The residency is one of the most stressful periods in medical practice and entails different psycopathological disorders. Individual type of adaptation plays an important role in the pychological response to this situations.

The aims are to describe the coping behaviour used during the residency period, and to analyze the factors related to them.

Cross-sectional study in 145 residents, in wich we valued sociodemographic data, psychopatholy (GHQ Goldberg), personality dimensions (16PF-A Cattell), psychic antecedent and coping behaviour (Lazarus and Folkman, 1986). A descriptive, comparative and a Pearson correlation study was performed.

The sociodemographic variables and the frecuency of the coping behaviors used are detailed in table. We described their relation with personality features and sociodemographic variables, and the coping associated with psychic antecedent and psychopathology.

The more used behaviors were those directed towards Planful problem-solving, Seeking social support, Self-controlling, Positive reappraisal, Confrontive coping and Distraction. Coping behaviour are related with various factors that probably caused them, being personality features outstanding. The socio-demographic variables also are related, and in women are more frecuent Seeking social support and Selfaward.

Although this study, due to its transversal structure, can not establish a causal relationship between coping behaviour and the presence of psychopathology, we observed that the latest one was associated with Selfblame, Distancing and Avoidance behaviors and could be considered as inefficient strategies. In those with personal psychic antecedents, Distraction and Selfaward behaviors were outstanding, although this mechanisms were not related to psychopathology.

Four psychotherapies have been recognized as effective with scientific evidence for the treatment of BPD, but are long term techniques. It is necessary to explore new time limited psychotherapies in order to be more accessible.

We have developed a specific manualized psychotherapy for BPD named Psychic Representation focused Psychotherapy (PRFP)

To assess the efficacy of the PRFP in BPD in an outpatient care setting compared to a control group receiving psychiatric treatment “as usual” in several specific symptoms and in diminishing the disability due to the illness.

60 subjects with BPD were randomized to one of the two treatment groups. The study group has received PRFP with 20 sessions on a weekly basis; the control group has received treatment “as usual”. Both groups may receive psychopharmacological treatment. The assessment is done in four time-points: at baseline, after the psychotherapy or conventional treatment (six months), and at a six and twelve month's follow-up period.

Preliminary results of the first 30 patients (control group 17, experimental group 13, without significant differences, Age 18–35 years; 70% women) assessed at the baseline and at the end of the intervention (six months). Experimental group reached a statistically significant clinical improvement over the controls in all measured variables: Scales: SCL-90; Zanarini ; MDRS; Barrat; STAI anxiety state; Rosemberg self-esteem and SASS social adaptation.

The preliminary results are encouraging and reveal that this method could be effective. This study state the interest in develop more studies about time limited psychotherapy for BPD.

Pegylated-interferon (pegIFN) plus ribavirin (RBV) is for the moment the licensed therapy for chronic hepatitis C. Neuropsychiatric symptoms are frequent side effects due to the treatment with pegIFN, registered in 30 to 80% of instances, with mood rather than psychotic disorders usually observed.

A manic profile with psychotic symptoms in a HCV infected patient receiving pegIFN-RBV is presented to discuss management strategies and outcome.

A 47 year-old male patient, former IDU and alcohol abuser, without past psychiatric history, was under therapy for chronic hepatitis C (genotype 1a, HCV-RNA 6.2 log IU/mL, Child A5). After 14 weeks of pegIFN-RBV treatment the patient attends ER with insomnia, uneasiness, expansive mood, irritability, extravagant and disinhibited behaviour, and detrimental and self-referential delusion.

The patient was admitted at the Psychiatric Unit to receive amisulpride (400 mg/12h) plus lorazepam (1 mg/8h), with psychotic symptoms regressing over following days. It was agreed that psychotropic drugs would be maintained at the same doses until the end pegIFN-RBV therapy, intended for 48 weeks.

Antipsychotic therapy may be effective and allows VHC infected patients with manic symptoms related to pegIFN-RBV therapy to continue their treatment.

For the last years, there has been a wordlwide increase in the interest of oriental culture, specifically in the practice of meditation. This technic is known to produce cognitive and sensorial perception alterations, with changes in the hormonal and autonomic system. This practice has been associated with different mental disorders such as neurotic manifestations, dissociative and regressive, and even psychotic symptoms.

To analyze the influence of an intensive practice of different styles of meditation in the psychopathology found in our patients (Hospital Clínico San Carlos, Madrid). To study the changes produced in the Central Nervous System measured by Electroencephalogram, PET, and functional MRI.

To define the clinical characteristics, prognois, risk factors and biological changes of an “atypical” psycopathologic case which seems to be more frequent everyday.

Extensive bibliographic review focused in psychopathology related to different types of meditation, either auto or hetero-induced (exorcism). We analyzed the patients attended in our hospital with psychotic semiology after having practiced intensive types of meditacion (transcendental meditation, buddhist meditation, tai-chi, qi-gong, tiao-shen, etc).

We found serious psychotic and affective symptomatology produced after using these technics. It is important to point out the risk factors and traumatic experiences characteristic of these processes.

There is clinical and epidemiological evidence that a group of psychosis, with different symptomatology and prognosis than schizophrenia and affective disorders, exists. We describe a syndrome of sudden manifestation, with flourished symptoms and serious behavior alterations, etc.

The study of associated stigma to mental illness has been usually oriented to psychotic disorders. Recently the ASPEN Project has had the objective to study the stigma associated to depressive disorders worldwide. As a part of this study, we present the results of a Spanish sample. We assessed the nature and severity of experienced and anticipated discrimination reported by adults with major depressive disorder. Additionally we investigated whether experienced discrimination is related to severity of depression and level of self-esteem.

In a cross-sectional survey, 100 adult outpatients with a diagnosis of major depressive disorder were interviewed with the discrimination and stigma scale (version 12; DISC-12). Severity of depression and self-esteem were measured by the Hamilton-17 questionnaire and the Rosemberg scale respectively. Socio-demographic variables were included.

the life domains where the major proportion of individuals experienced discrimination were: treatment by their family (46%), in marriage or divorce (30%) and been avoided or shunned by people who know their mental health problem (34%). 60% of patients anticipated discrimination avoiding intimate relationships and 67% of them concealed or hid his/her mental health problem from others. Experienced discrimination had a significant association with the severity of depression and with a low level of self-esteem.

Discrimination related to depression acts as a barrier in some fields of social life. Non-disclosure of depression could be a barrier to seeking help. It is important to consider effective actions to prevent stigma and discrimination in the field of depressive disorders. INDIGO-ASPEN Group
