  • The impact of food aid interventions on food insecurity, diet quality and mental health in households with children in high-income countries: a systematic review

  • Objective:

    Households with children accessing food aid in high-income countries are often food insecure. We aimed to review the evidence on food aid interventions in households with children and impact on food insecurity, diet quality and mental health.

    Design:

    A systematic search was conducted using Web of Science, MEDLINE, CINAHL and PsycINFO. Articles published from January 2008 to July 2022 including cross-sectional, cohort and interventional studies in high-income countries were eligible.

    Setting:

    Food aid is defined as the use of interventions providing free food items by community and/or charitable organisations.

    Participants:

    Two-parent, lone parent or households with a primary caregiver with at least one child ≤ 18 years.

    Results:

    From a total of 10 394 articles, nine were included. Food banks, mobile pantry combined with a free meal for children, backpack provision during school term and food parcel home delivery interventions were evaluated. Food bank models offering additional support such as community programmes, health and social services, cooking classes and free meals for children, client-choice-based models and programmes providing convenient access were associated with improved food security and diet quality (increased intake of wholegrains, fruit and vegetables). One study reported an improvement in mental health and food bank access at the end of 18 months but not at earlier timepoints and one study reported no change in parents’ mental health.

    Conclusions:

    Accessing food aid was linked to improved diet quality and reduced food insecurity in some studies. Allowing clients to choose food items and providing support services were most effective.

  • Humiliation as a Harm of Sexual Violence: Feminist versus Neoliberal Perspectives

  • This essay provides an account of humiliation as a manifestation of the relationship one has to oneself. This account elucidates two important insights: first, that all sexual violence and not only public gang rape humiliates and, second, that appeals to the neoliberal notion of resilience undermine feminist efforts to counter sexual violence. The first part of the essay provides an overview of the idea of a relation of self to self and its significance, presents humiliation specifically as a manifestation of the self‐relation, and then turns to feminist analyses of sexual violence and its effects in order to illustrate how sexual violence against women humiliates. The second part of the essay illustrates that, given the nature and function of humiliation, neoliberal discourse generally and that of resilience more specifically reproduce the individuation and exclusion that characterize humiliation and are thus detrimental to feminist efforts toward countering sexual violence against women. I conclude by encouraging feminist anti‐sexual violence efforts that at least counter individuation and at best promote collective and inclusive solidarity.

    Foucault the experimenter

    Michel Foucault was not a systematic thinker. He referred to himself as an “experimenter” as opposed to a “theorist” (1991a: 27); eschewed the labelling of his work in terms of existing categories; and asserted that “thinking differently” and self-transformation, rather than “validating what is already known”, lay at the core of his philosophical work (1990b: 910). “I don't feel that it is necessary to know exactly what I am,” Foucault states in a 1982 interview:

    The main interest in life and work is to become someone else that you were not in the beginning. If you knew when you began a book what you would say at the end, do you think you would have the courage to write it? What is true for writing and for a love relationship is true also for life. The game is worthwhile insofar as we don't know what will be the end.

    In addition to being unsystematic, Foucault's work also challenges fundamental aspects of the Western philosophical tradition. As he sees it, philosophers have exerted much intellectual time and effort and devoted many pages to creating a dualistic and overly simplified worldview that valorizes aspects of human existence that provide us with a false sense of our own ability to gain certainty about the world, and to thereby become masters of it and ourselves. This worldview imbues us with a false and misguided sense of security that, nonetheless, because it is preferable to the threat that uncertainty appears to pose, ensures the reproduction and eventual systematizing of the same faulty thinking.

    Previous chapters have illustrated that for Foucault subjectivity is not a state we occupy but rather an activity we perform. Moreover, it is an activity that always takes place within a context of constraint. We constitute ourselves as subjects (we are enabled) by way of various “practices of the self”, which include activities like writing, diet, exercise and truth-telling. At the same time, we are constituted (we are constrained) in so far as the way in which we undertake these practices is shaped by institutions such as schools, courts of law, hospitals and the state security apparatus, as well as by the more general prevailing norms and values of the society in which we live.

    Put differently, subjectivity is not distinct from but is rather formed in and through relations of power. There are not emancipatory institutions and norms that enable us, on the one hand, and oppressive or normalizing institutions and norms that constrain us, on the other; rather, we are simultaneously enabled and constrained by the same institutions and norms. We therefore find ourselves confronted with the task of figuring out when and how we are enabled and when and how we are constrained, of determining ways in which existing practices have the potential to loosen constraints and thus resist normalization, and of employing those practices not only for that purpose, but also in order to develop new and different practices – new and different ways of relating to ourselves and others. We need, in other words, to be able to reflect critically on the very process of becoming a subject.

    Neighbourhood differences in access to fresh fruit and vegetables may explain social inequalities in diet. Investigations have focused on variations in cost and availability as barriers to the purchase and consumption of fresh produce; investigations of quality have been neglected. Here we investigate whether produce quality systematically varies by food store type, rural–urban location and neighbourhood deprivation in a selection of communities across Scotland.

    Design

    Cross-sectional survey of twelve fresh fruit and vegetable items in 288 food stores in ten communities across Scotland. Communities were selected to reflect a range of urban–rural settings and a food retail census was conducted in each location. The quality of twelve fruit and vegetable items within each food store was evaluated. Data from the Scottish Executive were used to characterise each small area by deprivation and urban–rural classification.

    Results

    Quality of fruit and vegetables within the surveyed stores was high. Medium-sized stores, stores in small town and rural areas, and stores in more affluent areas tended to have the highest-quality fresh fruit and vegetables. Stores where food is secondary, stores in urban settings and stores in more deprived areas tended have the lowest-quality fresh produce. Although differences in quality were not always statistically significant, patterns were consistent for the majority of fruit and vegetable items.

    Conclusions

    The study provides evidence that variations in food quality may plausibly be a micro-environmental mediating variable in food purchase and consumption and help partially explain neighbourhood differences in food consumption patterns.
