Jusqu'il y a 15 ou 20 ans, des nombreux nutritionnistes étaient intéressés principalement dans les effets de l'alimentation sur des paramètres de santé comme la pression sanguine et les concentrations de cholestérol dans le sérum. Ces études ont signifié sans doute une contribution très important à la compréhension actuelle de la relation entre l'alimentation et la santé. Cependant, et pour de nombreuses raisons, il n'y a pas eu beaucoup de monde qui a essayé d'expliquer ces effets au niveau moléculaire. Toutefois, il paraîtra qu'aujourd'hui la tendance a était renversé; un grand nombre de travaux de recherche sont dédié a l'exploration des effets des components de l'alimentation à niveau moléculaire. Ce type de recherche a était fait possible grâce, entre autres facteurs, à l'implémentation de techniques des sciences plus fundamentaux à la recherche dans la nutrition. Ce aussi la disponibilité des séquences du génome qui a accéléré ce changement d'intéret. Les objectives de ce projet sont l'obtention d'une information précis sur les réponses métaboliques et moléculaires des cellules et des tissues (ou même de l'organisme entier) aux components diététiques. Dans ces études, les interactions entre l'alimentation et l'héritage génétique, ainsi qu'entre l'alimentation et des conditions physiologiques et psychologiques doivent aussi etre adressées. Cependant, il n'est pas seulement important l'obtention d'information sur les mécanismes, mais aussi sur les conséquences fonctionnelles pour l'organisme. Une dernière question se pose sur l'usage de cette information pour le développement de tests capable de prescrire des conseils diététiques aux subpopulations spécifiques. Ces questions stimulantes peuvent juste etre abordées à travers d'une approximation intégrée combinant l'expertise de plusieurs disciplines.

Résumé

La distribution intracellulaire des micronutriments ainsi que leur absorption sont importantes pour les fonctions cellulaires. Dans certains cas la distribution des micronutriments ou des protéines associées est déterminée par des mécanismes liés à l'expression des gènes. La région 3' non traduite (3'UTR) de l'ARNm de la métallothioneine-1 détermine la localisation de ce message et, par conséquent, la localisation intracellulaire de la protéine qu'il code. En utilisant des cellules transfectées nous avons montré que la métallothioneine-1 est transportée vers le noyau ou elle exerce un rôle dans la protection contre le stress oxydant et les dommages causés à l'ADN. Quand l'apport nutritionnel en Se est limité, l'expression des sélénoproteines est altérée. Toutefois celle-ci n'est pas affectée de fac¸on identique pour toutes les sélénoproteines; le Se disponible étant utilisé de fac¸on prioritaire pour la synthèse de certaines d'entre elles. Cet ordre de priorité met en jeu des différences dans la traduction et la stabilité de leur ARNm qui sont sous le controle de séquences dans la région 3' non traduite. Potentiellement, des variations génétiques affectant ces mécanismes régulateurs peuvent moduler les besoins en nutriments. Des polymorphismes génétiques ont été décrits dans le 3'UTR des ARNm de deux sélénoproteines; l'un d'entre eux affectant la synthèse de la sélénoproteine correspondante. Ces exemples illustrent comment des approches moléculaires peuvent contribuer à accroître notre compréhension du métabolisme et des besoins en nutriments à différents niveaux. Premièrement, elles permettent d'étudier les effets régulateurs des gènes et de leurs produits. Ensuite, la compréhension de ces effets peut fournir un modèle pour étudier le métabolisme des nutriments au niveau cellulaire. Ainsi, lorsque des effets essentiels sont identifiés, la connaissance du génome humain et les bases de données sur les polymorphismes génétiques constituent des outils complémentaires pour définir l'étendue de la variation génétique des gènes revêtant une importance nutritionnelle. Enfin, la fonctionnalité de ces variations peut être définie et des sous-groupes de la population, possédant des besoins nutritionnels différents, peuvent etre identifiés.

Résumé

La vitamine E, l'antioxydant liposoluble le plus important, fut découverte à l'Université de Californie à Berkeley en 1922. Depuis sa découverte, les études sur les tocophérols et les tocotrienols que constitue cette vitamine, ont été centrées pour la plupart sur leurs propriétés antioxydantes. En 1991, le groupe de Angelo Azzi (Boscoboinik et al. 1991a,b) fut le premier à décrire les fonctions autres que les antioxydantes et de transmission de signaux de l'α-tocophérol, en démontrant la régulation par la vitamine E de l'activité de la protéine kinase C dans les cellules de muscle lisse. Au niveau de la transcription, l'²-tocophérol module l'expression de la protéine de transfert hépatique de l'α-tocophérol, ainsi que l'expression du ge`ne alpha1 du collage`ne du foie, du ge`ne de la collagénase et du ge`ne de l'α-tropomyosine. Récemment, un facteur de transcription dépendant du tocophérol (la protéine associée au tocophérol) a été découvert. Il a été démontré sur des cellules cultivées que la vitamine E inhibe l'inflammation, l'adhésion cellulaire, l'agrégation des plaquettes et la prolifération des cellules de muscle lisse. Les avancées récentes de la biologie moléculaire et des techniques génomiques ont conduit à la découverte de nouveaux ge`nes et des mécanismes de transduction des signaux sensibles à la vitamine E.

Alors que l'alimentation de l'homme a beaucoup évoluée depuis ses origines, le génome humain est resté très stable. Pourtant, de très nombreux gènes ont des polymorphismes connus. En fait, on considère maintenant que les principales pathologies humaines (maladies cardiovasculaires, diabète, obésité et cancers) résultent d'une interaction entre des facteurs de succeptibilité génétique et des facteurs de l'environement, dont l'alimentation. Dans le domaine du métabolisme des lipoprotéines et des maladies cardiovasculaires, des polymorphimes de plusieurs gènes ont été identifiés et associés aux niveaux des paramètres lipidiques ou aà des réponses variables aux régimes, comme pour les apoprotéines (apo) E, B, A-IV et C-III, le LDL récepteur, la protéine microsomiale de transport (MTP), la protéine de liason des acides gras (FABP), la protéine de transport des esters de cholésterol (CETP), la lipoprotéine lipase ou la lipase hepatique. Re´sume´Les allergies alimentaires repre´sentent un sujet important pour la sante´. La pre´valence chez les adultes d'Europe Occidentale est estime´e entre 1 et 2 %, tandis que parmi les enfants elle est plus e´leve´e (autour de 5 %). La plupart des allergies confirme´es sont associe´es à une gamme de produits relativement re´duite, dont le lait de vache, les ufs, les noix, les cacahue´tes, le ble´, les poissons et les fruits de mer. Cependant, il est connu que la pre´valence des allergies aux aliments spe´cifiques varie selon la zone ge´ographique, en raison principalement des diffe´rences dans les habitudes alimentaires. Bien que nous n'en ayons pas la preuve formelle, on suppose que le nombre de cas d'allergie alimentaire, comme celui des autres formes de maladies atopiques, est en augmentation. Il n'y a aucun doute que la pre´disposition ge´ne´tique est un de´terminant important. Toutefois, il est connu que l'acquisition de la sensibilisation aux prote´ines alimentaires et par la suite au proble`me d'allergie est influence´e par un e´ventail de facteurs environnementaux, ainsi que par le moment, la dure´e et l'intensite´ de l'exposition. De plus, la nature de l'allerge`ne lui-meme peut avoir un rle tre`s important sur la se´ve´rite´ et la persistance de la maladie clinique. Notre objectif est ici de discuter de l'importance de quelques-unes de ces variables dans le contexte des re´ponses allergiques immunoglobuline E-dependant.

Résumé

La génomique de la volaille a bénéficié des avancements technologiques rapides acquis en génomique humaine et sur les organismes modèles. Certains outils et certains approches sont maintenant bien établis chez le poulet, y compris les cartes et marqueurs (génétiques comme physiques), mapping loci pour caractéres quantitatifs, mapping comparative, ressources expressed sequence tag et bacterial artificial chromosome, et mapping physique. De plus, la phase suivante de la découverte génétique, la génomique fonctionnelle, est en cours. Les progre`s dans le mapping de loci pour caracte`res quantitatifs de croissance et d'adiposité seront discutés pour illustrer ces nouvelles technologies et ces nouvelles approches dans l'étude de la génétique et de la physiologie avicole.

Re´sume´Parmi les ge`nes dont les produits jouent un rle critique dans la re´gulation de la re´ponse immune, on trouve les familles de ge`nes de l'antige`ne leucocyte humain (ALH) et de la cytokine. Les ge`nes ALH sont les plus polymorphes du ge´nome humain, et ce polymorphisme entraine des diffe´rences fonctionnelles des mole´cules ALH exprime´es, qui se traduit par des diffe´rences individuelles dans la manie`re dont des peptides antige´niques se pre´sentent aux cellules T. De plus, un nombre conside´rable de polymorphismes des ge`nes associe´s a la cytokine ont e´te´ identifie´s, la plupart d'entre eux se produisent au niveau du promoteur de ces ge`nes, ce que dans de nombreux cas se traduit par l'expression diffe´rentielle in vitro de leurs respectifs produits ge´ne´tiques pro- ou anti-inflammatoire. Des polymorphismes ALH particuliers aboutirent a` des associations bien de´finies avec un grand nombre de maladies immunologiquement medie´es, parmi lesquelles quelques-unes avec des facteurs de risque die´te´tique. Par exemple, des individus de ge´notype HLA-DQA1*0501, DQB1*0201 ont un risque deux cent fois plus grand de de´velopper une intole´rance au gluten alimentaire (maladie ce´liaque), et des facteurs additionnels en relation avec l'ALH peuvent influencer le de´veloppement d'un lymphome maligne dans une maladie ce´liaque existante. De mme, des alle`les HLA-DRB1 qui partage une se´quence motive commune constituent le premier facteur de risque ge´ne´tique de l'arthrite rhumatode. L'influence de polymorphismes associe´s a une expression de cytokine diffe´rentiel sur la susceptibilite´ aux maladies pre´sent dans ce moment un grand inte´rt. La plupart de l'attention a e´tait centre´ les associations avec la susceptibilite´ de maladies medie´es immunologiquement be´nignes, parmi eux un grand nombre de maladies de l'intestin. Toutefois, un travail re´cent dans notre laboratoire indique que les polymorphismes de la cytokine peuvent influencer la susceptibilite´ a` certains cancers (ainsi que son pronostic), entre eux le me´lanome de peau maligne et des tumeurs solides qui peuvent tre affecte´s par l'alimentation, comme le cancer de prostate (collaboration avec le CRC/BPG e´tude familial du cancer de prostate Royaume-Uni). De plus, des travails pre´liminaires sugge`rent que la modulation par l'alimentation des niveaux d'expression de certaines cytokines dans des sujets humains peut tre de´pendent du ge´notype.

A la différence des mutations génétiques rares mais séveères qui causent les maladies monogéniques, les facteurs génétiques qui modulent la susceptibilité individuelle aux maladies multifactorielles (maladies cardiovasculaires, cancer, diabeète) sont des formes fréquentes, fonctionnellement différentes, des ge`nes (polymorphismes) qui ont généralement un effet modeste au niveau individuel, mais en raison de leur grande fréquence dans la population, peuvent être associés à un risque attribuable élevé. Les facteurs environnementaux peuvent révéler ou faciliter l'expression phénotypique de ces ge`nes de susceptibilité. En effet, dans le cas des maladies communes, les effets génétiques peuvent etre considérablement amplifiés en présence de facteurs déclenchants. On sait maintenant que la plupart des geènes de susceptibilité aux maladies communes n'ont pas un rôle étiologique primaire dans la prédisposition a` la maladie mais agissent plutot comme des modificateurs dans la réponse a` des facteurs exoge`nes tels que le stress, l'environnement, la maladie, la prise de médicament, le régime alimentaire. Une meilleure caractérisation des interactions entre facteurs génétiques et environnementaux constitue un élément clé dans la compréhension de la pathogéneèse des maladies multifactorielles. Cet article présente trois exemples d'interaction gène-environnement dans le domaine des maladies coronariennes.

The ultimate goal of an organism is to maximise its inclusive fitness, and an important sub-goal must be the optimisation of the lifetime pattern of food intake, in order to meet the nutrient demands of survival, growth and reproduction. The conventional assumption that fitness is maximised by maximising daily food intake, subject to physical and physiological constraints, has been challenged recently. Instead, it can be argued that fitness is maximised by balancing benefits and costs over the organism's lifetime. The fitness benefits of food intake are a function of its contribution to survival, growth (including necessary body reserves) and reproduction. Against these benefits must be set costs. These costs include not only extrinsic foraging costs and risks, such as those due to predation, but also intrinsic costs associated with food intake, such as obesity and oxidative metabolism that may reduce vitality and lifespan. We argue that the aggregate of benefits and costs form the fitness function of food intake and present examples of such an approach to predicting optimal food intake.

It is widely believed that body fatness (and hence total body mass) is regulated by a lipostatic feedback system. This system is suggested to involve at least one peripheral signalling compound, which signals to the brain the current size of body fat stores. In the brain the level of the signal is compared with a desirable target level, and food intake and energy expenditure are then regulated to effect changes in the size of body fat stores. There is considerable support for this theory at several different levels of investigation. Patterns of body-mass change in subjects forced into energy imbalance seem to demonstrate homeostasis, and long-term changes in body mass are minor compared with the potential changes that might result from energy imbalance. Molecular studies of signalling compounds have suggested a putative lipostatic signal (leptin) and a complex network of downstream processing events in the brain, polymorphisms of which lead to disruption of body-mass regulation. This network of neuropeptides provides a rich seam of potential pharmaceutical targets for the control of obesity. Despite this consistent explanation for the observed phenomena at several different levels of enquiry, there are alternative explanations. In the present paper we explore the possibility that the existence of lipostatic regulation of body fatness is an illusion generated by the links between body mass and energy expenditure and responses to energy imbalance that are independent of body mass. Using computer-based models of temporal patterns in energy balance we show that common patterns of change in body mass following perturbation can be adequately explained by this ‘non-lipostatic’ model. This model has some important implications for the interpretations that we place on the molecular events in the brain, and ultimately in the search for pharmaceutical agents for alleviation of obesity.

Since the discovery of the protein product of the ob/ob gene, leptin, knowledge of the neurochemical pathways involved in the regulation of feeding has increased enormously. Our understanding of the mechanisms regulating food intake in man has also progressed greatly over a similar time span. Previous research into the regulation of food intake has largely proceeded through a reductionist approach, defining ever-smaller components of these mechanisms. This research strategy has been very productive and instructive, and has yielded a great deal of information on the specific putative components linking energy status and food intake. However, to fully understand the regulation of feeding it is important that these components are systematically reconstructed to investigate relevant interactions. In the present review recent data relating to interactions between systems proposed to be involved in feeding regulation will be highlighted. The review will be directed predominantly (but not exclusively) towards the regulation of human feeding.

Human food intake is driven by necessity. We eat to live, but as Brillat-Savarin and others have noted throughout history, in affluent societies eating is a pleasure and becomes more than a means to an end.Eating signifies lifestyle choice and it has considerable meaning in our society beyond the acquisition of essential energy and nutrients. Thus, it is that the study of human food intake, particularly food choice, in contrast to food intake in other animals, tends to be skewed towards measures of behavioural, social and environmental influences rather than on precise physiological processes reflecting metabolism and nutrient partitioning. The dichotomy between physiological and psychological measures is a false one, since all behaviours are necessarily expressed through physiological systems. However, in the field of human food intake research the dichotomy refers to the divergent strands of interest in either psychological or physiological processes underlying intake and appetite. The present review considers both psychological and physiological measures in promoting our understanding of the human appetite system. The overall conclusion is that the burgeoning interest in identifying appetite suppressant drugs to combat obesity and in genotyping alongside behavioural phenotyping will close the gap between psychological and physiological perspectives on human food intake.

Many experiments have shown that foraging animals are sensitive to the riskiness, or variance, associated with alternative food sources. For example, when offered a choice of a constant feeding option that always offers three seeds, and a risky option that offers either no seeds or six seeds with equal probability, most animals tested will be either risk-averse or risk-prone, preferring either the fixed or variable option respectively. Whether animals are risk-averse or risk-prone appears to depend on a range of factors, including the energetic status of the forager, the type of variance associated with the feeding options and even the number of feeding options between which the animal is choosing. These behavioural phenomena have attracted much theoretical interest, and a range of different explanations have been suggested, some based on a consideration of the psychological mechanisms involved in decision making, and others on a consideration of the Darwinian fitness consequences of risk-averse or risk-prone behaviour for the forager. A brief review of the recent literature on risk-sensitive foraging will be presented, focusing on results from the two experimental systems with which I have been involved: wild rufous hummingbirds (Selasphorus rufus) foraging on artificial flowers; European starlings (Sturnus vulgaris) foraging in operant boxes in the laboratory. It will be argued that to understand the foraging decisions of animals account needs to be taken of both the psychological mechanisms underlying decision-making and the fitness consequences of different decisions for the forager.

Present-day human eating behaviour in industrialised society is characterised by the consumption of high-energy-density diets and often unstructured feeding patterns, largely uncoupled from seasonal cycles of food availability. Broadly similar patterns of feeding are found among advantaged groups in economically-emerging and developing nations. Such patterns of feeding are consistent with the evolutionary ecological understanding of feeding behaviour of hominids ancestral to humans, in that human feeding adaptations are likely to have arisen in the context of resource seasonality in which diet choice for energy-dense and palatable foods would have been selected by way of foraging strategies for the maximisation of energy intake. One hallmark trait of human feeding behaviour, complex control of food availability, emerged with Homo erectus (19 × 106-200 000 years ago), who carried out this process by either increased meat eating or by cooking, or both. Another key trait of human eating behaviour is the symbolic use of food, which emerged with modern Homo sapiens (100 000 years ago to the present) between 25 000 and 12 000 years ago. From this and subsequent social and economic transformations, including the origins of agriculture, humans have come to use food in increasingly elaborate symbolic ways, such that human eating has become increasingly structured socially and culturally in many different ways.

Both jurisdictions of Ireland have high rates of chronic degenerative diseases, particularly of the cardiovascular system, and Irish migrants have worse health profiles, often lasting at least two generations. The influence of socio-demographic variation over the life course, and what role diet plays, has not been well researched in epidemiological terms. There is a long history of an unusual Irish diet. Estimated dietary fat intake (% total energy intake) in 1863 was only 9, but had reached 30 in 1948 and 34 in 1999. Conversely, carbohydrate intake has fallen steadily over 150 years. From 1948 onwards household budget survey data illustrate patterns of increasing urbanisation and socio-economic gradients in food availability. The National Survey of Lifestyles, Attitudes and Nutrition, (n 6539, 622 % response rate) provides clear evidence of inverse social-class gradients in intake of fruit and vegetables and dairy products and in reported patterns of healthy eating. Median carbohydrate and vitamin C levels are higher among social classes 1-2 and mean saturated fat intake is lower. International comparisons indicate a continuing, if narrowing, north-south gradient across Europe. Data from the Boston-Ireland study suggest a crossover in both dietary intake patterns and risk of heart disease in Ireland and the USA in the 1970s. Contemporary comparative data of middle-aged Irish and American women demonstrate patterns of diet intake and inactivity consistent with the modern epidemic of obesity and non-insulin-dependent diabetes. Thus, dietary variations within and between countries and over time are consistent with chronic disease patterns in contemporary Ireland.

The search for the causes of CHD has been guided by a `destructive' model, which proposes that influences acting in adult life, such as smoking, obesity or high saturated fat intakes, lead to an acceleration of age-related destructive processes, including a rise in blood pressure and the formation of atheroma. One explanation for the failure of the model to account for, or indeed to prevent rising epidemics of CHD, is that individuals are heterogeneous in their responses to such influences. This heterogeneity in response is linked to different paths of early growth. The recent discovery that individuals who develop CHD grew differently from other individuals during fetal life and in childhood has led to a new `developmental' model for the disease. Reduced fetal growth followed by poor growth in infancy leads to an increased risk of development of CHD, and its associated conditions, stroke, hypertension and impaired glucose tolerance. These effects are compounded by accelerated weight gain, which may represent 'compensatory growth' in childhood.

Glucose intolerance represents a spectrum of abnormalities, including impaired fasting glucose, impaired glucose tolerance and type 2 diabetes. It is a major public health challenge worldwide, with rapidly increasing prevalence rates in both developed and developing countries. This global epidemic of diabetes is largely driven by the globalisation of Western culture and lifestyles. Specifically, there is now evidence from large-scale observational studies, and from intervention studies, of powerful synergistic interactions between diet, obesity, exercise, smoking and alcohol in the development of glucose intolerance. It is estimated that > 90 % of cases of type 2 diabetes in the population could be prevented with the adoption of a prudent diet (high in cereal fibre and polyunsaturated fatty acids and low in trans-fatty acids and glycaemic load), avoidance of overweight and obesity (BMI < 25 kg/m2), engagement in moderate to vigorous physical activity for at least 05 h/d, non-smoking and moderate alcohol consumption. These findings are biologically plausible and have major public health implications. They form the basis for a clear, simple and coherent message for health promotion and public policy. However, to make progress on these issues health will need to be placed at the centre of public policy and relevant vested interests tackled, notably in the food, entertainment, tobacco and automobile industries.

Socially-and culturally-patterned differences in food habits exist both between and within European populations. Daily individual food availability data, collected through the national household budget surveys (HBS) and harmonized in the context of the Data Food Networking (DAFNE) project, were used to assess disparities in food habits of seven European populations and to evaluate dietary changes within a 10-year interval. The availability of selected food items was further estimated according to the educational level of the household head and, based only on the Greek HBS data, according to quintiles of the household's food purchasing capacity. Results for overall food availability support the north-south differentiation in food habits. Generally, the availability of most food items, including foods such as vegetable fats, animal lipids and sugar products, has decreased over the 10 years. Households in which the head was in the higher education categories reported lower availability for most food items, with the exception of low-fat milk, fresh fruit, animal lipids and soft drinks; the latter showing a sharp increase even within southern European households. The household's food purchasing capacity can be used as an indicator of socio-economic status, with higher values being associated with lower status. Greek households of lower social class follow a healthier diet in terms of greater availability of vegetable oils, fresh vegetables, legumes, fish and seafood. Data from the DAFNE databank may serve as a tool for identifying and quantifying variation in food habits in Europe, as well as for providing information on the socio-economic determinants of food preferences.

Development of effective intervention strategies to meet the needs of people with ethnic minority origins is dependent on two factors: an understanding of the modifiable risk factors which can form the basis of intervention; an understanding of the relevant health behaviours so that appropriate strategies can be designed. The present paper briefly reviews the evidence concerning the part that nutritional and dietary factors play in the aetiology of the observed patterns of disease in these groups and the limitations of the data as a basis for intervention. Consideration is also given to the available information concerning factors influencing health behaviour (particularly eating behaviour) and the applicability of commonly-used models of behaviour change to people of ethnic minority origin. Finally, the results of nutrition intervention programmes will be examined with a view to identifying lessons for the future.

Budget standards are specified baskets of goods and services which, when priced, can represent predefined living standards. ‘Low cost but acceptable’ (LCA) is a minimum income standard, adequate to provide warmth and shelter, a healthy and palatable diet, social necessities, social integration, avoidance of chronic stress and the maintenance of good health (physical, mental and social) in a context of free access to good-quality health care, good-quality education and social justice. The LCA food budget standard identifies a basket of foods and corresponding menus which provides (for a given household composition) a palatable diet that is consistent with prevailing cultural norms, and that satisfies existing criteria for health in relation to dietary reference values, food-based dietary guidelines and safe levels of alcohol consumption. Two previous studies that explored the relationship between diet and food expenditure in low-income households suggested that the amount spent on food was a good predictor of dietary adequacy, growth and health in children. The current paper will focus on diet and measures of deprivation in 250 low-income households in London. Households were screened for material deprivation (e.g. no car, no fixed line telephone, in receipt of Income Support) using a doorstep questionnaire. Diet was assessed using four 24 h recalls based on the ‘triple pass’ method. Expenditure on food and other aspects of household circumstances were assessed by face-to-face interview. Food expenditure in these households was characterized in relation to food budget standards. Further analyses explored the relationships between food expenditure and dietary adequacy, growth in children and measures of deprivation.
