OBJECTIVES/GOALS: Several lines of studies have supported the existence of periodontitis (inflammation of the gums) as a risk factor for cardiovascular disease (CVD). The goal of this study is to evaluate the relationship between periodontitis and CVD among Hispanic, African American, and Caucasian populations. METHODS/STUDY POPULATION: We analyzed data from the National Health and Nutrition Examination Survey, 1999-2004 (NHANES). The population was all adults with a periodontal exam. Periodontal Disease was defined as mild, moderate, and severe (2 loss of attachments of at least 3mm, 2 sites with probing depth of at least 4mm, or one site with probing depth of at least 5mm). Cardiovascular disease was defined by a questionnaire regarding prevalence of any of 5 diagnosis (congestive heart failure [CHF], coronary artery disease [CAD], angina, heart attack or stroke). Data were analyzed using multinomial regression in SAS version 9.3 taking into consideration the design and weight. RESULTS/ANTICIPATED RESULTS: The study included 3375 adults; 13% were Hispanic and 10% were Blacks, 58% had > high school education, 81% were insured, 11% were heavy alcohol drinkers, 27% were smokers, 13% were physically inactive, 14% had periodontitis, 62% visited dentist last year, 2% had CHD, and 1.5% had CHF or stroke. In the multiple multinomial regression, overall, people with periodontitis were more likely to have both CHD (AOR = 2.0, 95% CI = 1.1-3.8, p<0.05) and CHF or stroke (AOR = 1.8, 95% CI = 1.01, 3.0, p<0.05) than to have no heart condition. There was a racial/ethnic difference in the relationship between periodontitis and cardiovascular disease but it was not statistically significant (p>0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: Overall, people with periodontitis were more likely to have CHD, CHF or stroke than to have no heart condition, but with no significant effect of racial/ethnic group. This study provides a foundation to future studies on the connection of periodontitis and CVD in relation to ethnic/racial groups.

OBJECTIVES/GOALS: Diabetes is a prevalent chronic illness that imposes health-related burdens including nephropathy, retinopathy and sleep disorders. The goal of this study was to examine the relation between both sleep disorders and sleep duration and diabetic chronic kidney disease (CKD) and retinopathy. METHODS/STUDY POPULATION: We analyzed data from the National Health and Nutrition Examination Survey 2005-2016 related to diabetic nephropathy and retinopathy, sleep disorders and duration, demographics, and risk factors among diabetics. The subjects were adults with diabetes type 2. Multiple logistic regression analysis was performed to look at the relationship between diabetic complications (CKD and retinopathy) and sleep disorders and sleep duration adjusting for demographics and risk factors. RESULTS/ANTICIPATED RESULTS: Of the 4087 diabetics, 45% had CKD, 19% had retinopathy, and 15% had sleep disorders. CKD and retinopathy were not associated with sleep disorders (p>0.05) but CKD was associated with sleep duration (Adjusted odds ration = 1.014, 95% confidence interval = 1.001-1.027, p<0.05). Cardiovascular disease was a predictor of both CKD and nephropathy (P<0.05). Other predictors of CKD and nephropathy were age >60 years, Non-Hispanic Black, hypertension, low education level, and living under 200% of the Federal Poverty Level (P<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT : Among diabetics, CKD and retinopathy were not associated with sleep disorders, and only CKD was associated with sleep duration. These findings may impact the management of diabetes in the future, since it has effects on a range of other health conditions.

OBJECTIVES/GOALS: Asthma is a life-long, chronic lung disease that inflames and narrows the airways. Its effects on quality of life in children can be exacerbated. The goal of this study was to investigate the link between asthma, family structure and demographics and how it impacts quality of life in children. METHODS/STUDY POPULATION: We analyzed data from a cross sectional study of the 2016-2017 National Survey of Children’s Health, NSCH, to assess the relationship between determinants of health variables and the outcome variable of parent’s report of child’s perceived health status (quality of life). The study population was children under the age of 18. Data were analyzed using descriptive, bivariate analysis using Chi square, and multiple logistic regression of quality of life and family structure adjusting for confounding variables. RESULTS/ANTICIPATED RESULTS: The study included 5,687 children. Significant predictors of asthmatic children’s quality of life were severity of asthma, self-perceived mental/physical health status of adults, neighborhood safety for children (p <0.05). The interaction between family structure and asthma severity was significant indicating that asthma severity was an effect modifier. Among children with mild asthma, predictors of quality of life were self-perceived mental/physical health status of adults in the household, neighborhood safety of children, physical activity status of children (p<0.05). Among children with severe asthma, predictors were family structure and physical/mental health of adults (p<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: This study suggests children with severe asthma who are born to single mothers with lower parental reporting of physical/mental health status had a lower quality of life. A longitudinal study could be implemented to target these three measures to improve quality of life among these children. Also, a culturally adapted intervention involving community, parents, and providers is needed to improve the quality of life of the children with asthma.

OBJECTIVES/GOALS: Diabetes mellitus is a common metabolic disease. Uncontrolled diabetes can lead to complications. The objective of this study is to examine the racial/ethnic variation in the relation between diabetes control and healthy eating, food security, exercise, and access to health care. METHODS/STUDY POPULATION: We analyzed data related to diabetes control, demographics, insurance, healthy eating, food security and physical activity for 949 diabetics from the National Health and Nutrition Examination Survey (NHANES). The population examined was adults with diabetes mellitus. Diabetes control was classified as fair control [HbA1c 7-<8 and fasting glucose <126 mg/dL], good control [HbA1c<7 and fasting glucose <110 mg/dL] or uncontrolled [HbA1c8 and fasting glucose <110 mg/dL]. We used multinomial logistic regression controlling for confounders to analyze the data overall and for each racial/ethnic group and report adjusted odds ratios (AOR) and 95% confidence limits (CL). RESULTS/ANTICIPATED RESULTS: Of the 949 diabetics, 14.7% were Blacks, 15.9% were Hispanics, 11.0% had fair control, 61.0% had good control, 14.2% were uninsured, 18.1% had low/very low food security, and 39.7% were inactive. Overall, uninsured subjects had a lower chance of fair diabetes control (AOR = 0.2, 95% CL = 0.1-0.9, p = 0.04), but this relationship was significant only for Hispanics and Blacks (p<0.05). Whites with low/very low food security were less likely to have fair control (AOR = 0.2, 95% CI = 0.001-0.9, p = 0.04). Blacks who were inactive, and insufficiently active Hispanics had a lower chance of good/excellent diabetes control (AOR = 0.5, 95% CI = 0.2-0.9, p = 0.03; AOR = 0.3, 95% CI = 0.1-0.7, p = 0.007 respectively). DISCUSSION/SIGNIFICANCE OF IMPACT: The results show the importance of insurance coverage, food security and physical activity in diabetes control among different racial/ethnic groups. They indicate a need for affordable health care and for culturally-relevant interventions that include physical activity and food security.

OBJECTIVES/SPECIFIC AIMS: The aim of the study is to examine the relation between dental fluorosis, serum and water levels of fluoride and Chronic Kidney Disease (CKD) among children. A link between dental fluorosis, fluoride level and CKD can be an indicator of the blind danger of fluoride toxicity that poses a great threat to the human health. METHODS/STUDY POPULATION: Dental fluorosis, serum and water levels of fluoride and CKD were examined in children 6-19 years old, using data from the National Health and Nutrition Examination Survey1999-2012 and 2013-2016. We used multiple logistic regression to adjust for the confounders (demographics, insurance, dental visit, and co-morbidity) to assess the relation between dental fluorosis, serum and water levels of fluoride and CKD. STATA 14.0 was used to analyze the data (sample design and weight). P < 0.05 is statistically significant. RESULTS/ANTICIPATED RESULTS: The prevalence of CKD was 13.9% and dental fluorosis was 34.3%. In the multivariate model, plasma fluoride level was independently associated with CKD (Adjusted Odds Ratio (AOR) = 1.68, 95% Confidence Interval (CI) = 1.06-2.68, p = 0.029) but not with dental fluorosis (AOR = 1.4, 95% CI = 0.87-2.2, p = 0.17) or water fluoride level (AOR = 0.91, 95% CI = 0.59-1.396, p = 0.659). DISCUSSION/SIGNIFICANCE OF IMPACT: Results indicated that serum fluoride level is independently associated with CKD but dental fluorosis and water fluoride level were not related to CKD. Increase awareness and screening for fluorosis in children are needed for early detection and prevention of organ damage. Prospective studies related to fluorosis and tissue damage are needed.

OBJECTIVES/SPECIFIC AIMS: To examine the relationship between healthy eating, physical activity (PA), sleep problem and hours of sleep and cognitive function among elderly population and the racial/ethnic differences in this relation. METHODS/STUDY POPULATION: We analyzed data from National Health and Nutrition Examination Survey 2014-2016 for 882 population 60 years and older. Cognitive status was measured by the Digit Symbol Substitution (DSS) exercise score and the Consortium to Establish a Registry for Alzheimer’s Disease (CERAD) total score. Healthy eating index (HEI), PA, and sleep problem and hours of sleep were assessed by questionnaire. The association between cognitive function and HEI, PA, sleep problem and hours of sleep were assessed by linear regression after adjusting for age, gender, race/ethnicity, poverty level, lipid profile, fasting glucose level, alcohol, body mass index, stroke and education. Data were analyzed using Stata 14 considering design and sample weight and p<0.05 is statistically significant. RESULTS/ANTICIPATED RESULTS: CERAD total score was associated with HEI (Adjusted B = 0.07, 95% Confidence Interval (CI) = 0.01-0.13, p = 0.02) and not associated with physical activity or sleep problem or hours of sleep (p > 0.05). Animal fluency score was associated only with HEI (Adjusted B = 0.05, 95% CI = 0.01-0.09, p = 0.02). DDS score was not associated with HEI, PA, or sleep problem (p > 0.05) but associated with hours of sleep (p = 0.03). Stratified analysis by race/ethnicity showed that CERAD total score was associated with HEI only in White (Adjusted B = 0.08, 95% CI = 0.01-0.15, p = 0.02). DISCUSSION/SIGNIFICANCE OF IMPACT: CERAD total score was associated with HEI and not associated with PA or sleep problem. Promoting healthy eating is important for improving cognition in elderly population. Culturally sensitive and linguistically appropriate programs that involve community and care providers are needed to promote healthy eating for elderly population.

OBJECTIVES/SPECIFIC AIMS: Lead (Pb) exposure can seriously affect nervous system and kidney. Young children are vulnerable to Pb exposure. However, the role of low-level Pb exposure in asthma in children and the age and racial disparity is not well studied. The objectives are to examine the relation between Pb level and asthma status and to determine the age and racial/ethnic differences in this relation. METHODS/STUDY POPULATION: We analyzed data from National Health and Nutrition Examination Survey 1999-2016 for 22,885 children 1-15 years old. Asthma information was collected by questionnaire. Blood lead level was measured using mass spectrometry. The association between blood Pb level and asthma status was assessed by logistic regression after adjusting for children’ age, gender, race/ethnicity, insurance status, and source of care; household poverty, mother’s age and smoking status. Data were analyzed using Stata 14 considering design and sample weight and p<0.05 is statistically significant. RESULTS/ANTICIPATED RESULTS: Pb level was associated with asthma status (Adjusted Odds Ratio (AOR)=1.4, 95% Confidence Interval (CI) = 1.2-1.7, p < 0.001). Stratified analysis by age showed that blood Pb level is related to asthma only in children 1-5 years old (AOR = 1.3, 95% CI = 1.1-1.5, p = 0.004). There was no racial/ethnic difference in this association. DISCUSSION/SIGNIFICANCE OF IMPACT: Pb level is associated with asthma status in children especially young children. Health risk of low Pb is a concern. Preventive measures by reducing potential sources of Pb should be introduced early.

OBJECTIVES/SPECIFIC AIMS: Previous studies showed that 52% of smokers were unsuccessful in quitting smoking. Smoking in alcoholics is 2-3 times that of the general population with 50%-80% of alcoholics smoking regularly. Studies have linked several genetic variants to addiction. We examined the relation between successful quitting smoking, alcohol use, and genetic data for CYP2A6, CYP2B6, DRD2, DRD1 and GABRB1 alleles. METHODS/STUDY POPULATION: We analyzed data from NHANES III 1988-1994 for socioeconomic factors, physical activity, body mass index (BMI), alcohol status, successful quit smoking, and genetic data for CYP2A6, CYP2B6, DRD2, DRD1 and GABRB1 alleles. Multivariate logistic regression was used to examine the association between successful quit smoking and genotypes adjusting for other variables. Data were analyzed using SAS version 9.3 (design & weight). RESULTS/ANTICIPATED RESULTS: Of the 2,269 smokers, 57% were current smokers, 35% were heavy drinkers, 24% were both smokers & heavy drinkers and 41% successfully quitted smoking. Successfully quit smoking was associated with CYP2A6 (rs28399433-TG) (adjusted odds ratio (AOR) = 3.6, 95% confidence interval (CI) = 1.1-11.9, p = 0.03), CYP2B6 (rs2279343-AA and AG) (AOR = 2.3, 95%CI = 1.5-3.5, p = 0.0003 for AA & AOR = 2.3, 95%CI = 1.2-4.2, p = 0.01 for AG) and DRD1 (rs4532-AA) (AOR = 2.2, 95%CI = 1.01-4.6, p = 0.04). Among heavy drinkers, those with CYP2A6 (rs28399433-TG) and CYP2B6 (rs2279343-AA and AG) were more likely to successfully quit smoking and those with CYP2A6 (rs5031017-GG) and GABRB1 (rs1442099-CC) were less likely to successfully quit smoking (p<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: We concluded that while rs28399433-TG, rs2279343-AA & AG positively impacted the success to quit smoking, rs5031017-GG & rs1442099-CC negatively impacted the success in quitting smoking both overall and specifically in heavy drinker smokers.

OBJECTIVES/SPECIFIC AIMS: Our objective is to examine the relationship between depression, childhood asthma and family structure. METHODS/STUDY POPULATION: We analyzed data from The National Survey of Children’s Health 2011-2012 for the variables family structure, parent’s smoking, education, poverty, available emotional support, and children’s age, gender, race/ethnicity, asthma, physical/mental health, access to care, and insurance. Data were analyzed using multiple logistic regression in STATA 14 (sampling and weight). RESULTS/ANTICIPATED RESULTS: Of the 95,000 participants, 3.8% had depression, 5.1% had anxiety, 8.5% had emotional, developmental, or behavioral problem, 14.6% had asthma and 19% were single-parent family. Asthmatics had higher chance for depression relative to non-asthmatics (Adjusted Odds Ratio (AOR)=1.5, 95% confidence interval (CI)=1.2-1.8), p=0.001). Those living in a single-family home had higher chance for depression than those living in 2-parents homes (AOR=2.5, 95% CI=1.9-3.2), p=0.001). Other predictors were lack of emotional support, age, poverty <100%, poor health, and smoking parent (p<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: We concluded that depression was associated with asthma in children from poor single family who lack an emotional support. Management of asthma should include screening and management of psychosocial factors.

OBJECTIVES/SPECIFIC AIMS: To examine the racial/ethnic variation in the relation between metabolic syndrome (MetS) components and cardiovascular disease (CVD) as well as examine the role of uric acid as a predictor of CVD among population with MetS. METHODS/STUDY POPULATION: We analyzed National Health and Nutrition Examination Surveys data (1999–2010) for adults aged >20 years with MetS. Using the ATP III clinical criteria for diagnosing MetS, subjects were classified as having MetS if they had ≥3 of the following: waist circumference ≥40 inches for men or ≥35 inches for women, triglyceride ≥150 mg/dL, HDL-C for men ≤40 mg/dL; women ≤50 mg/dL, pre-hypertension, or fasting glucose ≥110 mg/dL. We used multiple logistic regression in STATA 14 survey module to examine the relation between MetS components and CVD adjusting for age, gender, race/ethnicity, education, smoking, alcohol, albuminuria, glomerular filtration rate, C-reactive protein, uric acid and white blood count. To assess the racial/ethnic variation, we examined the same model in each race/ethnic group. RESULTS/ANTICIPATED RESULTS: Of the 3212 subjects, 78% were Whites, 10% were Blacks, and 15% had CVD. MetS components, CVD, and uric acid varied significantly by race/ethnicity (p<0.05). In the multivariate model, HDL-C level [odds ratio (OR)=1.5; 95% confidence interval (CI)=1.1–2.0], triglyceride level (OR=2.0; CI=1.4–2.9), and elevated uric acid (OR=1.4; CI=1.1–1.9) were independently related to CVD (p<0.05). While CVD was independently related to HDL-C, triglyceride, and elevated uric acid in Whites (p<0.05), it was associated with pre-hypertension and triglyceride in Blacks (p<0.05) and no predictors in Hispanics (p>0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: Elevated uric acid, HDL-C, and triglyceride levels are significant independent predictors of CVD among population with MetS. These predictors varied by race/ethnicity. Health care providers should be vigilant in the management of MetS components and control of uric acid level in each racial/ethnic group to prevent the CVD risk among the population with MetS.

OBJECTIVES/SPECIFIC AIMS: To diversify the biomedical research workforce by training postdoctoral scholars and junior faculty from 6 Minority Serving Institutions (MSIs) on practical research skills such as Critical and Creative Thinking, Formulating the Problem, Asking the Right Question, Grant Writing, and Team Science METHODS/STUDY POPULATION: In collaboration with our partners, we identified 11 topics where trainees lack research funding. Next, we identified instructors for these topics. We converted the topics to online module with modules ranging from 2 to 8 weeks. In working with an online education expert, we developed innovative online training using Moodle as the content management system. Scholars complete readings, videos, self-assessments and participate in discussion board each week. In addition, we have weekly synchronous sessions for each module. All scholars are required to take the grant writing module and 8 other modules. After each module, trainees complete a brief survey to evaluate the module. The leaders at the MSI participated in an intensive face-to-face training session on how to be a career coach so that they could be career coaches for the LEADS Scholars at their home institutions. RESULTS/ANTICIPATED RESULTS: In the first year, we selected 13 LEADS Scholars. All but 3 scholars elected to take every module. The 3 scholars did not enroll in the Peer Reviewing module. Results of the brief survey at the end of each module indicate that the scholars value each of the modules and rate them very highly. When 1 scholar wanted to leave the program, we decided to have a conference call with all of the LEADS Scholars to determine what was working and what was not working with the program. All scholars recognized the value of LEADS. Some scholars felt that the weekly synchronous session was too demanding as they have competing demands on their time. We consulted with the leadership at the MSI and decided to modify the requirements of the program such that every synchronous call was not required for successful completion of the module and to earn a badge. Scholars need to have at least 9 badges to earn a certificate. In addition to the training, we decided that scholars would also benefit from mock reviews of their grants. This will help them submit successful grants. We learned that the best way to serve the needs of the scholars is to work iteratively with the scholars and leadership to develop a successful program that most effectively meets their needs of the scholars and helps them launch a successful career. DISCUSSION/SIGNIFICANCE OF IMPACT: Postdoctoral scholars and junior faculty from MSI need practical research training to help launch their research career. We suspect that this is true of many institutions and plan to develop these modules so that they can be widely disseminated to other institutions.

OBJECTIVES/SPECIFIC AIMS: Examine mental health service use and its correlates among depressed group in a national sample of population≥20 years old. METHODS/STUDY POPULATION: Analysis of data for adult≥20 years old from the NHANES 2006–2012. Depression was assessed using the 9-item PHQ. The use of mental health and antidepressant drug were used to indicate the service use. We utilized multiple logistic regressions to determine the independent association between service use and each independent variable (demographics, health status, food security, chronic conditions, and depression severity) controlling for other independent variables. Data were presented as adjusted odds ratio (AOR), 95% confidence interval (95% CI), and p-value of statistical significance. p-value of<0.05 indicates statistical significance. RESULTS/ANTICIPATED RESULTS: Of the 17,824 subjects, 22% had mild to severe depression. Among the depressed group, 25% used antidepressant, 17% used mental health service. For the use of mental health services among the depressed group, African-American (AA), ≥60 years old, uninsured and foreign born were less likely to use the mental health service relative to other groups [AOR=0.58 (95% CI=0.45–0.75), 0.21 (95% CI=0.14–0.33), 0.61 (95% CI=0.45–0.83), 0.41 (95% CI=0.17–0.99), respectively, p<0.05]. For the use of antidepressant drug among the depressed group, AA, Hispanics, uninsured and foreign born were less likely to use antidepressant drug relative to other groups [OR=0.26 (95% CI=0.20–0.33), 0.42 (95% CI=0.31–0.57), 0.41 (95% CI=0.31–0.56), 0.20 (95% CI=0.10–0.78), respectively, p<0.05). For the use of mental health services and/or antidepressant drug among the depressed group, 40–59 years old, AA, Hispanics, uninsured, foreign born were less likely to use mental health services and/or antidepressant drug relative to other groups [OR=0.52 (95% CI=0.38–0.72), 0.35 (95% CI=0.28–0.43), 0.52 (95% CI=0.40–0.69), 0.53 (95% CI=0.41–0.68), 0.30 (95% CI=0.13–0.68), respectively, p<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: Our study showed that minority (AA and Hispanics), foreign born and uninsured with depression were less likely to use mental health services and/or antidepressant drug relative to other groups. Culturally and linguistically adapted intervention that involves community and providers to increase awareness about depression and the available services/treatment among minority, immigrant, and uninsured population are needed.
