To explore whether and how group cognitive-behavioural therapy (GCBT) plus medication differs from medication alone for the treatment of generalized anxiety disorder (GAD).
170 patients were randomly assigned to the GCBT plus duloxetine (n=89) or duloxetine group (n=81). The primary outcomes were Hamilton Anxiety Scale (HAMA) response and remission rates. The explorative secondary measures included score reductions from baseline in the HAMA total, psychic and somatic anxiety subscales (HAMA-PA, HAMA-SA), the Hamilton Depression Scale (HAMD), the Severity Subscale of Clinical Global Impression Scale (CGI-S), Global Assessment Function (GAF), and the 12-item Short-Form Health Survey (SF-12). Assessments were conducted at baseline, 4-weeks, 8-weeks and 3-monthsfollow-up.
At 4-weeks, HAMA response (GCBT group 57.0% vs. control group 24.4%, p=0.000, Cohen d=0.90) and remission rates (GCBT group 21.5%vs. control group 6.2%, p=0.004; d=0.51), and most secondary outcomes (all p<0.05, d=0.36-0.77) showed that the combined therapy was superior. At 8-weeks, all the primary and secondary significant differences found at 4-weeks were maintained with smaller effect sizes (p<0.05, d=0.32-0.48) . At 3-months follow-up, the combined therapy was only significantly superior in the HAMA total (p<0.045, d=0.43) and HAMA-PA score reductions (p<0.001, d=0.77). Logistic regression showed superiority of the combined therapy for HAMA response rates (OR=2.12, 95% CI 1.02-4.42, p=0.04); and remission rates (OR=2.80, 95% CI 1.27-6.16, p=0.01).
Compared with duloxetine alone, GCBT plus duloxetine showed significant treatment response for GAD over a shorter period of time, particularly for psychic anxiety symptoms, which may suggest GCBT was effective in changing cognitive style.
Disturbances in lipid metabolism are at the core of several health issues facing modern society, including fatty liver and obesity. The SREBP-1 is one important transcription factor regulating lipid metabolism, but the relevant mechanism still remains unknown. The present study determined the transcriptional regulation of SREBP-1 and its target genes (including accα, fas and scd1) in a freshwater teleost, grass carp Ctenopharyngodon idella. We cloned and characterized the 1988 bp, 2043 bp, 1632 bp and 1889 bp sequences of srebp-1, accα, scd1, and fas promoters, respectively. A cluster of putative binding sites of transcription factors, such as SP, YY1, NF-Y, SREs and E-box element, were predicted on their promoter regions. nSREBP-1 overexpression reduced srebp-1 promoter activity, increased scd1 and fas promoter activity, but did not influence accα promoter activity. The site mutation and EMSA analysis indicated that srebp-1, fas and scd1 promoters, but not accα promoter, possessed SREs. In CIK cells of grass carp, nSREBP-1 overexpression significantly reduced srebp-1 mRNA expression, and up-regulated miR-29 mRNA expression. The 3’UTR of srebp-1 possessed the potential miR-29 binding site and miR-29 up-regulated the luciferase activity of srebp-1 3’UTR and srebp-1 mRNA expression, implying a self-activating loop of SREBP-1 and miR-29 in grass carp. Based on these results above, we found two novel transcriptional mechanisms for SREBP-1 in grass carp: (1) the auto-regulation sited on the SREBP-1 promoter regions was suppressive; (2) there was a self-activating loop of SREBP-1 and miR-29.
We assessed inheritance of resistance to sugarcane brown rust (Puccinia melanocephala) in selfing F1 populations of wild sugarcane germplasm Erianthus rockii ‘Yundian 95-19’ and E. rockii ‘Yundian 95-20’. We tested parent and selfing F1 individuals for the brown rust resistance gene, Bru1, that has been shown to confer resistance to brown rust in sugarcane. The Bru1 gene was not detected in E. rockii ‘Yundian 95-19’, E. rockii ‘Yundian 95-20’ or their selfing F1 individuals, and we found there was segregation of resistance in the two selfing F1 populations (segregation ratio: 3:1). The results confirmed resistance in E. rockii ‘Yundian 95-19’ and E. rockii ‘Yundian 95-20’ to sugarcane brown rust is controlled by a novel, single dominant gene.
Applying artificial intelligence to materials research requires abundant curated experimental data and the ability for algorithms to request new experiments. ESCALATE (Experiment Specification, Capture and Laboratory Automation Technology)—an ontological framework and open-source software package—solves this problem by providing an abstraction layer for human- and machine-readable experiment specification, comprehensive and extensible (meta-) data capture, and structured data reporting. ESCALATE simplifies the initial data collection process, and its reporting and experiment generation mechanisms simplify machine learning integration. An initial ESCALATE implementation for metal halide perovskite crystallization was used to perform 55 rounds of algorithmically-controlled experiment plans, capturing 4336 individual experiments.
The antibacterial hydrogels can be widely used in the biomedical area owing to their excellent properties. The main limitation of antibacterial hydrogels is their poor mechanical strength. In this study, the novel hydrogels were fabricated with a mixture of silk fibroin (SF), chitosan (CH), agarose (AG), and silver nanoparticles (SNPs) via facile reaction condition without inorganic substances. The mechanical property of these fabricated hydrogels can be modulated by the concentration of SF or AG. The rheological studies demonstrated enhanced elasticity of CH-doped hydrogels. Because of the presence of CH and Ag in hydrogels, the antimicrobial property against gram-positive and gram-negative bacteria was exhibited. Cytocompatibility test proved the very low toxic nature of the hydrogels. In addition, these composite hydrogels have a smaller porosity, higher swelling ratio, and good compatibility, indicating their great potential for biomedical application.
The parent-child relationship undergoes substantial reorganization over the transition to adolescence. Navigating this change is a challenge for parents because teens desire more behavioral autonomy as well as input in decision-making processes. Although it has been demonstrated that changes in parental socialization approaches facilitates adolescent adjustment, very little work has been devoted to understanding the underlying mechanisms supporting parents’ abilities to adjust caregiving during this period. Guided by self-regulation models of parenting, the present study examined how parental physiological and cognitive regulatory capacities were associated with hostile and insensitive parent conflict behavior over time. From a process-oriented perspective, we tested the explanatory role of parents’ dysfunctional child-oriented attributions in this association. A sample of 193 fathers, mothers, and their early adolescent (ages 12–14) participated in laboratory-based research assessments spaced approximately 1 year apart. Parental physiological regulation was measured using square root of the mean of successive differences during a conflict task; cognitive regulation was indicated by set-shifting capacity. Results showed that parental difficulties in vagal regulation during parent-adolescent conflict were associated with increased hostile conflict behavior over time; however, greater set-shifting capacity moderated this association for fathers only. In turn, father's dysfunctional attributions regarding adolescent behavior mediated the moderating effect. The results highlight how models of self-regulation and social cognition may explain the determinants of hostile parenting with differential implications for fathers during adolescence.
Subcutaneous immunotherapy is an effective and safe treatment for allergic rhinitis and allergic asthma. Different symptom scores are used to evaluate the efficacy of subcutaneous immunotherapy in clinical trials.
A total of 58 allergic rhinitis patients sensitised to house dust mites, with or without mild asthma, were included. Symptom score, medication score, visual analogue scale score and quality of life were assessed before and after 6, 12 and 24 months of subcutaneous immunotherapy.
After two years of subcutaneous immunotherapy, asthma symptom scores nearly reached zero, whereas the scores remained higher for nasal symptoms. The changes in asthma symptom scores were markedly different (p < 0.05) and occurred faster than the changes in nasal symptom scores when compared between monosensitised and polysensitised groups. Significant reductions in visual analogue scale score and medication score were demonstrated after subcutaneous immunotherapy.
Two-year subcutaneous immunotherapy with house dust mite vaccine is an effective treatment for both monosensitised and polysensitised allergic patients. The changes in asthma symptom scores were markedly different and occurred quicker than the changes in nasal symptom scores in Chinese house dust mite allergic patients.
Few studies have examined the association of various types of Fe with colorectal cancer risk. The aim of this study was to investigate different forms and sources of Fe in relation to colorectal cancer risk in a Chinese population. A total of 2138 patients with colorectal cancer and 2144 sex- and age-matched (5-year interval) controls were recruited from July 2010 to November 2017. Dietary information was assessed by face-to-face interviews using a validated FFQ. Multivariable logistic regression was used to estimate the OR and 95 % CI on models. Intake of Fe from plants and Fe from white meat were inversely associated with the risk of colorectal cancer, while haem Fe and Fe from red meat were positively associated with colorectal cancer risk. The multivariable OR for the highest quartile v. the lowest quartile were 0·72 (95 % CI 0·59, 0·87, Ptrend<0·001) for Fe from plants, 0·54 (95 % CI 0·45, 0·66, Ptrend<0·001) for Fe from white meat, 1·26 (95 % CI 1·04, 1·53, Ptrend=0·005) for haem Fe and 1·83 (95 % CI 1·49, 2·24, Ptrend<0·001) for Fe from red meat intake, respectively. However, no significant association was found between the consumption of total dietary Fe, non-haem Fe, Fe from meat and colorectal cancer risk. This study showed that lower intake of Fe from plants and white meat, as well as higher intake of haem Fe and Fe from red meat, were associated with colorectal cancer risk in a Chinese population.
In this paper, we consider a nonlinear elliptic system which is an extension of the single equation derived by investigating the stationary states of the nonlinear Schrödinger equation. We establish the existence and uniqueness of solutions to the Dirichlet problem on the ball and entire space as the parameters within certain regions. In addition, a complete structure of different types of solutions for the radial case is also provided.
Polarization mode dispersion (PMD) in fibers for high-power lasers can induce significant frequency modulation to amplitude modulation (FM-to-AM) conversion. However, existing techniques are not sufficiently flexible to achieve efficient compensation for such FM-to-AM conversion. By analyzing the nonuniform transmission spectrum caused by PMD, we found that the large-scale envelope of the transmission spectrum has more serious impacts on the amount of AM. In order to suppress the PMD-induced FM-to-AM conversion, we propose a novel tunable spectral filter with multiple degrees of freedom based on a half-wave plate, a nematic liquid crystal, and an axis-rotated polarization-maintaining fiber. Peak wavelength, free spectral range (FSR), and modulation depth of the filter are decoupled and can be controlled independently, which is verified through both simulations and experiments. The filter is utilized to compensate for the PMD-induced FM-to-AM conversion in the front end of a high-power laser facility. The results indicate that, for a pulse with phase-modulation frequency of 22.82 GHz, the FM-to-AM conversion could be reduced from 18% to 3.2% within a short time and maintained below 6.5% for 3 h. The proposed filter is also promising for other applications that require flexible spectral control such as high-speed channel selection in optical communication networks.
Dilated Cardiomyopathy is a serious heart disorder that may induce sudden cardiac death and heart failure. Significant progress has been made in understanding the molecular basis of dilated cardiomyopathy. In previous studies, mutations in more than fifty genes have been identified in dilated cardiomyopathy patients. The purpose of this study was to detect the genetic lesion in a family from the central south of China affected by severe dilated cardiomyopathy.
Whole-exome sequencing combined with cardiomyopathy-related genes list were used to analyse the mutations of the proband. Co-segregation analysis was performed by Sanger sequencing.
Results and conclusions
Two novel heterozygous mutations – Myosin Binding Protein C: p.L1014RfsX6 and Titin: p.R9793X – were identified in the proband. The deletion mutation c.3041delT/p.L1014RfsX6 caused a premature stop codon at position 1020 in exon 28 of the Myosin Binding Protein C. The nonsense mutation, c.29377 C>T/ p. R9793X, of Titin was located in the highly evolutionarily conserved domain, resulting in truncation of the Titin protein as well. Co-segregation analysis further revealed that the Myosin Binding Protein C mutation came from his mother and the Titin mutation came from his father. Both mutations are reported in dilated cardiomyopathy patients for the first time. Our study not only provides a unique example of the genes and molecular mechanisms involved in dilated cardiomyopathy but also expands the spectrum of Myosin Binding Protein C and Titin mutations and contributes to the genetic diagnosis and counselling of dilated cardiomyopathy patients.
Global Navigation Satellite Systems (GNSSs) are easily influenced by the external environment. Signals may be lost or become abnormal thereby causing outliers. The filter gain of the standard Kalman filter of a loosely coupled GNSS/inertial navigation system cannot change with the outliers of the GNSS, causing large deviations in the filtering results. In this paper, a method based on a χ2-test and a dynamically adjusting filter gain method are proposed to detect and separately to suppress GNSS observation outliers in integrated navigation. An indicator of an innovation vector is constructed, and a χ2-test is performed for this indicator. If it fails the test, the corresponding observation value is considered as an outlier. A scale factor is constructed according to this outlier, which is then used to lower the filter gain dynamically to decrease the influence of outliers. The simulation results demonstrate that the observation outlier processing method does not affect the normal values under normal circumstances; it can also discriminate between single and continuous outliers without errors or omissions. The impact time of outliers is greatly reduced, and the system performance is improved by more than 90%. Experimental results indicate that the proposed methods are effective in suppressing GNSS observation outliers in integrated navigation.
A high power laser system was used to drive the ignition of inertial confinement fusion (ICF), of which the high energy, the uniform focal spot, the accurate laser waveform, and the synchronization between the laser beams are key parameters. To accomplish this, global laser characteristics control should be assured, which was the main purpose of the injection laser system. In this paper, the key technological progress involved in the improvement of the performance of the injection laser of SG-II is reported, including frequency domain control, time domain control, near-field spatial shaping, pre-amplifier technology, and the optical parametric chirped pulse amplification pump source.
Wave radiation and diffraction by a circular cylinder submerged below an ice sheet with a crack are considered based on the linearized velocity potential theory together with multipole expansion. The solution starts from the potential due to a single source, or the Green function satisfying both the ice sheet condition and the crack condition, as well as all other conditions apart from that on the body surface. This is obtained in an integral form through Fourier transform, in contrast to what has been obtained previously in which the Green function is in the series form based on the method of matched eigenfunction expansion in each domain on both sides of the crack. The multipole expansion is then constructed through direct differentiation of the Green function with respect to the source position, rather than treating each multipole as a separate problem. The use of the Green function enables the problem of wave diffraction by the crack in the absence of the body to be solved directly. For the circular cylinder, wave radiation and diffraction problems are solved by applying the body surface boundary condition to the multipole expansion, through which the unknown coefficients are obtained. Extensive results are provided for the added mass and damping coefficient as well as the exciting force. When the cylinder is away from the crack, a wide spacing approximation method is used, which is found to provide accurate results apart from when the cylinder is quite close to the crack.
To develop an artificial intelligence (AI)-based algorithm which can automatically detect food items from images acquired by an egocentric wearable camera for dietary assessment.
To study human diet and lifestyle, large sets of egocentric images were acquired using a wearable device, called eButton, from free-living individuals. Three thousand nine hundred images containing real-world activities, which formed eButton data set 1, were manually selected from thirty subjects. eButton data set 2 contained 29 515 images acquired from a research participant in a week-long unrestricted recording. They included both food- and non-food-related real-life activities, such as dining at both home and restaurants, cooking, shopping, gardening, housekeeping chores, taking classes, gym exercise, etc. All images in these data sets were classified as food/non-food images based on their tags generated by a convolutional neural network.
A cross data-set test was conducted on eButton data set 1. The overall accuracy of food detection was 91·5 and 86·4 %, respectively, when one-half of data set 1 was used for training and the other half for testing. For eButton data set 2, 74·0 % sensitivity and 87·0 % specificity were obtained if both ‘food’ and ‘drink’ were considered as food images. Alternatively, if only ‘food’ items were considered, the sensitivity and specificity reached 85·0 and 85·8 %, respectively.
The AI technology can automatically detect foods from low-quality, wearable camera-acquired real-world egocentric images with reasonable accuracy, reducing both the burden of data processing and privacy concerns.
The amplitude and period of the first frequency increased, but the amplitude and period of the second frequency decreased. Two new frequencies (10.5011 and 2.0484 c/d) were found. The frequency (2.0484 c/d) might be an interaction term from which resonances can occur and then lead amplitude and period variations.
