This study describes changes in dining practices and provider perspectives on meal-related challenges due to the coronavirus disease (COVID-19) pandemic. An online survey was disseminated between July and September 2020 through stakeholder networks and social media with 1,036 respondents. Altered dining practices included residents eating in rooms (54.3%), spacing residents in common areas for meals (69.3%), and disposable dish use (44.9%). The most common mealtime challenges were reduced socializing opportunities at meals (29.3%), inadequate staffing (22.8%), reduced family/volunteer help (16.7%), and assisting residents to eat (10.5%). Many participants (72.2%) felt conflict balancing safety and relationship-centred care. Geographic region, home size, building age, respondent’s job title, pre-pandemic relationship-centred practices, and mealtime satisfaction, and some pandemic-initiated practices were associated with mealtime challenges and feeling conflicted in binary logistic regression analyses. Considering trade-offs between safety and relational aspects of mealtimes during the pandemic is crucial.

This SHEA white paper identifies knowledge gaps and challenges in healthcare epidemiology research related to coronavirus disease 2019 (COVID-19) with a focus on core principles of healthcare epidemiology. These gaps, revealed during the worst phases of the COVID-19 pandemic, are described in 10 sections: epidemiology, outbreak investigation, surveillance, isolation precaution practices, personal protective equipment (PPE), environmental contamination and disinfection, drug and supply shortages, antimicrobial stewardship, healthcare personnel (HCP) occupational safety, and return to work policies. Each section highlights three critical healthcare epidemiology research questions with detailed description provided in supplementary materials. This research agenda calls for translational studies from laboratory-based basic science research to well-designed, large-scale studies and health outcomes research. Research gaps and challenges related to nursing homes and social disparities are included. Collaborations across various disciplines, expertise and across diverse geographic locations will be critical.

This study determines the prevalence of inadequate micronutrient intakes consumed by long-term care (LTC) residents. This cross-sectional study was completed in thirty-two LTC homes in four Canadian provinces. Weighed and estimated food and beverage intake were collected over 3 non-consecutive days from 632 randomly selected residents. Nutrient intakes were adjusted for intra-individual variation and compared with the Dietary Reference Intakes. Proportion of participants, stratified by sex and use of modified (MTF) or regular texture foods, with intakes below the Estimated Average Requirement (EAR) or Adequate Intake (AI), were identified. Numbers of participants that met these adequacy values with use of micronutrient supplements was determined. Mean age of males (n 197) was 85·2 (sd 7·6) years and females (n 435) was 87·4 (sd 7·8) years. In all, 33 % consumed MTF; 78·2 % (males) and 76·1 % (females) took at least one micronutrient pill. Participants on a MTF had lower intake for some nutrients (males=4; females=8), but also consumed a few nutrients in larger amounts than regular texture consumers (males=4; females =1). More than 50 % of participants in both sexes and texture groups consumed inadequate amounts of folate, vitamins B6, Ca, Mg and Zn (males only), with >90 % consuming amounts below the EAR/AI for vitamin D, E, K, Mg (males only) and K. Vitamin D supplements resolved inadequate intakes for 50–70 % of participants. High proportions of LTC residents have intakes for nine of twenty nutrients examined below the EAR or AI. Strategies to improve intake specific to these nutrients are needed.

Ready-to-use modified-texture food (rMTF) products are commercially available and may have greater appeal than conventional in-house or commercial bulk modified-texture food (cMTF) products. A nine-month pilot study using a prospective interrupted time-series design where participants (n = 42) served as their own controls investigated the impact of cMTF + rMTF on weight goals, weight, food intake, and co-morbidity. Seventy-four per cent of participants achieved their weight goals at the end of six months on rMTF and, although insignificant, participants did have a trend towards weight gain while on rMTF (OR 3.5 p = .16). Main-plate food intake (grams) was not significantly different over time, but a downwards trajectory suggests decreased consumption that was compensated for by a significantly higher fat intake during the intervention period (p = .01). Increased co-morbidity and a decreasing volume of food consumed are common in older adults with dysphagia, and enhanced food products are needed to meet nutrient needs. Methodological issues encountered in this study can provide guidance for future work.

Little is known about how persons with dementia and their care partners respond to mealtime changes that occur throughout the dementia journey. By interviewing 27 persons living with dementia and their 28 care partners, we explored the meaning and experience of change surrounding mealtimes. Participants adjusted to mealtime change by adapting to an evolving life, as a result of a dynamic process of becoming aware of change, attaching meaning to change, and responding to change. Seminal events compounded by a sense of things being different triggered awareness of mealtime changes. Meaning was attached to mealtime changes, observed through emotions experienced and diverse strategies developed to support mealtime values. Responding to change ranged from resisting, to being in a holding pattern, to transforming and adapting. Understanding how individuals and families adjust to mealtime changes, and the strategies they develop, provides critical insights for supporting families throughout the dementia journey.

Food insecurity in older adults is influenced by financial constraints, functional disability, and isolation. Twenty-eight social- and community-service providers participated in four focus groups to report (a) perceptions and experiences with food insecurity in their older clients, (b) beliefs about their potential role(s) in promoting food security, and (c) opinions about constraints that influenced these roles. A constant comparison analysis identified key themes. The formal caregivers reported six roles for improving food security: (a) monitoring, (b) coordination, and (c) promoting services, (d) education, (e) advocacy, and (f) providing a social environment. The final theme summarizes these roles as “the need for personalization of service”. Social and community service providers are involved in roles that can promote the health of older adults by addressing their food insecurity. Social service providers need to be acknowledged and supported in this health promotion role.

Information on the characteristics of seniors who use supplements is lacking. A convenience sample of community-living seniors (n = 128) was recruited from 10 sites to identify factors associated with supplement use (vitamin-mineral and/or herbal preparations). Data on nutritional variables, health history, and supplement use were collected with an interview-administered questionnaire. A Propensity to Use Supplements scale was developed and was associated with increased use of herbal supplements. At the bivariate level, few characteristics distinguished users and non-users. Multivariate modelling revealed the Propensity scale and a low Body Mass Index to be factors related to the total number of supplements used. When individual scale items were examined at the bivariate level, food supply concerns, health beliefs, and a proactive role in one's own health were associated with increased supplement use. Initial results suggest seniors' attitudes towards supplements may be an important factor contributing to their decision to use supplements. Further work is required in a more diverse, randomly selected, older Canadian population.

Older-adult community programs are significant partners in the identification of need and delivery of health care for seniors. At present there is no systematic screening for nutritional risk in Ontario, and the interest and resources of community programs to screen is unknown. From three Ontario organizational membership lists, 200 programs were randomly selected; 136 key informants completed and returned the survey. A diverse sample of programs was included. Most were providing some form of nutrition programming, with the most common being meal provision. Two thirds (67.7%) were collecting some form of nutrition information: 56.4 per cent had an assessment questionnaire with nutrition information, and 21.8 per cent had clients subjectively assess their own nutritional risk. Most providers were interested in the nutritional health of their clients, and over half were interested in formally screening for nutritional risk. Barriers to screening were also identified. It is clear that nutrition is an area of priority for community programs and that nutrition screening is desired. Barriers to ethical screening need to be addressed prior to implementation of a systematic screening program.

The purpose of this study was to determine the extent to which functional ability was associated with dietary intake in community-living seniors with some activity limitation. In this cross-sectional survey, 145 seniors were recruited from 15 recreation, day programs or seniors' apartment complexes. Dietary intake was assessed with a food frequency questionnaire and evaluated for adequacy with a Diet Score. An Activity Score was derived from 15 questions on basic and instrumental activities of daily living. Demographic, health, psychosocial, functional and behavioural factors that could influence diet intake were measured with standardized, validated scales. Diet Score was significantly and positively associated with income, hearing, education, satisfaction with use of time, number of medications and age. Activity Score, the belief that health affects activity level, shopping behaviour and smoking were negatively and significantly associated with Diet Score. Many factors influence the dietary intake of seniors, including extent of functional limitations. This analysis suggests that the most dependent seniors living in the community consume better diets than those seniors with few limitations; this finding is probably a result of informal and formal support services for these functionally dependent seniors.
