we find that the unsteady jet produced greater added momentum in the boundary layer and added vorticity when compared to a momentum-matched steady jet. Both the steady and unsteady jets produced similar jet penetration characteristics.

OBJECTIVES/GOALS: Osteoarthritis (OA) is a cartilage destroying disease. We are investigating abaloparatide (ABL) activation of parathyroid hormone receptor type 1 (PTH1R), which is expressed by articular chondrocytes in OA. We propose ABL treatment is chondroprotective in murine PTOA via stimulation of matrix production and inhibition of chondrocyte maturation. METHODS/STUDY POPULATION: 16-week-old C57BL/6 male mice received destabilization of the medial meniscus (DMM) surgery to induce knee PTOA. Beginning 2 weeks post-DMM, 40 μg/kg of ABL (or saline) was administered daily via subcutaneous injection and tissues were harvested after 6 weeks of daily injections and 8 weeks after DMM surgery. Harvested joint tissues were used for histological and molecular assessment of OA using three 5 μm thick sagittal sections from each joint, 50 μm apart, cut from the medial compartment of injured knees. Safranin O/Fast Green tissue staining and immunohistochemistry-based detection of type 10 collagen (Col10) and lubricin (Prg4) was performed using standard methods. Histomorphometric quantification of tibial cartilage area and larger hypertrophic-like cells was performed using the Osteomeasure system. RESULTS/ANTICIPATED RESULTS: Safranin O/Fast Green stained sections showed a decreased cartilage loss in DMM joints from ABL-treated versus saline-treated mice. Histomorphometric analysis of total tibial cartilage area revealed preservation of cartilage tissue on the tibial surface. Immunohistochemical analyses showed that upregulation of Col10 in DMM joints was mitigated in the cartilage of ABL-treated mice, and chondrocyte expression of Prg4 was increased in uncalcified cartilage areas in ABL-treated group. The Prg4 finding suggests a matrix anabolic effect that may counter OA cartilage loss. Quantification of chondrocytes in uncalcified and calcified tibial cartilage areas revealed a reduction in the number of larger hypertrophic-like cells in ABL treated mice, suggesting deceleration of hypertrophic differentiation. DISCUSSION/SIGNIFICANCE: Cartilage preservation/regeneration therapies would fill a critical unmet need. We demonstrate that an osteoporosis drug targeting PTH1R decelerates PTOA in mice. ABL treatment was associated with preservation of cartilage, decreased Col10, increased Prg4, and decreased number of large hypertrophic-like chondrocytes in the tibial cartilage.

Congenital left ventricle to coronary sinus fistula is a rare entity. We report a case of an infant with prenatal finding of left ventricle to right atrial shunt. The anatomy was defined by multi-modality imaging. Baseline electrocardiogram was notable for a Wolff–Parkinson–White pattern. He underwent successful catheter device closure of the left ventricle to coronary sinus fistula. The patient developed supraventricular tachycardia and underwent successful ablation of the accessory pathway.

Online intermediaries such as social network sites or search engines are playing an increasingly central role in democracy by acting as mediators between information producers and citizens. Academic and public commentators have raised persistent concerns that algorithmic recommender systems would negatively affect the provision of political information by tailoring content to the predispositions and entertainment preferences of users. At the same time, recent research indicates that intermediaries foster exposure to news that people would not use as part of their regular media diets. This study investigates these unresolved questions by combining the web browsing histories and survey responses of more than 7,000 participants from six major democracies. The analysis shows that despite generally low levels of news use, using online intermediaries fosters exposure to nonpolitical and political news across countries and personal characteristics. The findings have implications for scholarly and public debates on the challenges that high-choice digital media environments pose to democracy

In view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients.
